Deaths, disability-adjusted life years and years of life lost due to elevated systolic blood pressure in Poland: estimates for the Global Burden of Disease Study 2016
Introduction. High systolic blood pressure (SBP) is a well-known risk factor for major adverse cardiovascular outcomes; however, data regarding disease burden due to high SBP in the Polish population are scarce.
Material and methods. We extracted and analyzed the latest country-, gender-, age- and year-specific estimates from the Global Burden of Disease (GBD) Study 2016 for SBP-related mortality, years of life lost (YLLs), disability-adjusted life years (DALYs), and attributable risk factors in Poland in 2016. In the GBD 2016, the term ‘high SBP’ refers to SBP of at least 110−115 mmHg.
Results. High SBP was attributable to (per 100,000) 106,043.16 deaths (95% UI [Uncertainty Interval]: 88,207–121,849) that was 27.22% of all deaths in Poland in 2016; 1,751,844.69 DALYs (95% UI: 1,525,188–1,966,25) and 1,497,959.71 YLLs (95% UI: 1,287,279–1,497,959). In males, DALYs attributable to high SBP were higher by 34% and YLLs by 23%, while in females death rates were higher by 14%. SBP was highly attributable to ischemic heart disease, stroke, and chronic kidney disease (63.7%, 63,1%, and 59.1%, respectively). In the GBD hierarchy, high SBP was the most common risk factor, followed by smoking, high body mass index (BMI), high total cholesterol levels, alcohol use, and high fasting plasma glucose levels.
Conclusions. In Poland, SBP of at least 110–115 mmHg remains one of the largest risks for loss of good health; greater than smoking, high cholesterol levels, or high BMI. With the population aging globally, the burden due to high SBP is expected to increase further
Czy nieprawidłowe rozpoznanie zespołu Wolframa jako cukrzycy typu 1 i jej powikłań może być przyczyną rzadkiego rozpoznawania tego zespołu?
Wolfram syndrome (WS), also known as DIDMOAD (Diabetes Insipidus, Diabetes Mellitus, Optic Atrophy and Deafness), is a rare autosomal recessive syndrome (1/770,000 in the United Kingdom), characterised by juvenile onset of diabetes mellitus, optic nerve atrophy, diabetes insipidus, sensorineural deafness, renal tract and neurological abnormalities, and primary gonadal atrophy. WS is caused mainly by biallelic mutations in the WFS1 gene, which encodes wolframin. Wide tissue distribution of wolframin and many mutations in the wolframin gene resulting in Wolfram syndrome may contribute to different phenotypes and the unusual combinations of clinical features. We describe a female patient with Wolfram syndrome diagnosed at the age of 25, with a previous false diagnosis of type 1 diabetes mellitus and misdiagnosed diabetic complications. The patient was found to be a compound heterozygote for two novel mutations in exon 8 of WFS1 gene: a 2-bp deletion AT at nt 1539 leading to a frameshift (Y513fs) and a single-base substitution 1174C > T resulting in a stop codon (Q392X). A detailed analysis of the patient’s medical history and a review of the literature suggest that many cases of Wolfram syndrome may remain undiagnosed due to misdiagnosis as type 1 diabetes mellitus and incorrect interpretation of clinical symptoms of neurodegenerative abnormalities, especially in their early stages. WS jest spowodowany głównie mutacją w genie WFS1, który koduje wolframinę. Obecność wolframiny w wielu tkankach organizmu oraz wiele różnych mutacji w genie wolframiny, których skutkiem jest wystąpienie zespołu Wolframa, może stanowić przyczynę różnych fenotypów tego zespołu oraz różnych kombinacji cech klinicznych. W poniższej publikacji opisano przypadek pacjentki z zespołem Wolframa, której choroba początkowo była błędnie zdiagnozowana jako cukrzyca typu 1 i jej powikłania. W badaniach genetycznych wykazano, że pacjentka była heterozygotą w zakresie dwóch nowych mutacji w egzonie 8 genu WFS1: 2-bp delecji AT w regionie nt 1539, prowadzącej do mutacji zmiany ramki odczytu (Y513fs) oraz substytucji pojedynczej zasady 1174 C > T, czego skutkiem był stop kodon (Q392X). Ze szczegółowej analizy historii medycznej pacjentki oraz przeglądu piśmiennictwa wynika, że duża liczba przypadków zespołu Wolframa może zostać niewłaściwie rozpoznana jako cukrzyca typu 1 lub zaburzenia neurodegeneracyjne, zwłaszcza w początkowej fazie ich rozwoju. Pineal cyst-related sleep disorders – a narrative review
The pineal gland is an endocrine gland, which is responsible for the human circadian rhythm, through the regulation of its hormone – melatonin. The most common pathology of a pineal gland is pineal cyst – its population incidence is estimated as 1-23%, depending on the publication. The most common symptoms of a pineal cyst include headache (87%), visual field defects (54%), nausea/vomiting (34%), and dizziness/vertigo (31%). Among the majority of patients with the asymptomatic pineal cyst, the wait-and-see strategy is proper.
