    Central Texas Extreme Weather and Climate Change Vulnerability Assessment of Regional Transportation Infrastructure

    This report presents the results of a Climate Resilience Pilot Project conducted by the Capital Area Metropolitan Planning Organization (CAMPO), the City of Austin Office of Sustainability, and sponsored in part by the Federal Highway Administration (FHWA). The specific purpose of this study was to assess the potential vulnerability of a limited selection of critical transportation assets in the CAMPO region to the effects of extreme weather and climate; to highlight lessons learned in the process, and to outline potential next steps toward enhancing the resilience of the region\u2019s transportation infrastructure. The assets evaluated include roadways, bridges, and rail, and the climate-related stressors considered were flooding, drought, extreme heat, wildfire, and extreme cold (icing). Commensurate with the region\u2019s Long-Range Transportation Plan (LRTP) \u2013 under development at the time of writing), the year 2040 was selected as the analysis horizon

    DTFH61-16-D-00052/Task 10The purpose of this study is to explore the potential to significantly enhance equity and reduce congestion experienced by city bus riders due to transportation network company (TNC) operations (most especially within corridors with significant bus service) through select TNC or government policy measures. The study analyzes data from a survey conducted by a large TNC, coupled with appended demographic and trip data, to ascertain the impacts of changing private party and shared party TNC pricing and trip times to encourage TNC customers to choose a shared instead of a ridealone trip. Compressing the difference in duration between shared and drive-alone trips (including wait time for initial pick up) by 5 minutes is shown to roughly double the proportion of TNC trips that customers choose to share

    An assessment of opportunities and challenges for public sector involvement in the maternal health voucher program in Uganda

    This is an open access article distributed under the terms of the Creative Commons Attribution License (http://creativecommons.org/licenses/by/2.0), which permits unrestricted use, distribution, and reproduction in any medium, provided the original work is properly cited.Background: Continued inequities in coverage, low quality of care, and high out-of-pocket expenses for health services threaten attainment of Millennium Development Goals 4 and 5 in many sub-Saharan African countries. Existing health systems largely rely on input-based supply mechanisms that have a poor track record meeting the reproductive health needs of low-income and underserved segments of national populations. As a result, there is increased interest in and experimentation with results-based mechanisms like supply-side performance incentives to providers and demand-side vouchers that place purchasing power in the hands of low-income consumers to improve uptake of facility services and reduce the burden of out-of-pocket expenditures. This paper describes a reproductive health voucher program that contracts private facilities in Uganda and explores the policy and implementation issues associated with expansion of the program to include public sector facilities. Methods: Data presented here describes the results of interviews of six district health officers and four health facility managers purposefully selected from seven districts with the voucher program in southwestern Uganda. Interviews were transcribed and organized thematically, barriers to seeking RH care were identified, and how to address the barriers in a context where voucher coverage is incomplete as well as opportunities and challenges for expanding the program by involving public sector facilities were investigated. Results: The findings show that access to sexual and reproductive health services in southwestern Uganda is constrained by both facility and individual level factors which can be addressed by inclusion of the public facilities in the program. This will widen the geographical reach of facilities for potential clients, effectively addressing distance related barriers to access of health care services. Further, intensifying ongoing health education, continuous monitoring and evaluation, and integrating the voucher program with other services is likely to address some of the barriers. The public sector facilities were also seen as being well positioned to provide voucher services because of their countrywide reach, enhanced infrastructure, and referral networks. The voucher program also has the potential to address public sector constraints such as understaffing and supply shortages.Conclusions: Accrediting public facilities has the potential to increase voucher program coverage by reaching a wider pool of poor mothers, shortening distance to service, strengthening linkages between public and private sectors through public-private partnerships and referral systems as well as ensuring the awareness and buy-in of policy makers, which is crucial for mobilization of resources to support the sustainability of the programs. Specifically, identifying policy champions and consulting with key policy sectors is key to the successful inclusion of the public sector into the voucher program

