    The experience of sleep for women with borderline personality disorder: An occupational perspective

    Introduction: Sleep difficulties are prevalent in borderline personality disorder (BPD); however, the experience of sleep for individuals with a diagnosis of BPD has not been considered from an occupational therapy perspective. Method: This study adopted a qualitative methodology. Semi-structured interviews were utilised to explore how sleep is experienced by four women with a diagnosis of BPD, in an inpatient setting. Interviews were recorded and transcribed verbatim. Findings: Through thematic analysis four themes were identified: ‘there isn’t a proper pattern or structure to my sleeps’, ‘sleep is getting away for me’, ‘before I came in [to the unit] my life-work balance was all chaotic’ and ‘I know the routine, but it doesn’t really seem to affect me’. These findings described the variety of problems experienced by participants in regards to sleep and the implications of sleep difficulties on occupational performance and participation. Conclusion: Women with BPD on an inpatient unit experienced a variety of sleep difficulties which impact on their ability to engage in occupations. Sleep and sleep difficulties should be explicitly addressed by occupational therapists and sleep interventions should be individually tailored; further research is required to examine occupational therapy interventions for sleep

    Identification and characterization of Dlc1 isoforms in the mouse and study of the biological function of a single gene trapped isoform

    <p>Abstract</p> <p>Background</p> <p>The Dlc1 (deleted in liver cancer 1) tumour suppressor gene codes for a RhoGTPase activating protein that is found inactivated in many tumour types. Several transcriptional isoforms have been described but the functional significance and tissue distribution of each form is presently poorly understood. Also, differences in the number of isoforms and splice variants reported still exist between different mammalian species. In order to better understand the number and function of the different variants of the Dlc1 gene in the mouse, we have carried out a detailed analysis. Extensive 3' RACE experiments were carried out in order to identify all possible Dlc1 isoforms and splice variants in the mouse. In addition, we have generated a gene trapped mouse that targets one of these isoforms in order to study its biological function. The effect of this gene trap insertion on the splicing of other isoforms has also been studied.</p> <p>Results</p> <p>In addition to the known 6.1 and 6.2 Kb transcripts of Dlc1, our study revealed the existence of a novel 7.6 Kb transcriptional isoform in the mouse, which corresponds to the human 7.4 Kb (KIAA1723) cDNA transcript. A gene trapped embryonic cell line, with an insertion between Exon 1 and 2 of the 6.1 Kb transcriptional isoform, was used to generate a transgenic mouse. This line showed a significant reduction in the expression of the trapped isoform. However, reduced expression of the other isoforms was not seen. Mice heterozygous for the gene trapped allele were phenotypically normal, but homozygous mutant embryos did not survive beyond 10.5 days post coitum. Dlc1<sup>gt/gt </sup>embryos showed defects in the brain, heart, and placental blood vessels. Cultured serum-free mouse embryo cells from Dlc1 deficient embryos had elevated RhoA activity and displayed alterations in the organization of actin filaments and focal adhesions. The Dlc1 deficient cells also exhibited increased wound closure in an <it>in vitro </it>scratch assay.</p> <p>Conclusions</p> <p>The mouse has three major transcriptional isoforms of the Dlc1 gene that are differentially expressed in various tissues. A mouse with exon 1 of the 6.1 Kb transcript gt resulted in hypomorphic expression of Dlc1 protein and an embryonic lethal phenotype in the homozygous condition, which indicates that this isoform plays a major role in mouse development. The Dlc1 deficient cells showed altered cytoskeleton structure, increased RhoA activity and cellular migration.</p

    Rare case of autonomic instability of the lower limb presenting as painless Complex Regional Pain Syndrome type I following hip surgery: two case reports

    <p>Abstract</p> <p>Introduction</p> <p>According to the International Association for the Study of Pain criteria of 1994, pain is a diagnostic requirement for Complex Regional Pain Syndrome type I. However, other authors have suggested that patients can rarely present with the sensory and vascular symptoms of Complex Regional Pain Syndrome without pain. This entity has not been reported following hip surgery in the English medical literature.</p> <p>Case presentation</p> <p>We present two cases of Complex Regional Pain Syndrome-like symptoms following hip surgery and with the total absence of pain. The first case was a 29-year-old Caucasian woman who had a reattachment of the greater trochanter following non-union of an intertrochanteric osteotomy of the hip. Five weeks later, the patient presented with features of Complex Regional Pain Syndrome but with the absence of pain. The second patient was a 20-year-old Caucasian woman who had undergone an open debridement and repair of a torn acetabular labrum. Ten days later, the patient presented with features suggestive of Complex Regional Pain Syndrome which was again painless. Both patients were non-weight bearing at presentation and the symptoms resolved following recommencement of weight bearing.</p> <p>Conclusions</p> <p>The authors believe these symptoms are manifestations of vascular changes to the lower limb as a result of non-weight bearing status. Painless Complex Regional Pain Syndrome-like symptoms may occur in patients who are kept non-weight bearing following hip surgery. However, vascular insufficiency and deep venous thrombosis must be excluded before this diagnosis is made. If the clinical situation permits, early weight bearing may relieve symptoms. Orthopaedic and vascular surgeons should be aware of this entity when a postoperative patient presents to them with the above clinical picture. This is also relevant to general practitioners who are likely to see the patients in the postoperative period.</p

    Generation of intense quasi-electrostatic fields due to deposition of particles accelerated by petawatt-range laser-matter interactions

