    Preparation of cyclodextrin nanoparticles and evaluation of its effect on the capacitation of bovine spermatozoa used in the in vitro fertilization

    This study was conducted to produce nanosized cyclodextrin (NCD) and assess its effect on bovine spermatozoa during In vitro fertilization (IVF) to optimize the capacitation media for successful IVF. Therefore, Four cyclodextrin formulations were prepared and characterized. Data analysis revealed the best formula (F2) showed a smallest particle size (15 nm), zeta potential (-37 mv), and higher yield percentages (95%) was selected for spem capacitation. Motile spermatozoa were separated from frozen-thawed semen by a swim-up procedure and capacitated in IVF-TALP medium with different formulae of NCD or CD or without treatments (control) and incubated for 3hours(hr) at 38°C and evaluated every one (hr) interval. Data analysis revealed that the formulation of cyclodextrin nanoparticles (F2) after (2hr) incubation in the media gave best effect on sperm capacitation and acrosme reaction (AR) and effect of sperm treated with NCD on fertilization rate was evaluated. The results showed that the proportion of Oocytes fertilized was increased significantly in F2 (60%) than in the control (35%), and cyclodextrin group (50%) groups (p<0.05). It could be inferred from this investigation that cyclodextrin nanoparticles can be used for biomedical interventions in bovine spermatozoa. NCD improve sperm motility, viability, and (AR), also fertilization rate of sperm treated with NCD increase. So NCD gave positive effect on sperm functions during IVF.

    Preoperative unrestricted clear fluid and association with postoperative nausea and vomiting: Review article

    Background: One of the most distressing side effects of general anaesthesia is postoperative nausea and vomiting (PONV). Recovery room and hospital stay and overall health care costs can be increased by PONV. Reduced frequency of PONV is still significant even after the introduction of shorter surgical procedures, shorter acting anesthesias and newer anti-emetic medicines; many anti-emetic treatments are expensive and do not totally eliminate PONV with some adverse effects; yet. As a precaution against regurgitation and aspiration of stomach contents, pre-operative fasting reduces stomach volume and acidity. The current standards in Europe, on the other hand, advocate the consumption of oral clear fluids up to two hours prior to the onset of general anaesthetic induction. In order to limit the danger of regurgitation and aspiration, patients are advised to fast prior to general anaesthesia.Objective: Review of free oral consumption of clear fluids before the anaesthesia and their effects on nausea and vomiting beyond surgery.Methods: The databases were searched for articles published in English in 4 data bases [PubMed – Medline - Google scholar- science direct] and Boolean operators (and, or, not) had been used such as [Preoperative Clear Fluid, Postoperative Nausea and Vomiting] and in peer-reviewed articles between 2001 and 2021.Conclusion: The incidence of postoperative nausea and vomiting was lowered when patients drank freely clear fluids prior to the scheduled induction of day case anaesthesia

    Effect of Low-Level Laser Therapy in Treatment of Chemotherapy Induced Oral Mucositis

    Introduction: Oral mucositis (OM) is an unavoidable condition of the oral cavity that accompanies chemotherapy for various malignant cases. Chemotherapy-induced oral Mucositis (COM) is a frequent complication due to mucotoxic drugs and is known to deteriorate the general health significantly, while negatively affecting the quality of life (QOL). Studies have reported that low-level laser therapy (LLLT) promotes the tissue healing. The objective of the present study was to explore the efficacy of gallium-arsenide (GaAs) laser in treating COM and its impact on inflammatory cytokine levels in patients receiving chemotherapy for various malignancies.Methods: A total of 80 patients with COM received LLLT 6 days/week. OM was graded according to the World Health Organization (WHO) grading scale. The outcome parameters were the serum levels of tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6) measured before, during and after administration of LLLT.Results: After LLLT, a significant decrease was found in the mean values of mucositis grade from 2.35 ± 0.695 to 1.13 ± 0.333 after (P < 0.001). A significant reduction in the level of TNF-α was found after LLLT among breast cancer patients (P = 0.0045), but not in head and neck cancer and lymphoma patients. A significant reduction was also found in IL-6 level after treatment among head and neck and breast cancer patients (P = 0.0307 and 0.019, respectively).Conclusion: The use of GaAs LLLT in treating COM in patients with various malignancies is well tolerated by patients, it results in improvement of mucositis, however; mechanism of action does not seem to be completely linked to the change of pro or anti-inflammatory cytokines

