Probability estimates for the unique childhood leukemia cluster in Fallon, Nevada, and risks near other U.S. Military aviation facilities.
A unique cluster of childhood leukemia has recently occurred around the city of Fallon in Churchill County, Nevada. From 1999 to 2001, 11 cases were diagnosed in this county of 23,982 people. Exposures related to a nearby naval air station such as jet fuel or an infectious agent carried by naval aviators have been hypothesized as potential causes. The possibility that the cluster could be attributed to chance was also considered. We used data from the Surveillance, Epidemiology, and End Results Program (SEER) to examine the likelihood that chance could explain this cluster. We also used SEER and California Cancer Registry data to evaluate rates of childhood leukemia in other U.S. counties with military aviation facilities. The age-standardized rate ratio (RR) in Churchill County was 12.0 [95% confidence interval (CI), 6.0-21.4; p = 4.3 times symbol 10(-9)]. A cluster of this magnitude would be expected to occur in the United States by chance about once every 22,000 years. The age-standardized RR for the five cases diagnosed after the cluster was first reported was 11.2 (95% CI, 3.6-26.3). In contrast, the incidence rate was not increased in all other U.S. counties with military aviation bases (RR = 1.04; 95% CI, 0.97-1.12) or in the subset of rural counties with military aviation bases (RR = 0.72; 95% CI, 0.48-1.08). These findings suggest that the Churchill County cluster was unlikely due to chance, but no general increase in childhood leukemia was found in other U.S. counties with military aviation bases
Risk Assessment and Comparative Effectiveness of Left Ventricular Assist Device and Medical Management in Ambulatory Heart Failure Patients The ROADMAP Study 2-Year Results
OBJECTIVES The authors sought to provide the pre-specified primary endpoint of the ROADMAP (Risk Assessment and Comparative Effectiveness of Left Ventricular Assist Device and Medical Management in Ambulatory Heart Failure Patients) trial at 2 years. BACKGROUND The ROADMAP trial was a prospective nonrandomized observational study of 200 patients (97 with a left ventricular assist device [LVAD], 103 on optimal medical management [OMM]) that showed that survival with improved functional status at 1 year was better with LVADs compared with OMM in a patient population of ambulatory New York Heart Association functional class IIIb/IV patients. METHODS The primary composite endpoint was survival on original therapy with improvement in 6-min walk distance \u3e= 75 m. RESULTS Patients receiving LVAD versus OMM had lower baseline health-related quality of life, reduced Seattle Heart Failure Model 1-year survival (78% vs. 84%; p = 0.012), and were predominantly INTERMACS (Interagency Registry for Mechanically Assisted Circulatory Support) profile 4 (65% vs. 34%; p \u3c 0.001) versus profiles 5 to 7. More LVAD patients met the primary endpoint at 2 years: 30% LVAD versus 12% OMM (odds ratio: 3.2 [95% confidence interval: 1.3 to 7.7]; p = 0.012). Survival as treated on original therapy at 2 years was greater for LVAD versus OMM (70 +/- 5% vs. 41 +/- 5%; p \u3c 0.001), but there was no difference in intent-to-treat survival (70 +/- 5% vs. 63 +/- 5%; p = 0.307). In the OMM arm, 23 of 103 (22%) received delayed LVADs (18 within 12 months; 5 from 12 to 24 months). LVAD adverse events declined after year 1 for bleeding (primarily gastrointestinal) and arrhythmias. CONCLUSIONS Survival on original therapy with improvement in 6-min walk distance was superior with LVAD compared with OMM at 2 years. Reduction in key adverse events beyond 1 year was observed in the LVAD group. The ROADMAP trial provides risk-benefit information to guide patient- and physician-shared decision making for elective LVAD therapy as a treatment for heart failure. The Spectrum of Goldstini and Modulini
When supersymmetry is broken in multiple sectors via independent dynamics,
the theory furnishes a corresponding multiplicity of "goldstini" degrees of
freedom which may play a substantial role in collider phenomenology and
cosmology. In this paper, we explore the tree-level mass spectrum of goldstini
arising from a general admixture of F-term, D-term, and almost no-scale
supersymmetry breaking, employing non-linear superfields and a novel gauge
fixing for supergravity discussed in a companion paper. In theories of F-term
and D-term breaking, goldstini acquire a mass which is precisely twice the
gravitino mass, while the inclusion of no-scale breaking renders one of these
modes, the modulino, massless. We argue that the vanishing modulino mass can be
explained in terms of an accidental and spontaneously broken "global"