According to the newest meta-analyze, the prevalence of sleep disturbances among patients with pineal cyst estimates to be 17%. The results of direct melatonin level measurements in pineal cysts remain unclear. The study focused on sleep disturbances in pediatric patients with pineal cysts and showed a significantly higher score on Sleep Disturbance Scale for Children (SDSC) in the domains of disorders of excessive sleepiness and disorders of initiating and maintaining sleep than the control groups. The impact of lesion on sleep quality correlated with its size.
The pineal cyst may be considered a rare, and potentially reversible, cause of sleep disorders. Nevertheless, according to the present reports (often opposite to each other), this thesis and the mechanism of its occurrence need to be further researched
Verifi cation of myths on diabetic diet
Wstęp. W środowisku pacjentów chorych na cukrzycękrąży wiele mitów dotyczących produktów spożywczych,które mają obniżać stężenie glukozy we krwilub przynajmniej go nie podnosić, mimo że zawierająwęglowodany. Celem badania było określenie stanuwiedzy na temat wpływu popularnych i często stosowanych produktów spożywczych na wartość glikemiiwśród chorych na cukrzycę oraz wśród średniegopersonelu medycznego.Materiał i metody. Grupa badana obejmowała 162 chorych na cukrzycę, członków Polskiego StowarzyszeniaDiabetyków, oraz grupę kontrolną 89 zdrowych pielęgniarek z dwóch szczecińskich szpitali. Badanychpoproszono o wypełnienie ankiety weryfi kującej ichwiedzę na temat wpływu na glikemię takich produktów,jak: grejpfrut, miód, kawa zbożowa, czekolada dladiabetyków, zupa mleczna z płatkami oraz karkówka.Wyniki. Najwięcej błędnych odpowiedzi zarównowśród chorych, jak i personelu zostało zaznaczonychw przypadku grejpfruta (49% chorych i 72% personelu,p < 0,01) oraz karkówki (67% chorych i 71% personelu,NS). Najwięcej prawidłowych odpowiedzi pojawiło sięw przypadku miodu (73% chorych i 78% personelu, NS)oraz zupy mlecznej (71% chorych oraz 63% personelu, NS). Wiek, płeć, czas trwania cukrzycy nie mają wpływu na poziom wiedzy pacjentów z zakresu oddziaływania wybranych produktów spożywczych na wartość glikemii.Poziom wiedzy chorych leczonych insuliną stosowanąw monoterapii lub w leczeniu skojarzonym z lekami doustnymi jest wyższy niż osób stosujących leki doustne. Wśród personelu medycznego stwierdzono, że osoby młodsze charakteryzują się większą wiedzą z zakresu diety cukrzycowej niż osoby starsze.Wnioski. 1. Poziom edukacji pacjentów oraz średniegopersonelu szpitala z zakresu wpływu popularnychproduktów na glikemię chorego jest niezadowalający.2. Na poziom wyedukowania pacjentów nie wpływawiek, płeć oraz czas trwania cukrzycy, natomiast jest onnieco lepszy u chorych leczonych insuliną. 3. Młodszywiek personelu szpitalnego wiąże się z nieco lepszympoziomem wiedzy na temat diety cukrzycowej.Background. Among diabetic patients, there are many myths concerning food products which are believed tolower or — at least — not infl uence the glucose level,even though they contain carbohydrates. The aim ofthis study was to assess the knowledge of diabeticpatients and hospital nurses concerning popular foodproducts and their impact on blood glucose levels.Material and methods. The study group consistedof 162 patients with diabetes (DM), members of thePolish Diabetes Association; the control group (C) consisted of 89 healthy nurses from two hospitals inSzczecin, Poland. Participants were asked to completea questionnaire on products common in diabetic dietand their infl uence on blood glucose levels. The products were: grapefruit, honey, wheat coffee, diabeticchocolate, milk soup with fl akes and pork chuck steak.Results. The highest percentage of wrong answers wasgiven for pork (DM 67%; C 71%, ns) and grapefruit (DM49%; C 72%, p < 0.01), while the most correct answerswere for honey (DM 73%; C 78%, ns) and milk soup (DM 71%; C 63%, ns). There was no relationship between the level of knowledge of diabetic diet and age, sex or duration of DM. Patients on insulin monotherapy or combinations of insulin with oral agents are more educated than patients treated with drugs. Negative correlation between age and the level of knowledge was observed in the group of nurses.Conclusions. 1. The level of knowledge concerningproducts commonly used in diabetic diet among patientswith diabetes and hospital nurses is low. 2. Thelevel of patient’s education is not related to sex, age,or duration of diabetes but it is little higher in subjectstreated with insulin. 3. Younger age of medical staffcorrelates with their higher level of knowledge
Management of diabetes mellitus in patients with type 2 diabetes residing in a rural district of westpomeranian province
WSTĘP. Zdecydowana większość chorych na cukrzycę
typu 2 (ok. 95% wszystkich przypadków cukrzycy)
pozostaje, zwłaszcza na terenach wiejskich, pod
wyłączną opieką lekarzy rodzinnych. W badaniu
UKPDS udowodniono, że lepsze wyrównanie cukrzycy
typu 2 istotnie zmniejsza ryzyko rozwoju przewlekłych
powikłań cukrzycowych. Celem przedstawionego
badania była ocena wyrównania metabolicznego
chorych na cukrzycę typu 2, zamieszkujących
gminę rolniczą w województwie zachodniopomorskim.
