    PENTOXIFYLLINE IN ACUTE ISCHEMIC STROKE PATIENTS WITH BLOOD HYPERVISCOSITY

    Objective: This study evaluated the efficacy of pentoxifylline, a hemorrheologic agent, for treating acute ischemic stroke in patients with bloodhyperviscosity.Methods: Patients were randomly allocated within 3 days of stroke onset to study or control treatment. All the patients received the standard acuteischemic stroke treatment. The study treatment was intravenous pentoxifylline 1200 mg/day for 5 consecutive days and oral pentoxifylline 800 mgin two divided doses for 23 days. Blood viscosity was evaluated on days 7 and 30. The outcomes were assessed by the National Institutes of HealthStroke Scale (NIHSS), modified Rankin Scale, and Barthel index (BI) criteria.Results: The median baseline viscosity was 6.89 poise in the study and 6.46 poise in the control groups and had decreased in all the patients ondays 7 and 30. The decrease in the study group on day 7 (1.13 poise) was greater than that in the control group (0.57 poise); the difference was notsignificant. The NIHSS score improved by 32% in the study group (relative risk [RR]=1.00, 95% confidence interval [CI]: 0.421–3.556; p=1.00). At1-month, 67% of the study group had a good functional outcome (RR=1.026, 95% CI: 0.656–1.605; p=0.909).Conclusion: Pentoxifylline decreased blood viscosity and improved clinical outcome in this patient series

    Evaluating Resistance to Acetyl Salicylic Acid Using Platelet Function Test in Patients with Ischemic Stroke at Cipto Mangunkusumo Hospital

    Aim: to identify the prevalence of laboratoric ASA resistance using platelet function tests in patients with ischemic stroke at Cipto Mangunkusumo Hospital and its associated factors. Methods: this study was a cross-sectional study involving 50 patients with ischemic stroke who only received ASA treatment. Evaluation of resistance to ASA was performed using Verifynow® platelet function test. ASA resistance was defined as ASA reaction unit (ARU) ≥550. Results: there were 7 patients with ASA resistance. The mean age of subjects in ASA resistance group was 51.3±9.2 years; while in ASA responsive group was 57.8±9.7 years. In ASA resistance group, there were 85.7% male patients, 57.1% active smoker and 100% subjects with hypertension. Conclusion: the prevalence of laboratoric ASA resistance in patients with ischemic stroke evaluated using platelet function test at Cipto Mangunkusumo Hospital is 14%. The prevalence is more likely to occur in male patients who were active smoker, at younger age and with comorbidity of hypertension.     An ultra-cool white dwarf serendipitously found with COMBO-17

    We report the discovery of an ultra-cool white dwarf in the COMBO-17 survey. So far, only seven objects have been discovered in this rare category of white dwarfs, which are characterized by strong flux depression in the far-red and near-infrared part of the spectrum, presumably due to collisionally induced absorption (CIA). The new object COMBO-17 J114356.08-0144032 has very similar colours to LHS 3250, which was the first of its kind to be recognized. However, at R=21.5 it is the faintest and possibly most distant such object discovered to date. It is the only such object in COMBO-17 at R<23; due to the small sky coverage of 0.78 square degree this chance discovery can not provide any constraints on the abundance of faint ultra-cool white dwarfs. We speculate on the basis of the proper motion that this new object is probably a member of the Galactic (thin) disk.Comment: accepted for publication by A&A Letters, 4 pages in print forma

    BLOOD VISCOSITY AS A DETERMINING FACTOR OF ISCHEMIC STROKE OUTCOMES EVALUATED WITH NIHSS AND MRS ON DAY 7 AND 30 POST-THROMBOLYSIS

    Objective: This study aimed to analyze blood viscosity as a determining factor of ischemic stroke outcomes evaluated with National Institutes of Health Stroke Scale (NIHSS) and modified Rankin Scale (mRS) on day 7 and 30 post-thrombolysis. Methods: This study was a 4-months cohort study taking place in Cipto Mangunkusumo General Hospital from January to April 2017. Subjects were collected at the Emergency Department or Neurology Outpatient Department. Eligible patients gave informed consent. Patients underwent numerous examinations, including blood viscosity test using digital microcapillary (DM) instrument. Outcomes of patients were identified on day 7 and day 30 post-thrombolysis using NIHSS and mRS, respectively. Results: Most acute ischemic stroke patients (88.6%) had blood hyperviscosity. 9.1% patients had poorer neurologic deficit on day 7 evaluated with NIHSS and 18.2% patients had poor outcome on day 30 evaluated with mRS. All patients with normal blood viscosity did not have a poorer neurologic deficit on day-7-evaluation. Conclusion: Blood viscosity determines the outcomes of acute ischemic stroke patients on day 7 and day 30 post-thrombolysis

