    ABSTRACT The route selection in a ship operation is important thing, a ship is expected to operate safely, effectively and efficiently. Tanjung Perak Marine Navigation Service ship beside carried out task to maintenance buoy and other navigation aid also have a task to mobilization of lighthouse keeper in their work area. The ideal condition according to the provisions of the Directorate of Navigation is that every ship must sail at least 118 days per year. However, due to the limited fuel oil budget, these ideal conditions could not be achieved, where the pattern of routes I and II carried out by Class I ships in the form of logistics delivery and roster schedule of lighthouse keeper, which were originally 4 times per year, only became 2 times per year. Whereas class II ships only carry out maintenance on route III and IV. For the V route pattern, namely Lebaran, Christmas and New Year, transportation currently only carried out by class I ships, however class II ships in this study are simulated to also be able to serve route V taking into account the availability of passenger capacity and fulfillment of ship sailing time. This research was conducted to analyze several existing route options, in order to obtain a route that can be used as a reference in terms of ship operations that meet existing limitations such as the need for fuel, fresh water and in accordance with the roster schedule for each lighthouse keeper. Several other things are taken into consideration in selecting routes and officer schedules including the weather factor, the amount of cargo to be transported and unloaded, as well as the type of maintenance work that is in an Navigation Aid (SBNP) Due to these dynamic condition, the research method used in this study is Discrete event simulation. The simulation begins with modelling 5 (five) existing route with 4 (four) navigation ship. Simulation is carried out based on type of ship, route scenario, location of lighthouse and assignment schedule of lighthouse keeper, as well as existing limitation.     Homegarden commercialization: extent, household characteristics, and effect on food security and food sovereignty in Rural Indonesia

    Homegardens have long been recognized for contributing to household food security, nutritional status, and ecological sustainability in especially poor, rural areas in low-income countries. However, as markets and policies drive the commercialization of food and farming systems, and of rural livelihoods in general, it becomes increasingly difficult for small-holder farmers to maintain homegarden plots. Rather than autonomous spaces to grow food for self-consumption, farmers are transforming the land around their dwellings into an income-generating space by planting commercial crops for sale in urban and processing markets. The objective of this study was to examine homegarden commercialization in the Upper Citarum Watershed of West Java, Indonesia, and its effects on food security and food sovereignty. We employed a mixed-method approach to survey 81 village households involved in agricultural production. For quantitative analysis, we calculated a “homegarden commercialization index,” and developed indicator frameworks to examine relationships between commercialization, household food security, and food-related decision-making. Accompanied by insights from qualitative interviews, our results show that homegardens are highly commercialized, which contributes to the spread of monocultural production in the region.     Collective modes of asymmetric nuclear matter in Quantum HadroDynamics

    We discuss a fully relativistic Landau Fermi liquid theory based on the Quantum Hadro-Dynamics (QHDQHD) effective field picture of Nuclear Matter ({\it NM}). From the linearized kinetic equations we get the dispersion relations of the propagating collective modes. We focus our attention on the dynamical effects of the interplay between scalar and vector channel contributions. A beautiful ``mirror'' structure in the form of the dynamical response in the isoscalar/isovector degree of freedom is revealed, with a complete parallelism in the role respectively played by the compressibility and the symmetry energy. All that strongly supports the introduction of an explicit coupling to the scalar-isovector channel of the nucleon-nucleon interaction. In particular we study the influence of this coupling (to a ÎŽ\delta-meson-like effective field) on the collective response of asymmetric nuclear matter (ANMANM). Interesting contributions are found on the propagation of isovector-like modes at normal density and on an expected smooth transition to isoscalar-like oscillations at high baryon density. Important ``chemical'' effects on the neutron-proton structure of the mode are shown. For dilute ANMANM we have the isospin distillation mechanism of the unstable isoscalar-like oscillations, while at high baryon density we predict an almost pure neutron wave structure of the propagating sounds.Comment: 18 pages (LATEX), 8 Postscript figures, uses "epsfig

    Efficacy and Safety of Three Antiretroviral Regimens for Initial Treatment of HIV-1: A Randomized Clinical Trial in Diverse Multinational Settings