MATERIAŁ I METODY. Badanie przeprowadzono w Poradni
Lekarza Rodzinnego NZOZ „Asklepios” w Bobolicach,
pod opieką której pozostaje 279 pacjentów
chorych na cukrzycę stanowiących 3,3% zarejestrowanych
tam osób. Na badanie zaproszono wszystkich
chorych, spośród których zgłosiło się 249 osób
(89%). Wiek badanych wynosił 32-91 lat (śr. 66,5 ±
± 12,4 roku). Wskaźnik masy ciała (BMI) oznaczony
u 174 chorych wynosił 32,3 ± 6,4 kg/m2. U wszystkich
badanych rano, na czczo pobrano krew żylną
w celu oznaczenia stężeń HbA1c oraz glukozy. WYNIKI. Średnie stężenie HbA1c w badanej próbie
wynosiło 7,82 ± 1,95%, średnie stężenie glukozy na
czczo - 156 ± 66 mg/dl. Stężenie HbA1c wynoszące
maksymalnie 6,5% stwierdzono u 31% osób uczestniczących
w badaniu. Pożądaną glikemię na czczo
wynoszącą maksymalnie 110 mg/dl, stwierdzono
u 25% chorych. Jedynie u 17% badanych oba parametry,
HbA1c oraz glikemia na czczo, spełniały kryteria
wyrównania wyznaczone przez PTD 2007. Stężenie
HbA1c poniżej 7%, zalecane przez ADA, osiągnęło
38% uczestników badania, zaś glikemię na czczo
w przedziale 90-130 mg/dl zaobserwowano u 37%
badanych. Oba zalecane przez ADA kryteria wyrównania
spełniło 22% chorych. Uzyskane wyniki porównano
z danymi pochodzącymi z innych ośrodków,
w których oceniano chorych zamieszkujących miasta.
WNIOSKI. Wyrównanie metaboliczne chorych na
cukrzycę typu 2 zamieszkujących tereny wiejskie jest
niezadowalające i wydaje się gorsze niż u pacjentów
mieszkających w miastach. Być może wpływa
na to większy stopień otyłości i nadwagi obserwowany
u tych chorych, a także mniejsza dostępność
opieki specjalistycznej.INTRODUCTION. The great majority of patients with
type 2 diabetes, which accounts for 95% of all dia betic subjects, are treated only by general practitioners,
especially in rural areas. The UKPDS study demonstrated
that a better control of type 2 diabetes
considerably reduces the risk of chronic diabetic complications.
The main goal of the study was to assess
metabolic control in type 2 diabetes patients, residents
of a rural district of Westpomeranian Province.
MATERIAL AND METHODS. The study was conducted
in the General Practitioner’s Clinic NZOZ „Asklepios”
in Bobolice, who provides medical care for 279
diabetics who account for 3.3% of all registered subjects.
All diabetic patients were invited to participate
in the study, and 249 subjects volunteered (89%). The
age of investigated subjects ranged from 32 to
91 years (mean 66.5 ± 12.4 years). The mean body
mass index (BMI), calculated for 174 patients, was 32.3 ± 6.4 kg/m². In the morning after an overnight fast, in
all participants venous blood samples were taken for
measurement of HbA1c and serum glucose level.
RESULTS. The mean HbA1c level was 7.82 ± 1.95%
and mean fasting serum glucose 156 ± 66 mg/dl.
