Determinants of Leukocyte Margination in Rectangular Microchannels
Microfabrication of polydimethylsiloxane (PDMS) devices has provided a new set of tools for studying fluid dynamics of blood at the scale of real microvessels. However, we are only starting to understand the power and limitations of this technology. To determine the applicability of PDMS microchannels for blood flow analysis, we studied white blood cell (WBC) margination in channels of various geometries and blood compositions. We found that WBCs prefer to marginate downstream of sudden expansions, and that red blood cell (RBC) aggregation facilitates the process. In contrast to tubes, WBC margination was restricted to the sidewalls in our low aspect ratio, pseudo-2D rectangular channels and consequently, margination efficiencies of more than 95% were achieved in a variety of channel geometries. In these pseudo-2D channels blood rheology and cell integrity were preserved over a range of flow rates, with the upper range limited by the shear in the vertical direction. We conclude that, with certain limitations, rectangular PDMS microfluidic channels are useful tools for quantitative studies of blood rheology
Maximal Spontaneous Photon Emission and Energy Loss from Free Electrons
Free electron radiation such as Cerenkov, Smith--Purcell, and transition
radiation can be greatly affected by structured optical environments, as has
been demonstrated in a variety of polaritonic, photonic-crystal, and
metamaterial systems. However, the amount of radiation that can ultimately be
extracted from free electrons near an arbitrary material structure has remained
elusive. Here we derive a fundamental upper limit to the spontaneous photon
emission and energy loss of free electrons, regardless of geometry, which
illuminates the effects of material properties and electron velocities. We
obtain experimental evidence for our theory with quantitative measurements of
Smith--Purcell radiation. Our framework allows us to make two predictions. One
is a new regime of radiation operation---at subwavelength separations, slower
(nonrelativistic) electrons can achieve stronger radiation than fast
(relativistic) electrons. The second is a divergence of the emission
probability in the limit of lossless materials. We further reveal that such
divergences can be approached by coupling free electrons to photonic bound
states in the continuum (BICs). Our findings suggest that compact and efficient
free-electron radiation sources from microwaves to the soft X-ray regime may be
achievable without requiring ultrahigh accelerating voltages.Comment: 7 pages, 4 figure
A Single-Institution Experience with Metallic Ureteral Stents: A Cost-Effective Method of Managing Deficiencies in Ureteral Drainage
IntroductionThe limitations of traditional ureteral stents in patients with deficiencies in ureteral drainage have resulted in frequent stent exchanges. The implementation of metallic stents was introduced to improve the patency rates of patients with chronic upper urinary tract obstruction, obviating the need for frequent stent exchanges. We report our clinical experiences with the use of metallic ureteral stents in the management of poor ureteral drainage.Materials and MethodsFifty patients underwent metallic ureteral stent placement from 2009 to 2012. Stent failure was defined as an unplanned stent exchange, need for nephrostomy tube placement, increasing hydronephrosis with stent in place, or an elevation in serum creatinine. Stent life was analyzed using the Kaplan-Meier methodology, as this was a time dependent continuous variable. A cost analysis was similarly conducted.ResultsA total of 97 metallic stents were placed among our cohort of patients: 63 in cases of malignant obstruction, 33 in the setting of cutaneous ureterostomies, and 1 in an ileal conduit urinary diversion. Overall, stent failure occurred in 8.2% of the stents placed. Median stent life was 288.4 days (95% CI: 277.4-321.2 days). The estimated annual cost for traditional polymer stents (exchanged every 90 days) was 13,128, while the estimated cost for metallic stents was 5,313.ConclusionOur results indicate that metallic ureteral stent placement is a technically feasible procedure with minimal complications and is well tolerated among patients. Metallic stents can be left in situ for longer durations and provide a significant financial benefit when compared to traditional polymer stents
Protecting eyewitness evidence: Examining the efficacy of a self-administered interview tool
Given the crucial role of eyewitness evidence, statements should be obtained as soon as possible after an incident. This is not always achieved due to demands on police resources. Two studies trace the development of a new tool, the Self-Administered Interview (SAI), designed to elicit a comprehensive initial statement. In Study 1, SAI participants reported more correct details than participants who provided a free recall account, and performed at the same level as participants given a Cognitive Interview. In Study 2, participants viewed a simulated crime and half recorded their statement using the SAI. After a delay of 1 week, all participants completed a free recall test. SAI participants recalled more correct details in the delayed recall task than control participants
Incomplete oedipism and chronic suicidality in psychotic depression with paranoid delusions related to eyes
Self-enucleation or oedipism is a term used to describe self-inflicted enucleation. It is a rare form of self-mutilation, found mainly in acutely psychotic patients. We propose the term incomplete oedipism to describe patients who deliberately and severely mutilate their eyes without proper enucleation. We report the case of a 32-year-old male patient with a five-year history of psychotic depression accompanied by paranoid delusions centered around his belief that his neighbors criticized him and stared at him. A central feature of his clinical picture was an eye injury that the patient had caused by pouring molten lead into his right eye during a period of deep hopelessness and suicidality when the patient could not resolve his anhedonia and social isolation. Pharmacotherapy and psychotherapy dramatically improved his disorder
Comparison of symptom-based versus self-reported diagnostic measures of anxiety and depression disorders in the GLAD and COPING cohorts
Background:
Understanding and improving outcomes for people with anxiety or depression often requires large sample sizes. To increase participation and reduce costs, such research is typically unable to utilise “gold-standard” methods to ascertain diagnoses, instead relying on remote, self-report measures.
