    Management of merged external/internal root resorption using CEM cement: a case report

    Chronic pulpal inflammation and infection are the main predisposing factors for internal and external root resorption (IRR & ERR); however, merging of IRR and ERR is a rare lesion which rigorously alters the anatomy of root canals. This study reports a case of merged IRR and ERR in an asymptomatic maxillary left central incisor in a 33-year old Caucasian woman that was managed by one-visit root canal therapy (RCT) using calcium-enriched mixture (CEM) cement. Radiographic examination showed a short root with under-filled root canal obturation associated with ERR/IRR and an apical lesion. After thorough chemo-mechanical preparation, the root canal was obturated with CEM cement; one week later, the access cavity was permanently restored. Clinical/radiographic examinations at 1-year follow-up revealed uneventful healing, reestablishment of lamina dura and stabilization of the resorptive defects. The treatment outcome demonstrates that one-visit RCT using CEM cement may be a viable treatment option in cases with merged external/internal root resorption. Further clinical trials with a larger number of cases are suggested to document a higher level of evidenc

    The effects of vitamin D on Immunoresponsive gene 1 and KrĂĽppel-like Factor 2 protein expression in the lung due to the cadmium poisoning

    Background: Cadmium, a well-known hazardous heavy metal and non-essential component, has several negative health effects. The long-term use of cadmium toxin to develop a pulmonary model, evaluation of Irg1 gene expression and KLF-2 protein and serum IL4 levels following model induction, and evaluation of vitamin D's therapeutic effects in reducing pulmonary and hepatic complications in a mice model have not been studied.Methods: A total of 40 healthy female C57 black 6 mice weighing 20–25 g and approximately 6–8 weeks’ old were purchased from animal husbandry, Pasteur Institute of Iran. After induction of the model, the mice were assigned to the following groups such as Group 1 (G1): mice were euthanized the day after induction. Group 2(G2): mice were ethically killed 21 days after induction. Group 3(G3) mice were treated with vitamin D and euthanized 5 days after treatment. Group 4(G4): mice were treated with vitamin D and euthanized 21 days after treatment. Vitamin D3 with a concentration of 300,000 units per milliliter, which is equivalent to 7.5 mg per 1 microliter, and 13.5 μl of the main stock is equivalent to 100 ng, which is used for 1 kg of mice. Blood samples were collected to measure serum glutamic-oxaloacetic transaminase, serum glutamic-pyruvic transaminase, and alkaline phosphatase to evaluate liver toxicity.Results: Based on the results obtained, serum SGPT levels in vitamin D treatment groups did not show a significant decrease compared to cadmium groups (p >0.001). The ALP biomarker in the groups treated with vitamin D was decreased significant in comparison to untreated model groups (p<0.001). While any significant differences were not observed between the Cd model and the Cd model treated with Vitamin D.Conclusion: This study proved that administration vitamin D to some extent reduces the toxic effects of cadmium on the liver and lung.Keywords: Ameliorated; Vitamin D; Protein expression; Lung; Cadmiu

    Comparison of Efficacy and Complication of Alteplase Injection in Acute Ischemic Stroke

    Background and Aim: Alteplase is a thrombolytic drug that is produced by recombinant DNA technology. Tissue plasminogen activator enzyme which converts plasminogen to the active form of plasmin is also produced by the same technology; it causes fibrinolysis and clot dissolution. This study aimed to compare the efficacy and complications of Alteplase injection in patients with acute ischemic stroke (AIS( during the first 3 hours and&nbsp; 3-4.5 hours after the onset of symptoms. Methods: In this study, patients with AIS who were referred to Golestan Hospital of Ahvaz city during 2018-2019 were selected. Information was collected by a checklist. Results: The results showed that the mean Modified Rankin Scale&nbsp;(mRS) for 3 months and 6 months (p-value: 0.91 for 3&nbsp;months and p-value: 0.80 for 6&nbsp;months) and National Institutes of Health Stroke Scale (NIHSS) (p-value: 0.21) were not significantly different between both groups; statistically, no significant relationship was observed between them. The incidence of complications after treatment was almost similar, in both groups.     Accelerated versus Conventional Corneal Collagen Cross-Linking for Progressive Keratoconus

