Status of sonic boom methodology and understanding
In January 1988, approximately 60 representatives of industry, academia, government, and the military gathered at NASA-Langley for a 2 day workshop on the state-of-the-art of sonic boom physics, methodology, and understanding. The purpose of the workshop was to assess the sonic boom area, to determine areas where additional sonic boom research is needed, and to establish some strategies and priorities in this sonic boom research. Attendees included many internationally recognized sonic boom experts who had been very active in the Supersonic Transport (SST) and Supersonic Cruise Aircraft Research Programs of the 60's and 70's. Summaries of the assessed state-of-the-art and the research needs in theory, minimization, atmospheric effects during propagation, and human response are given
Assessment of Stream Restoration Structures in Streams of North-Central Pennsylvania
Interest in stream restoration has increased over the last two decades, leading to a growth in the industry to the point that it has become a 1 billion dollar per year enterprise (Bernhart et al 2005, Thompson 2002). In north-central Pennsylvania alone, over $9 million has been spent since 1999 on stream restoration projects, from the designing stage to actual construction and reconstruction of the sites. Even though a extensive amount of money is being spent on the construction of these projects, very little to no post-monitoring is taking place. Without post-monitoring of the projects, it is unknown if they actually work.
After a successful statewide stream restoration assessment was completed in North Carolina by the advisor of this project, it was discussed to undertake a similar one for the state of Pennsylvania starting with the north-central region. This includes Bradford, Cambria, Cameron, Centre, Columbia, Lycoming, McKean, Montour, Northumberland, Potter, Sullivan, Tioga, and Union Counties. Within these 13 counties over 60 restoration projects have been implemented since the 1990s ranging in type from FGM structures to Fish and Boat Commission habitat structures. Twenty-two of the restoration sites comprising of 58,255 feet of restoration work were selected for individual site assessment during March 2008-May 2009. Over 300 structures were assessed during this period of time for structural integrity and the degree to which the adjacent bed and banks had been affected by unintended erosion or deposition.
Approximately 75% of the structures have sustained some structural damage (ranking \u3e1) or erosion or deposition (ranking \u3e1). Thirty-five percent of the structures have sustained significant damage (ranking \u3e2) or significant erosion or deposition (ranking \u3e3). Most of the damage (63% of all structures) is related to erosion or deposition which can impact the functionality of the structure. Many of the streams in north-central Pennsylvania are experiencing pulses of aggradation of gravel-cobble size clasts throughout the stream system. This pulse of gravel is contributing to the partial burial of individual structures and may fill in the pool that was created by the structure. Out of the four highly used structure types (cross vanes, j-hooks, log vanes, and rock vanes), j-hooks and rock vanes sustained the highest percentage of damage compared to the other two structures. Much more work needs to be completed in this region before we can fully grasp an understanding for failure of the various structures
The in vivo efficacy of two administration routes of a phage cocktail to reduce numbers of Campylobacter coli and Campylobacter jejuni in chickens
<p>Abstract</p> <p>Background</p> <p>Poultry meat is one of the most important sources of human campylobacteriosis, an acute bacterial enteritis which is a major problem worldwide. <it>Campylobacter </it><it>coli </it>and <it>Campylobacter </it><it>jejuni </it>are the most common <it>Campylobacter </it>species associated with this disease. These pathogens live in the intestinal tract of most avian species and under commercial conditions they spread rapidly to infect a high proportion of the flock, which makes their treatment and prevention very difficult. Bacteriophages (phages) are naturally occurring predators of bacteria with high specificity and also the capacity to evolve to overcome bacterial resistance. Therefore phage therapy is a promising alternative to antibiotics in animal production. This study tested the efficacy of a phage cocktail composed of three phages for the control of poultry infected with <it>C. coli </it>and <it>C. jejuni</it>. Moreover, it evaluated the effectiveness of two routes of phage administration (by oral gavage and in feed) in order to provide additional information regarding their future use in a poultry unit.