    Background:Antiretroviral regimens with simplified dosing and better safety are needed to maximize the efficiency of antiretroviral delivery in resource-limited settings. We investigated the efficacy and safety of antiretroviral regimens with once-daily compared to twice-daily dosing in diverse areas of the world.Methods and Findings:1,571 HIV-1-infected persons (47% women) from nine countries in four continents were assigned with equal probability to open-label antiretroviral therapy with efavirenz plus lamivudine-zidovudine (EFV+3TC-ZDV), atazanavir plus didanosine-EC plus emtricitabine (ATV+DDI+FTC), or efavirenz plus emtricitabine-tenofovir-disoproxil fumarate (DF) (EFV+FTC-TDF). ATV+DDI+FTC and EFV+FTC-TDF were hypothesized to be non-inferior to EFV+3TC-ZDV if the upper one-sided 95% confidence bound for the hazard ratio (HR) was ù‰€1.35 when 30% of participants had treatment failure.An independent monitoring board recommended stopping study follow-up prior to accumulation of 472 treatment failures. Comparing EFV+FTC-TDF to EFV+3TC-ZDV, during a median 184 wk of follow-up there were 95 treatment failures (18%) among 526 participants versus 98 failures among 519 participants (19%; HR 0.95, 95% CI 0.72-1.27; p = 0.74). Safety endpoints occurred in 243 (46%) participants assigned to EFV+FTC-TDF versus 313 (60%) assigned to EFV+3TC-ZDV (HR 0.64, CI 0.54-0.76; p<0.001) and there was a significant interaction between sex and regimen safety (HR 0.50, CI 0.39-0.64 for women; HR 0.79, CI 0.62-1.00 for men; p = 0.01). Comparing ATV+DDI+FTC to EFV+3TC-ZDV, during a median follow-up of 81 wk there were 108 failures (21%) among 526 participants assigned to ATV+DDI+FTC and 76 (15%) among 519 participants assigned to EFV+3TC-ZDV (HR 1.51, CI 1.12-2.04; p = 0.007).Conclusion: EFV+FTC-TDF had similar high efficacy compared to EFV+3TC-ZDV in this trial population, recruited in diverse multinational settings. Superior safety, especially in HIV-1-infected women, and once-daily dosing of EFV+FTC-TDF are advantageous for use of this regimen for initial treatment of HIV-1 infection in resource-limited countries. ATV+DDI+FTC had inferior efficacy and is not recommended as an initial antiretroviral regimen.Trial Registration:http://www.ClinicalTrials.gov NCT00084136

    Construction status and prospects of the Hyper-Kamiokande project

    The Hyper-Kamiokande project is a 258-kton Water Cherenkov together with a 1.3-MW high-intensity neutrino beam from the Japan Proton Accelerator Research Complex (J-PARC). The inner detector with 186-kton fiducial volume is viewed by 20-inch photomultiplier tubes (PMTs) and multi-PMT modules, and thereby provides state-of-the-art of Cherenkov ring reconstruction with thresholds in the range of few MeVs. The project is expected to lead to precision neutrino oscillation studies, especially neutrino CP violation, nucleon decay searches, and low energy neutrino astronomy. In 2020, the project was officially approved and construction of the far detector was started at Kamioka. In 2021, the excavation of the access tunnel and initial mass production of the newly developed 20-inch PMTs was also started. In this paper, we present a basic overview of the project and the latest updates on the construction status of the project, which is expected to commence operation in 2027

    Prospects for neutrino astrophysics with Hyper-Kamiokande

    Hyper-Kamiokande is a multi-purpose next generation neutrino experiment. The detector is a two-layered cylindrical shape ultra-pure water tank, with its height of 64 m and diameter of 71 m. The inner detector will be surrounded by tens of thousands of twenty-inch photosensors and multi-PMT modules to detect water Cherenkov radiation due to the charged particles and provide our fiducial volume of 188 kt. This detection technique is established by Kamiokande and Super-Kamiokande. As the successor of these experiments, Hyper-K will be located deep underground, 600 m below Mt. Tochibora at Kamioka in Japan to reduce cosmic-ray backgrounds. Besides our physics program with accelerator neutrino, atmospheric neutrino and proton decay, neutrino astrophysics is an important research topic for Hyper-K. With its fruitful physics research programs, Hyper-K will play a critical role in the next neutrino physics frontier. It will also provide important information via astrophysical neutrino measurements, i.e., solar neutrino, supernova burst neutrinos and supernova relic neutrino.     The evolving SARS-CoV-2 epidemic in Africa: Insights from rapidly expanding genomic surveillance