Values of HbA1c ≥ 6.5% were observed in 31% of
participants. Recommended glucose level £ 110 mg/dl
was observed in 25% patients. Only in 17% of subjects
both parameters, HbA1c and fasting glycaemia
met the criteria recommended by Polish Diabetes
Association 2007. Values of HbA1c < 7%, recommended
by the American Diabetes Association (ADA)
were observed in 38% of subjects while fasting glycaemia
90-130 mg/dl in 37% of cases. Both criteria,
recommended by the ADA were met in 22% of diabetic
patients. Obtained results were compared with
the published data concerning outpatients’ clinics
from urban areas.
CONCLUSION. Metabolic control of type 2 diabetic
subjects, residents of the rural area in North-West
part of Poland, is unsatisfactory and seems to be
worse than in the patients from urban areas. This
may be a result of higher degree of obesity and overweight
of these patients as well as worse access to
specialist care
Ocena jakości życia u pacjentów z chorobą Gravesa-Basedowa i postępującą naciekową oftalmopatią tarczycową w trakcie skojarzonego leczenia metyloprednizolonem i radioterapią przestrzeni pozagałkowych
Wstęp: Celem pracy była ocena jakości życia u chorych z naciekową postacią oftalmopatii tarczycowej (GO, Graves’ opthalmopathy)
w trakcie pulsacyjnego leczenia metylprednizolonem w połączeniu z radioterapią oczodołów oraz próba odniesienia wyników leczenia
oftalamopatii do zmian w jakości życia.
Materiał i metody: Badaniem objęto grupę 29 pacjentów w wieku 25-74 lat (śr. wieku: 52 ± 6 lat) z naciekową GO. Podstawą kwalifikacji
chorych do leczenia oftalmopatii było uzyskanie eutyreozy, postępujący charakter zmian ocznych, stopień zaawansowania zmian ocznych
oceniany w oparciu o klasyfikację NO SPECS mieszczący się przynajmniej w klasie 3c, indeks oftalmopatii według Donaldson ≥ 4 punktów.
Za aktywną postać GO przyjmowano wartości klinicznego wskaźnika aktywności (CAS, clinical activity score) ≥ 4. Podczas leczenia,
u chorych zastosowano 6 cykli soli sodowej metyloprednizolonu w dawce 1,0 g/dobę podczas jednogodzinnych wlewów dożylnych,
przez kolejne trzy dni w tygodniu. Między 2. a 4. cyklem Solu-Medrolu prowadzono radioterapię tkanek pozagałkowych promieniami X
o energii 10 MeV. Grupę kontrolną utworzono ze zdrowych ochotników, dobranych w stosunku do grupy badanej pod względem płci, wieku, posiadanego wykształcenia i uzależnienia od nikotyny. Składała się ona z 53 osób, w wieku 21-75 lat (śr. wieku: 52,4 ± 14 lat).
Badania jakości życia przeprowadzano, opierając się na kwestionariuszu MOS SF-36.
Wyniki: Pacjenci z GO gorzej oceniali jakość życia w stosunku do osób zdrowych w zakresie ogólnej sprawności, ograniczeń fizycznych
i emocjonalnych w pełnieniu funkcji, stanu zdrowia, witalności, funkcjonowania społecznego, zdrowia psychicznego oraz występowania
i nasilenia bólu. Nie wykazano korelacji pomiędzy jakością życia a wiekiem, płcią, czasem trwania choroby Gravesa-Basedowa i oftalmopatii.
Podobnie nie stwierdzono zależności pomiędzy aktywnością i zaawansowaniem klinicznym zmian ocznych a jakością życia. Zastosowanie
skojarzonej terapii GO spowodowało znamienne zmniejszenie stopnia zaawansowania zmian ocznych i obniżenie aktywności
choroby. Po leczeniu pacjenci wskazali na poprawę jakości życia w zakresie ograniczeń fizycznych w odgrywaniu ról, występowania
i nasilenia bólu oraz witalności. Pozostałe parametry jakości życia nie różniły się istotnie statystycznie.
Wnioski: Oftalamopatia tarczycowa powoduje znaczne pogorszenie jakości życia. Stopień zaawansowania klinicznego i aktywność oftalmopatii
nie wykazują związku z jakością życia. Skuteczności leczenia oftalmopatii nie można oceniać, kierując się zmianami w jakości życia pacjentów.Introduction: The aim of the study was to assess quality of life (QoL) in patients with infiltrative form of Graves’ ophthalmopathy (GO)
during the combined pulse treatment with methylprednisolone and orbital radiotherapy, and also to search for the relation between the
results of ophthalmopathy treatment and changes in QoL.