</p> <p>Results</p> <p>The results indicate that experimental colonisation of chicks was successful and that the birds showed no signs of disease even at the highest dose of <it>Campylobacter </it>administered. The phage cocktail was able to reduce the titre of both <it>C. coli </it>and <it>C. jejuni </it>in faeces by approximately 2 log<sub>10 </sub>cfu/g when administered by oral gavage and in feed. This reduction persisted throughout the experimental period and neither pathogen regained their former numbers. The reduction in <it>Campylobacter </it>titre was achieved earlier (2 days post-phage administration) when the phage cocktail was incorporated in the birds' feed. <it>Campylobacter </it>strains resistant to phage infection were recovered from phage-treated chickens at a frequency of 13%. These resistant phenotypes did not exhibit a reduced ability to colonize the chicken guts and did not revert to sensitive types.</p> <p>Conclusions</p> <p>Our findings provide further evidence of the efficacy of phage therapy for the control of <it>Campylobacter </it>in poultry. The broad host range of the novel phage cocktail enabled it to target both <it>C. jejuni </it>and <it>C. coli </it>strains. Moreover the reduction of <it>Campylobacter </it>by approximately 2 log<sub>10</sub>cfu/g, as occurred in our study, could lead to a 30-fold reduction in the incidence of campylobacteriosis associated with consumption of chicken meals (according to mathematical models). To our knowledge this is the first report of phage being administered in feed to <it>Campylobacter-</it>infected chicks and our results show that it lead to an earlier and more sustainable reduction of <it>Campylobacter </it>than administration by oral gavage. Therefore the present study is of extreme importance as it has shown that administering phages to poultry via the food could be successful on a commercial scale.</p
The Development of a Novel Interprofessional Education Curriculum for third year medical and pharmacy students
Abstract
Introduction:
The Liaison Committee on Medical Education and the Accreditation Council for Pharmacy Education, agencies responsible for the accreditation of medical and pharmacy schools respectively, require interprofessional education (IPE) to be integrated into both curricula. Institutions are given the autonomy to design and implement this requirement, however research is equivocal in regards to when and how best to implement IPE. The development of a new IPE curriculum is often met with a number of challenges, such as a lack of faculty support and resources.
Methods:
This study describes a newly created pilot IPE curriculum developed with minimal existing organizational IPE structure and resources, led by faculty champions from two complementary healthcare professions, Internal Medicine and Pharmacy. The validated 10-item Student Perceptions of Interprofessional Clinical Education- Revised (SPICE-R) instrument was used to assess the medical and pharmacy students’ attitudes towards interprofessional healthcare teams and the team approach to patient care.
Results:
Overall, students demonstrated a statistically significant increase in their perception of interprofessional healthcare teams and team approach to patient care.
Conclusion:
Prior to this IPE curriculum no formal IPE curriculum existed in this setting. This IPE curriculum was successfully implemented with minimal existing resources, the use of faculty champions and student’s perception of IPE improved using the validated SPICE-R instrument. IPE curriculum integration at our institution is in various stages of development. As IPE integration moves forward this pilot can serve as one example of how IPE could be implemented
NRG Oncology-Radiation Therapy Oncology Group Study 1014: 1-Year Toxicity Report From a Phase 2 Study of Repeat Breast-Preserving Surgery and 3-Dimensional Conformal Partial-Breast Reirradiation for In-Breast Recurrence.
PURPOSE: To determine the associated toxicity, tolerance, and safety of partial-breast reirradiation.
METHODS AND MATERIALS: Eligibility criteria included in-breast recurrence occurring \u3e1 year after whole-breast irradiation, \u3c3 \u3ecm, unifocal, and resected with negative margins. Partial-breast reirradiation was targeted to the surgical cavity plus 1.5 cm; a prescription dose of 45 Gy in 1.5 Gy twice daily for 30 treatments was used. The primary objective was to evaluate the rate of grade ≥3 treatment-related skin, fibrosis, and/or breast pain adverse events (AEs), occurring ≤1 year from re-treatment completion. A rate of ≥13% for these AEs in a cohort of 55 patients was determined to be unacceptable (86% power, 1-sided α = 0.07).
