    A “near miss" congenital eventration of the right hemidiaphragm in a neonate: a case report

    Focal eventration involving the posterior segment of the hemidiaphragm is a rare congenital anomaly. We report of a 10-day-old infant who presented with significant respiratory insufficiency and failure to show any responses to standard treatment. The diagnosis of focal eventration of the diaphragm was not anticipated until ultrasonographic examination revealed the defect. Diaphragmatic plication resulted in complete resolution of symptoms. A high level clinical awareness is crucial as a relatively simple surgical procedure could avert long term life-threatening complications

    Feasibility study of smartphone application for self-monitoring dietary intake among cancer patients

    Objective: Evidence accumulates that optimal nutrition status positively influences cancer treatment outcomes. A "smartphone application" (intelligent dietitian support apps, iDSA) was developed to act as a tool to assist patients in terms of dietary monitoring. This study aimed to examine the feasibility of iDSA for self-monitoring dietary intake among cancer patients. Method: This was a pilot study. Eligible patients were approached and recruited. Dietitian entered individual estimated energy protein requirement into iDSA after installation. Participants recorded and monitored own daily dietary intake. Dietitian arranged for a two-week follow up to monitor nutritional status (weight and dietary intake). Results: This study enrolled 14 participants, six males and eight females, with a mean age of 36.4 ± 10.1 years. iDSA improved participants’ nutritional outcomes significantly; weight gained 1.2 ± 0.2 kg, increased energy intake 215 ± 100 kcal/day and protein intake 8 ± 5.1 g/day. There were 92.9% of participants agreed or totally agreed that they were able to monitor and increase dietary intake during using iDSA. However, about 57.1% reported that it was burdensome to record their diet daily and sometimes they forgot to record their food intake. Conclusion: Self-monitoring dietary intake via intelligent dietitian support apps was feasible among cancer patients. With compliance to iDSA resulted in an increase in dietary intake and body weight after two-week. iDSA usability was rated good and can be used to study dietary intake among cancer patients

    Antibacterial performance of Ag nanoparticles and AgGO nanocomposites prepared via rapid microwave-assisted synthesis method

    Silver nanoparticles and silver-graphene oxide nanocomposites were fabricated using a rapid and green microwave irradiation synthesis method. Silver nanoparticles with narrow size distribution were formed under microwave irradiation for both samples. The silver nanoparticles were distributed randomly on the surface of graphene oxide. The Fourier transform infrared and thermogravimetry analysis results showed that the graphene oxide for the AgNP-graphene oxide (AgGO) sample was partially reduced during the in situ synthesis of silver nanoparticles. Both silver nanoparticles and AgGO nanocomposites exhibited stronger antibacterial properties against Gram-negative bacteria (Salmonella typhi and Escherichia coli) than against Gram-positive bacteria (Staphyloccocus aureus and Staphyloccocus epidermidis). The AgGO nanocomposites consisting of approximately 40 wt.% silver can achieve antibacterial performance comparable to that of neat silver nanoparticles

    Effect of parboiling on in vitro physiological antioxidant capacity of brown rice

    Parboiling process has been widely implemented in brown rice processing, but its effect on in vitro physiological antioxidant capacity of brown rice was not known. In this study, an in vitro method simulating the human physiological conditions was used to investigate the effect of parboiling on antioxidant capacity of brown rice in three Bario rice varieties. In this method, bacterial inocula were prepared from rat cecal contents. Results showed that parboiling process gave significant impacts on in vitro physiological antioxidant capacity of brown rice. The process improved total phenolic content at small intestine (Adan Halus), DPPH scavenging activity at both small and large intestines (Adan Halus and Bario Merah) and ferrous ion-chelating activity at large intestine (Bario Hitam). However, changes in antioxidant capacity were variety dependent, possibly due to different bran pigmentation. These suggested that parboiling process could improve physiological antioxidant capacity with in vitro simulation at small and large intestines by selecting a suitable rice variety and parboiled brown rice could offer good antioxidant properties to maintain physiological health

    Utilization of agro-industrial residues from palm oil industry for production of lignocellulolytic enzymes by curvularia clavata

    Production of crude palm oil generates a huge amount of palm oil mill effluent (POME) and oil palm empty fruit bunch (OPEFB), leading to adverse environmental impacts. This study seeks to explore the ability of Curvularia clavata in the utilization of such waste to produce lignocellulolytic enzymes such as carboxymethyl cellulase, xylanase, manganese peroxidase, laccase and lignin peroxidase. Raw POME generated from the milling process is discharged to the raw pond, followed by generation of anaerobic POME that is an intermediate step in treatment process. Utilization of raw POME produces higher enzyme activities compared to anaerobic POME. This simultaneously resulted in detoxification and significant removal of COD (67 %), BOD3 (45 %), total polyphenolic (50 %) and ammoniacal nitrogen (61 %) of POME. Under submerged fermentation of OPEFB, C. clavata produces 3569 U/mL of xylanase, which has a potential application in the production of xylanase

