    Mechanical support concept of the DEMO breeding blanket

    The DEMO tokamak architecture is based on large vertical breeding blanket (BB) segments that are accessed from a maintenance hall above the tokamak and are vertically replaced through large upper ports of the vacuum vessel (VV). The feasibility of the BB segments mechanical supports is a prerequisite of this vertical segment architecture. Their design directly impacts on the removal kinematics and the remote handling operations required for release and engagement. The supports must withstand large forces acting on the BB in particular due to electromagnetic (EM) loads. At the same time, they must ensure a sufficiently precise positioning of the BB first wall. Their design also takes into account the significant thermal expansion of the blanket segments that are operated at high temperature avoiding excessive support reaction forces. The BB support concept described in this article does not require fasteners or electrical straps to the VV and therefore much reduces the complexity of the BB remote replacement – a valuable characteristic that would make this concept a milestone in meeting one of the goals defined for the DEMO project: to develop a maintainable fusion power plant design [1]. Each blanket segment is individually supported by the VV without any physical contact to the other blankets or in-vessel components. It relies instead on vertical pre-compression inside the VV due to obstructed thermal expansion and radial pre-compression due to the ferromagnetic force acting on the BB material in the toroidal magnetic field. The verification process did not identify show stoppers. Nonetheless, a further evolution of the concept is required including design improvements to mitigate the high stress levels found in the inboard blankets during plasma disruptions. The fact that no excessively high support reaction forces or large BB deflections were found suggests though that the further development of the concept could be successful

    Do virtual patients prepare medical students for the real world? Development and application of a framework to compare a virtual patient collection with population data

    Background: An important aspect of virtual patients (VPs), which are interactive computer-based patient scenarios, is authenticity. This includes design aspects, but also how a VP collection represents a patient population and how a patient is presented in a VP scenario. Therefore, our aim was to analyze VP scenarios integrated into the combined internal medicine and surgery curriculum at the University of Munich (LMU) and compare the results with data from the population in Germany. Method: We developed a coding framework with four main categories: patient data, patient representation, diagnoses, and setting. Based on the framework we analyzed 66 VP and compared the results with data from the German healthcare system. Results: Especially in the categories of patient data and patient representation, the VPs presented an unrealistic image of the real world;topics such as unemployment, disability, or migration background were almost non-existent. The diagnoses of the VPs and the onset of diseases were comparable with the healthcare data. Conclusions: An explanation for the lack of representativeness of the patient data and representation might be a trend to create VPs based on fictional patient stories with VP authors trying to minimize complexity and cognitive load for the students. We suggest raising awareness among VP authors concerning personalized representations of patients without overwhelming their students. Our framework can support educators to assess the authenticity and diversity of a VP collection

    European integration assessed in the light of the 'rules vs. standards debate'

    The interplay of various legal systems in the European Union (EU) has long triggered a debate on the tension between uniformity and diversity of Member States' (MS) laws. This debate takes place among European legal scholars and is also paralleled by economic scholars, e.g. in the ambit of the 'theory of federalism'. This paper takes an innovative perspective on the discrepancy between 'centralized' and 'decentralized' law-making in the EU by assessing it with the help of the rules versus standards debate. When should the EU legislator grant the national legislator leeway in the formulation of new laws and when should all be fixed ex ante at European level? The literature on the 'optimal shape of legal norms' shall be revisited in the light of law-making in the EU, centrally dealing with the question how much discretion shall be given to the national legislator; and under which circumstances. This paper enhances the established decisive factors for the choice of a rule or a standard in a national setting (complexity, volatility, judges' specialization and frequency of application) by two new crucial factors (switching costs and the benefit of uniformity in terms of information costs) in order to assess law-making policies at EU level

    Pathogenesis of Candida albicans Infections in the Alternative Chorio-Allantoic Membrane Chicken Embryo Model Resembles Systemic Murine Infections

    Alternative models of microbial infections are increasingly used to screen virulence determinants of pathogens. In this study, we investigated the pathogenesis of Candida albicans and C. glabrata infections in chicken embryos infected via the chorio-allantoic membrane (CAM) and analyzed the virulence of deletion mutants. The developing immune system of the host significantly influenced susceptibility: With increasing age, embryos became more resistant and mounted a more balanced immune response, characterized by lower induction of proinflammatory cytokines and increased transcription of regulatory cytokines, suggesting that immunopathology contributes to pathogenesis. While many aspects of the chicken embryo response resembled murine infections, we also observed significant differences: In contrast to systemic infections in mice, IL-10 had a beneficial effect in chicken embryos. IL-22 and IL-17A were only upregulated after the peak mortality in the chicken embryo model occurred; thus, the role of the Th17 response in this model remains unclear. Abscess formation occurs frequently in murine models, whereas the avian response was dominated by granuloma formation. Pathogenicity of the majority of 15 tested C. albicans deletion strains was comparable to the virulence in mouse models and reduced virulence was associated with significantly lower transcription of proinflammatory cytokines. However, fungal burden did not correlate with virulence and for few mutants like bcr1Δ and tec1Δ different outcomes in survival compared to murine infections were observed. C. albicans strains locked in the yeast stage disseminated significantly more often from the CAM into the embryo, supporting the hypothesis that the yeast morphology is responsible for dissemination in systemic infections. These data suggest that the pathogenesis of C. albicans infections in the chicken embryo model resembles systemic murine infections but also differs in some aspects. Despite its limitations, it presents a useful alternative tool to pre-screen C. albicans strains to select strains for subsequent testing in murine models
