    Excerpt: On a rainy early spring morning in a modest brick Presbyterian church just outside the Fruitvale neighborhood of Oakland, California, sixtyfour worshippers gather. The entire worship is in Spanish. During the sermon, the pastor makes a passing reference to how few of the attenders now live in Oakland proper, that many have to drive farther than ever for church services. The implicit message: the leadership of the church realizes that gentrification of San Francisco has spilled over the Bay Bridge and now threatens the availability of affordable housing throughout Oakland. In response, the congregation has started programs that offer legal advice for responding to rent-hiking landlords and identifying housing options around the city. Though resources and attenders tend to be somewhat scarce, the leadership has creatively organized in an effort to address the structural and policy concerns of housing. Beyond that, the pastor proudly notes that this church readily offers immigration status services, computer classes, and English classes

    Does mental health service integration affect compulsory admissions?

    Background: Over recent years, the number of compulsory admissions in many countries has increased, probably as a result of the shift from inpatient to outpatient mental health care. This might be mitigated by formal or collaborative relationships between services. <br><br> Methods: In a retrospective record linkage study, we compared two neighboring districts, varying in level of service integration. Two periods were combined: 1991–1993 and 2001–2003. We included patients aged 18–60, who had a first emergency compulsory admission (n=830). Their psychiatric history was assessed, and service-use after admission was monitored over a 12-month follow-up. <br><br> Results: Over a 10-year period, compulsory admission rates increased by 47%. Difference in relative increase between the integrated and non-integrated services was 14%. Patient characteristics showed different profiles in the two districts. Length of stay was >10 days shorter in the integrated district, where the proportion of involuntary readmissions decreased more, and where aftercare was swift and provided to about 10% more patients than in the non-integrated district. <br><br> Conclusions: Services outcomes showed better results where mental healthcare was more integrated. However, limited effects were found and other factors than integration of services may be more important in preventing compulsory admissions

    Rates of ecological knowledge learning in Pemba, Tanzania: Implications for childhood evolution

    Humans live in diverse, complex niches where survival and reproduction are conditional on the acquisition of knowledge. Humans also have long childhoods, spending more than a decade before they become net producers. Whether the time needed to learn has been a selective force in the evolution of long human childhood is unclear, because there is little comparative data on the growth of ecological knowledge throughout childhood. We measured ecological knowledge at different ages in Pemba, Zanzibar (Tanzania), interviewing 93 children and teenagers between 4 and 26 years. We developed Bayesian latent-trait models to estimate individual knowledge and its association with age, activities, household family structure and education. In the studied population, children learn during the whole pre-reproductive period, but at varying rates, with the fastest increases in young children. Sex differences appear during middle childhood and are mediated by participation in different activities. In addition to providing a detailed empirical investigation of the relationship between knowledge acquisition and childhood, this study develops and documents computational improvements to the modelling of knowledge development

    Vapour sorption and permeation properties of poly (dimethylsiloxane) films

    Sorption and permeation of several organic vapours in poly (dimethylsiloxane) were investigated. Solubility and permeability coefficients measured show a strong dependence of the applied vapour activity typical for these systems, contrary to the behaviour of permanent gases. However, as irregularities in the series of chloromethanes show, highest sorption does not necessarily lead to the highest permeability value. A possible explanation of this effect can be found in the concentration dependent diffusion behaviour of poly (dimethylsiloxane) toward these organic vapours. Thus, from the kinetics of solvent uptake, i.e. the sorption isotherms over time, the diffusion coefficients, which were corrected for swelling and thermodynamic activity of the vapour, were calculated for the different vapour activities

    Evaluation of behavioral changes and subjective distress after exposure to coercive inpatient interventions

    BACKGROUND: There is a lack of evidence to underpin decisions on what constitutes the most effective and least restrictive form of coercive intervention when responding to violent behavior. Therefore we compared ratings of effectiveness and subjective distress by 125 inpatients across four types of coercive interventions. METHODS: Effectiveness was assessed through ratings of patient behavior immediately after exposure to a coercive measure and 24 h later. Subjective distress was examined using the Coercion Experience Scale at debriefing. Regression analyses were performed to compare these outcome variables across the four types of coercive interventions. RESULTS: Using univariate statistics, no significant differences in effectiveness and subjective distress were found between the groups, except that patients who were involuntarily medicated experienced significant less isolation during the measure than patients who underwent combined measures. However, when controlling for the effect of demographic and clinical characteristics, significant differences on subjective distress between the groups emerged: involuntary medication was experienced as the least distressing overall and least humiliating, caused less physical adverse effects and less sense of isolation. Combined coercive interventions, regardless of the type, caused significantly more physical adverse effects and feelings of isolation than individual interventions. CONCLUSIONS: In the absence of information on individual patient preferences, involuntary medication may be more justified than seclusion and mechanical restraint as a coercive intervention. Use of multiple interventions requires significant justification given their association with significant distress

