    Rotational mal-alignment after reamed intramedullary nailing for tibial shaft fracture

    Abstract Intra-medullary (IM) nailing is standard of care for unstable tibial shaft fractures. Malrotation is very common but it is under-recognised, inpart because of variation in normal anatomy and partly due to difficulty in accurately assessing rotation. This study was planned to evaluate the frequency of rotational mal-alignment after reamed tibia IMnailing. This cross-sectional study was conducted at Aga khan University Hospital, Karachi, and comprised patients with tibia shaft fractures managed with IMnailing from January to December 2014. All the patients were assessed intra-operatively for rotational alignment using the knee and ankle fluoroscopic images. There were 81 patients with a mean age of 38±16.9 years. There were 64(79%) male patients. Overall the incidence of malrotation was in 20(24.7%) cases. Rotational mal-alignment is one of the preventable complications after IMnailing which can be assesed intra-operatively under fluoroscope

    Problems of Political Development in Bangladesh

    After World War II the revival of the spirit of nationalism led to the emergence of many new states. Freed from the colonial domination, the new states were keenly interested in developing their societies as fast as possible. Many problems stand in the way of development. These problems are the problems of stability, unity, integration and socioeconomic problems. This study is concerned about the problems of political development in Bangladesh, which came into existence in 1971, as a result of the problems of of national integration in Pakistan. This study identifies some problems which stand in the way of political development in Bangladesh. These problems are: (1) the problems of integration, (2) the problems of unity and stability, (3) the lack of well-organized political parties, (4) the problems of bureaucracy, and (5) socio-economic problems. After the creation of Bangladesh it was assumed that there would be no problem of integration in Bangladesh. But an analysis of the political situations will show that problems of integration still remain, though in different character, such as the lack of integration between the elites and the mass, the lack of integrative ideology, the separatist tendency in the tribal people, etc. Absence of stability is creating problems in the effective undertaking of any developmental plan. The chief causes of instability are: factionalism, frustration of the people, threat of India, loss of charisma, etc. Too many political parties with too many ideologies stand in the way of creating a stable government. The parties do not have any organizational basis, and are based on the personality of the leader. The parties are not effective in aggregating the peoples\u27 demands and grievances. The lack of balance in the role of the bureaucracy is also a problem. On the one hand, if the bureaucrats are given more power, they hinder the growth of political institutions. On the other hand, if they are given less power, they play an inactive role hindering the execution of policies. Bangladesh suffers from both the problems of bureaucracy. The most pre-dominant problems which hamper political development in Bangladesh are socio-economic problems. The population problem, food problem, and unemployment create mass frustrations causing instability and disunity within the country. The attachment to the traditional values hinders the way of change. This study analyzes in detail all these problems and how they stand in the way of political development

    A Phonemic and Acoustic Analysis of Hindko Fricatives

    The Hindko language is mainly spoken in the province of Khyber Pukhtunkhaw, Pakistan. It relates to the Indo-Aryan family of languages. This paper focusses on the phonemic and acoustic analysis of Hindko fricatives. The phonemic analysis identifies that Hindko has eight fricatives. The acoustic analysis aims to explore temporal and spectral characteristics of these fricatives. The acoustic analysis discloses that Hindko fricatives are distinguishable on the basis of the temporal properties such as friction duration and spectral cues like turbulence noise

    Receptor modification as a therapeutic approach against viral diseases

    Poliovirus causes flaccid paralysis through the destruction of motor neurons in the CNS. Susceptibility to its infection is mainly due to the interaction in between the surface capsid proteins and its receptors on the host cell surface, important for binding, penetration and other necessary events during early infection. Receptor modification is a new approach to treat viral diseases by the modification of target proteins structure. Binding domains are modified in an effective way to make it difficult for the virus to recognize it. In this study, tolerant and intolerant induced mutations in the poliovirus receptor, VP1 and VP2 were identified and substituted in the seed sequence to get the modified versions. Substitutions causing changes in initial folding were short listed and further analyzed for high level folding, physiochemical properties and interactions. Highest RMSD values were observed in between the seed and the mutant K90F (3.265 â„«) and Q130W (3.270â„«) respectively. The proposed substitutions were found to have low functional impact and thus can be further tested and validated by the experimental researchers. Interactions analyses proved most of the substitutions having decreased affinity for both the VP1 and VP2 and thus are of significant importance against poliovirus. This study will play an important role for bridging computational biology to other fields of applied biology and also will provide an insight to develop resistance against viral diseases. It is also expected that same approach can also be applicable against other viruses like HCV, HIV and other in near future

    Abberent expression analysis of LMNA gene in hutchinson-gilford progeria syndrome

