Nosokomiale Harnwegsinfektionen des Erwachsenen
Zusammenfassung: Die Harnwegsinfektion ist die hĂ€ufigste Infektion des Erwachsenen im Spital. Sie ist meistens mit dem Tragen von Harnwegskathetern assoziiert. Entscheidend sind daher die richtige Indikationsstellung fĂŒr die Kathetereinlage, die frĂŒhzeitige Wiederentfernung des Katheters sowie die korrekte Diagnosestellung zur Vermeidung unnötiger antibiotischer Behandlungen. Die therapeutischen Ăberlegungen sollten die KomorbiditĂ€ten und potenzielle Behinderungen des Urinabflusses mit einschlieĂen. In der Behandlung stehen Chinolone, Breitspektrumpenizilline und Cephalosporine der 3.Generation im Vordergrund. Bei den ökonomischen Ăberlegungen sollte v.a. der höhere Preis von i.v.-Antibiotika im Vergleich zu per os mit einflieĂe
Pure hydrogen low-temperature plasma exposure of HOPG and graphene: Graphane formation?
Single- and multilayer graphene and highly ordered pyrolytic graphite (HOPG) were exposed to a pure hydrogen low-temperature plasma (LTP). Characterizations include various experimental techniques such as photoelectron spectroscopy, Raman spectroscopy and scanning probe microscopy. Our photoemission measurement shows that hydrogen LTP exposed HOPG has a diamond-like valence-band structure, which suggests double-sided hydrogenation. With the scanning tunneling microscopy technique, various atomic-scale charge-density patterns were observed, which may be associated with different C-H conformers. Hydrogen-LTP-exposed graphene on SiOâ has a Raman spectrum in which the D peak to G peak ratio is over 4, associated with hydrogenation on both sides. A very low defect density was observed in the scanning probe microscopy measurements, which enables a reverse transformation to graphene. Hydrogen-LTP-exposed HOPG possesses a high thermal stability, and therefore, this transformation requires annealing at over 1000 °C
The Value of Flexibility in a Carbon Neutral Power System
In this paper, we use a formulation of the generation expansion planning
problem with hourly temporal resolution, to investigate the impact of the
availability of different sources of flexibility on a carbon-neutral central
European power system (with a focus on Switzerland) for the year 2040. We
assess the role of flexible generation, load shifting and imports on the
investment and operation of existing and newly built units. Our results show
that including load shifting as part of the optimization could reduce the need
for investments in both RES and conventional technologies. The combination of
newly built flexible generators (gas turbines) and load shifting increases the
flexibility of the simulated power system and results in the overall lowest
system costs. The reduction of cross-border transmission capacity between
Switzerland and its neighbors has a significant impact on the domestic

Advantages and disadvantages of different nasal CPAP systems in newborns
Objective: To compare three different systems of continuous positive airway pressure (CPAP): the naso-pharyngeal tube and two-prong systems in newborns, focusing on duration of CPAP, side effects and cost. Design: Randomized clinical study. Patients: Between July 2000 and September 2001 newborns were randomized to three different CPAP systems. Forty infants in two weight groups (>2500g and 1250-2500g; 20 patients in each group) were included. Results: In the group >2500g the median duration of CPAP was 1.1days (range 0.25-14.3days). The median time on a naso-pharyngeal CPAP was 1day (range 0.25-14.3days), on Hudson prongs 1.6days (range 0.5-3.3days) and on the Infant Flow system 0.7days (range 0.3-13.6days; p>0.05 for comparison between groups, Fisher's exact test). With naso-pharyngeal CPAP, 2 patients developed moderate nasal injuries. On Hudson, 2 patients developed moderate and three mild nasal injuries. One patient on the Infant Flow showed mild and one moderate nasal injuries. In the weight group 1250-2500g the median duration of CPAP was 1.1days (range 0.1-7.0days). The median time on the naso-pharyngeal tube was 0.9days (range 0.1-7days), on Hudson prongs 1.1days (range 0.7-6.6days) and on the Infant Flow system 1.3days (range 0.25-5.9days; p>0.05 for comparison between groups, Fisher's exact test). With a naso-pharygeal tube, one infant developed mild and one moderate nasal injuries. On Hudson prongs, two had moderate nasal injuries. On Infant Flow, one newborn showed a severe nasal injury and two mild injuries. None of the patients developed a pneumothorax. Conclusion: The naso-pharyngeal tube is an easy, safe and economical CPAP system usable with every common ventilator. For very low birth weight newborns, a prong system may have advantage
Anisotropic Etching of Graphite and Graphene in a Remote Hydrogen Plasma
We investigate the etching of a pure hydrogen plasma on graphite samples and
