    Dropout from exercise programs for seniors: a prospective cohort study

    This study examines dropout incidence, moment of dropout, and switching behavior in organized exercise programs for seniors in the Netherlands, as determined in a prospective cohort study (with baseline measurements at the start of the exercise program and follow-up after 6 months; N = 1,725, response rate 73%). Participants were community-living individuals 50+ who participated in different forms of organized exercise programs. The average dropout incidence was 0.15 per 6 months, which is lower than that for the general population. The dropout incidence and the timing of dropout differed substantially between the exercise programs. In total, 31% of people who dropped out of one type of exercise program switched to another type of exercise. The type of program and exercise had a strong effect on differences in this switching behavior. It is recommended that switching behavior be monitored in future studie

    A symmetry-preserving second-order time-accurate PISO-based method

    A new conservative symmetry-preserving second-order time-accurate PISO-based pressure-velocity coupling for solving the incompressible Navier-Stokes equations on unstructured collocated grids is presented in this paper. This new method for implicit time stepping is an extension of the conservative symmetry-preserving incremental-pressure projection method for explicit time stepping and unstructured collocated meshes of Trias et al. [35]. In order to assess and compare both methods, we have implemented them within one unified solver in the open source code OpenFOAM where we use a Butcher array to prescribe the Runge-Kutta method. Thus, by changing the entries of the Butcher array, explicit and diagonally implicit Runge-Kutta schemes can be combined into one solver. We assess the energy conservation properties of the implemented discretisation methods and the temporal consistency of the selected Runge-Kutta schemes using Taylor-Green vortex and lid-driven cavity flow test cases. Finally, we use a more complex turbulent channel flow test case in order to further assess the performance of the presented new conservative symmetry-preserving incremental-pressure PISO-based method. Although both implemented methods are based on a symmetry-preserving discretisation, we show they still produce a small amount of numerical dissipation when the total pressure is directly solved from a Poisson equation. When an incremental-pressure approach is used, where a pressure correction is solved from a Poisson equation, both methods are effectively fully-conservative. For high-fidelity simulations of incompressible turbulent flows, it is highly desirable to use fully-conservative methods. For such simulations, the presented numerical methods are therefore expected to have large added value, since they pave the way for the execution of truly energy-conservative high-fidelity simulations in complex geometries. Furthermore, both methods are implemented in OpenFOAM, which is widely used within the CFD community, so that a large part of this community can benefit from the developed and implemented numerical methods

    The impact of obesity on cardiac troponin levels after prolonged exercise in humans

    Elevated cardiac troponin I (cTnI), a marker for cardiac damage, has been reported after high-intensity exercise in healthy subjects. Currently, little is known about the impact of prolonged moderate-intensity exercise on cTnI release, but also the impact of obesity on this response. 97 volunteers (55 men and 42 women), stratified for BMI, performed a single bout of walking exercise (30–50 km). We examined cTnI-levels before and immediately after the exercise bout in lean (BMI < 25 kg/m2, n = 30, 57 ± 19 years), overweight (25 ≤ BMI < 30 kg/m2, n = 29, 56 ± 11 years), and obese subjects (BMI ≥ 30 kg/m2, n = 28, 53 ± 9 years). Walking was performed at a self-selected pace. cTnI was assessed using a high-sensitive cTnI-assay (Centaur; clinical cut-off value ≥0.04 μg/L). We recorded subject characteristics (body weight, blood pressure, presence of cardiovascular risk) and examined exercise intensity by recording heart rate. Mean cTnI-levels increased significantly from 0.010 ± 0.006 to 0.024 ± 0.046 μg/L (P < 0.001). The exercise-induced increase in cTnI was not different between lean, overweight and obese subjects (two-way ANOVA interaction; P = 0.27). In 11 participants, cTnI was elevated above the clinical cut-off value for myocardial infarction. Logistic regression analysis identified exercise intensity (P < 0.001), but not BMI, body fat percentage or waist circumference to significantly relate to positive troponin tests. In conclusion, prolonged, moderate-intensity exercise results in a comparable increase in cTnI-levels in lean, overweight and obese subjects. Therefore, measures of obesity unlikely relate to the magnitude of the post-exercise elevation in cTnI