    Hutchinson-Gilford progeria syndrome (HGPS) is caused by de novo dominant point mutations of the genes encoding nuclear lamina proteins, leading towards premature aging. A protein sequence is subjected to mutations in nature which can affect the function and folding pattern of the protein by different ways. Mutations involved in HGPS were identified and were substituted in the seed sequence retrieved from the UniProt database to get the mutated versions. Tertiary structure of the Lamin A protein was previously unpredicted so was performed for all the mutated as well as for the seed protein to analyze the effects of mutations on the protein structure, folding and interactions. All the predicted models were refined and validated through multiple servers for multiple parameters. The validated 3D structure of seed protein was then successfully submitted to the Protein Model Database and was assigned with the PMDB ID PM0077829. All the predicted structures were superimposed with a root mean square deviation value of 7.0 â„« and a high Dali Z-score of 1.9. It was observed that mutations affected physiochemical properties as well as instability index and thus is affecting the domains in specific and the whole structure in general. It was further analyzed that HGPS is the result of affected Lamin a protein interactions with other integral and binding proteins in the inner nuclear membrane affecting the link in between the nuclear membrane and the network of the lamina

    Locking compression plate in musculoskeletal oncology\u27a nriend in need\u27

    Abstract BACKGROUND: We are presenting our experience in the use of locking compression plate (LCP) after juxta-articular oncological resections in addition to its use in pathologic fracture. METHODS: A retrospective audit of skeletal reconstruction using LCP in 25 cases of long bone tumors was performed from 2008 to 2010. Reconstruction following limb salvage surgery was done in 17 patients and internal fixation of pathological fracture was done in 8 patients. All patients were available for \u3e 12 months of follow-up, and thus assessed for union at the resected ends. RESULTS: There were 8 males and 17 females in the study. The average age at the time of surgery was 30 years (range, 9 to 66 years). The minimum follow-up was 12 months (range, 12 to 32 months). All patients except three went on to heal successfully. Complications occurred in those three patients: wound infection in one, nonunion in another, and periprosthetic fracture in the other patient. In the remaining patients, union was achieved at an average of 6.5 months after reconstruction in curative resection and 4.75 months after fixation of pathological fractures. CONCLUSIONS: Joint sparing limb salvage surgery was made successfully possible after sekeletal reconstruction with LCP. Its use was also quite effective in pathological fractures with poor bone quality. Use of locking plates for musculoskeletal oncological reconstruction resulted in a good and predictable rate of union

    PAKISTAN’S TRADE POLICIES, NON-TARIFF MEASURES AND CONCERNS OF STAKEHOLDERS

    Pakistan can increase its exports by 12 billion dollars till 2024 if integration with international market is improved, and strong linkages are developed among value chains (ITC, 2020). But this potential development is facing impediments due to transparency issues, imposition of non-tariff measures, ambiguous regulations at home and lack of information among both importers and exporters. This paper is dedicated to the discussion of Pakistan’s trade policies, tariff structure, rules governing non-tariff measures, and discussion about the concerns of business community and other stakeholders on NTMs. Application of NTMs in Pakistan, and their shortcomings are also discussed in detail. Then based on these discussions some recommendations are forwarded. Study shows that most of the hurdles faced by traders are home borne. If regulations are made easier at home and mutual recognition of standards is acquired with partner countries, then across the border trade can become much easier

    Sequence and structure based assessment of nonsynonymous SNPs in hypertrichosis universalis

    Hairs are complex structures, making a protective layer and serves different biological functions. TRPS1, a transcription factor is one of the candidate genes causing congenital hypertrichosis, an excessive hair growth at inappropriate body parts. SNPs of TRPS1 were retrieved from dbSNP which were screened by SIFT and PolyPhen servers based on their functional impacts. Out of the screened SNPs, rs181507248 and rs146506752 were predicted as intolerant and damaging by both the servers. The predicted tertiary structure of the native TRPS1 after refinement and validation was successfully submitted to the Protein Model Database and was assigned with PMDB ID PM0077843, as it was previously unpredicted. It was observed through the structure based analysis that, the SNPs rs181507248 and rs146506752 caused significant changes in the secondary and tertiary structures as well as the physiochemical properties of TRPS1 protein. It can thus be concluded that the changed properties due to these single nucleotide polymorphisms effect the interactions of TRPS1 which result in congenital hypertrichosis

    Morbidity of colostomy reversal

    Abstract OBJECTIVE: To determine morbidity after colostomy reversal at a tertiary care hospital. METHODS: The retrospective case series was conducted at the Aga Khan University Hospital, Karachi, and comprised records of patients aged 15 years or more who underwent colostomy reversal from January 2003 to December 2011. Data was collected regarding demographics, procedure dates, indication, as well as type and location of colostomy. Details of colostomy reversal, including pre-operative, intra-operative and post-operative variables were recorded. RESULTS: The mean age of the 96 patients was 40±16 years; 72(75%) of them being males. The most frequent indications for fashioning of colostomy were bowel perforation in 53 (55.2%) and malignancy in 9(9.3%) patients. Intra-operative complications occurred in 5(5.2%) with bowel perforation in 3(3.1%) and bleeding in 2(2%) patients. Overall, 40(41.6%) patients had post-operative local complications; the most common being wound infection in 19(19.8%) followed by incisional hernia 15(15.6%). Patients who experienced post-operative complications had significantly longer hospital stay compared to those without complications (9±2.8 vs. 7±2.4days; p=0.038). CONCLUSIONS: Colostomy reversal was associated with non-negligible morbidity. The most common complications were wound infection and incisional hernia
