Increase in HIV sexual risk behaviour in homosexual men in Scotland, 1996–2002: prevention failure?
<b>Objective</b>: To investigate trends in homosexual men’s sexual risk behaviour for HIV infection in Scotland.
<b>Methods</b>: Cross sectional surveys in 1996, 1999, and 2002 were carried out in "gay" bars in Glasgow and Edinburgh, Scotland. 6508 men—2276 (79% response rate) in 1996, 2498 (78%) in 1999, and 1734 (62%) in 2002. <b>Results</b>: In 1996, 10.7% of men surveyed and in 1999, 11.2% reported unprotected anal intercourse (UAI) with casual partners, compared with 18.6% in 2002 (p<0.001).
There was also a significant increase in men reporting that they "knew" their casual partners’ HIV status, despite no increase in HIV testing among men who reported UAI with casual partners. In 2002, increases in UAI with more than one partner, in UAI with casual partners and in reporting seroconcordance remained significant after adjusting for confounding factors including HIV testing status and demographic characteristics.
<b>Conclusions</b>: High risk sexual behaviour among homosexual men in Scotland increased between 1999 and 2002. Men showed increased confidence of shared antibody status, despite no increase in HIV testing, or evidence of discussion of HIV status. Explanations for this must include consideration of a cultural shift in the perception of HIV and "prevention failure" on the part of governments and health agencies
Monopoles and instantons in SU(2) lattice gauge theory
We investigate the monopole-instanton correlation in SU(2) lattice gauge
theory using a renormalisation group inspired smoothing technique. We look at
the properties of monopole clusters and their correlation with instantons.
Since the action of the smoothed configurations is dominated by instantons we
compare the smoothed Monte Carlo lattices to artificially reconstructed
configurations with the same instanton content but no other fluctuations. Both
parallel and randomly rotated (in group space) instanton ensembles are

Coagulation and anticoagulation in idiopathic pulmonary fibrosis
Idiopathic pulmonary fibrosis (IPF) is an incurable, progressive interstitial lung disease with a prognosis that is worse than that of many cancers. Epidemiological studies have demonstrated a link between IPF and thrombotic vascular events. Coagulation and fibrinolytic systems play central roles in wound healing and repair, processes hypothesised to be abnormal within the IPF lung. Animal models of pulmonary fibrosis have demonstrated an imbalance between thrombosis and fibrinolysis within the alveolar compartment, a finding that is also observed in IPF patients. A systemic prothrombotic state also occurs in IPF and is associated with increased mortality, but trials of anticoagulation in IPF have provided conflicting results. Differences in methodology, intervention and study populations may contribute to the inconsistent trial outcomes. The new oral anticoagulants have properties that may prove advantageous in targeting both thrombotic risk and progression of lung fibrosis
The relation between questions indicating transient ischaemic attack and stroke in 20 years of follow up in men and women in the Renfrew/Paisley Study
<b>STUDY OBJECTIVE</b> Transient ischaemic attack (TIA) is often a precursor to stroke, so identification of people experiencing TIA could assist in stroke prevention by indicating those at high risk of stroke who would benefit most from intervention for other stroke risk factors. The objective of this study was to investigate whether answers to a simple questionnaire for TIA could predict the occurrence of stroke in the following 20 years.
DESIGNProspective cohort study, conducted between 1972 and 1976, with 20 years of follow up.
<b>SETTING</b> Renfrew and Paisley, Scotland.
<b>PARTICIPANTS</b> 7052 men and 8354 women aged 45-64 years at the time of screening completed a questionnaire and attended a physical examination. The questionnaire asked participants if they had ever, without warning, suddenly lost the power of an arm, suddenly lost the power of a leg, suddenly been unable to speak properly or suddenly lost consciousness. These four questions were taken as indicators of TIA and were related to subsequent stroke mortality or hospital admission.
<b>MAIN RESULTS</b> For women, each question was significantly related to stroke risk, whereas for men only the question on loss of power of arm was significantly related to stroke risk. Men and women answering two or more questions positively had double the relative rate of stroke compared with men and women answering none of the questions positively, even after adjusting for other risk factors for stroke.
