    Hydrogeological evaluation of geological formations in Ashanti Region, Ghana

    The success of groundwater exploitation in an area largely depends on prior knowledge of existing hydrogeological parameters such as borehole yield, overburden thickness, depth of boreholes, static water level, specific capacity and the quality of water. This study, therefore, employed Geographical Information System to assess some of these hydrogeological parameters in the Ashanti Region using the ordinary kriging interpolation method. Data on 2,788 drilled boreholes in the region were used and the assessment focused on the various geological formations in the region that comprised the Birimian, Granitoids, Tarkwaian and Voltaian formations. The study results indicate that the Birimian formation is generally within medium to high yielding potential zones (30 – 60 l/min or higher) with a high drilling success rate of 91% and average borehole depth of 53 m. The Granitoids are, mostly, within the low yielding zone (< 30 l/min) with isolated high yielding boreholes and have average borehole depth and drilling success rate of 50 m and 72 % respectively. The Tarkwaian formation, on other hand, is within the medium yield zone with a drilling success rate of 80 % whilst the Voltaian formation is mainly classified to fall within low yield zone with about 60% success rate although significant high yield zones occur within the sandstone formation underlying its westernmost part in the region. Groundwater in the region is generally potable except in a few locations within the Birimian and ranitoids,where there are problems with levels of iron, nitrate, manganese and pH. Keywords: Groundwater; Hydrogeological parameters; Geological formations; Geographical Information System; Ashanti Regio

    Case Study: Intervention in Sierra Leone

    Case Study Prepared for Intervention in Internal ConflictSierra Leone is a case of state collapse, in which the conflicts of the 1990s were not an independent event but merely the work of the maggots on a dead body. Thus no intervention could have done anything more, at best, than removing the momentary parasites taking advantage of the situation. It would require a longer, deeper, and more sustained effort of the Sierra Leoneans, necessarily with help from the international community, to restore a functioning political, economic and social structure necessary to prevent a recurrence of conflict. At the same time, it is noteworthy that this internal conflict was not an ethnic conflict, despite some secondary ethnic ramifications. The collapse of the Sierra Leonean state, already a weak creation of colonization and decolonization, took place under the long reign of Siaka Stevens (1968-85) and his All Peoples Congress (APC) drawing primarily on the interior Temne and Limba people from the northern part of the country, reacting against the previous predominance of the coastal Mende people from the south and east. Collapse was consummated under Stevens" handpicked, ineptsuccessor, Gen. Joseph Momoh, overthrown by dissatisfied junior officers led by Capt Valentine Strasser in April 1992. The main rebel groups operated under the name of the Revolutionary United Force (RUF), led by ex-cpl Foday Sankoh and Samuel Bokarie and operating with the active support of the rebel movement and then the government of Liberia under Charles Taylor. The rebellion expanded into neighboring countries and then wore down under the falling away of external and internal support

    Macrophages orchestrate the expansion of a proangiogenic perivascular niche during cancer progression

    Tumor-associated macrophages (TAMs) are a highly plastic stromal cell type that support cancer progression. Using single-cell RNA sequencing of TAMs from a spontaneous murine model of mammary adenocarcinoma (MMTV-PyMT), we characterize a subset of these cells expressing lymphatic vessel endothelial hyaluronic acid receptor 1 (Lyve-1) that spatially reside proximal to blood vasculature. We demonstrate that Lyve-1+ TAMs support tumor growth and identify a pivotal role for these cells in maintaining a population of perivascular mesenchymal cells that express α-smooth muscle actin and phenotypically resemble pericytes. Using photolabeling techniques, we show that mesenchymal cells maintain their prevalence in the growing tumor through proliferation and uncover a role for Lyve-1+ TAMs in orchestrating a selective platelet-derived growth factor–CC–dependent expansion of the perivascular mesenchymal population, creating a proangiogenic niche. This study highlights the inter-reliance of the immune and nonimmune stromal network that supports cancer progression and provides therapeutic opportunities for tackling the disease

    Genome-wide association study of survival from sepsis due to pneumonia: an observational cohort study