graphene flakes on SiO and hexagonal Boron-Nitride (hBN) substrates. The
pressure and distance dependence of the graphite exposure experiments reveals
the existence of two distinct plasma regimes: the direct and the remote plasma
regime. Graphite surfaces exposed directly to the hydrogen plasma exhibit
numerous etch pits of various size and depth, indicating continuous defect
creation throughout the etching process. In contrast, anisotropic etching
forming regular and symmetric hexagons starting only from preexisting defects
and edges is seen in the remote plasma regime, where the sample is located
downstream, outside of the glowing plasma. This regime is possible in a narrow
window of parameters where essentially all ions have already recombined, yet a
flux of H-radicals performing anisotropic etching is still present. At the
required process pressures, the radicals can recombine only on surfaces, not in
the gas itself. Thus, the tube material needs to exhibit a sufficiently low H
radical recombination coefficient, such a found for quartz or pyrex. In the
remote regime, we investigate the etching of single layer and bilayer graphene
on SiO and hBN substrates. We find isotropic etching for single layer
graphene on SiO, whereas we observe highly anisotropic etching for graphene
on a hBN substrate. For bilayer graphene, anisotropic etching is observed on
both substrates. Finally, we demonstrate the use of artificial defects to
create well defined graphene nanostructures with clean crystallographic edges.Comment: 7 pages, 4 color figure
Phenyl-triazine oligomers for light-driven hydrogen evolution
The design of stable, yet highly tunable organic photocatalysts which orchestrate multi-step electron transfer reactions is at the heart of the newly emerging field of polymer photocatalysis. Covalent triazine frameworks such as the archetypal CTF-1 have been theorized to constitute a new class of photocatalytically active polymers for light-driven water splitting. Here, we revisit the ionothermal synthesis of CTF-1 by trimerization of 1,4-dicyanobenzene catalyzed by the Lewis acid zinc chloride and demonstrate that the microporous black polymer CTF-1 is essentially inactive for hydrogen evolution. Instead, highly photoactive phenyl-triazine oligomers (PTOs) with higher crystallinity as compared to CTF-1 are obtained by lowering the reaction temperature to 300 °C and prolonging the reaction time to >150 hours. The low reaction temperature of the PTOs largely prevents incipient carbonization and thus results in a carbon-to-nitrogen weight ratio close to the theoretical value of 3.43. The oligomers were characterized by MALDI-TOF and quantitative solid-state NMR spectroscopy, revealing variations in size, connectivity and thus nitrile-to-triazine ratios depending on the initial precursor dilution. The most active PTO samples efficiently and stably reduce water to hydrogen with an average rate of 1076 (±278) ÎŒmol hâ1 gâ1 under simulated sunlight illumination, which is competitive with the best carbon nitride-based and purely organic photocatalysts. The photocatalytic activity of the PTOs is found to sensitively depend on the polymerization degree, thus suggesting a prominent role of the unreacted nitrile moieties in the photocatalytic process. Notably, PTOs even show moderate hydrogen production without the addition of any co-catalyst
Atypisches Erythema induratum Bazin bei tuberkulöser Osteomyelitis
Zusammenfassung: Hauttuberkulosen können sich mit sehr unterschiedlichen klinischen Bildern manifestieren und damit die Diagnosestellung erschweren. Wir stellen den Fall einer 79-jĂ€hrigen Patientin vor, mit einer atypischen PrĂ€sentation eines Erythema induratum Bazin (EIB) am Oberkörper und einer tuberkulösen Osteomyelitis des Olekranon links. Aus den Biopsien der EIB-Knoten konnte M.tuberculosis kulturell nachgewiesen werden. Das widerspricht der klassischen Vorstellung, dass das EIB als Folge einer HypersensitivitĂ€tsreaktion auf Mykobakterien entsteht, und unterstĂŒtzt die Hypothese, dass das EIB auch durch eine hĂ€matogene oder lymphogene Streuung von lebenden M.tuberculosis entstehen kan
Classical double-layer atoms: artificial molecules
The groundstate configuration and the eigenmodes of two parallel
two-dimensional classical atoms are obtained as function of the inter-atomic
distance (d). The classical particles are confined by identical harmonic wells
and repel each other through a Coulomb potential. As function of d we find
several structural transitions which are of first or second order. For first
(second) order transitions the first (second) derivative of the energy with
respect to d is discontinuous, the radial position of the particles changes
discontinuously (continuously) and the frequency of the eigenmodes exhibit a
