Marjolin\u27s Tumor Complicating Chronic Periprosthetic Infection of a Total Knee Arthroplasty
Marjolin\u27s tumor is a term used to describe a malignancy developing in the setting of a chronic wound, infection, or other tissue subject to chronic inflammatory changes. These malignancies usually present after many years of chronicity, and can range from lower grade basal cell carcinomas to high-grade sarcomas. We present the case of a squamous cell carcinoma that developed within a chronic periprosthetic infection of a total knee arthroplasty of 7 years duration. The intra-articular location, association with an orthopaedic implant, and brief latency period are all unique features of this case
The Structure of Screening in QED
The possibility of constructing charged particles in gauge theories has long
been the subject of debate. In the context of QED we have shown how to
construct operators which have a particle description. In this paper we further
support this programme by showing how the screening interactions arise between
these charges. Unexpectedly we see that there are two different gauge invariant
contributions with opposite signs. Their difference gives the expected result.Comment: 8 pages, LaTe
Affectionate communication, health, and relationships
A robust body of research attests to the mental and physical health correlates and consequences of affectionate communication. Like much research on personal relationships, however, this work may overrepresent certain portions of the population, may underrepresent others, and may not effectively account for intersections of identities. We define intersectionality as comprising the unique effects of two or more social identities interacting with each other. To assess this literature with an eye toward intersectionality and representation, the present article reports a systematic review of 86 individual empirical studies representing 26,013 participants. The review concludes that there is no explicit or implicit attention to intersectionality in the existing research on affectionate communication and health, and that U.S. Americans, women, younger individuals, white individuals, and students are overrepresented in research samples. The review ends with future directions to encourage more inclusive research on this topic
Section on Prospects for Dark Matter Detection of the White Paper on the Status and Future of Ground-Based TeV Gamma-Ray Astronomy
This is a report on the findings of the dark matter science working group for
the white paper on the status and future of TeV gamma-ray astronomy. The white
paper was commissioned by the American Physical Society, and the full white
paper can be found on astro-ph (arXiv:0810.0444). This detailed section
discusses the prospects for dark matter detection with future gamma-ray
experiments, and the complementarity of gamma-ray measurements with other
indirect, direct or accelerator-based searches. We conclude that any
comprehensive search for dark matter should include gamma-ray observations,
both to identify the dark matter particle (through the charac- teristics of the
gamma-ray spectrum) and to measure the distribution of dark matter in galactic

First Experimental Demonstration of Full-Duplex Optical Communications on a Single Laser Beam
We present the results of the first experimental demonstration a novel communications architecture that will be deployed on a Space Shuttle mission in 2003. This architecture can provide a very lightweight, low power consumption, low data rate communications link between the earth and LEO satellites. A unique characteristic of this system is that it provides full-duplex communications on a single beam is presented. The results of first experiments demonstrating this full duplex communications architecture are presented
Interpretation difficulty of normal versus abnormal radiographs using a pediatric example
Background: Radiograph teaching files are usually dominated by abnormal cases, implying that normal radiographs are easier to interpret. Our main objective was to compare the interpretation difficulty of normal versus abnormal radiographs of a set of common pediatric radiographs.Methods: We developed a 234-item digital case bank of pediatric ankle radiographs, recruited a convenience sample of participants, and presented the cases to each participant who then classified the cases as normal or abnormal. We determined and contrasted the interpretation difficulty of the normal and abnormal x-rays items using Rasch Measurement Theory. We also identified case features that were associated with item difficulty.Results: 139 participants (86 medical students, 7 residents, 29 fellows, 5 emergency physicians, and 3 radiologists) rated a minimum of 50 cases each, which resulted in 16,535 total ratings. Abnormal cases were more difficult (+0.99 logits) than were normal ones (-0.58 logits), difference 1.57 logits (95% CI 1.2, 2.0), but there was considerable overlap in difficulty scores. Patient variables associated with a more difficult normal radiograph included younger patient age (β = -0.16, 95% CI -0.22, -0.10), history of distal fibular tenderness (β = 0.55, 95% CI 0.17, 0.93), and presence of a secondary ossification centre (β = 0.84, 95% CI 0.27, 1.41).Conclusions: While abnormal images were more difficult to interpret, normal images did show a range of interpretation difficulties. We describe the Centers for Disease Control and Prevention's National Nosocomial Infections Surveillance system. Elements of the system critical for successful reduction of nosocomial infection rates include voluntary participation and confidentiality; standard definitions and protocols; identification of populations at high risk; site-specific, risk- adjusted infection rates comparable across institutions; adequate numbers of trained infection control professionals; dissemination of data to health-care providers; and a link between monitored rates and prevention efforts