RESULTS: Between 2010 and 2013, 65 patients were accrued, and the first 55 eligible and with 1 year follow-up were analyzed. Median age was 68 years. Twenty-two patients had ductal carcinoma in situ, and 33 had invasive disease: 19 ≤1 cm, 13 \u3e1 to ≤2 cm, and 1 \u3e2 cm. All patients were clinically node negative. Systemic therapy was delivered in 51%. All treatment plans underwent quality review for contouring accuracy and dosimetric compliance. All treatment plans scored acceptable for tumor volume contouring and tumor volume dose-volume analysis. Only 4 (7%) scored unacceptable for organs at risk contouring and organs at risk dose-volume analysis. Treatment-related skin, fibrosis, and/or breast pain AEs were recorded as grade 1 in 64% and grade 2 in 7%, with only 1 (

A qualitative exploration of the effect of visual field loss on daily life in home-dwelling stroke survivors
Objective: To explore the effect of visual field loss on the daily life of community-dwelling stroke survivors. Design: A qualitative interview study. Participants: Adult stroke survivors with visual field loss of at least six months’ duration. Methods: Semi-structured interviews were conducted with a non-purposive sample of 12 stroke survivors in their own homes. These were recorded, transcribed verbatim and analyzed with the framework method, using an inductive approach. Results: Two key analytical themes emerged. ‘Perception, experience and knowledge’ describes participant’s conflicted experience of having knowledge of their impaired vision but lacking perception of that visual field loss and operating under the assumption that they were viewing an intact visual scene when engaged in activities. Inability to recognize and deal with visual difficulties, and experiencing the consequences, contributed to their fear and loss of self-confidence. ‘Avoidance and adaptation’ were two typologies of participant response to visual field loss. Initially, all participants consciously avoided activities. Some later adapted to vision loss using self-directed head and eye scanning techniques. Conclusions: Visual field loss has a marked impact on stroke survivors. Stroke survivors lack perception of their visual loss in everyday life, resulting in fear and loss of confidence. Activity avoidance is a common response, but in some, it is replaced by self-initiated adaptive techniques

Uba1 functions in Atg7- and Atg3-independent autophagy
Autophagy is a conserved process that delivers components of the cytoplasm to lysosomes for degradation. The E1 and E2 enzymes encoded by Atg7 and Atg3 are thought to be essential for autophagy involving the ubiquitin-like protein Atg8. Here, we describe an Atg7- and Atg3-independent autophagy pathway that facilitates programmed reduction of cell size during intestine cell death. Although multiple components of the core autophagy pathways, including Atg8, are required for autophagy and cells to shrink in the midgut of the intestine, loss of either Atg7 or Atg3 function does not influence these cellular processes. Rather, Uba1, the E1 used in ubiquitination, is required for autophagy and reduction of cell size. Our data reveal that distinct autophagy programs are used by different cells within an animal, and disclose an unappreciated role for ubiquitin activation in autophagy
Identification and manipulation of the pleuromutilin gene cluster from Clitopilus passeckerianus for increased rapid antibiotic production
Semi-synthetic derivatives of the tricyclic diterpene antibiotic pleuromutilin from the basidiomycete Clitopilus passeckerianus are important in combatting bacterial infections in human and veterinary medicine. These compounds belong to the only new class of antibiotics for human applications, with novel mode of action and lack of cross-resistance, representing a class with great potential. Basidiomycete fungi, being dikaryotic, are not generally amenable to strain improvement. We report identification of the seven-gene pleuromutilin gene cluster and verify that using various targeted approaches aimed at increasing antibiotic production in C. passeckerianus, no improvement in yield was achieved. The seven-gene pleuromutilin cluster was reconstructed within Aspergillus oryzae giving production of pleuromutilin in an ascomycete, with a significant increase (2106%) in production. This is the first gene cluster from a basidiomycete to be successfully expressed in an ascomycete, and paves the way for the exploitation of a metabolically rich but traditionally overlooked group of fungi

Hyperglycemia and Adverse Pregnancy Outcome (HAPO) study: common genetic variants in GCK and TCF7L2 are associated with fasting and postchallenge glucose levels in pregnancy and with the new consensus definition of gestational diabetes mellitus from the International Association of Diabetes and Pregnancy Study Groups.
OBJECTIVE: Common genetic variants in GCK and TCF7L2 are associated with higher fasting glucose and type 2 diabetes in nonpregnant populations. However, their associations with glucose levels from oral glucose tolerance tests (OGTTs) in pregnancy have not been assessed in a large sample. We hypothesized that these variants are associated with quantitative measures of glycemia in pregnancy. RESEARCH DESIGN AND METHODS: We analyzed the associations between variants rs1799884 (GCK) and rs7903146 (TCF7L2) and OGTT outcomes at 24-32 weeks' gestation in 3,811 mothers of European (U.K. and Australia) and 1,706 mothers of Asian (Thailand) ancestry from the HAPO cohort. We also tested associations with offspring birth anthropometrics. RESULTS: The maternal GCK variant was associated with higher fasting glucose in Europeans (P = 0.001) and Thais (P 0.05). In both populations, both variants were associated with higher odds of gestational diabetes mellitus according to the new International Association of Diabetes and Pregnancy Study Groups recommendations (P = 0.001-0.08). CONCLUSIONS: Maternal GCK and TCF7L2 variants are associated with glucose levels known to carry an increased risk of adverse pregnancy outcome in women without overt diabetes. Further studies will be important to determine the variance in maternal glucose explained by all known genetic variants
