    To Study Correlation of Body Fat and Blood Lipids with Autonomic Nervous System Activity in Postmenopausal Indian Women

    Background: Obesity, physical inactivity, and altered estrogen metabolism play integrated role contributing to the disease risk profiles of postmenopausal women (PMW). Aim: To study the correlation of body fat and blood lipids with autonomic nervous system activity in PM Indian women. Subjects and Methods: A total of 60 PM sedentary women without any gross systemic disease were selected as a control group and 23 PMW, who were regularly doing morning walk for 1‑year was  selected as a study group. Body mass index (BMI), waist/hip (W/H) ratio were recorded; lipid profile analyzed, and conventional autonomic function tests were carried. Results were analyzed using SPSS version 16. Results: There was a significant difference in BMI, W/H ratio, lipid profile, autonomic function tests between the two groups. Aging and menopausal duration (MD) were positively correlated with pulse rate, systolic blood pressure, total cholesterol (TC), low‑density lipoprotein (LDL) cholesterol, orthostatic tolerance test (OTT), and negatively correlated with Valsalva ratio (VR), deep breath test (DBT), 30:15 relative risk (RR) intervals ratio, isometric hand‑grip test (HGT), cold pressor test (CPT). BMI and W/H ratio were positively correlated with TC, triglyceride, LDL cholesterol, OTT, and negatively correlated with isometric HGT, CPT. TC and LDL were negatively correlated with VR, deep breath difference test, 30:15 RR interval ratios, isometric HGT, CPT, and positively correlated with OTT. Multiple regression analysis revealed LDL cholesterol as the only significant independent explanatory variable of DBT. LDL cholesterol and age were the significant independent explanatory variables of  30:15 RR interval ratios, CPT. TC, LDL cholesterol, and age were the significant independent  explanatory variables of isometric HGT.Conclusions: Aging, MD, BMI, and W/H ratio all contribute significantly to worsening of autonomic functions and lipid profile. Multiple regression analysis revealed LDL cholesterol as the most significant contributor of worsening of autonomic functions in PM females and regular exercise can improve theseparameters significantly.KEY WORDS: Autonomic functions, blood lipids, body fat, menopaus

    Arsha (Hemorrhoids) is one of the most common conditions affecting peoples since time immemorial. The prevalence is equal in both sexes, peaks between the ages of 45 and 65 and declining thereafter. The main cause of hemorrhoids is constipation over a prolonged period of time and has to strain to pass stools, it can damage the lining of anal canal. Sushruta the Father of Surgery advocated four method of treatment of Arsha, which are Bheshaja, Kshara, Agni and Shastra. Under Sushrutas operative procedure local application of Pratisharaniya Kshara in the prolapse piles mass helps to reduce pile mass effectively (Ref.Su.Chi.6). It is safe, cheap, effective, simple, easily approachable procedure and ambulatory treatment and the patients acceptability is good to overcome such problem. Aims & Objective is to evaluate the clinical efficacy of Apamarga Pratisharaniya teekshna kshara in the management of Arsha. & to evaluate the safety of Apamarga Pratisharaniya teekshna kshara in the management of Arsha. Material and Methods: 30 patients were enrolled for the study within a period of one year and Teekshna pratisharaniya kshara was applied on piles mass intra anally under operative procedure with the help of a instrument proctoscope. Observation and result: Reduction in the piles mass evident by Inspection/ proctoscopy /Visual perception was observed remarkably.Bleeding and pain were also reduced remarkably in all the piles patient. 100% relief in bleeding, reduction of pile mass and total reduction in size of pile mass was recorded. Conclusion: Recently these Oldest technique advocated by Sushruta have been proven to be most effective and free from post-operative complications

    Isolation of antibacterial pentahydroxy flavones from the seeds of Mimusops elengi Linn

    In order to search for antimicrobial phytochemicals, two antibacterial compounds were detected from the seeds of Mimusops elengi Linn. The compounds were extracted by ethyl acetate and purified bycolumn chromatography. Their structures were elucidated through IR, 1H NMR and GC-EL-MS spectral analyses. After characterization, the compounds were identified as 2,3-dihyro-3,3’4’5,7- pentahydroxyflavone and 3,3’,4’,5,7-pentahydroxyflavone. The compounds showed strong inhibitory activity against Gram positive and Gram negative bacteria

    Effect of Progressive Muscle Relaxation on the Adverse Cardiovascular Profile in Women with Polycystic Ovarian Syndrome

