    Improvement of intraoperative antibiotic prophylaxis in prolonged cardiac surgery by automated alerts in the operating room.

    OBJECTIVE: To assess the impact of an automated intraoperative alert to redose prophylactic antibiotics in prolonged cardiac operations. DESIGN: Randomized, controlled, evaluator-blinded trial. SETTING: University-affiliated hospital. PATIENTS: Patients undergoing cardiac surgery that lasted more than 4 hours after the preoperative administration of cefazolin, unless they were receiving therapeutic antibiotics at the time of surgery. INTERVENTION: Randomization to an audible and visual reminder on the operating room computer console at 225 minutes after the administration of preoperative antibiotics (reminder group, n = 137) or control (n = 136). After another 30 minutes, the circulating nurse was required to indicate whether a follow-up dose of antibiotics had been administered. RESULTS: Intraoperative redosing was significantly more frequent in the reminder group (93 of 137; 68%) than in the control group (55 of 136; 40%) (adjusted odds ratio, 3.31; 95% confidence interval, 1.97 to 5.56; P < .0001). The impact of the reminder was even greater when compared with the 6 months preceding the study period (129 of 480; 27%; P < .001), suggesting some spillover effect on the control group. Redosing was formally declined for 19 of the 44 patients in the reminder group without redosing. The rate of surgical-site infection in the reminder group (5 of 137; 4%) was similar to that in the control group (8 of 136; 6%; P = .42), but significantly lower than that in the pre-study period (48 of 480; 10%; P = .02). CONCLUSION: The use of an automatic reminder system in the operating room improved compliance with guidelines on perioperative antibiotic prophylaxis

    Coeficiente de erodibilidade em sulcos e entressulcosde Argissolos coesos estimado pela cor do solo

    O objetivo deste trabalho foi estimar o coeficiente de erodibilidade em sulcos e entressulcos, por meio de parâmetros da cor do solo. Foram selecionadas três áreas cultivadas com soja, em Argissolos Amarelos coesos do Leste Maranhense. Em cada área, foram retiradas 121 amostras na profundidade de 0,00-0,20 m. Foram determinados os valores de espectroscopia de reflectância difusa na faixa do visível, para obtenção dos valores de matiz, valor e croma. A partir destes constituintes da cor, foi calculado o índice de avermelhamento para cada amostra. Foram desenvolvidos modelos para avaliar o efeito de cada componente da cor sobre a erodibilidade em sulcos e entressulcos. Esses componentes apresentaram R2 que variou de 0,37 a 0,70. Os modelos gerados obtiveram R2 de 0,70 e 0,73, para entressulcos e sulcos, respectivamente. A cor do solo determinada por espectroscopia de reflectância difusa pode ser utilizada como método alternativo para estimar o coeficiente de erodibilidade de Argissolos Amarelos coesos

    Nuclear moments, charge radii and spins of the ground and isomeric states in 175 Yb and 177 Yb

    This paper reports static moments and changes in mean-square charge radii of175, 177, 177mYb measured using collinear laser spectroscopy at the IGISOL facility. The moments are compared to predictions made using the Nilsson model to determine the purity of the multi-quasiparticle T1/2 = 11.4 s, Iπ = 8 state of176Yb and the ground state of177Yb. The ground-state spins of175, 177Yb and the T1/2 = 6.41 s, E = 331.5 keV isomeric state in177Yb, have been measured from the hyperfine structure to be 7/2, 9/2 and 1/2 respectively

    First results from laser spectroscopy on bunched radioactive beams from the JYFL ion-beam cooler

    A new RFQ ion-beam cooler and buncher, installed after the mass-separating magnet of the ion guide isotope separator, IGISOL, JYFL, has dramatically increased the scope of on-line laser spectroscopy at this facility. The device, operated in a bunching mode, has permitted new measurements on short-lived radionuclei in the Ti, Zr and Hf chains at a sensitivity two orders of magnitude greater than that previously achieved. The device has also opened new prospects for laser-based nuclear spectroscopy at the facility, particularly collinear resonance ionisation spectroscopy
