    The Changing Landscape for Stroke\ua0Prevention in AF: Findings From the GLORIA-AF Registry Phase 2

    Background GLORIA-AF (Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation) is a prospective, global registry program describing antithrombotic treatment patterns in patients with newly diagnosed nonvalvular atrial fibrillation at risk of stroke. Phase 2 began when dabigatran, the first non\u2013vitamin K antagonist oral anticoagulant (NOAC), became available. Objectives This study sought to describe phase 2 baseline data and compare these with the pre-NOAC era collected during phase 1. Methods During phase 2, 15,641 consenting patients were enrolled (November 2011 to December 2014); 15,092 were eligible. This pre-specified cross-sectional analysis describes eligible patients\u2019 baseline characteristics. Atrial fibrillation disease characteristics, medical outcomes, and concomitant diseases and medications were collected. Data were analyzed using descriptive statistics. Results Of the total patients, 45.5% were female; median age was 71 (interquartile range: 64, 78) years. Patients were from Europe (47.1%), North America (22.5%), Asia (20.3%), Latin America (6.0%), and the Middle East/Africa (4.0%). Most had high stroke risk (CHA2DS2-VASc [Congestive heart failure, Hypertension, Age  6575 years, Diabetes mellitus, previous Stroke, Vascular disease, Age 65 to 74 years, Sex category] score  652; 86.1%); 13.9% had moderate risk (CHA2DS2-VASc = 1). Overall, 79.9% received oral anticoagulants, of whom 47.6% received NOAC and 32.3% vitamin K antagonists (VKA); 12.1% received antiplatelet agents; 7.8% received no antithrombotic treatment. For comparison, the proportion of phase 1 patients (of N = 1,063 all eligible) prescribed VKA was 32.8%, acetylsalicylic acid 41.7%, and no therapy 20.2%. In Europe in phase 2, treatment with NOAC was more common than VKA (52.3% and 37.8%, respectively); 6.0% of patients received antiplatelet treatment; and 3.8% received no antithrombotic treatment. In North America, 52.1%, 26.2%, and 14.0% of patients received NOAC, VKA, and antiplatelet drugs, respectively; 7.5% received no antithrombotic treatment. NOAC use was less common in Asia (27.7%), where 27.5% of patients received VKA, 25.0% antiplatelet drugs, and 19.8% no antithrombotic treatment. Conclusions The baseline data from GLORIA-AF phase 2 demonstrate that in newly diagnosed nonvalvular atrial fibrillation patients, NOAC have been highly adopted into practice, becoming more frequently prescribed than VKA in Europe and North America. Worldwide, however, a large proportion of patients remain undertreated, particularly in Asia and North America.     This text aims to reveal how natives of Mexico learned the techniques to built masonry spanning structures such as arches and vaults. The guideline of this study starts by observing illustrations of the Florentine Codex, later it reflects on pre-Columbian masons skills by looking at the building solutions in the site of Mitla, and finally takes on the unique experience of reconstructing the ribbed vault of the open chapel of Teposcolula as case study. The weaving of these three components unveils those methods of teaching building techniques during the 16th century were done by example and practical knowledge.<br><br>Este texto pretende mostrar cómo los indígenas de México aprendieron las técnicas para construir estructuras de mampostería como arcos y bóvedas. El hilo conductor de este trabajo parte de la observación de ilustraciones del Códice Florentino, después refleja las habilidades albañiles prehispånicos observando las soluciones de construcción en el sitio de Mitla, y finalmente toma la experiencia de la reconstrucción de la bóveda de la capilla abierta de Teposcolula como caso de estudio. El tejido de estos tres componentes revela que los métodos de enseñanza de las técnicas de construcción en el siglo XVI se hicieron a través del ejemplo y del conocimiento pråctico

    Novel hydrothermal carbonization of cellulose catalyzed by montmorillonite to produce kerogen-like hydrochar

    The conversion of cellulose to petroleum-like fuel is a very challenging yet attractive route to developing biomass-to-fuel technology. Many attempts have been made in liquefaction, pyrolysis and gasification of cellulose to produce fuels or intermediate chemicals. Previous studies indicate that these processes are tough. Hence, the present work is concerned with the development of new technologies for the conversion of cellulose into materials which are analogies to the precursor of petroleum. Montmorillonite-catalyzed hydrothermal carbonization of microcrystalline cellulose for the production of kerogen-like hydrochar under mild conditions was investigated. It was revealed that the hydrothermal carbonization of microcrystalline cellulose alone resulted in hydrochar with type III kerogen-like structure, whereas in the presence of montmorillonite, the hydrothermal carbonization of microcrystalline cellulose yielded a hydrochar-mineral complex, of which the isolated organic fraction was oil-prone type II kerogen-like structure. Results suggested that further improved montmorillonite-aided biomass conversion to more oil-prone kerogen-like solid products could be an alternative efficient route to obtain biofuel and chemicals

    Chronic coronary syndromes without standard modifiable cardiovascular risk factors and outcomes: the CLARIFY registry

    Background and Aims: It has been reported that patients without standard modifiable cardiovascular (CV) risk factors (SMuRFs—diabetes, dyslipidaemia, hypertension, and smoking) presenting with first myocardial infarction (MI), especially women, have a higher in-hospital mortality than patients with risk factors, and possibly a lower long-term risk provided they survive the post-infarct period. This study aims to explore the long-term outcomes of SMuRF-less patients with stable coronary artery disease (CAD). Methods: CLARIFY is an observational cohort of 32 703 outpatients with stable CAD enrolled between 2009 and 2010 in 45 countries. The baseline characteristics and clinical outcomes of patients with and without SMuRFs were compared. The primary outcome was a composite of 5-year CV death or non-fatal MI. Secondary outcomes were 5-year all-cause mortality and major adverse cardiovascular events (MACE—CV death, non-fatal MI, or non-fatal stroke). Results: Among 22 132 patients with complete risk factor and outcome information, 977 (4.4%) were SMuRF-less. Age, sex, and time since CAD diagnosis were similar across groups. SMuRF-less patients had a lower 5-year rate of CV death or non-fatal MI (5.43% [95% CI 4.08–7.19] vs. 7.68% [95% CI 7.30–8.08], P = 0.012), all-cause mortality, and MACE. Similar results were found after adjustments. Clinical event rates increased steadily with the number of SMuRFs. The benefit of SMuRF-less status was particularly pronounced in women. Conclusions: SMuRF-less patients with stable CAD have a substantial but significantly lower 5-year rate of CV death or non-fatal MI than patients with risk factors. The risk of CV outcomes increases steadily with the number of risk factors