    Background: The altered stress reactivity in polycystic ovarian syndrome (PCOS) may constitute a link between depression, overweight, and cardiovascular risk factors. Aims: To study the effects of progressive muscle relaxation (PMR) on the stress levels of PCOS patients and their influence on the cardiovascular risk factors. Subjects and Methods: This prospective pilot project was conducted on 100 PCOS patients in a tertiary care hospital of West Bengal, after receiving approval from the Institutional Ethical Committee and informed consent of the subjects. The stress levels were assessed and conventional autonomic function tests and the lipid profi les were analyzed. The subjects were divided into two groups using an online randomizer. One group received medication, while the other group received medication and practiced progressive muscle relaxation (PMR) for three months. All parameters were re‑evaluated at the end of three months. Results: The perceived stress scale was significantly less in subjects practicing relaxation exercises, as compared to subjects only on medication. The waist/hip ratio, pulse rate, and systolic blood pressure were significantly lower, while there was no difference in the body mass index (BMI) and diastolic blood pressure. Results of the autonomic function tests showed a significant parasympathetic tilt in subjects practicing PMR. In patients with PCOS, who were on PMR, cholesterol, triglycerides, and low‑density cholesterol (LDL) were significantly lower and high‑density cholesterol HDL was significantly higher. Conclusions: PCOS patients are a high‑risk group for developing the metabolic syndrome and relaxation therapies may be recommended as an adjuvant therapy, to tilt the autonomic balance to parasympathetic dominance, to improve the cardiovascular profile.Keywords: Autonomic functions, cardiovascular system, polycystic ovarian syndrome, stres

    Cervical Cancer in Women with Unhealthy Cervix in a Rural Population of a Developing Country

    Background: Cervical cancer, the most common malignancy among Indian women, is the second most common and fi fth most fatal cancer in women world-wide.Aim: The study is aimed to determine the risk factors, incidence of cervical malignancy in women with grossly unhealthy cervix in a rural population and to get an overview of eff ectiveness of the existing screening programs.Subjects and Methods: This cross-sectional prospective pilot study was carried in a tertiary care hospital in a span of 6 months. A total of 300 females with grossly unhealthy cervix with suspicions of malignancy, who had never undergone cervical cytology or any other cervical neoplasia screening procedure, were included. Unaided visual inspection with Cusco’s speculum was performed followed by digital examination. Clinical staging was carried out in patients according to International Federation of Gynecology and Obstetrics (FIGO’S) Classifi cation. Cervical punch biopsy was taken under colposcopic guidance and histopathological examinations were done. Data were analyzed using SPSS, version 15.0 (Chicago Illinois, USA) and presented as simple percentages.Results: Among 300 females, 63.4% (190/300) were aged between 40 and 59 years. Nearly, 70.7% were illiterate and 52.6% had monthly family income between Rs. 2,000 and 5,000. Majority was married and 72.7% had parity between 1 and 3 and 58.7% had early marriages. Unaided visual examination of the women showed 62.7% of them had visible growth and 48.7% of them had bleeding erosions. Visible growths along with bleeding erosions were present in 11.3% cases. Histopathological examination of cervical biopsy specimens revealed mild, moderate and severe dysplasia in 14, 22 and 36 cases, respectively. A total of 212 patients had invasive squamous cell carcinoma. Only 16 patients had normal histopathology fi ndings. Nearly, 56.61% had Stage II disease; among them 27 had Stage IIa and 33 had Stage IIb disease, 26 patients had Stage I disease. Stage IIIa and IIIb have been found in 50 and 12 cases respectively. Four cases had cancer extending to urinary bladder and rectum (Stage IVa). Conclusions: Cervical carcinoma not only has a biomedical spectrum, but also has a wide cultural and socio-economic background. Extensive screening campaigns needs to be implemented with immediate eff ect to early diagnose cases to decrease the social burden of the disease.  Keywords: Africa, cervical cancer, screening procedure