    Effectiveness of Structured Nursing Teaching Program on Outcomes of Chronic Low Back Pain Patients Undergoing Radiofrequency Ablation

    Background: Chronic low back pain is a full disorder that considers an important health problem. Aim: To evaluate the effectiveness of structured teaching program on outcomes of chronic low back pain patients undergoing radiofrequency ablation. Methods: A quasi-experimental research design has been utilized. Setting: Anesthesia and pain management department at the main Assiut University Hospital. Sample: Sixty low back pain patients undergoing radiofrequency ablation. They were randomly assigned in two groups of 30 patients each. Tools: three tools were utilized: Tool I: Structured interview patients' questionnaire. Tool II: Visual analogue pain scale and Tool III: "Modified Oswestry low back pain disability questionnaire" in addition, structured nursing teaching program. Results: More than half (53.3%) of the case group their age was from 18 to < 55 years, and the rest of them (46.7%) were older adults aged 55 years and more. 80% of the case group had good knowledge post implementation of the program. (100%) of the case group had worst pain intensity level which decrease to (46.7%) post program. 40% of case group had sever disability, which decreased to 30% post implementation of the program. there were significant relation between pain analogue scale and Oswestry low back pain disability scale in pre and post structured nursing teaching program regarding to occupation Conclusion: Structured nursing teaching program had an effective strategy to improve the patients’ outcomes through increasing patients' knowledge, and decreasing both pain intensity and physical disability in the case group versus the control group. Recommendation: Supervised structured teaching program should be carried out for all patients undergoing radiofrequency ablation about pre-and post-procedure instructions and helpful hints for a healthy back when standing, walking, sitting, sleeping, and lifting.     Preparation of progesterone nanoparticles and evaluation of its effect on the capacitation of Bovine spermatozoa used in the in Vitro Fertilization

    Progesterone (P) has been reported to affect several sperm functions especially capacitation and acrosome reaction. The main problem of (P) is its low aqueous solubility. So formulation of progesterone nanoparticles (PN) will enhance its solubility. This study was conducted to produce nanosized progesterone (NP) and assess its biocompatibility. Therefore, nine progesterone formulations were prepared and characterized. Data analysis revealed only one formula of P showed nanosized particle (1-100 nm) with an average particle size (95±5 nm), and spherical shape as seen by Transmission Electron Microscope(TEM). Motile spermatozoa were separated from frozen-thawed semen by a swim-up procedure and capacitated in IVF-TALP medium with NP or P or without treatments (control) and incubated for 3h at 38°C and evaluated every 1 hour (h) interval. Ovarian oocytes were matured and fertilized in vitro with frozen-thawed bull sperm capacitated in vitro with NP or P or control (without NP, P) and incubated at 39C in 5% CO2 incubator for 24h and then examined for evidence of fertilization. In conclusion, this study demonstrates that nanosized progesterone is highly efficient for sperm capacitation. In addition to the use of nanosized progesterone in sperm capacitation produces more fertilized oocytes than the progesterone after In Vitro Fertilization (IVF)

    A Rare Association Between Leukocyte Adhesion Deficiency Type I and Psoriasis in Humans

    The β2 integrins are expressed exclusively on leukocytes and participate in many immune and inflammatory processes. This subfamily comprises four heterodimeric glycoproteins with a common β-subunit, designated β2 (CD18). Spontaneous mutations of the CD18 gene result in leukocyte adhesion deficiency type I (LAD-I). Low level of CD18 expression has also been implicated in the pathogenesis of psoriasis. We here describe a child with recurrent skin infections without pus formation, persistent gingivitis and periodontitis. His blood counts showed persistent leukocytosis (neutrophilia). CD11b expression was defective on neutrophils, while that of CD18 was normal. So, our patient represents a mild variant of LAD-I with possible dysfunctional CD18. Moreover, he developed psoriasis with reduced CD18 expression on CD4+ T-cells. Psoriasiform dermatitis has been described before in association with LAD-I, however, clinically and histologically confirmed psoriasis in association with LAD-I has been described only in CD18 hypomorphic mice. Therefore, our patient represents the first clinically and histopathologically documented association between LAD-I and psoriasis in humans. It lends support to the role of β2 integrins in the etiopathogenesis of psoriasis