    We demonstrate here for the first time that charge emitted by laser-target interactions at petawatt peak-powers can be efficiently deposited on a capacitor-collector structure far away from the target and lead to the rapid (tens of nanoseconds) generation of large quasi-static electric fields over wide (tens-of-centimeters scale-length) regions, with intensities much higher than common ElectroMagnetic Pulses (EMPs) generated by the same experiment in the same position. A good agreement was obtained between measurements from a classical field-probe and calculations based on particle-flux measurements from a Thomson spectrometer. Proof-of-principle particle-in-cell simulations reproduced the measurements of field evolution in time, giving a useful insight into the charging process, generation and distribution of fields. The understanding of this charging phenomenon and of the related intense fields, which can reach the MV/m order and in specific configurations might also exceed it, is very important for present and future facilities studying laser-plasma-acceleration and inertial-confinement-fusion, but also for application to the conditioning of accelerated charged-particles, the generation of intense electric and magnetic fields and many other multidisciplinary high-power laser-driven processes

    A Multi-Scale Feasibility Study into Acid Mine Drainage (AMD) Monitoring Using Same-Day Observations

    This is the final version. Available on open access from MDPI via the DOI in this recordGlobally, many mines emit acid mine drainage (AMD) during and after their operational life cycle. AMD can affect large and often inaccessible areas. This leads to expensive monitoring via conventional ground-based sampling. Recent advances in remote sensing which are both non-intrusive and less time-consuming hold the potential to unlock a new paradigm of automated AMD analysis. Herein, we test the feasibility of remote sensing as a standalone tool to map AMD at various spatial resolutions and altitudes in water-impacted mining environments. This was achieved through the same-day collection of satellite-based simulated Sentinel-2 (S2) and PlanetScope (PS2.SD) imagery and drone-based UAV Nano-Hyperspec (UAV) imagery, in tandem with ground-based visible and short-wave infrared analysis. The study site was a historic tin and copper mine in Cornwall, UK. The ground-based data collection took place on the 30 July 2020. Ferric (Fe(III) iron) band ratio (665/560 nm wavelength) was used as an AMD proxy to map AMD pixel distribution. The relationship between remote-sensed Fe(III) iron reflectance values and ground-based Fe(III) iron reflectance values deteriorated as sensor spatial resolution decreased from high-resolution UAV imagery (<50 mm2 per pixel; r2 = 0.78) to medium-resolution PlanetScope Dove-R (3 m2 per pixel; r2 = 0.51) and low-resolution simulated Sentinel-2 (10 m2 per pixel; r2 = 0.23). A fractioned water pixel (FWP) analysis was used to identify mixed pixels between land and the nearby waterbody, which lowered spectral reflectance. Increases in total mixed pixels were observed as the spatial resolution of sensors decreased (UAV: 2.4%, PS: 3.7%, S2: 8.5%). This study demonstrates that remote sensing is a non-intrusive AMD surveying tool with varying degrees of effectiveness relative to sensor spatial resolution. This was achieved by identifying and successfully mapping a cross-sensor Fe(III) iron band ratio whilst recognizing water bodies as reflectance inhibitors for passive sensors.European Space AgencyUniversity of Exete

    Effects of an irregular bedtime schedule on sleep quality, daytime sleepiness, and fatigue among university students in Taiwan

    <p>Abstract</p> <p>Background</p> <p>An irregular bedtime schedule is a prevalent problem in young adults, and could be a factor detrimentally affecting sleep quality. The goal of the present study was to explore the association between an irregular bedtime schedule and sleep quality, daytime sleepiness, and fatigue among undergraduate students in Taiwan.</p> <p>Methods</p> <p>A total of 160 students underwent a semi-structured interview and completed a survey comprising 4 parts: Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Fatigue Severity Scale (FSS), and a rating of irregular bedtime frequency. Participants were grouped into 3 groups in terms of irregular bedtime frequency: low, intermediate, or high according to their 2-week sleep log. To screen for psychological disorders or distress that may have affected responses on the sleep assessment measures, the Chinese health questionnaire-12 (CHQ-12) was also administered.</p> <p>Results</p> <p>We found an increase in bedtime schedule irregularity to be significantly associated with a decrease in average sleep time per day (Spearman r = -0.22, p = 0.05). Multivariate regression analysis revealed that irregular bedtime frequency and average sleep time per day were correlated with PSQI scores, but not with ESS or FSS scores. A significant positive correlation between irregular bedtime frequency and PSQI scores was evident in the intermediate (partial r = 0.18, p = 0.02) and high (partial r = 0.15, p = 0.05) frequency groups as compared to low frequency group.</p> <p>Conclusion</p> <p>The results of our study suggest a high prevalence of both an irregular bedtime schedule and insufficient sleep among university students in Taiwan. Students with an irregular bedtime schedule may experience poor sleep quality. We suggest further research that explores the mechanisms involved in an irregular bedtime schedule and the effectiveness of interventions for improving this condition.</p

    The phases of deuterium at extreme densities

    We consider deuterium compressed to higher than atomic, but lower than nuclear densities. At such densities deuterium is a superconducting quantum liquid. Generically, two superconducting phases compete, a "ferromagnetic" and a "nematic" one. We provide a power counting argument suggesting that the dominant interactions in the deuteron liquid are perturbative (but screened) Coulomb interactions. At very high densities the ground state is determined by very small nuclear interaction effects that probably favor the ferromagnetic phase. At lower densities the symmetry of the theory is effectively enhanced to SU(3), and the quantum liquid enters a novel phase, neither ferromagnetic nor nematic. Our results can serve as a starting point for investigations of the phase dynamics of deuteron liquids, as well as exploration of the stability and dynamics of the rich variety of topological objects that may occur in phases of the deuteron quantum liquid, which range from Alice strings to spin skyrmions to Z_2 vortices.Comment: 9 pages, 6 figures; v2: fixed typo
