Coarctation of the Aorta in the Adult
Coarctation of the aorta is a complex systemic cardiovascular disease. The most prominent feature is the stenosis at the aortic isthmus that can be removed surgically or by catheter intervention. However, there are often concomitant cardiovascular problems like a bicuspid aortic valve, vascular wall abnormalities, arterial hypertension and vascular aneurysms. Therefore, regular life-long medical follow-up is necessary including modern imaging techniques, even after successful primary treatment
Giant dielectric constants at the approach to the insulator-metal transition
We have measured the real and imaginary parts of the dielectric susceptibility of insulating samples of P-doped Si at millikelvin temperatures at 400 MHz using a resonant transmission cavity. We find that the real part is enhanced by more than two orders of magnitude over the isolated donor polarizability, and we determine the exponent which describes the critical divergence of the real part at the insulator-metal transition by fitting the temperature dependence of the corresponding imaginary part. The form of the observed divergence remains unexplained theoretically
Intracardiac thrombus formation after the Fontan operation
AbstractObjectives: Intracardiac thrombus formation is suspected to be a specific sequela after the Fontan operation and is difficult to determine by means of routine transthoracic echocardiography. The aim of our study was to evaluate the occurrence of intracardiac thrombi in the different types of Fontan modifications and to identify predisposing risk factors. Methods: We evaluated 52 patients who had undergone a Fontan-type operation and were free of symptoms regarding thrombosis as determined by transesophageal echocardiography. Results: In 17 (33%) patients thrombus formation could be found without clinical evidence of thromboembolic complications. Neither underlying morphologic disease nor age at operation, type of Fontan operation, sex, follow-up interval, arrhythmias, or laboratory or hemodynamic findings could be identified as predisposing risk factors. Conclusion: In patients having had a Fontan operation with inadequate or without anticoagulation medication, we would recommend routine transesophageal echocardiography to exclude eventual thrombi. Because of the high incidence of thrombi, we suggest oral anticoagulation therapy in all patients. (J Thorac Cardiovasc Surg 2000;119:745-52

Contacts between the endoplasmic reticulum and other membranes in neurons
The cytoplasm of eukaryotic cells is compartmentalized by intracellular membranes that define subcellular organelles. One of these organelles, the endoplasmic reticulum, forms a continuous network of tubules and cisternae that extends throughout all cell compartments, including neuronal dendrites and axons. This network communicates with most other organelles by vesicular transport, and also by contacts that do not lead to fusion but allow cross-talk between adjacent bilayers. Though these membrane contacts have previously been observed in neurons, their distribution and abundance has not been systematically analyzed. Here, we have carried out such analysis. Our studies reveal new aspects of the internal structure of neurons and provide a critical complement to information about interorganelle communication emerging from functional and biochemical studies
Emergency hospital admissions and three-year survival of adults with and without cardiovascular surgery for congenital cardiac disease
AbstractObjective:This study determined the quantity and nature of emergencies leading to unscheduled hospital admissions of adults with congenital cardiac disease and their mid-term survival.ResultsDuring 1 year, 429 adults with congenital cardiac diseases were admitted 571 times, and 124 admissions (22%) of 95 patients (22%) were emergency admissions. Fifteen of the 95 patients were seen for the first time in 1 of the participating centers. The underlying anomalies were Fallot's tetralogy and pulmonary atresia (n = 26/7), univentricular heart after Fontan procedure (n = 25), atrial septal defect (n = 18), Eisenmenger syndrome (n = 12), complete transposition (n = 11), and others (n = 25). Indications for admission were cardiovascular complications (n = 103; 83%) (arrhythmia, cardiac failure, syncope, pacemaker problems, pericardial tamponade, and sudden death), infections (n = 8, 6%) (endocarditis, pacemaker infection, pneumonia, and cerebral abscess), acute chest pain (n = 7; 6%), and acute abdominal pain (n = 4; 3%). All patients required immediate emergency care, and 16 patients (17%) required urgent cardiovascular or abdominal surgery. Six patients died during the hospital stay. During a follow-up of 2.9 years (SD 0.8), 16 (18%) of the discharged patients died, and 2 additional patients underwent heart or heart-lung transplantation.ConclusionAdults with congenital cardiac disease often experience serious emergency situations with a high in-hospital and mid-term post-hospital mortality. Care given by physicians with special expertise is important in this specific group of patients

Low field vortex matter in YBCO: an atomic beam magnetic resonance study
We report measurements of the low field structure of the magnetic vortex
lattice in an untwinned YBCO single-crystal platelet. Measurements were carried
out using a novel atomic beam magnetic resonance (ABMR) technique. For a 10.7 G
field applied parallel to the c-axis of the sample, we find a triangular
lattice with orientational order extending across the entire sample. We find
the triangular lattice to be weakly distorted by the a-b anisotropy of the
material and measure a distortion factor, f = 1.16. Model-experiment
comparisons determine a penetration depth, lambda_ab = 140 (+-20) nm. The paper
includes the first detailed description of the ABMR technique. We discuss both
technical details of the experiment and the modeling used to interpret the

Eisenmenger syndrome is the most severe form of pulmonary arterial hypertension and arises on the basis of congenital heart disease with a systemic-to-pulmonary shunt. Due to the chronic slow progressive hypoxemia with central cyanosis, adult patients with the Eisenmenger syndrome suffer from a complex and multisystemic disorder including coagulation disorders (bleeding complications and paradoxical embolisms), renal dysfunction, hypertrophic osteoarthropathy, heart failure, reduced quality of life and premature death