<b>CONCLUSIONS</b> A simple questionnaire for TIA could help predict stroke over 20 years of follow up. Targeting men and women who report TIA with early treatment could help to prevent strokes from occurring
Radiative corrections to the lattice gluon action for highly improved staggered quarks (HISQ) and the effect of such corrections on the static potential
We perform a perturbative calculation of the influence of dynamical HISQ
fermions on the perturbative improvement of the gluonic action in the same way
as we have previously done for asqtad fermions. We find the fermionic
contributions to the radiative corrections in the Luescher-Weisz gauge action
to be somewhat larger for HISQ fermions than for asqtad. Using one-loop
perturbation theory as a test, we estimate that omission of the fermion-induced
radiative corrections in dynamical asqtad simulations will give a measurable
effect. The one-loop result gives a systematic shift of about -0.6% in (r_1/a)
on the coarsest asqtad improved staggered ensembles. This is the correct sign
and magnitude to explain the scaling violations seen in Phi_B on dynamical

Helmet-mounted pilot night vision systems: Human factors issues
Helmet-mounted displays of infrared imagery (forward-looking infrared (FLIR)) allow helicopter pilots to perform low level missions at night and in low visibility. However, pilots experience high visual and cognitive workload during these missions, and their performance capabilities may be reduced. Human factors problems inherent in existing systems stem from three primary sources: the nature of thermal imagery; the characteristics of specific FLIR systems; and the difficulty of using FLIR system for flying and/or visually acquiring and tracking objects in the environment. The pilot night vision system (PNVS) in the Apache AH-64 provides a monochrome, 30 by 40 deg helmet-mounted display of infrared imagery. Thermal imagery is inferior to television imagery in both resolution and contrast ratio. Gray shades represent temperatures differences rather than brightness variability, and images undergo significant changes over time. The limited field of view, displacement of the sensor from the pilot's eye position, and monocular presentation of a bright FLIR image (while the other eye remains dark-adapted) are all potential sources of disorientation, limitations in depth and distance estimation, sensations of apparent motion, and difficulties in target and obstacle detection. Insufficient information about human perceptual and performance limitations restrains the ability of human factors specialists to provide significantly improved specifications, training programs, or alternative designs. Additional research is required to determine the most critical problem areas and to propose solutions that consider the human as well as the development of technology
Crew workload strategies in advanced cockpits
Many methods of measuring and predicting operator workload have been developed that provide useful information in the design, evaluation, and operation of complex systems and which aid in developing models of human attention and performance. However, the relationships between such measures, imposed task demands, and measures of performance remain complex and even contradictory. It appears that we have ignored an important factor: people do not passively translate task demands into performance. Rather, they actively manage their time, resources, and effort to achieve an acceptable level of performance while maintaining a comfortable level of workload. While such adaptive, creative, and strategic behaviors are the primary reason that human operators remain an essential component of all advanced man-machine systems, they also result in individual differences in the way people respond to the same task demands and inconsistent relationships among measures. Finally, we are able to measure workload and performance, but interpreting such measures remains difficult; it is still not clear how much workload is too much or too little nor the consequences of suboptimal workload on system performance and the mental, physical, and emotional well-being of the human operators. The rationale and philosophy of a program of research developed to address these issues will be reviewed and contrasted to traditional methods of defining, measuring, and predicting human operator workload. POPCORN: a Supervisory Control Simulation for Workload and Performance Research
A multi-task simulation of a semi-automatic supervisory control system was developed to provide an environment in which training, operator strategy development, failure detection and resolution, levels of automation, and operator workload can be investigated. The goal was to develop a well-defined, but realistically complex, task that would lend itself to model-based analysis. The name of the task (POPCORN) reflects the visual display that depicts different task elements milling around waiting to be released and pop out to be performed. The operator's task was to complete each of 100 task elements that ere represented by different symbols, by selecting a target task and entering the desired a command. The simulated automatic system then completed the selected function automatically. Highly significant differences in performance, strategy, and rated workload were found as a function of all experimental manipulations (except reward/penalty)