Aims:
Assess the comparability of remote diagnostic methods for anxiety and depression disorders commonly used in research.
Method:
Participants from the UK-based GLAD and COPING NBR cohorts (N = 58,400) completed an online questionnaire between 2018 and 2020. Responses to detailed symptom reports were compared to DSM-5 criteria to generate symptom-based diagnoses of major depressive disorder (MDD), generalised anxiety disorder (GAD), specific phobia, social anxiety disorder, panic disorder, and agoraphobia. Participants also self-reported any prior diagnoses from health professionals, termed self-reported diagnoses. “Any anxiety” included participants with at least one anxiety disorder. Agreement was assessed by calculating accuracy, Cohen’s kappa, McNemar’s chi-squared, sensitivity, and specificity.
Results:
Agreement between diagnoses was moderate for MDD, any anxiety, and GAD, but varied by cohort. Agreement was slight to fair for the phobic disorders. Many participants with self-reported GAD did not receive a symptom-based diagnosis. In contrast, symptom-based diagnoses of the phobic disorders were more common than self-reported diagnoses.
Conclusions:
Agreement for MDD, any anxiety, and GAD was higher for cases in the case-enriched GLAD cohort and for controls in the general population COPING NBR cohort. For anxiety disorders, self-reported diagnoses classified most participants as having GAD, whereas symptom-based diagnoses distributed participants more evenly across the anxiety disorders. Further validation against gold standard measures is required
Effect of tube diameter and capillary number on platelet margination and near-wall dynamics
The effect of tube diameter and capillary number on platelet
margination in blood flow at tube haematocrit is investigated.
The system is modelled as three-dimensional suspension of deformable red blood
cells and nearly rigid platelets using a combination of the lattice-Boltzmann,
immersed boundary and finite element methods. Results show that margination is
facilitated by a non-diffusive radial platelet transport. This effect is
important near the edge of the cell-free layer, but it is only observed for , when red blood cells are tank-treading rather than tumbling. It is also
shown that platelet trapping in the cell-free layer is reversible for . Only for the smallest investigated tube ()
margination is essentially independent of . Once platelets have reached the
cell-free layer, they tend to slide rather than tumble. The tumbling rate is
essentially independent of but increases with . Tumbling is suppressed
by the strong confinement due to the relatively small cell-free layer thickness

The Predictive Nature of Individual Differences in Early Associative Learning and Emerging Social Behavior
Across the first year of life, infants achieve remarkable success in their ability to interact in the social world. The hierarchical nature of circuit and skill development predicts that the emergence of social behaviors may depend upon an infant's early abilities to detect contingencies, particularly socially-relevant associations. Here, we examined whether individual differences in the rate of associative learning at one month of age is an enduring predictor of social, imitative, and discriminative behaviors measured across the human infant's first year. One-month learning rate was predictive of social behaviors at 5, 9, and 12 months of age as well as face-evoked discriminative neural activity at 9 months of age. Learning was not related to general cognitive abilities. These results underscore the importance of early contingency learning and suggest the presence of a basic mechanism underlying the ontogeny of social behaviors