    INTRODUCTION Investment in Africa over the past year with regard to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) sequencing has led to a massive increase in the number of sequences, which, to date, exceeds 100,000 sequences generated to track the pandemic on the continent. These sequences have profoundly affected how public health officials in Africa have navigated the COVID-19 pandemic. RATIONALE We demonstrate how the first 100,000 SARS-CoV-2 sequences from Africa have helped monitor the epidemic on the continent, how genomic surveillance expanded over the course of the pandemic, and how we adapted our sequencing methods to deal with an evolving virus. Finally, we also examine how viral lineages have spread across the continent in a phylogeographic framework to gain insights into the underlying temporal and spatial transmission dynamics for several variants of concern (VOCs). RESULTS Our results indicate that the number of countries in Africa that can sequence the virus within their own borders is growing and that this is coupled with a shorter turnaround time from the time of sampling to sequence submission. Ongoing evolution necessitated the continual updating of primer sets, and, as a result, eight primer sets were designed in tandem with viral evolution and used to ensure effective sequencing of the virus. The pandemic unfolded through multiple waves of infection that were each driven by distinct genetic lineages, with B.1-like ancestral strains associated with the first pandemic wave of infections in 2020. Successive waves on the continent were fueled by different VOCs, with Alpha and Beta cocirculating in distinct spatial patterns during the second wave and Delta and Omicron affecting the whole continent during the third and fourth waves, respectively. Phylogeographic reconstruction points toward distinct differences in viral importation and exportation patterns associated with the Alpha, Beta, Delta, and Omicron variants and subvariants, when considering both Africa versus the rest of the world and viral dissemination within the continent. Our epidemiological and phylogenetic inferences therefore underscore the heterogeneous nature of the pandemic on the continent and highlight key insights and challenges, for instance, recognizing the limitations of low testing proportions. We also highlight the early warning capacity that genomic surveillance in Africa has had for the rest of the world with the detection of new lineages and variants, the most recent being the characterization of various Omicron subvariants. CONCLUSION Sustained investment for diagnostics and genomic surveillance in Africa is needed as the virus continues to evolve. This is important not only to help combat SARS-CoV-2 on the continent but also because it can be used as a platform to help address the many emerging and reemerging infectious disease threats in Africa. In particular, capacity building for local sequencing within countries or within the continent should be prioritized because this is generally associated with shorter turnaround times, providing the most benefit to local public health authorities tasked with pandemic response and mitigation and allowing for the fastest reaction to localized outbreaks. These investments are crucial for pandemic preparedness and response and will serve the health of the continent well into the 21st century

    Tocilizumab in patients admitted to hospital with COVID-19 (RECOVERY): a randomised, controlled, open-label, platform trial

    Background: In this study, we aimed to evaluate the effects of tocilizumab in adult patients admitted to hospital with COVID-19 with both hypoxia and systemic inflammation. Methods: This randomised, controlled, open-label, platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]), is assessing several possible treatments in patients hospitalised with COVID-19 in the UK. Those trial participants with hypoxia (oxygen saturation &lt;92% on air or requiring oxygen therapy) and evidence of systemic inflammation (C-reactive protein ≄75 mg/L) were eligible for random assignment in a 1:1 ratio to usual standard of care alone versus usual standard of care plus tocilizumab at a dose of 400 mg–800 mg (depending on weight) given intravenously. A second dose could be given 12–24 h later if the patient's condition had not improved. The primary outcome was 28-day mortality, assessed in the intention-to-treat population. The trial is registered with ISRCTN (50189673) and ClinicalTrials.gov (NCT04381936). Findings: Between April 23, 2020, and Jan 24, 2021, 4116 adults of 21 550 patients enrolled into the RECOVERY trial were included in the assessment of tocilizumab, including 3385 (82%) patients receiving systemic corticosteroids. Overall, 621 (31%) of the 2022 patients allocated tocilizumab and 729 (35%) of the 2094 patients allocated to usual care died within 28 days (rate ratio 0·85; 95% CI 0·76–0·94; p=0·0028). Consistent results were seen in all prespecified subgroups of patients, including those receiving systemic corticosteroids. Patients allocated to tocilizumab were more likely to be discharged from hospital within 28 days (57% vs 50%; rate ratio 1·22; 1·12–1·33; p&lt;0·0001). Among those not receiving invasive mechanical ventilation at baseline, patients allocated tocilizumab were less likely to reach the composite endpoint of invasive mechanical ventilation or death (35% vs 42%; risk ratio 0·84; 95% CI 0·77–0·92; p&lt;0·0001). Interpretation: In hospitalised COVID-19 patients with hypoxia and systemic inflammation, tocilizumab improved survival and other clinical outcomes. These benefits were seen regardless of the amount of respiratory support and were additional to the benefits of systemic corticosteroids.