    Septic Abortion Managed in a Tertiary Hospital in West Bengal

    Background: Septic abortion is still a challenging problem and a major cause of maternal mortality and morbidity in developing countries.Aim: The present study was conducted to estimate incidence, causes, risk factors, associated microorganisms, and treatment modalities of cases of septic abortion managed in a tertiary health care in West Bengal.Subjects and Methods: It was a cross‑sectional study carried out in Burdwan Medical College and Hospital in West Bengal between July 2011 and June 2012. On admission a thorough history was taken, examination was done, and investigations were carried. Depending on the examination findings the cases were grouped into three grades‑grade I: The infection localized in the uterus; grade II: The infection spread beyond the uterus to the parametrium, tubes, and ovaries or pelvic peritoneum; grade III: Generalized peritonitis and/or endotoxic shock or jaundice or acute renal failure. Statistical analysis was done by using Epi Info™ software (Version 3.5.1, CDC) after proper arrangement of all the data in tabular form and presented as simple percentage.Results: During the period, 1297 cases of abortion were admitted among which 107 were septic abortions, thus giving the incidence of the latter as 8.2% (107/1297). Forty‑three percent cases of septic abortion (46/107) were in grade I, 21.5% (23/107) in grade II, and 35.5% (38/107) in grade III. The modal age group of the patients was 15‑25 years (49.5%). Majority of the subjects were married (77.6%; 83/107) and multiparous (60.7%; 65/107). Similarly, most of them (71%; 76/107) were from low socioeconomic class. Ninety percent of the patients (96/107) had induced abortion. The common clinical features at presentation were pallor (88.8%; 95/107) and fever (86.0%; 92/107). Escherichia coli (62.6%; 67/107) and Klebsiella pneumonia (32.75%; 35/107) were the most common organism isolated. The case fatality from septic abortion in this study was 13.1% and the condition accounted for 15.4% of total maternal mortality within the period reviewed.Conclusion: The incidence of septic abortion in this study is relatively high, and majority of the cases resulted from attempted termination of pregnancy. It is a significant contributor to maternal mortality. Promotion of family planning and legalization of abortion services will go a long way in reducing the incidence of septic abortion and itsassociated complications. Keywords: Asia, management, maternal mortality, outcome, septic abortio

    A Prospective Study of Doppler Velocimetry in Pregnancy-induced Hypertension in a Rural Population of a Developing Country

    Background: Pregnancy-induced hypertension (PIH) remains a great challenge to obstetricians. Doppler velocimetry can detect fetal compromise much before other antepartum tests.Aim: The aim of this study is to detect the changes of uterine artery, umbilical artery and middle cerebral artery in PIH by Doppler velocimetry.Subjects and Methods: This prospective study was conducted on hundred subjects with PIH. Doppler studies were carried, and parameters recorded in uterine, umbilical and middle cerebral artery (MCA) were Systolic/Diastolic ratio, Resistance Index, Cerebro Placental Index (CPI). Fetal outcomes were monitored. Statistical analysis was performed using Epi InfoTM software (Version 3.5.1, CDC, Atlanta). Test for significance was done with student’s t-test and Chi-square where applicable. A P- value of<0.05 was considered as significant.Results: Among the 100 subjects, 76 (76%) of fetuses had abnormal and 24% normal umbilical artery Doppler velocimetry; 62% had abnormal and 38% normal MCA Doppler velocimetry; 64% fetuses had abnormal and 36% normal CPI. In 95% of subjects having abnormal umbilical Doppler studies, caesarean section had to be done for acute fetal distress. Incidence of caesarean section was 61% in abnormal MCA group and 63% in abnormal CPI group. Among 14 patients who had abnormal uterine artery Doppler, four developed pre-eclampsia, 2 IUGR. In patients with notches in uterine artery Doppler, 38% developed pre-eclampsia, 38% had IUGR, 13% babies were still born and 25% of newborns required NICU admission. In umbilical artery Doppler, when S/D ratio was abnormal, 60% developed pre-eclampsia, 40% had IUGR and 40% of newborns had to be admitted in NICU.Conclusion: Doppler study for fetal surveillance in pregnancy-induced hypertension is a very useful device and abnormal umbilical artery and uterine artery velocimetry seems to have worse pregnancy outcomes in the present study. Notch as a single parameter is the best indicator with highest sensitivity and positive predicative values.     Giant cells: multiple cells unite to survive

    Multinucleated Giant Cells (MGCs) are specialized cells that develop from the fusion of multiple cells, and their presence is commonly observed in human cells during various infections. However, MGC formation is not restricted to infections alone but can also occur through different mechanisms, such as endoreplication and abortive cell cycle. These processes lead to the formation of polyploid cells, eventually resulting in the formation of MGCs. In Entamoeba, a protozoan parasite that causes amoebic dysentery and liver abscesses in humans, the formation of MGCs is a unique phenomenon and not been reported in any other protozoa. This organism is exposed to various hostile environmental conditions, including changes in temperature, pH, and nutrient availability, which can lead to stress and damage to its cells. The formation of MGCs in Entamoeba is thought to be a survival strategy to cope with these adverse conditions. This organism forms MGCs through cell aggregation and fusion in response to osmotic and heat stress. The MGCs in Entamoeba are thought to have increased resistance to various stresses and can survive longer than normal cells under adverse conditions. This increased survival could be due to the presence of multiple nuclei, which could provide redundancy in case of DNA damage or mutations. Additionally, MGCs may play a role in the virulence of Entamoeba as they are found in the inflammatory foci of amoebic liver abscesses and other infections caused by Entamoeba. The presence of MGCs in these infections suggests that they may contribute to the pathogenesis of the disease. Overall, this article offers valuable insights into the intriguing phenomenon of MGC formation in Entamoeba. By unraveling the mechanisms behind this process and examining its implications, researchers can gain a deeper understanding of the complex biology of Entamoeba and potentially identify new targets for therapeutic interventions. The study of MGCs in Entamoeba serves as a gateway to exploring the broader field of cell fusion in various organisms, providing a foundation for future investigations into related cellular processes and their significance in health and disease