    Environmental assessment and health impact assessment

    HIA and EA are approaches and processes that support better, healthier, and more sustainable policy development and decisionmaking. When undertaken well, and when valued and applied, they can also help to support better, more informed, transparent and democratic policy development and decision- making processes. However, they are not a panacea; rather, they constitute one important piece of the policy development and decision- making puzzle. Public health practitioners need to increase their knowledge and understanding of EA and HIA. They should improve their links with EA and HIA specialists. They also need to proactively and consistently undertake and commission assessments of health either within EA or as stand alone HIAs. Public health practitioners would also do well to oversee and scrutinize the scope of work for, and the findings of, EAs and HIAs that are commissioned and undertaken by others in the localities in which they work. By doing so, public health practitioners can help to advance the agenda of improving health for all by acting on the upstream determinants of health and bringing together key actors across society

    Objective: This study evaluated the efficacy of thrombolysis with 0.6 mg/kg intravenous alteplase for acute ischemic stroke patients within 6 h of stroke onset. Methods: This cross-sectional study collected data of patients with ischemic stroke received intravenous thrombolytic therapy with 0.6 mg/kg alteplase within 6 h of onset in Cipto Mangunkusumo General Hospital (Rumah Sakit Cipto Mangunkusumo [RSCM]) between November 2014 and August 2017. Efficacy of the thrombolytic therapy was evaluated using the National Institutes of Health Stroke Scale (NIHSS) and modified Rankin Scale (mRS). NIHSS evaluated on 24 h and 7 d post thrombolytic therapy portrayed clinical outcomes of patients while mRS evaluated on day 30 post-thrombolysis portrayed the functional outcome of patients. Results: The median NIHSS score decreased on 24 h and 7 d post-thrombolysis. 33.3% patients experienced a reduction of NIHSS score ≥4 on 24 h post thrombolytic therapy. On day 7 following thrombolysis, 57.4% patients had a good clinical outcome. On day 30 follow-up, 55.6% patients had a good functional outcome. Conclusion: Thrombolysis using 0.6 mg/kg intravenous alteplase within 6 h of onset is effective for acute ischemic stroke patients

    Non-SMC condensin I complex proteins control chromosome segregation and survival of proliferating cells in the zebrafish neural retina

    <p>Abstract</p> <p>Background</p> <p>The condensation of chromosomes and correct sister chromatid segregation during cell division is an essential feature of all proliferative cells. Structural maintenance of chromosomes (SMC) and non-SMC proteins form the condensin I complex and regulate chromosome condensation and segregation during mitosis. However, due to the lack of appropriate mutants, the function of the condensin I complex during vertebrate development has not been described.</p> <p>Results</p> <p>Here, we report the positional cloning and detailed characterization of retinal phenotypes of a zebrafish mutation at the <it>cap-g </it>locus. High resolution live imaging reveals that the progression of mitosis between prometa- to telophase is delayed and that sister chromatid segregation is impaired upon loss of CAP-G. CAP-G associates with chromosomes between prometa- and telophase of the cell cycle. Loss of the interaction partners CAP-H and CAP-D2 causes cytoplasmic mislocalization of CAP-G throughout mitosis. DNA content analysis reveals increased genomic imbalances upon loss of non-SMC condensin I subunits. Within the retina, loss of condensin I function causes increased rates of apoptosis among cells within the proliferative ciliary marginal zone (CMZ) whereas postmitotic retinal cells are viable. Inhibition of p53-mediated apoptosis partially rescues cell numbers in <it>cap-g </it>mutant retinae and allows normal layering of retinal cell types without alleviating their aberrant nuclear sizes.</p> <p>Conclusion</p> <p>Our findings indicate that the condensin I complex is particularly important within rapidly amplifying progenitor cell populations to ensure faithful chromosome segregation. In contrast, differentiation of postmitotic retinal cells is not impaired upon polyploidization.</p