    Differential electromagnetic attack on an FPGA implementation of elliptic curve cryptosystems

    This paper describes a differential electromagnetic analysis attack performed on a hardware implementation of an elliptic curve cryptosystem. We describe the use of the distance of mean test. The number of measurements needed to get a clear idea of the right guess of the key-bit is taken as indication of the success of the attack. We can find the right key-bit by using only 2000 measurements. Also we give a electromagnetic model for the FPGA we use in our experiments. The amplitude, the direction and the position of the current on the FPGA’s lines with respect to the position of the antenna have an influence on the measured electromagnetic radiation in the FPGA’s surrounding area

    The evaluation of a frame-of-reference training programme for intern psychometrists

    Orientation: The use of assessment centres (ACs) has drastically increased over the past decade. However, ACs are constantly confronted with the lack of construct validity. One aspect of ACs that could improve the construct validity significantly is that of assessor training. Unfortunately untrained or poorly trained assessors are often used in AC processes. Research purpose: The purpose of this research was to evaluate a frame-of-reference (FOR) programme to train intern psychometrists as assessors at an assessment centre. Motivation of study: The role of an assessor is important in an AC; therefore it is vital for an assessor to be able to evaluate and observe candidates’ behaviour adequately. Commencing with this training in a graduate psychometrist programme gives the added benefit of sending skilled psychometrists to the workplace. Research design, approach and method: A quantitative research approach was implemented, utilising a randomised pre-test-post-test comparison group design. Industrial Psychology postgraduate students (N = 22) at a South African university were used and divided into an experimental group (n = 11) and control group (n = 11). Three typical AC simulations were utilised as pre- and post-tests, and the ratings obtained from both groups were statistically analysed to determine the effect of the FOR training programme. Main findings: The data indicated that there was a significant increase in the familiarity of the participants with the one-on-one simulation and the group discussion simulation. Practical/managerial implications: Training intern psychometrists in a FOR programme could assist organisations in the appointment of more competent assessors. Contribution/value-add: To design an assessor training programme using FOR training for intern psychometrists in the South African context, specifically by incorporating this programme into the training programme for Honours students at universities

    Breakdown of continuity in public mental healthcare in the Netherlands: a longitudinal case study

    <strong>Introduction:</strong> Continuity of care for long-term service-dependent patients in the public mental health system requires intensive collaboration between all agencies involved. Understanding the ways in which various aspects of continuity of care interact may reveal help to find out more about how care de­livered over time improves outcomes. <strong>C</strong><strong>ase study:</strong> Based on medical records, an addicted couple was monitored for number and type of contacts with health and social services. Over the years, 81 social workers or nurses, spread over 25 health and social services, have been involved in the rehabilitation process. Breakdown of continuity of care is linked to lack of information, missing procedures and guidelines, fragile relationships with the patient, and a reluctant public health approach. <strong>Conclu</strong><strong>sion: </strong>Prominent among relevant factors is the absence of protocols governing the transfer of patients between the various links in the continuum of mental healthcare services. High-quality follow-up after admission is partly a matter of professional principle in ensuring that problems in the chain of services are discussed.     Dysregulation of Glucagon Secretion by Hyperglycemia-Induced Sodium-Dependent Reduction of ATP Production

Mice lacking fumarase (Fh1) in their β cells (Fh1βKO mice) develop progressive hyperglycemia and dysregulated glucagon secretion similar to that seen in diabetic patients (too much at high glucose and too little at low glucose). The glucagon secretion defects are corrected by low concentrations of tolbutamide and prevented by the sodium-glucose transport (SGLT) inhibitor phlorizin. These data link hyperglycemia, intracellular Na+ accumulation, and acidification to impaired mitochondrial metabolism, reduced ATP production, and dysregulated glucagon secretion. Protein succination, reflecting reduced activity of fumarase, is observed in α cells from hyperglycemic Fh1βKO and β-V59M gain-of-function KATP channel mice, diabetic Goto-Kakizaki rats, and patients with type 2 diabetes. Succination is also observed in renal tubular cells and cardiomyocytes from hyperglycemic Fh1βKO mice, suggesting that the model can be extended to other SGLT-expressing cells and may explain part of the spectrum of diabetic complications.Peer reviewe