    Oxidative DNA damage preventive activity and antioxidant potential of plants used in Unani system of medicine

    <p>Abstract</p> <p>Background</p> <p>There is increasing recognition that many of today's diseases are due to the "oxidative stress" that results from an imbalance between the formation and neutralization of reactive molecules such as reactive oxygen species (ROS) and reactive nitrogen species (RNS), which can be removed with antioxidants. The main objective of the present study was to evaluate the antioxidant activity of plants routinely used in the Unani system of medicine. Several plants were screened for radical scavenging activity, and the ten that showed promising results were selected for further evaluation.</p> <p>Methods</p> <p>Methanol (50%) extracts were prepared from ten Unani plants, namely <it>Cleome icosandra, Rosa damascena, Cyperus scariosus, Gardenia gummifera, Abies pindrow, Valeriana wallichii, Holarrhena antidysenterica, Anacyclus pyrethrum, Asphodelus tenuifolius </it>and <it>Cyperus scariosus</it>, and were used to determine their total phenolic, flavonoid and ascorbic acid contents, in vitro scavenging of DPPH<sup>·</sup>, ABTS<sup>·+</sup>, NO, <sup>·</sup>OH, O<sub>2</sub><sup>.- </sup>and ONOO<sup>-</sup>, and capacity to prevent oxidative DNA damage. Cytotoxic activity was also determined against the U937 cell line.</p> <p>Results</p> <p>IC<sub>50 </sub>values for scavenging DPPH<sup>·</sup>, ABTS<sup>·+</sup>, NO, <sup>·</sup>OH, O<sub>2</sub><sup>.- </sup>and ONOO<sup>- </sup>were in the ranges 0.007 ± 0.0001 - 2.006 ± 0.002 mg/ml, 2.54 ± 0.04 - 156.94 ± 5.28 μg/ml, 152.23 ± 3.51 - 286.59 ± 3.89 μg/ml, 18.23 ± 0.03 - 50.13 ± 0.04 μg/ml, 28.85 ± 0.23 - 537.87 ± 93 μg/ml and 0.532 ± 0.015 - 3.39 ± 0.032 mg/ml, respectively. The total phenolic, flavonoid and ascorbic acid contents were in the ranges 62.89 ± 0.43 - 166.13 ± 0.56 mg gallic acid equivalent (GAE)/g extract, 38.89 ± 0.52 - 172.23 ± 0.08 mg quercetin equivalent (QEE)/g extract and 0.14 ± 0.09 - 0.98 ± 0.21 mg AA/g extract. The activities of the different plant extracts against oxidative DNA damage were in the range 0.13-1.60 μg/ml. Of the ten selected plant extracts studied here, seven - <it>C. icosandra, R. damascena, C. scariosus, G. gummifera, A. pindrow, V. wallichii </it>and <it>H. antidysenterica - </it>showed moderate antioxidant activity. Finally, potentially significant oxidative DNA damage preventive activity and antioxidant activity were noted in three plant extracts: <it>C. icosandra, R. damascena </it>and <it>C. scariosus</it>. These three plant extracts showed no cytotoxic activity against U937 cells.</p> <p>Conclusions</p> <p>The 50% methanolic extracts obtained from different plant parts contained significant amounts of polyphenols with superior antioxidant activity as evidenced by the scavenging of DPPH<sup>·</sup>, ABTS<sup>·+</sup>, NO, <sup>·</sup>OH, O<sub>2</sub><sup>.- </sup>and ONOO<sup>-</sup>. <it>C. icosandra, R. damascena </it>and <it>C. scariosus </it>showed significant potential for preventing oxidative DNA damage and radical scavenging activity, and the <it>G. gummifera, A. pindrow, V. wallichii, H. antidysenterica, A. pyrethrum, A. tenuifolius </it>and <it>O. mascula </it>extracts showed moderate activity. The extracts of <it>C. icosandra, R. damascena </it>and <it>C. scariosus </it>showed no cytotoxicity against U937 cells. In conclusion, these routinely used Unani plants, especially <it>C. icosandra, R. damascena </it>and <it>C. scariosus</it>, which are reported to have significant activity against several human ailments, could be exploited as potential sources of natural antioxidants for plant-based pharmaceutical industries.</p
