    EcoPlex: Empowering compact wireless sensor platforms via roaming and interoperability support

    Abstract—EcoPlex is an infrastructure that enables simple wireless platforms to participate seamlessly in a feature-rich, wireless ad hoc network. EcoPlex consists of gateways that are responsible for handoff support for mobility and high data rate without burdening the simple nodes to implement multi-hop protocols. The gateways also create virtual identities for simpler nodes to enable their participation in the feature-rich network without adding complexity to them. We demonstrate the feasibility of this idea with the ultra-compact wireless sensor platform called Eco to participate as virtual nodes in a fully general ZigBee network. Experimental results show EcoPlex to be efficient and scalable. The enhanced mobility and interoperability are added to the Eco platform at the infrastructure level, all with minimal node complexity.     Oncologic impact of delay between diagnosis and radical nephroureterectomy

    PurposeThis study aimed to evaluate the oncological outcome of delayed surgical wait time from the diagnosis of upper tract urothelial carcinoma (UTUC) to radical nephroureterectomy (RNU).MethodsIn this multicenter retrospective study, medical records were collected between 1988 and 2021 from 18 participating Taiwanese hospitals under the Taiwan UTUC Collaboration Group. Patients were dichotomized into the early (≤90 days) and late (>90 days) surgical wait-time groups. Overall survival, disease-free survival, and bladder recurrence-free survival were calculated using the Kaplan–Meier method and multivariate Cox regression analysis. Multivariate analysis was performed using stepwise linear regression.ResultsOf the 1251 patients, 1181 (94.4%) were classifed into the early surgical wait-time group and 70 (5.6%) into the late surgical wait-time group. The median surgical wait time was 21 days, and the median follow-up was 59.5 months. Our study showed delay-time more than 90 days appeared to be associated with worse overall survival (hazard ratio [HR] 1.974, 95% confidence interval [CI] 1.166−3.343, p = 0.011), and disease-free survival (HR 1.997, 95% CI 1.137−3.507, p = 0.016). This remained as an independent prognostic factor after other confounding factors were adjusted. Age, ECOG performance status, Charlson Comorbidity Index (CCI), surgical margin, tumor location and adjuvant systemic therapy were independent prognostic factors for overall survival. Tumor location and adjuvant systemic therapy were also independent prognostic factors for disease-free survival.ConclusionsFor patients with UTUC undergoing RNU, the surgical wait time should be minimized to less than 90 days. Prolonged delay times may be associated with poor overall and disease-free survival

    Coping strategies and stressors in patients with hemodialysis

    Objectives: To investigate the stress related to undergoing hemodialysis (HD) and the relationship between these stresses and the coping strategies used by patients with end-stage renal disease. Methods: We used the Hemodialysis Stressor Scale and the Jalowiec Coping Scale to interview 2642 patients (mean age 57 years; 53.5 % female) receiving HD. The Hemodialysis Stressor Scale measures the level of stress related to stressor subscales: daily activity, physical condition, dependency on medical staff, fluid and food restriction, role ambiguity, blood vessel problems, and reproductive system functioning. The Jalowiec Coping Scale identifies the use of the following coping strategies: problem-oriented, emotion-oriented, support seeking, avoidance, and isolated thoughts. Data were analyzed using Hierarchical Linear Modeling. Results: Daily activity subscale scores were positively associated with using emotion-oriented, avoidance, and isolated thoughts as coping styles and negatively related to support seeking from professionals. The higher the perceived stress related to physical symptoms, dependency on medical staff, and blood vessel problems, the more the patients used emotion-oriented, support seeking, avoidance, and isolated thoughts to cope. Fluid and food restriction and role ambiguity subscales were found to be positively associated with emotion-oriented, avoidance, and isolated thoughts coping strategies. Reproductive system functioning was positively associated with emotion-oriented, avoidance, and isolated thoughts coping strategies. Patients on HD seldom use problem-oriented strategy to ease their stresses. Support seeking was another infrequently used coping strategy. Conclusions: The most commonly used coping strategies in our patients were emotion-oriented, avoidance, and isolated thoughts. The choice of coping strategy depended on the types of stressor.     EcoExec: An Interactive Execution Framework for Ultra Compact Wireless Sensor Nodes

    Abstract—EcoExec is a host-assisted interactive execution environment for wireless sensing systems. Users can interact with sensor nodes by viewing attributes and invoking functions via a command-line interface. Functions that are not resident in the node’s firmware are automatically compiled on the host, packaged up and downloaded to the node, linked, and executed, all seamlessly and transparently to the user. By packaging these features in a dynamically object-oriented programming environment such as Python, EcoExec enables programmers to experiment with features of the wireless sensor nodes and to rapidly develop application software. Most importantly, EcoExec empowers resource-constrained wireless sensor platforms with rich functionalities that would otherwise be prohibitive, thanks to its host-assisted execution feature with code swapping over the network. Experimental results on actual wireless sensor platforms show EcoExec to perform effectively with negligible observed overhead to the user. I

    Gender Differences In Stress And Coping Among Elderly Patients On Hemodialysis

    The study investigated whether gender differences in the stressor, coping strategies, and how they associate stress and copings among 875 elderly patients undergoing hemodialysis in Taiwan. Chi-square tests, MANOVA and Structural Equation Modeling were used to attain the research purposes. The results found that the women had reported higher stress in response to physical and vessel problems and higher scores in using emotion-oriented and support-seeking coping strategies, while the men reported higher stress in reproductive system functioning and higher score in using avoidance as a coping strategy. Furthermore, the results from SEM demonstrated that after controlling for patient characteristics, no statistical difference was found between the two groups with regard to their linking of stress and coping strategies. © 2008 Springer Science+Business Media, LLC

    The Influence Of Facility Ownership Structure On Individual Responding To Stress: A Multilevel Model

    Few studies have investigated the effect of health-care facility ownership on the relationship between patient stressors and coping strategies. The purpose of this study was to investigate whether haemodialysis (HD) patient stressors and coping strategies differ by type of health-care facility ownership, and whether such ownership has a cross-level moderating effect between stressors and coping strategies. We used the Haemodialysis Stressor Scale and the Jalowiec Coping Scale; primary data were collected by interviewing 2642 HD patients 15 years or older on dialysis for at least three months from 27 HD centres. One-way analysis of variance and hierarchical linear modelling were used to attain the research purposes. HD patients from religious-based hospitals had higher stress related to their physical symptoms, dependency on medical staff, role ambiguity and blood vessel problems than those differently owned facilities. Patients in veterans and army (VA) hospitals had higher stress related to food and fluid restriction and dependency on medical staff than private centres. Patients in religious-based hospitals had significantly higher coping scores, followed by VA and private HD centres. Religion-based ownership might serve as a cross-level moderator for patients perceiving role ambiguity stress and using problem-oriented, support seeking and isolated thought-coping strategies