    We aimed to compare the effect of accelerated and conventional corneal collagen cross-linking (CXL) on visual, refractive, and topographic parameters in patients with progressive keratoconus. Between December 2014 and February 2016, at Imam Khomeini Hospital, Ahvaz Jundishapur University of Medical Sciences, Iran, we compared 37 eyes of 21 patients treated by conventional CXL (CCXL; 3 mW/cm2 in 30 minutes) with 34 eyes of 18 patients treated by accelerated CXL (ACXL; 18 mW/cm2 in 5 minutes) based on generalizing estimation equation analysis in terms of corrected distance visual acuity (CDVA), uncorrected distance visual acuity (UDVA), corneal endothelial cell indices, and topographic parameters before and at 3, 6 and 12 months after the operation. The mean UDVA and spherical equivalent changes were similar in the two groups, but an improvement in CDVA was only observed in the CCXL group (P = 0.003). Keratometry (minimum and maximum) was significantly decreased in the CCXL group (P = 0.043 and P = 0.008, respectively). Indices of keratoconus progression—surface asymmetry index (SAI), keratoconus prediction index (KPI), and keratoconus index (KCI)—were significantly lower in the CCXL group than in the ACXL group (P = 0.002, P &lt; 0.001, and P &lt; 0.001, respectively). The thinnest corneal thickness (TCT) was not significantly different between the two groups (P = 0.15). The reduction of corneal endothelial cell density was also similar between the two groups; however, polymorphism and polymegethism were significantly lower in the ACXL group than in the CCXL group. In conclusion, we showed that although ACXL at 18 mW/cm2 slowed keratoconus progression safely during a 1-year follow-up, CCXL at 3 mW/cm2 may be superior in the prevention of keratoconus progression

    Low presence of papillomavirus and its lack of correlation with clinicopathological factors in breast cancer: a case control study

    Background and Objectives: Breast cancer is currently the most commonly diagnosed neoplasm in women worldwide. There is evidence that human papillomavirus (HPV) infection may play a key role in breast cancer aggressiveness, but results are conflicting across studies. The aim of this study was to investigate the presence of the HPV viral genome in benign and malignant breast tissue samples and its clinicopathological characteristics of cancer. Materials and Methods: In this case-control study, 100 formalin-fixed paraffin-embedded (FFPE) of breast cancer and 100 blocks of non-cancerous breast tissue were selected as a control group from the pathology department of Imam Khomeini Hospital in Ahvaz from 2020-2022. The presence of HPV was detected using nested PCR including MY09/11 primers and sequencing were performed for virus genotyping. Results: The present study enrolled 100 subjects each in two cancer and control groups with a mean age of 52.81±13.23 and 35.77±11.65, respectively. The risk of cancer in HPV-infected patients is almost 5 times higher than in HPV-negative individuals, it is not statistically significant (OR =4.99, 95% CI 0.35 to 72.15, p=0.238). The prevalence of HPV in the cancer and control groups was 7% and 1%, respectively and HPVs detected in two groups were of the HPV 16 genotype. Although the chance of ER and PR expression, lymphvascular involvement, perineural invasion, and higher tumor grade was higher in HPV-positive subjects than in HPV-negative subjects, this was not statistically significant (OR>1, p>0.05). Conclusion: Based on studies reporting the existence of sequences of different high-risk HPV types (oncogenes) in breast cancer tissues, this study confirmed the hypothesis of a possible infectious cause in the development of breast cancer. So far, however, the results have been controversial and inconclusive. Further studies with large sample sizes are needed to demonstrate the link between HPV and breast cancer

    The study of annual cycle of gonads developments and sex steroids of gray mullet, Mugil cephalus in Golestan Province

    Grey mullet, Mugil cephalus is one of the most famous and valuable fish on the fisheries and aquaculture industry. This species is the most suitable to introducing the southern, northern and central regions of our country due to high resistant to large variation of water salinity and temperature. On the monthly sampling period biometric data including total length and total weight recorded and length and weight classes for every sex were determined. To understanding of physiological functions and sexual development of reproductive organs, this investigation carried out with Grey mullet imported from Hong Kong by 1993. Findings of this research can be used for management of artificial breeding of this species. Daily variation of pond water temperature was recorded. Sex steroid hormones and other plasma metabolites such as Ca^2+, cholesterol, triglycerides and total proteins were measured by blood sampling. Gonad samples were prepared in order to histological studies. Oocyte growth was studied by measurement of egg diameters. Measured values of cholesterol, triglycerides of blood plasma revealed that these metabolites have seasonal variation and severely deposited on the summer and mobilized on the spawning seasons. Findings of this research suggested that egg diameters, values of sex steroid hormones (estradiol and progesterone) and Calcium concentrations of fish blood plasma shortening increased a long with day length shortening
