    Primary angiosarcoma of breast

    Primary breast sarcomas, except for phyllodes tumour, are very rare entities, accounting for \u3c 0.1% of all malignant neoplasms. Angiosarcoma of breast is infrequent malignancy and differential diagnosis from other sarcomatous and angiomatous breast tumours holds importance. Two cases of primary angiosarcoma of breast were encountered. One involved a 32 years lady who was treated by wide local excision and six cycles of chemotherapy. The other occurred in a 54 years old lady who was treated with mastectomy, did not receive any radiation or chemotherapy and was later lost to follow-up. Neither of the patient had history of previous breast surgery, chemotherapy or radiotherapy

    Frequency of precancerous lesions in endoscopic gastric biopsies in chronic gastritis

    Objective: To determine the frequency of precancerous lesions in endoscopic gastric biopsies of patients with chronic gastritis.STUDY Design: A case series. Place and duration of study: Department of Histopathology, Liaquat National Hospital, Karachi, from July 2008 to January 2009.METHODOLOGY: Over 6 months, 375 endoscopic gastric biopsies of patients with age group of 15-65 years having endoscopic chronic gastritis were included. From final biopsy report, basic information like patient demographics and presence of precancerous lesions i.e. activity (chronic active gastritis), atrophy (atrophic gastritis), intestinal metaplasia and dysplasia were recorded on proforma. Results were described as proportions and frequency. Results: The frequency of precancerous lesions in endoscopic gastric biopsies of patients with chronic gastritis in Karachi was markedly high. Most common lesion was chronic active gastritis as depicted by activity (48.3%); dysplasia (1.3%) was the least common. Proportion of more aggressive precancerous lesions were markedly higher in older age group (\u3e 40 years). Conculsion: he precancerous lesions are frequent in endoscopic gastric biopsies of patients with chronic gastritis

    Hemodynamic Comparison between modified saddle block and subarachnoid block for Transurethral Resection of Prostate

    BACKGROUND:   Spinal anesthesia is often the mode of anesthesia in transurethral resection of prostate (TURP) albeit with risk of hypotension. However, hemodynamic derangement is often less pronounced in saddle block. This study was conducted to compare the mean fall in Mean arterial Pressure (MAP) and Heart rate (HR) between modified saddle block and subarachnoid block for TURP.  OBJECTIVE:   To compare the mean fall in MAP and HR between modified saddle block and subarachnoid block for TURP.  METHODS:   In this randomized controlled trial 120 patients undergoing elective TURP were included and divided into two groups. Group A Patients received subarachnoid block and Group B received modified saddle block. Baseline hemodynamics were recorded before and then at 5 minutes after anesthesia. All the collected data was entered into SPSS version 22 and analyzed. Quantitative data like age, BMI, ASA-PS score, duration of surgery and baseline hemodynamics were presented as means and standard deviations. The fall of mean arterial pressure (MAP) & heart rate (HR) were compared among groups by independent t test and P value < 0.05 was considered as statistically significant.   RESULTS:  Mean fall in MAP was 8.98±1.28 mmHg and 3.13±0.68 mmHg (p<0.001) in group A and B, respectively. Mean fall in HR was 7.17±0.98 and 2.78±0.59 (p<0.001) in group A and group B, respectively.  CONCLUSION:   Modified saddle block resulted in significantly decreased fall in MAP and HR as compared with subarachnoid block for TURP.  KEY WORDS:   Modified Saddle Block; Subarachnoid Block; Mean Arterial Pressure &nbsp

    A Trainee’s Approach for Effective Operating Room Learning

    Introduction: Surgical training has gone through a paradigm shift in recent years. The trends have shifted from an apprenticeship model to a new era of self-teaching and an attitude-driven learning. In this study, we aim to comprehensively investigate the knowledge and attitude pertaining to the perception of preparation required for the operating room and how the residents prepare themselves for it. Method: A quantitative study was conducted via predefined questionnaire. Surgical residents at a single tertiary care institution were asked to complete the survey via Google forms that was distributed by means of email to residents of the general surgery department. Inclusion and exclusion criteria were defined. Results: Forty surgical residents opted to participate in the survey. The majority of the residents had a similar perception of operating room preparation. The most prominent theme that emerged following the discussion was that the residents learned through trial and error, and advice solicited from co-residents, which in the majority of the cases, were senior level resident. The resources defined by the residents were similar among majority and factors that affected their preparation were variable. An important aspect highlighted through the discussion was that the level of postgraduate training affected the preparation done by residents in order to better perform in operation theatres. Conclusion: Knowledge and attitude regarding operating room preparation was quite similar among the surgical residents. They varied in their practice for preparation

    Comparative analysis of AC and DC distribution system with respect to harmonic distortion considering daily load profile

    Electrical energy is the most efficient and the cleanest form of energy at the moment that is being transmitted and distributed amongst end-users. From its earlier days, the AC system was preferred as an economical solution for transmission and distribution. However, the development in the power electronics technology and the evolution of highly efficient power electronic converters have established the resurgence of DC power system. Furthermore, the trend is shifting towards DC loads as various energy efficient appliances, such as DC inverter air conditioners, operate on DC nowadays. This further advocates the shift towards the DC power system. This research works is an effort to perform the comparative analysis of AC Distribution System (ACDS) and DC Distribution System (DCDS), with regards to power quality and harmonic distortion in particular. The comparison is performed considering load profile and load variation on daily basis. Simulations are performed in MATLAB. It has been concluded at the end that ACDS is better than DCDS in terms of power quality as total harmonic distortion of the DCDS under the same loading and same load variation during the whole day was significantly higher than that of ACDS

    Pilots Optimization and Surface Area Effects on Channel Estimation in RIS Aided MIMO System

    Reconfigurable intelligent surface (RIS) is an emerging tool for 5G and wireless communication technologies that have attracted researchers' interest. However, the passive nature and the high number of reflecting elements in RIS result in a large pilot overhead, which makes channel estimation challenging in multi-user multiple-input multiple-output (MU-MIMO) wireless communication systems. Previous works have shown an improvement in reducing the pilot overhead by exploiting the structured sparsity in rows and columns, which was further improved by compensating offset among users in angular cascaded channels of RIS aided system. To further reduce the pilot overhead, we analyze and adopt coherence-optimized pilots for channel estimation and propose an algorithm to analyze the combined effect of low-coherence pilots with an optimum size of RIS elements for a given number of users, transmit antennas, and normalized error threshold performance. The simulation results illustrate better NMSE performance as compared to contemporary techniques
