    The effective nursing educational plan for the elderly at hospital discharge. - through the questionnaires for the nurses and the elderly patients -

    近年の急速な高齢化社会にともない複数の疾病や種々の障害をもちながら自宅で療養する高齢者が増加してきている。このような状況でいったん入院療養した高齢者が自宅へ帰り生活をしていくにはかなりの困難が予測される。高齢者が退院し自宅での生活にスムーズに適応できるか否かは看護婦の退院指導の良否に関わってくる。そこで指導側の看護婦の指導計画や指導方法、内容について調査すると共に受け手の患者の指導希望内容との比較を行い有効な退院指導のあり方を検討した。患者の心配事や援助や相談の希望内容と看護婦の重要とする指導内容はほぼ同様の内容であった。その内容は日常生活に関すること、病気や健康に関することが多かった。看護婦の退院指導の計画でカンファレンスをしている看護婦はしていない看護婦に比べ指導内容の経済的なこと、趣味や生きがいに関する事、生活環境、家族の協力に関することを重要とすると回答したものが有意に多かった。The elderly with chronic disease or handicaps who receive nursing home care is currently increasing. They may have some difficulties in their daily life after hospital discharge. The proper nursing educational plan may determine their post-hospital life without anxiety. The purpose of this study is to find out the effective nursing educational plan for them. Therefore, we investigated the contents of the nursing educational plan for their hospital discharge and what elderly patients need in their post-hospital life. As a result, both contents were similar. Those were about their daily life, disease, health. And there were significantly defferences between well-discussed plan and no discussed plan in the items of economy, hobbies, purpose of life, home environment, family cooperation. Results from this study suggest that more detailed discussion among nurses at hospital discharge is necessary to determine the individual nursing educational plan of elderly patients in their post-hospital life

    Discovery of the First Low-Luminosity Quasar at z > 7

    We report the discovery of a quasar at z = 7.07, which was selected from the deep multi-band imaging data collected by the Hyper Suprime-Cam (HSC) Subaru Strategic Program survey. This quasar, HSC J124353.93+010038.5, has an order of magnitude lower luminosity than do the other known quasars at z > 7. The rest-frame ultraviolet absolute magnitude is M1450 = -24.13 +/- 0.08 mag and the bolometric luminosity is Lbol = (1.4 +/- 0.1) x 10^{46} erg/s. Its spectrum in the optical to near-infrared shows strong emission lines, and shows evidence for a fast gas outflow, as the C IV line is blueshifted and there is indication of broad absorption lines. The Mg II-based black hole mass is Mbh = (3.3 +/- 2.0) x 10^8 Msun, thus indicating a moderate mass accretion rate with an Eddington ratio 0.34 +/- 0.20. It is the first z > 7 quasar with sub-Eddington accretion, besides being the third most distant quasar, known to date. The luminosity and black hole mass are comparable to, or even lower than, those measured for the majority of low-z quasars discovered by the Sloan Digital Sky Survey, and thus this quasar likely represents a z > 7 counterpart to quasars commonly observed in the low-z universe.Comment: Accepted for publication in ApJ Letter

    Acompanhamento do tratamento de retinopatia diabética associada a edema macular: um relato de caso

