    Examining the Purdue Pegboard Test for Occupational Therapy Practice

    Background: Occupational therapy ethics require that therapists use current assessment tools that provide useful comparison data. When an assessment only has normative data that is more than 40 years old, it cannot be considered current. The purpose of this study was to examine the past and current use of the Purdue Pegboard Test by occupational therapists and other professionals and to determine if it is beneficial to conduct a large normative study on the Purdue Pegboard Assembly Task (PPAT) in order to bring the test up to date. Method: This was a psychometric study of inter-rater reliability and a small normative study of the PPAT with 150 healthy working adults from MI. Descriptive statistics were used for normative means, standard deviations, and standard errors of measurement. Results: Inter-rater reliability was measured using the intra-class correlation coefficient for the mean of all student-rating teams of seven occupational therapy students. The result of the psychometric study determined the ICC was above .99. During the normative study, 150 participants performed the PPAT for three trials. Norms for gender and ages 18-49 and 50-62 are presented. Conclusion: The result of the inter-rater reliability test determined that OT students can be reliable raters for the PPAT. The normative study collected current norms for healthy working adults in MI, but validity testing and a larger normative study is needed to bring the psychometrics of the PPAT up to date to be generalized for current use by occupational therapists

    Interprofessional education to implement patient falls education in hospitals: Lessons learned

    Aim: The aim of this study was to design, deliver and evaluate an interprofessional education programme for healthcare professionals on how to implement a modified version of the safe recovery programme to prevent falls in hospitalized patients. Design: Mixed methods design incorporating pre- and post education surveys and individual semi-structured interviews. Methods: Thirty-four health professional participants attended a 1-h face-to-face or ZoomÂź interprofessional education session to learn how to deliver an evidence-based patient falls prevention education strategy, the modified Safe Recovery Programme. Results: A 1-hour education session was insufficient to build full confidence to deliver the Safe Recovery Programme. There was no statistically significant change in participant views on interprofessional collaboration. Participants recommended prior consultation and preparation before delivery of IPE, with additional opportunities for discussion and feedback during implementation with patients. The findings highlight the importance of interprofessional education for evidence-based interventions in hospitals.     Bangladesh Norms for a Gender-Specific Functional Fine Dexterity Test (FFDT)

    Background: Good psychometrics and cultural relevance are needed for evidence-based practice. Occupational therapy (OT) assessment tools in Bangladesh have been developed outside of Bangladesh and have not been validated or normed for Bangladeshis. This normative and psychometric study was to provide culturally relevant norms for in Bangladesh for bilateral fine motor. Method: The Functional Fine Dexterity Test (FFDT) consists of two functional/self-care gender-specific task instruments: a shirt with five buttons for males and a pinning board for females. Raters were trained in timing these tasks. Results: Intra-Class Correlation (ICC) scores were \u3e .85 for all rater teams. Participants were timed three times while they completed the task. Convergent validity was examined using a Pearson’s Product-Moment correlation to compare the average of three trials of the FFDT and three trials of the Nine Hole Peg Test (NHPT). Male and female norms were developed for the FFDT using descriptive statistics. The sample included 180 Bangladesh participants. Convergent validity, when compared to the NHPT, ranged from r = .4 to .67 for males (ages 18-29 and 40-49), p \u3c .05; and r = .53 to .76 for females (aged 18-39), p \u3c .05. FFDT norms were developed for gender and age categories. Conclusion: The FFDT is a valid test to use for evaluating fine motor dexterity in Bangladesh. This is the first OT instrument to be studied for culturally relevancy

    Twenty-one lessons: preventing domestic violence in the Caribbean

    Domestic violence infringes the basic right to security of the individual and affects society as a whole – it impacts on demography, education and health systems, the economy, political participation and the overall security of a country. Domestic violence is a universal problem which affects all parts of the social fabric; it transcends ethnic, gender, religious, generational and economic lines. In Barbados and the Eastern Caribbean, beyond the ratification of a number of international instruments, domestic violence is widely recognised as a persistent challenge. On average, more than one in three women in the Caribbean experience gender-based violence in their lifetime; child sexual abuse is also a concern. The causes behind domestic violence in Barbados and the Eastern Caribbean are complex and deeply entrenched in cultural and behavioural norms. The prevention of domestic violence is vital if protection rights are to be effectively safeguarded. To this end, a sustainable and community driven approach -involving civil society and the various stakeholders - is crucial in effecting changes in cultural attitudes towards domestic violence. Early and continuous education about gender based violence and a focus on the prosecution and rehabilitation of perpetrators are both necessary to address the problem at its root. In addition, it is important that the relevant legal framework - and accompanying measures - is fortified and effectively enforced, so that the safety of victims and their families is adequately provided for. A co-ordinated approach between the various stakeholders is important to allow public and private mitigation and response mechanisms toward both protective and restorative safety nets (European Instrument for Democracy and Human Rights (EIDHR), ‘Towards a Future Free from Domestic Violence’, Guidelines for grant applicants 2014)

    Analysis of graft survival in a trial of stem cell transplant in ALS

    Objective The first US Food and Drug Administration–approved clinical trial to treat amyotrophic lateral sclerosis ( ALS ) with neural stem cell–based therapy is in progress. The goal of the current study was to identify and assess the survival of human spinal cord–derived neural stem cells ( HSSC s) transplanted into the spinal cord in patients with ALS . Methods Spinal cords transplanted with HSSCs were examined from six autopsy cases. Homogenized tissues were interrogated for the presence of donor versus recipient DNA using real‐time PCR methods ( qPCR ). Fluorescence in situ hybridization (FISH) was performed using DNA probes for XY chromosomes to identify male donor HSSCs in one female case, and immunohistochemistry (IHC) was used to characterize the identified donor cells. Results Genomic DNA from donor HSSC s was identified in all cases, comprising 0.67–5.4% of total tissue DNA in patients surviving 196 to 921 days after transplantation. In the one female patient a “nest” of cells identified on H&E staining were XY ‐positive by FISH , confirming donor origin. A subset of XY ‐positive cells labeled for the neuronal marker NeuN and stem cell marker SOX 2. Interpretation This is the first study to identify human neural stem cells transplanted into a human spinal cord. Transplanted HSSC s survived up to 2.5 years posttransplant. Some cells differentiated into neurons, while others maintained their stem cell phenotype.
