    Supplementation of polyunsaturated fatty acids, magnesium and zinc in children seeking medical advice for attention-deficit/hyperactivity problems - an observational cohort study

    <p>Abstract</p> <p>Background</p> <p>Polyunsaturated fatty acids are essential nutrients for humans. They are structural and functional components of cell membranes and pre-stages of the hormonally and immunologically active eicosanoids. Recent discoveries have shown that the long-chained omega-3 fatty acids eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) also play an important role in the central nervous system. They are essential for normal brain functioning including attention and other neuropsychological skills.</p> <p>Materials and methods</p> <p>In our large observational study we monitored 810 children from 5 to 12 years of age referred for medical help and recommended for consuming polyunsaturated fatty acids (PUFA) in combination with zinc and magnesium by a physician over a period of at least 3 months. The food supplement ESPRICO<sup>® </sup>(further on referred to as the food supplement) is developed on the basis of current nutritional science and containing a combination of omega-3 and omega-6 fatty acids as well as magnesium and zinc. Study objective was to evaluate the nutritional effects of the PUFA-zinc-magnesium combination on symptoms of attention deficit, impulsivity, and hyperactivity as well as on emotional problems and sleep related parameters. Assessment was performed by internationally standardised evaluation scales, i.e. SNAP-IV and SDQ. Tolerance (adverse events) and acceptance (compliance) of the dietary therapy were documented.</p> <p>Results</p> <p>After 12 weeks of consumption of a combination of omega-3 and omega-6 fatty acids as well as magnesium and zinc most subjects showed a considerable reduction in symptoms of attention deficit and hyperactivity/impulsivity assessed by SNAP-IV. Further, the assessment by SDQ revealed fewer emotional problems at the end of the study period compared to baseline and also sleeping disorders. Mainly problems to fall asleep, decreased during the 12 week nutritional therapy. Regarding safety, no serious adverse events occurred. A total of 16 adverse events with a possible causal relationship to the study medication were reported by 14 children (1.7%) and only 5.2% of the children discontinued the study due to acceptance problems. Continuation of consumption of the food supplement was recommended by the paediatricians for 61.1% of the children.</p> <p>Conclusion</p> <p>Our results suggest a beneficial effect of a combination of omega-3 and omega-6 fatty acids as well as magnesium and zinc consumption on attentional, behavioural, and emotional problems of children and adolescents. Thus, considering the behavioural benefit in combination with the low risk due to a good safety profile, the dietary supplementation with PUFA in combination with zinc and magnesium can be recommended.</p

    New long-term mass-balance series for the Swiss Alps

    In this study we present 19 new or re-analysed series of glacier-wide seasonal mass balance for the Swiss Alps based on direct measurements. The records partly start around 1920 and continue until today. Previously unpublished and unevaluated observations of point winter and annual balance are compiled from various sources and archives. These highly valuable datasets have not yet been consistently evaluated and were thus unavailable to the scientific community. Using distributed modelling for spatial interpolation and extrapolation and homogenization of the point measurements, we infer continuous series of area-averaged mass balance. The results are validated against independent decadal ice volume changes from photogrammetric surveys. Six of the new seasonal series cover 60 years and more and add a substantial amount of information on the variations of regional glacier mass change. This will strengthen the worldwide collection of glacier monitoring data, especially for the data-sparse period before the 1980s. We compare our results to existing long-term series and present an updated assessment of mass-balance variability and glacier sensitivity throughout the European Alps

    Pollen grains induce a rapid and biphasic eczematous immune response in atopic eczema patients

    Introduction: Eczematous reactions to type I allergy-inducing antigens are documented in a subgroup of patients with atopic eczema. Yet, the underlying immunological mechanisms are not well understood. Material and Methods: To delineate the effect of native pollen grains on human skin of healthy and atopic individuals we performed patch tests (atopy patch test with native pollen grains, PPT). Nickel patch tests (NPT) served as an established model of contact dermatitis. Skin site biopsies were taken 6 - 96 h after allergen application and investigated immunohistochemically. Results: Histology of positive patch tests showed an influx of mononuclear cells (predominantly CD4+, CD25+, CD45RO+). This influx was detected earlier in the PPT reaction than in the immune response to nickel. A biphasic cytokine response could be detected in the PPT: IL-5 dominated in the early, IFN-gamma in the late phase. The NPT was continuously dominated by IFN-gamma. Dendritic cell subpopulations imitated the earlier kinetics of the mononuclear infiltrate. Discussion: Thus, pollen grains induce eczematous reactions in susceptible individuals. This reaction appears clinically and immunohistochemically similar to the contact hypersensitivity reaction to nickel but follows a faster kinetic and a biphasic course: Th2 and IgE in the early (24 h) and Th1 predominance in the late (96 h) phase.     The impact of Saharan dust and black carbon on albedo and long-term mass balance of an Alpine glacier

    Light-absorbing impurities in snow and ice control glacier melt as shortwave radiation represents the main component of the surface energy balance. Here, we investigate the long-term effect of snow impurities, i.e., mineral dust and black carbon (BC), on albedo and glacier mass balance. The analysis was performed over the period 1914–2014 for two sites on Claridenfirn, Swiss Alps, where an outstanding 100-year record of seasonal mass balance measurements is available. Information on atmospheric deposition of mineral dust and BC over the last century was retrieved from two firn/ice cores of high-alpine sites. A combined mass balance and snow/firn layer model was employed to assess the effects of melt and accumulation processes on the impurity concentration at the surface and thus on albedo and glacier mass balance. Compared to pure snow conditions, the presence of Saharan dust and BC lowered the mean annual albedo by 0.04–0.06 depending on the location on the glacier. Consequently, annual melt was increased by 15–19 %, and the mean annual mass balance was reduced by about 280–490 mm w.e. BC clearly dominated absorption which is about 3 times higher than that of mineral dust. The upper site has experienced mainly positive mass balances and impurity layers were continuously buried whereas at the lower site, surface albedo was more strongly influenced by re-exposure of dust and BC-enriched layers due to frequent years with negative mass balances