    Constraining properties of the black hole population using LISA

    LISA should detect gravitational waves from tens to hundreds of systems containing black holes with mass in the range from 10 thousand to 10 million solar masses. Black holes in this mass range are not well constrained by current electromagnetic observations, so LISA could significantly enhance our understanding of the astrophysics of such systems. In this paper, we describe a framework for combining LISA observations to make statements about massive black hole populations. We summarise the constraints that LISA observations of extreme-mass-ratio inspirals might be able to place on the mass function of black holes in the LISA range. We also describe how LISA observations can be used to choose between different models for the hierarchical growth of structure in the early Universe. We consider four models that differ in their prescription for the initial mass distribution of black hole seeds, and in the efficiency of accretion onto the black holes. We show that with as little as 3 months of LISA data we can clearly distinguish between these models, even under relatively pessimistic assumptions about the performance of the detector and our knowledge of the gravitational waveforms.Comment: 12 pages, 3 figures, submitted to Class.     Detection of chromosome aberrations in the human interphase nucleus by visualization of specific target DNAs with radioactive and non-radioactive in situ hybridization techniques: diagnosis of trisomy 18 with probe L1.84

    The localization of chromosome 18 in human interphase nuclei is demonstrated by use of radioactive and nonradioactive in situ hybridization techniques with a DNA clone designated L1.84. This clone represents a distinct subpopulation of the repetitive human alphoid DNA family, located in the centric region of chromosome 18. Under stringent hybridization conditions hybridization of L1.84 is restricted to chromosome 18 and reflects the number of these chromosomes present in the nuclei, namely, two in normal diploid human cells and three in nuclei from cells with trisomy 18. Under conditions of low stringency, cross-hybridization with other subpopulations of the alphoid DNA family occurs in the centromeric regions of the whole chromosome complement, and numerous hybridization sites are detected over interphase nuclei. Detection of chromosome-specific target DNAs by non-radioactive in situ hybridization with appropriate DNA probes cloned from individual chromosomal subregions presents a rapid means of identifying directly numerical or even structural chromosome aberrations in the interphase nucleus.     Study protocol of the TIRED study:A randomised controlled trial comparing either graded exercise therapy for severe fatigue or cognitive behaviour therapy with usual care in patients with incurable cancer

    Background: Fatigue is a common and debilitating symptom for patients with incurable cancer receiving systemic treatment with palliative intent. There is evidence that non-pharmacological interventions such as graded exercise therapy (GET) or cognitive behaviour therapy (CBT) reduce cancer-related fatigue in disease-free cancer patients and in patients receiving treatment with curative intent. These interventions may also result in a reduction of fatigue in patients receiving treatment with palliative intent, by improving physical fitness (GET) or changing fatigue-related cognitions and behaviour (CBT). The primary aim of our study is to assess the efficacy of GET or CBT compared to usual care (UC) in reducing fatigue in patients with incurable cancer. Methods: The TIRED study is a multicentre three-armed randomised controlled trial (RCT) for incurable cancer patients receiving systemic treatment with palliative intent. Participants will be randomised to GET, CBT, or UC. In addition to UC, the GET group will participate in a 12-week supervised exercise programme. The CBT group will receive a 12-week CBT intervention in addition to UC. Primary and secondary outcome measures will be assessed at baseline, post-intervention (14 weeks), and at follow-up assessments (18 and 26 weeks post-randomisation). The primary outcome measure is fatigue severity (Checklist Individual Strength subscale fatigue severity). Secondary outcome measures are fatigue (EORTC-QLQ-C30 subscale fatigue), functional impairments (Sickness Impact Profile total score, EORTC-QLQ-C30 subscale emotional functioning, subscale physical functioning) and quality of life (EORTC-QLQ-C30 subscale QoL). Outcomes at 14 weeks (primary endpoint) of either treatment arm will be compared to those of UC participants. In addition, outcomes at 18 and 26 weeks (follow-up assessments) of either treatment arm will be compared to those of UC participants. Discussion: To our knowledge, the TIRED study is the first RCT investigating the efficacy of GET and CBT on reducing fatigue during treatment with palliative intent in incurable cancer patients. The results of this study will provide information about the possibility and efficacy of GET and CBT for severely fatigued incurable cancer patients

    Observation of the Dynamic Beta Effect at CESR with CLEO

    Using the silicon strip detector of the CLEO experiment operating at the Cornell Electron-positron Storage Ring (CESR), we have observed that the horizontal size of the luminous region decreases in the presence of the beam-beam interaction from what is expected without the beam-beam interaction. The dependence on the bunch current agrees with the prediction of the dynamic beta effect.