    BACKGROUND: Sepsis continues to be a major cause of death, disability, and health-care expenditure worldwide. Despite evidence suggesting that host genetics can influence sepsis outcomes, no specific loci have yet been convincingly replicated. The aim of this study was to identify genetic variants that influence sepsis survival. METHODS: We did a genome-wide association study in three independent cohorts of white adult patients admitted to intensive care units with sepsis, severe sepsis, or septic shock (as defined by the International Consensus Criteria) due to pneumonia or intra-abdominal infection (cohorts 1-3, n=2534 patients). The primary outcome was 28 day survival. Results for the cohort of patients with sepsis due to pneumonia were combined in a meta-analysis of 1553 patients from all three cohorts, of whom 359 died within 28 days of admission to the intensive-care unit. The most significantly associated single nucleotide polymorphisms (SNPs) were genotyped in a further 538 white patients with sepsis due to pneumonia (cohort 4), of whom 106 died. FINDINGS: In the genome-wide meta-analysis of three independent pneumonia cohorts (cohorts 1-3), common variants in the FER gene were strongly associated with survival (p=9·7 × 10(-8)). Further genotyping of the top associated SNP (rs4957796) in the additional cohort (cohort 4) resulted in a combined p value of 5·6 × 10(-8) (odds ratio 0·56, 95% CI 0·45-0·69). In a time-to-event analysis, each allele reduced the mortality over 28 days by 44% (hazard ratio for death 0·56, 95% CI 0·45-0·69; likelihood ratio test p=3·4 × 10(-9), after adjustment for age and stratification by cohort). Mortality was 9·5% in patients carrying the CC genotype, 15·2% in those carrying the TC genotype, and 25·3% in those carrying the TT genotype. No significant genetic associations were identified when patients with sepsis due to pneumonia and intra-abdominal infection were combined. INTERPRETATION: We have identified common variants in the FER gene that associate with a reduced risk of death from sepsis due to pneumonia. The FER gene and associated molecular pathways are potential novel targets for therapy or prevention and candidates for the development of biomarkers for risk stratification.     Mapping Helminth Co-Infection and Co-Intensity: Geostatistical Prediction in Ghana

    Urinary schistosomiasis and hookworm infections cause considerable morbidity in school age children in West Africa. Severe morbidity is predominantly observed in individuals infected with both parasite types and, in particular, with heavy infections. We investigated for the first time the distribution of S. haematobium and hookworm co-infections and distribution of co-intensity of these parasites in Ghana. Bayesian geostatistical models were developed to generate a national co-infection map and national intensity maps for each parasite, using data on S. haematobium and hookworm prevalence and egg concentration (expressed as eggs per 10 mL of urine for S. haematobium and expressed as eggs per gram of faeces for hookworm), collected during a pre-intervention baseline survey in Ghana, 2008. In contrast with previous findings from the East Africa region, we found that both S. haematobium and hookworm infections are highly focal, resulting in small, localized clusters of co-infection and areas of high co-intensity. Overlaying on a single map the co-infection and the intensity of multiple parasite infections allows identification of areas where parasite environmental contamination and morbidity are at its highest, while providing an evidence base for the assessment of the progress of successive rounds of mass drug administration (MDA) in integrated parasitic disease control programs

    Treatment response after relapse in a placebo-controlled maintenance trial in schizophrenia

    While placebo-controlled studies continue to be required by regulatory authorities for the licensing of new drugs for schizophrenia to demonstrate maintenance of effect, the long-term risks to participants are largely unknown. We compared the response to treatment with paliperidone palmitate before and after relapse in such a study. This was a post-hoc analysis of 97 patients with schizophrenia who relapsed while receiving placebo in a multinational relapse prevention clinical trial. Patients underwent an initial open-label treatment phase of 33. weeks (comprising a 9-week transition phase to switch patients to paliperidone palmitate, a 12-week flexible-dose phase and a 12-week fixed-dose phase); a double-blind phase of variable duration during which stabilized patients were randomized 1:1 to either continue paliperidone palmitate or receive placebo; and an optional 52-week open-label flexible-dose extension phase. There was a small but significant increase in PANSS total scores after eight months of treatment following relapse (56.7[12.68]) compared with prerelapse endpoint (54.5[11.74]) (p = 0.026). Fourteen of 97 (14.4%) patients who had initially responded favorably to treatment met predefined nonresponse criteria in the postrelapse treatment phase, suggesting that treatment refractoriness may evolve in a subset of patients after relapse. However, relapses occurred in 18% of patients randomized to ongoing treatment in the double-blind phase, raising the possibility that treatment failure may also evolve in patients receiving continuous treatment. These findings may help inform decisions regarding the future of placebo-controlled trials in schizophrenia. © 2012 Elsevier B.V