    Mobility on Demand (MOD) Sandbox Demonstration: Los Angeles County and Puget Sound First and Last Mile Partnership with Via Evaluation Report

    This report presents the results of an independent evaluation of the Los Angeles County and Puget Sound First and Last Mile Partnership with Via demonstration, part of the Federal Transit Administration (FTA) Mobility on Demand (MOD) Sandbox program. The demonstration, a joint pilot project in Los Angeles County and the Puget Sound region, executed an on-demand service in partnership with Via providing first- and last-mile access to transit stations within the Los Angeles and Seattle metropolitan regions. Upon launch, customers were able to request subsidized Via rides to or from the participating transit stations within specified zones and times of the day. The Los County Metropolitan Transportation Authority (LA Metro), King County Metro, Sound Transit, and Via (an on-demand transportation network company or TNC) worked together to develop and deploy two analogous pilot projects designed to test the viability of public-private partnerships to deliver access to and from core lines within the public transit networks. Three zones were selected in the Los Angeles region, and five zones were selected in the Puget Sound region. The evaluation of this MOD Sandbox project included 13 hypotheses that explored a number of potential impacts from the project, including mobility, accessibility, public transit ridership, fuel consumption, safety, costs, and lessons learned

    Factors influencing place of delivery for women in Kenya: an analysis of the Kenya Demographic and Health Survey, 2008/2009

    Background Maternal mortality in Kenya increased from 380/100000 live births to 530/100000 live births between 1990 and 2008. Skilled assistance during childbirth is central to reducing maternal mortality yet the proportion of deliveries taking place in health facilities where such assistance can reliably be provided has remained below 50% since the early 1990s. We use the 2008/2009 Kenya Demographic and Health Survey data to describe the factors that determine where women deliver in Kenya and to explore reasons given for home delivery. Methods Data on place of delivery, reasons for home delivery, and a range of potential explanatory factors were collected by interviewer-led questionnaire on 3977 women and augmented with distance from the nearest health facility estimated using health facility Global Positioning System (GPS) co-ordinates. Predictors of whether the woman’s most recent delivery was in a health facility were explored in an exploratory risk factor analysis using multiple logistic regression. The main reasons given by the woman for home delivery were also examined. Results Living in urban areas, being wealthy, more educated, using antenatal care services optimally and lower parity strongly predicted where women delivered, and so did region, ethnicity, and type of facilities used. Wealth and rural/urban residence were independently related. The effect of distance from a health facility was not significant after controlling for other variables. Women most commonly cited distance and/or lack of transport as reasons for not delivering in a health facility but over 60% gave other reasons including 20.5% who considered health facility delivery unnecessary, 18% who cited abrupt delivery as the main reason and 11% who cited high cost. Conclusion Physical access to health facilities through distance and/or lack of transport, and economic considerations are important barriers for women to delivering in a health facility in Kenya. Some women do not perceive a need to deliver in a health facility and may value health facility delivery less with subsequent deliveries. Access to appropriate transport for mothers in labour and improving the experiences and outcomes for mothers using health facilities at childbirth augmented by health education may increase uptake of health facility delivery in Kenya

    Initiation of Breastfeeding and Factors Associated with Prelacteal Feeds in Central Nepal

    Background: Prelacteal feeds and delayed initiation of breastfeeding may lead to undernutrition of the infant but are still prevalent in many countries.Objective: A prospective cohort community-based study was conducted in central Nepal to ascertain the rate of early breastfeeding initiation and factors associated with the introduction of prelacteal feeds.Methods: Breastfeeding information was collected from 639 women who recently gave birth in the Kaski district of central Nepal. Backward stepwise logistic regression analysis was performed to determine factors associated with the use of prelacteal feeds.Results: The incidence of prelacteal feeds was 9.1%, with infant formula being the most common prelacteal food. Approximately 67% and 90% of mothers breastfed within 1 hour and 4 hours of delivery, respectively. Women who reside in urban areas (odds ratio [OR] = 2.68; 95% confidence interval [CI], 1.35-5.39), first-time mothers (OR = 2.15; 95% CI,1.15-4.02), and those who underwent cesarean section (OR = 10.10; 95% CI, 5.47-18.67) were more likely to give prelacteal feeds to their infants.Conclusion: The early initiation of breastfeeding with colostrum as the first feed was common in the study area. The introduction of prelacteal feeds was associated with urban residency, first-time motherhood, and cesarean delivery