Material and methods: The study involved 29 patients aged 25-74 (the mean age: 52 ± 6 years) with infiltrative form of GO. They were
classified for ophthalmopathy treatment on the basis of the following factors: the obtained euthyreosis, progressive character of eye
changes, the level of eye changes determined on the basis of NO SPECS classification (at least class 3c), ophthalmopathy index (OI)
according to Donaldson ≥ 4. GO was diagnosed as active if CAS (clinical activity score) ≥ 4. During the treatment, the patients received
6 cycles of methylprednisolone sodium succinate in doses of 1,0 g/24 h given as one-hour-long intravenous infusions for three successive
days in a week. Between the 2nd and 4th cycle of Solu-Medrol, orbital radiotherapy with 10 MeV X-rays was performed. The control group
was made up of healthy volunteers selected with regard to sex, age, educational background and nicotine addiction so as they corresponded
with the study group. It involved 53 individuals aged 21-75 (the mean age: 52,4 ± 14 years). QoL was assessed by means of the MOS
SF-36 questionnaire.
Results: Patients with GO evaluated their QoL lower than healthy individuals, which referred to physical functioning, physical and
emotional role functioning, general health, vitality, social functioning, mental health and bodily pain. No correlation was found between
quality of life and such factors as age, sex, or duration time of Graves disease and ophthalmopathy. Analogically, no relation was observed
between the activity and stage of clinical development of eye changes and QoL. The use of the combined GO therapy contributed to
a considerable decrease in the development of eye changes and the disease activity. After treatment, the patients’ QoL improved which
referred to physical role functioning, bodily pain, and vitality. Other QoL parameters did not statistically significantly differ.
Conclusions: GO causes a considerable worsening of QoL. The stage of clinical development and activity of GO find no reflection in QoL.
Effectiveness of treatment for GO cannot be evaluated on the basis of changes in QoL
Mechanical thrombectomy in acute stroke – Five years of experience in Poland
Objectives
Mechanical thrombectomy (MT) is not reimbursed by the Polish public health system. We present a description of 5 years of experience with MT in acute stroke in Comprehensive Stroke Centers (CSCs) in Poland.
Methods and results
We retrospectively analyzed the results of a structured questionnaire from 23 out of 25 identified CSCs and 22 data sets that include 61 clinical, radiological and outcome measures.
Results
Most of the CSCs (74%) were founded at University Hospitals and most (65.2%) work round the clock. In 78.3% of them, the working teams are composed of neurologists and neuro-radiologists. All CSCs perform CT and angio-CT before MT. In total 586 patients were subjected to MT and data from 531 of them were analyzed. Mean time laps from stroke onset to groin puncture was 250±99min. 90.3% of the studied patients had MT within 6h from stroke onset; 59.3% of them were treated with IV rt-PA prior to MT; 15.1% had IA rt-PA during MT and 4.7% – emergent stenting of a large vessel. M1 of MCA was occluded in 47.8% of cases. The Solitaire device was used in 53% of cases. Successful recanalization (TICI2b–TICI3) was achieved in 64.6% of cases and 53.4% of patients did not experience hemorrhagic transformation. Clinical improvement on discharge was noticed in 53.7% of cases, futile recanalization – in 30.7%, mRS of 0–2 – in 31.4% and mRS of 6 in 22% of cases.
Conclusion
Our results can help harmonize standards for MT in Poland according to international guidelines
Myelodysplastic syndrome and pulmonary alveolar proteinosis in a 6-year-old girl with mutation of the ZBTB24 gene
In this article, we report a case of a 6-year-old girl, initially diagnosed with myelodysplastic syndrome (MDS), who presented a compound and uncharacteristic set of symptoms that included low level of lymphocytes, hypogammaglobulinemia, developmental delay and facial anomalies. An unconventional course of the disease gave premises to extended genetic diagnostic using next-generation sequencing, that provided the result of heterozygous ZBTB24 c.[1222T > G] cysteine-to-glycine missense mutation, which is characteristic of immunodeficiency, centromeric instability and facial dysmorphism syndrome type 2 (ICF2). During the treatment of hematologic disorders, the patient underwent allogenic stem cell transplantation (allo-HSCT). After performing allo-HSCT, another rare syndrome has been developed in form of pulmonary alveolar proteinosis (PAP).
In this paper, we describe the whole diagnostic process, usage of drugs that include immunoglobulins, rituximab, steroids, azathioprine and performed therapeutic procedures, such as allo-HSCT and lung lavages.
The unique value of this case lays in the coexistence of several rare diseases and response of the patient to multi-thread treatment