    Diabetic retinopathy (DR) is a retinal microvascular complication affecting the retina that compromises visual acuity and can lead to a significant risk of blindness and it results from inadequate and irregular control of diabetes mellitus. Here, we report the case of a 44-year-old male patient with type 2 diabetes mellitus and diabetic retinopathy associated with macular edema. The patient also presents erectile dysfunction as another complication of diabetes mellitus and uses the drug Tadalafil occasionally. We seek to understand. We seek to understand the prognosis of the case described and the importance of adequate ophthalmological monitoring and treatment. PubMed and Scientific Eletronic Library (SciELO) were used as search engines for the selection of articles, using the keywords "Diabetic Retinopathy, Diabetes complications, Diabetes Mellitus, Macular Edema". This article characterizes as an observational, descriptive study which was conducted at a private ophthalmological center located at Ponta Grossa - PR. Data collection involved thorough examination of the patient's medical record, as well as photographic results of imaging exams. Analysis of the case highlighted the need for further research into the outcomes, manifestations, progression and efficacy of treatments for DR. Furthermore, the association with the drug Tadalafil needs more studies and investigation aiming to elucidate the possible consequences with different ocular and vascular disorders.A retinopatia diabética (RD) é uma complicação microvascular retiniana que compromete a acuidade visual e traz como principal risco a cegueira e é resultante do controle inadequado e irregular da diabetes mellitus. Relatamos um caso de um paciente do sexo masculino, de 44 anos, portador de diabetes mellitus tipo 2 e retinopatia diabética associada à edema macular. O paciente apresenta também disfunção erétil como outra complicação da diabetes mellitus e faz uso eventual do medicamento Tadalafila. Buscou-se entender o prognóstico do caso descrito e a importância de um acompanhamento e tratamento oftalmológico adequado. Foram utilizados como motores de busca os indexadores PubMed e Scientific Eletronic Library (SciELO) para seleção dos artigos, através dos unitermos “Retinopatia Diabética; Complicações do Diabetes; Diabetes Mellitus; Edema macular.’’ O artigo caracteriza-se como um estudo observacional, do tipo descritivo, que foi realizado em um centro oftalmológico particular do município de Ponta Grossa - PR. A coleta de dados ocorreu por meio da análise do prontuário do paciente, bem como resultados fotográficos de exames realizados. a Análise do caso identificou a necessidade de mais estudos e pesquisas para buscar entender as consequências, manifestações, evolução e eficácia do tratamento da RD. Além disso, a associação com o medicamento Tadalafila precisa ser mais bem estudada e investigada com objetivo de elucidar as possíveis consequências para os pacientes portadores de diversas alterações vasculares oculares

    Symptomatic periesophageal vagal nerve injury by different energy sources during atrial fibrillation ablation

    BackgroundSymptomatic gastric hypomotility (SGH) is a rare but major complication of atrial fibrillation (AF) ablation, but data on this are scarce.ObjectiveWe compared the clinical course of SGH occurring with different energy sources.MethodsThis multicenter study retrospectively collected the characteristics and clinical outcomes of patients with SGH after AF ablation.ResultsThe data of 93 patients (67.0 ± 11.2 years, 68 men, 52 paroxysmal AF) with SGH after AF ablation were collected from 23 cardiovascular centers. Left atrial (LA) ablation sets included pulmonary vein isolation (PVI) alone, a PVI plus a roof-line, and an LA posterior wall isolation in 42 (45.2%), 11 (11.8%), and 40 (43.0%) patients, respectively. LA ablation was performed by radiofrequency ablation, cryoballoon ablation, or both in 38 (40.8%), 38 (40.8%), and 17 (18.3%) patients, respectively. SGH diagnoses were confirmed at 2 (1–4) days post-procedure, and 28 (30.1%) patients required re-hospitalizations. Fasting was required in 81 (92.0%) patients for 4 (2.5–5) days; the total hospitalization duration was 11 [7–19.8] days. After conservative treatment, symptoms disappeared in 22.3% of patients at 1 month, 48.9% at 2 months, 57.6% at 3 months, 84.6% at 6 months, and 89.7% at 12 months, however, one patient required surgery after radiofrequency ablation. Symptoms persisted for >1-year post-procedure in 7 patients. The outcomes were similar regardless of the energy source and LA lesion set.ConclusionsThe clinical course of SGH was similar regardless of the energy source. The diagnosis was often delayed, and most recovered within 6 months, yet could persist for over 1 year in 10%

Some users have pointed out some issues concerning sentence delimitation in the BCCWJ. To address these issues, we - NINJAL - performed a complete survey and correction, beginning in 2013 and ending in 2014. This article reports the revision work on sentence delimitation in the BCCWJ. The problems with the BCCWJ DVD Version 1.0 derive from the string-based definition. We could not obtain any morpheme information for the sentence delimitation task because of the task parallelism between sentence delimitation annotation and morpheme annotation. The method used this time was morpheme based. We present the morpheme-based annotation guidelines, annotation environment, and basic statistics of the corpus correction