    The New Swiss Glacier Inventory SGI2016: From a Topographical to a Glaciological Dataset

    Glaciers in Switzerland are shrinking rapidly in response to ongoing climate change. Repeated glacier inventories are key to monitor such changes at the regional scale. Here we present the new Swiss Glacier Inventory 2016 (SGI2016) that has been acquired based on sub-meter resolution aerial imagery and digital elevation models, bringing together topographical and glaciological approaches and knowledge. We define the process, workflow and required glaciological adaptations to compile a highly detailed inventory based on the digital Swiss Topographic Landscape model. The SGI2016 provides glacier outlines (areas), supraglacial debris cover and ice divides for all Swiss glaciers referring to the years 2013–2018. The SGI2016 maps 1,400 individual glacier entities with a total surface area of 961 ± 22 km2, whereof 11% (104 km2) are debris-covered. It constitutes the so far most detailed cartographic representation of glacier extent in Switzerland. Interpretation in the context of topographic parameters indicates that glaciers with moderate inclination and low median elevation tend to have highest fractions of supraglacial debris. Glacier-specific area changes since 1973 show the largest relative changes for small and low-elevation glaciers. The analysis further indicates a tendency for glaciers with a high share of supraglacial debris to show larger relative area changes. Between 1973 and 2016, an area change rate of –0.6% a−1 is found. Based on operational data sets and the presented methodology, the Swiss Glacier Inventory will be updated in 6- yr time intervals, leading to a high consistency in future glacier change assessments

    Multidimensional Geriatric Assessment: Back to the Future Multidimensional Preventive Home Visit Programs for Community-Dwelling Older Adults: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

    Background. Multidimensional preventive home visit programs aim at maintaining health and autonomy of older adults and preventing disability and subsequent nursing home admission, but results of randomized controlled trials (RCTs) have been inconsistent. Our objective was to systematically review RCTs examining the effect of home visit programs on mortality, nursing home admissions, and functional status decline. Methods. Data sources were MEDLINE, EMBASE, Cochrane CENTRAL database, and references. Studies were reviewed to identify RCTs that compared outcome data of older participants in preventive home visit programs with control group outcome data. Publications reporting 21 trials were included. Data on study population, intervention characteristics, outcomes, and trial quality were double-extracted. We conducted random effects meta-analyses. Results. Pooled effects estimates revealed statistically nonsignificant favorable, and heterogeneous effects on mortality (odds ratio [OR] 0.92, 95% confidence interval [CI], 0.80-1.05), functional status decline (OR 0.89, 95% CI, 0.77-1.03), and nursing home admission (OR 0.86, 95% CI, 0.68-1.10). A beneficial effect on mortality was seen in younger study populations (OR 0.74, 95% CI, 0.58-0.94) but not in older populations (OR 1.14, 95% CI, 0.90-1.43). Functional decline was reduced in programs including a clinical examination in the initial assessment (OR 0.64, 95% CI, 0.48-0.87) but not in other trials (OR 1.00, 95% CI, 0.88-1.14). There was no single factor explaining the heterogenous effects of trials on nursing home admissions. Conclusion. Multidimensional preventive home visits have the potential to reduce disability burden among older adults when based on multidimensional assessment with clinical examination.     The transcription factor SOX2 (3q26.3-q27) is an embryonic stem cell factor contributing to the induction of pluripotency in terminally differentiated somatic cells. Recently, amplification of the SOX2 gene locus has been described in squamous cell carcinoma (SCC) of different organ sites. Aim of this study was to investigate amplification and expression status of SOX2 in sinonasal carcinomas and to correlate the results with clinico-pathological data. A total of 119 primary tumor samples from the sinonasal region were assessed by fluorescence in-situ hybridization and immunohistochemistry for SOX2 gene amplification and protein expression, respectively. Of these, 59 were SSCs, 18 sinonasal undifferentiated carcinomas (SNUC), 10 carcinomas associated with an inverted papilloma (INVC), 19 adenocarcinomas (AD) and 13 adenoid cystic carcinomas (ACC). SOX2 amplifications were found in subsets of SCCs (37.5%), SNUCs (35.3%), INVCs (37.5%) and ADs (8.3%) but not in ACCs. SOX2 amplification resulted in increased protein expression. Patients with SOX2-amplified sinonasal carcinomas showed a significantly higher rate of tumor recurrences than SOX2 non-amplified tumors. This is the first study assessing SOX2 amplification and expression in a large cohort of sinonasal carcinomas. As opposed to AD and ACC, SOX2 amplifications were detected in more than 1/3 of all SCCs, SNUCs and INVCs. We therefore suggest that SNUCs are molecularly closely related to SCCs and INVCs and that these entities represent a subgroup of sinonasal carcinomas relying on SOX2 acquisition during oncogenesis. SOX2 amplification appears to identify sinonasal carcinomas that are more likely to relapse after primary therapy, suggesting that these patients might benefit from a more aggressive therapy regime