    Nurture the sprouting bud; do not uproot it. Using saving groups to save for maternal and newborn health: lessons from rural Eastern Uganda

    Background: Saving groups are increasingly being used to save in many developing countries. However, there is limited literature about how they can be exploited to improve maternal and newborn health. Objectives: This paper describes saving practices, factors that encourage and constrain saving with saving groups, and lessons learnt while supporting communities to save through saving groups. Methods: This qualitative study was done in three districts in Eastern Uganda. Saving groups were identified and provided with support to enhance members’ access to maternal and newborn health. Fifteen focus group discussions (FGDs) and 18 key informant interviews (KIIs) were conducted to elicit members’ views about saving practices. Document review was undertaken to identify key lessons for supporting saving groups. Qualitative data are presented thematically. Results: Awareness of the importance of saving, safe custody of money saved, flexible saving arrangements and easy access to loans for personal needs including transport during obstetric emergencies increased willingness to save with saving groups. Saving groups therefore provided a safety net for the poor during emergencies. Poor management of saving groups and detrimental economic practices like gambling constrained saving. Efficient running of saving groups requires that they have a clear management structure, which is legally registered with relevant authorities and that it is governed by a constitution. Conclusions: Saving groups were considered a useful form of saving that enabled easy access to cash for birth preparedness and transportation during emergencies. They are like ‘a sprouting bud that needs to be nurtured rather than uprooted’, as they appear to have the potential to act as a safety net for poor communities that have no health insurance. Local governments should therefore strengthen the management capacity of saving groups so as to ensure their efficient running through partnerships with non-governmental organizations that can provide support to such groups

This report covers existing guidance and best practices to recommend strategies for accurate, timely, and feasible measurement of pedestrian travel. Recommendations include: 1) expand the use of multi-day/multi-week counts to reduce estimation error rates, and rotate counts around the network; 2) validate equipment at installation and regularly thereafter; 3) tailor quality checks appropriate for low volume versus high volume locations; 4) compute bias compensation factors (e.g., occlusion adjustment factors) to account for limitations related to equipment and locations; and 5) conduct both short-duration and continuous counts to fully consider temporal and spatial aspects of pedestrian traffic patterns

    Low Compliance with Iron-Folate Supplementation Among Postpartum Mothers of Nepal: An Analysis of Nepal Demographic and Health Survey 2011

    One in five maternal deaths are directly attributable to anaemia in the world. The World Health Organization recommends iron supplementation from the second trimester of pregnancy to 45 days after delivery. The aim of this study was to determine the compliance rate of iron-folate consumption and the factors associated with iron-folate consumption among post-natal mothers in Nepal. This study utilised the data of Nepal Demographic and Health Survey (NDHS) 2011. The NDHS 2011 is a cross sectional and nationally representative survey. Of the 4,148 respondents, only 20.7 % consumed iron throughout the post-natal period for 45 days. Mothers who had higher and secondary education [adjusted Odd ratio (aOR) 3.101; 95 % CI (2.268–4.240)]; had attended four or more antenatal care visits [aOR 9.406; 95 % CI (5.552–15.938)]; lived in Far-western development region [aOR 1.822; 95 % CI (1.387–2.395)]; delivered in health facility [aOR 1.335; 95 % CI (1.057–1.687)]; and attended postnatal care [aOR 2.348; 95 % CI (1.859–2.965)] were more likely to take iron for 45 days of postpartum. Intervention to increase the compliance with the postpartum iron-folate supplementation are required to avoid adverse pregnancy outcomes associated with poor iron status with especial focus on the mothers who delivered at homes and did not attend postnatal check up
