    Effect of Low-Level Laser Therapy Versus Bioptron on Psoriasis

    Purpose: The current study was carried-out to examine the effectiveness of low-level laser therapy versus bioptron on psoriasis. Subjects and Methods: 60 patients of both sexes with psoriasis contributed in this study and they were aged from 30 to 60 years. They were randomized into two groups of the same number (A and B). Group (A) were given low level laser therapy (LLLT) and medical care, while group (B) were given bioptron light therapy (BLT) and the same medical care that received in group (A). The variables included Psoriasis Area Severity Index (PASI) and Dermoscopic finding were measured pre and post intervention program on psoriatic patients in both groups. Both groups were given treatment program 3 sessions a week for 8 weeks. Results: following 8 consecutive weeks, both groups showed statistically significant improvement in all outcome measures, however there was a significant difference between them post treatment, favoring group (A). Conclusion: Low level laser therapy and bioptron were beneficial treatment modalities in psoriatic patients and LLLT may be more effective in treating psoriasis

    Peguero Electrocardiographic Left Ventricular Hypertrophy Criteria and Risk of Mortality

    Background: Peguero electrocardiographic left ventricular hypertrophy (ECG-LVH) criteria are newly developed criteria that have shown better diagnostic performance than the traditional Cornell-voltage and Sokolow-Lyon criteria. However, prediction of poor outcomes rather than detection of increased left ventricular mass is becoming the primary use for ECG-LVH criteria which requires investigating any new ECG-LVH criteria in terms of prediction.Aims: To examine the prognostic significance of the newly developed Peguero ECG-LVH criteria.Methods: We compared the prognostic significance of Peguero ECG-LVH with Cornell-voltage and Sokolow-Lyon ECG-LVH criteria in 7,825 participants (age 59.8 ± 13.4 years; 52.7% women) from the third National Health and Nutrition Examination Survey who were free of major intraventricular conduction defects. ECG-LVH criteria were derived from digital ECG tracings processed at a central core laboratory.Results: At baseline, ECG-LVH was detected in 11.8% by Peguero; in 4.3% by Cornell voltage and in 6.4% by Sokolow-Lyon. During a median follow up of 13.8 years, 2,796 all-cause mortality events occurred. In multivariable models adjusted for demographics and cardiovascular risk factors, presence of Peguero ECG-LVH was associated with increased risk of all-cause mortality [HR (95% CI): 1.29 (1.16, 1.44)]. This association was not significantly different from the associations of Cornell voltage-LVH or Sokolow-Lyon LVH with all-cause mortality [HR (95%CI): 1.32 (1.12, 1.55) and 1.24 (1.07, 1.43), respectively; p-values for comparisons of these HRs with the HR of Peguero ECG-LVH 0.817 and 0.667, respectively]. Similar patterns of associations were observed with cardiovascular, ischemic heart disease and heart failure mortalities.Conclusion: Peguero ECG-LVH is predictive of increased risk of death similar to the traditional ECG-LVH criteria

    Infective Endocarditis After Transcatheter Aortic Valve Implantation: A Systematic Review

    Infective endocarditis (IE) after transcatheter aortic valve implantation (TAVI) is a rare but potentially fatal event. In this systematic review, we searched PubMed and Embase for large TAVI studies and registries to identify the incidence, presentation, microbiology, risk factors, and outcomes of IE in this population. After application of the selection criteria and quality assessment, 8 studies representing 255,310 TAVR cases and 4218 cases of IE qualified for this review. IE following TAVI is uncommon with an incidence of 0.87 to 1.7 events per 100 person-years. Most events occur in the first year following valve implantation. Staphylococcus , Enterococcus , and Streptococcus species are the most common pathogens. Risk factors include age, sex, concomitant comorbidities, and procedural factors. Outcomes are dismal, and surgical intervention is rare in this population

    Multifocal visual evoked potential for evaluation of open-angle glaucoma

    Background: To correlate multifocal visual evoked potential (mfVEP) findings with static automated perimetry (SAP) and spectral-domain optical coherence tomography (SD-OCT) in eyes with primary open angle glaucoma (POAG). Methods: This cross-sectional study included a consecutive sample of 40 eyes of 40 patients with POAG. The participants underwent a complete ophthalmologic assessment, axial length (AL) measurement, and assessments with SAP, SD-OCT, and mfVEP. Results: POAG cases were aged 49.70 ± 14.16 years (mean ± SD) and most were females (n = 24, 60%). For eyes of patients with POAG, the mfVEP upper-ring signal-to-noise ratio (SNR) showed a significant negative correlation with best-corrected logMAR visual acuity (r = - 0.33; P = 0.038), and a significant positive correlation with the superior hemifield of the visual field (VF) and the inferior-quadrant retinal nerve fiber layer (RNFL) thickness (r = + 0.34; P = 0.030; r = + 0.51; P < 0.001, respectively). Similarly, the mfVEP lower-ring SNR showed a significant negative correlation with best-corrected logMAR visual acuity (r = - 0.36; P = 0.024) and a significant positive correlation with the inferior hemifield of the VF and superior quadrant RNFL thickness (r = + 0.55; P < 0.001 and r = + 0.70; P < 0.001, respectively). Conclusions: mfVEP is a promising tool for objective assessment of the VF in patients with POAG, as it is positively correlated with the VF and OCT RNFL thickness. Future longitudinal studies with a larger sample size and a specific glaucoma subtype, along with multiple follow-up evaluations, are warranted to confirm our preliminary results
