    Professor of Sociology and Dean of the College Donald W. Hinrichs was interviewed on January 30, 2004 as the first part of two interviews by Michael J. Birkner. During these interviews, he discusses his childhood in Baltimore, going to college in the 1960\u27s, and his experience in the US Army during the Vietnam War. He also describes his time at Gettysburg, in particular the growth and development of the Sociology Department and his experience as a gay faculty member.     From Titanic to Costa Concordia—a century of lessons not learned

    The recent foundering of the Costa Concordia in January 2012 demonstrated that accidents can occur even with ships that are considered masterpieces of modern technology and despite more than 100 years of regulatory and technological progress in maritime safety. The purpose of this paper is, however, not to speculate about the concrete causes of the Costa Concordia accident, but rather to consider some human and organizational factors that were present in the Costa Concordia accident as well as in the foundering of the Titanic a century ago, and which can be found in many other maritime accidents over the years. The paper argues that these factors do not work in isolation but in combination and often together with other underlying factors. The paper critically reviews the focus of maritime accident investigations and points out that these factors do not receive sufficient attention. It is argued that the widespread confidence in the efficacy of new or improved technical regulations, that characterizes the recommendations from most maritime accident investigations, has led to a lack of awareness of complex interactions of factors and components in socio-technical systems. If maritime safety is to be sustainably improved, a systemic focus must be adopted in future accident investigations

    Making the case for Crew-Centered Design (CCD) in merchant shipping

    Since 2003, the International Maritime Organization (IMO) has emphasised the need to address and integrate ergonomics knowledge in a concerted manner. However, there is little guidance on the application of this knowledge in the design of merchant vessels. Utilizing a mixed methods approach, the paper identifies the need for crew-centered design (CCD), highlighting the importance of using concepts derived from Human-Centred Design (HCD) to be able to design work spaces and operational procedures that facilitate the work of the crew on board. Drawing upon results obtained from accident analysis (utilising the Technique for the Retrospective and predictive Analysis of Cognitive Errors - TRACEr) and 24 semi-structured interviews, this paper identifies areas in which it could be potentially beneficial to integrate end-users in the design of ships and shipborne operations. This paper further discusses why and how concepts rooted in HCD could be used to improve maritime workspace and interface design, as well as the design of procedures and shipborne operations. However, due to the specifics of the maritime domain, the authors propose that there is the need to expand the HCD perspective, making the design not only fit a single user, but to fit the crew as part of a maritime socio-technical system. The paper discusses that the case for CCD is imperative in the interest of safety, efficiency and even makes economic sense

    Detection of myocardial injury during transvenous implantation of automatic cardioverter-defibrillators

    AbstractOBJECTIVESThe present study was designed to assess the extent of myocardial injury in patients undergoing transvenous implantation of an automatic implantable cardioverter-defibrillator (ICD) using cardiac troponin I (cTNI), which is a highly specific marker of structural cardiac injury.BACKGROUNDDuring ICD implantation, repetitive induction and termination of ventricular fibrillation (VF) via endocardial direct current shocks is required to demonstrate the correct function of the device. Transthoracic electrical shocks can cause myocardial cell injury.METHODSMeasurements of total creatine kinase (CK), CK-MB, myoglobin, cardiac troponin T (cTNT) and cTNI were obtained before and after ICD implantation in 49 consecutive patients. Blood samples were drawn before and 2, 4, 8, and 24 h after implantation.RESULTSElevations of CK, CK-MB, myoglobin, cTNT and cTNI above cut-off level were found in 25%, 6%, 76%, 37% and 14% of patients, respectively, with peak cTNI concentrations ranging from 1.7 to 5.5 ng/ml. Cumulative defibrillation energy (DFE), mean DFE, cumulative VF time, number of shocks as well as prior myocardial infarction (MI) were found to be significantly related to a rise of cTNI. Mean DFE ≥ 18 J and a recent MI were identified as strong risk factors for cTNI rise.CONCLUSIONSDuring transvenous ICD implantation myocardial injury as assessed by cTNI rise occurs in about 14% of the patients. Peak cTNI concentrations are only minimally elevated reflecting subtle myocardial cell damage. Patients with a recent MI and a mean DFE ≥ 18 J seem to be prone to cTNI rise

    Maternal obesogenic diet enhances cholestatic liver disease in offspring

    Human and animal model data show that maternal obesity promotes nonalcoholic fatty liver disease in offspring and alters bile acid (BA) homeostasis. Here we investigated whether offspring exposed to maternal obesogenic diets exhibited greater cholestatic injury. We fed female C57Bl6 mice conventional chow (CON) or high fat/high sucrose (HF/HS) diet and then bred them with lean males. Offspring were fed 3,5-diethoxycarbonyl-1,4-dihydrocollidine (DDC) for 2 weeks to induce cholestasis, and a subgroup was then fed CON for an additional 10 days. Additionally, to evaluate the role of the gut microbiome, we fed antibiotic-treated mice cecal contents from CON or HF/HS offspring, followed by DDC for 2 weeks. We found that HF/HS offspring fed DDC exhibited increased fine branching of the bile duct (ductular reaction) and fibrosis but did not differ in BA pool size or intrahepatic BA profile compared to offspring of mice fed CON. We also found that after 10 days recovery, HF/HS offspring exhibited sustained ductular reaction and periportal fibrosis, while lesions in CON offspring were resolved. In addition, cecal microbiome transplant from HF/HS offspring donors worsened ductular reaction, inflammation, and fibrosis in mice fed DDC. Finally, transfer of the microbiome from HF/HS offspring replicated the cholestatic liver injury phenotype. Taken together, we conclude that maternal HF/HS diet predisposes offspring to increased cholestatic injury after DDC feeding and delays recovery after returning to CON diets. These findings highlight the impact of maternal obesogenic diet on hepatobiliary injury and repair pathways during experimental cholestasis