    Self-Care and Self-Management Among Adolescent T2DM Patients: A Review

    Uncontrolled hyperglycaemia can lead to macro- and microvascular complications. Adolescents with T2DM develop similar complications as in adults, including cardiovascular disease, stroke, myocardial infarction, renal insufficiency, and chronic renal failure. Although regular medical follow-up is essential to avoid long-term complications, patients with diabetes mellitus need to perform holistic self-care activities such as opting for a healthy diet, physical activity, self-monitoring, and proper medication. To the best of our knowledge, only a limited number of studies have focused on self-care activities and self-management, including self-care practices, supportive networks, and self-care education programs in adolescent with T2DM. Some of the studies focused on the appreciation of self-care in adolescents with T2DM. This review aimed to analyse self-care and self-management among adolescents with T2DM, and discuss the impact of self-care and self-management on glycaemic control. The difficulties faced by adolescents in self-managing their disease are also highlighted. Such information is essential for healthcare providers in promoting self-care practices among adolescents with T2DM. A thorough search of the literature was performed using three databases: Medline, Google Scholar, and Scopus. The articles focused on self-care and self-management of adolescents patients with T2DM aged between 12 and 19 years old were included. Findings from this review reveal that healthy food adaptation, adequate physical activity, proper medication practices, and regular glucose monitoring are the most common self-care practices. Parental involvement and clinician encouragement also contribute toward the practice of self-care and self-management among the adolescents with T2DM. In conclusion, independent self-management regimens and supportive networks for appropriate administration are vital factors to enhance clinical outcomes of adolescents with T2DM

    Heat moisture modified rice flours as additive in drilling fluids

    Drilling mud is a dense, viscous fluid mixture used in oil and gas drilling operations to bring rock cuttings to the earth's surface from the boreholes as well as to lubricate and cool the drill bit. Water-based mud is commonly used due to its relatively inexpensive and easy to dispose of. However, several components and additives in the muds become increasingly cautious and restricted. Starch was introduced as a safe and biodegradable additive into the water-based drilling fluid, in line with an environmental health concern. In this study, the suitability of four local rice flours and their heat moistures derivatives to be incorporated in the formulation of water-based drilling fluid was investigated. They were selected due to their natural amylose contents (waxy, low, intermediate, and high). They were also heat moisture treated to increase their amylose contents. Results showed that the addition of the rice flours into water-based mud significantly reduced the density, viscosity, and filtrate volume. However, the gel strength of the mud was increased. The rice flours, either native or heat moisture treated, could serve as additives to provide a variety of low cost and environmentally friendly drilling fluids to be incorporated and fitted into different drilling activity

    Cyclic Tetrapyrrolic Photosensitisers from the leaves of Phaeanthus ophthalmicus

    <p>Abstract</p> <p>Background</p> <p>Twenty-seven extracts from 26 plants were identified as photo-cytotoxic in the course of our bioassay guided screening program for photosensitisers from 128 extracts prepared from 64 terrestrial plants in two different collection sites in Malaysia - Royal Belum Forest Reserve in the State of Perak and Gunung Nuang in the State of Selangor. One of the photo-cytotoxic extracts from the leaves of <it>Phaeanthus ophtalmicus </it>was further investigated.</p> <p>Results</p> <p>The ethanolic extract of the leaves from <it>Phaeanthus ophtalmicus </it>was able to reduce the <it>in vitro </it>viability of leukaemic HL60 cells to < 50% when exposed to 9.6 J/cm<sup>2 </sup>of a broad spectrum light at a concentration of 20 μg/mL. Dereplication of the photo-cytotoxic fractions from <it>P. ophthalmicus </it>extracts based on TLC R<sub>f </sub>values and HPLC co-injection of reference tetrapyrrolic compounds enabled quick identification of known photosensitisers, pheophorbide-<it>a</it>, pheophorbide-<it>a </it>methyl ester, 13<sup>2</sup>-hydroxypheophorbide-<it>a </it>methyl ester, pheophytin-<it>a </it>and 15<sup>1</sup>-hydroxypurpurin 7-lactone dimethyl ester. In addition, compound <b>1 </b>which was not previously isolated as a natural product was also identified as 7-formyl-15<sup>1</sup>-hydroxypurpurin-7-lactone methyl ester using standard spectroscopic techniques.</p> <p>Conclusions</p> <p>Our results suggest that the main photosensitisers in plants are based on the cyclic tetrapyrrole structure and photosensitisers with other structures, if present, are present in very minor amounts or are not as active as those with the cyclic tetrapyrrole structure.</p