Single-Scale Natural SUSY
We consider the prospects for natural SUSY models consistent with current
data. Recent constraints make the standard paradigm unnatural so we consider
what could be a minimal extension consistent with what we now know. The most
promising such scenarios extend the MSSM with new tree-level Higgs interactions
that can lift its mass to at least 125 GeV and also allow for flavor-dependent
soft terms so that the third generation squarks are lighter than current bounds
on the first and second generation squarks. We argue that a common feature of
almost all such models is the need for a new scale near 10 TeV, such as a scale
of Higgsing or confinement of a new gauge group. We consider the question
whether such a model can naturally derive from a single mass scale associated
with supersymmetry breaking. Most such models simply postulate new scales,
leaving their proximity to the scale of MSSM soft terms a mystery. This
coincidence problem may be thought of as a mild tuning, analogous to the usual
mu problem. We find that a single mass scale origin is challenging, but suggest
that a more natural origin for such a new dynamical scale is the gravitino
mass, m_{3/2}, in theories where the MSSM soft terms are a loop factor below
m_{3/2}. As an example, we build a variant of the NMSSM where the singlet S is
composite, and the strong dynamics leading to compositeness is triggered by
masses of order m_{3/2} for some fields. Our focus is the Higgs sector, but our
model is compatible with a light stop (with the other generation squarks heavy,
or with R-parity violation or another mechanism to hide them from current
searches). All the interesting low-energy mass scales, including linear terms
for S playing a key role in EWSB, arise dynamically from the single scale
m_{3/2}. However, numerical coefficients from RG effects and wavefunction

CD300LF polymorphisms of inbred mouse strains confer resistance to murine norovirus infection in a cell type-dependent manner
Human norovirus is the leading cause of gastroenteritis worldwide, yet basic questions about its life cycle remain unanswered due to an historical lack of robust experimental systems. Recent studies on the closely related murine norovirus (MNV) have identified CD300LF as an indispensable entry factor for MNV. We compared the MNV susceptibilities of cells from different mouse strains and identified polymorphisms in murine CD300LF which are critical for its function as an MNV receptor. Bone marrow-derived macrophages (BMDMs) from I/LnJ mice were resistant to infection from multiple MNV strains which readily infect BMDMs from C57BL/6J mice. The resistance of I/LnJ BMDMs was specific to MNV, since the cells supported infection of other viruses comparably to C57BL/6J BMDMs. Transduction of I/LnJ BMDMs with C57BL/6J CD300LF made the cells permissible to MNV infection, suggesting that the cause of resistance lies in the entry step of MNV infection. In fact, we mapped this phenotype to a 4-amino-acid difference at the CC\u27 loop of CD300LF; swapping of these amino acids between C57BL/6J and I/LnJ CD300LF proteins made the mutant C57BL/6J CD300LF functionally impaired and the corresponding mutant of I/LnJ CD300LF functional as an MNV entry factor. Surprisingly, expression of the I/LnJ CD300LF in other cell types made the cells infectible by MNV, even though the I/LnJ allele did not function as an MNV receptor in macrophage-like cells. Correspondingly, I/LnJ CD300LF bound MNV virions in permissive cells but not in nonpermissive cells. Collectively, our data suggest the existence of a cell type-specific modifier of MNV entry
Gravitational Waves from Mesoscopic Dynamics of the Extra Dimensions
Recent models which describe our world as a brane embedded in a higher
dimensional space introduce new geometrical degrees of freedom: the shape
and/or size of the extra dimensions, and the position of the brane. These modes
can be coherently excited by symmetry breaking in the early universe even on
``mesoscopic'' scales as large as 1 mm, leading to detectable gravitational
radiation. Two sources are described: relativistic turbulence caused by a
first-order transition of a radion potential, and Kibble excitation of
Nambu-Goldstone modes of brane displacement. Characteristic scales and spectral
properties are estimated and the prospects for observation by LISA are
discussed. Extra dimensions with scale between 10 \AA and 1 mm, which enter the
3+1-D era at cosmic temperatures between 1 and 1000 TeV, produce backgrounds
with energy peaked at observed frequencies in the LISA band, between
and Hz. The background is detectable above instrument and
astrophysical foregrounds if initial metric perturbations are excited to a
fractional amplitude of or more, a likely outcome for the

Goldstini
Supersymmetric phenomenology has been largely bound to the hypothesis that
supersymmetry breaking originates from a single source. In this paper, we relax
this underlying assumption and consider a multiplicity of sectors which
independently break supersymmetry, thus yielding a corresponding multiplicity
of goldstini. While one linear combination of goldstini is eaten via the
super-Higgs mechanism, the orthogonal combinations remain in the spectrum as
physical degrees of freedom. Interestingly, supergravity effects induce a
universal tree-level mass for the goldstini which is exactly twice the
gravitino mass. Since visible sector fields can couple dominantly to the
goldstini rather than the gravitino, this framework allows for substantial
departures from conventional supersymmetric phenomenology. In fact, this even
occurs when a conventional mediation scheme is augmented by additional
supersymmetry breaking sectors which are fully sequestered. We discuss a number
of striking collider signatures, including various novel decay modes for the
lightest observable-sector supersymmetric particle, gravitinoless
gauge-mediated spectra, and events with multiple displaced vertices. We also

Low Mass X-ray Binaries and Globular Clusters in Early-Type Galaxies
(Abridged) A high fraction of the Low Mass X-ray Binaries (LMXBs) in
early-type galaxies are associated with globular clusters (GCs). Here, we
discuss the correlations between LMXBs and GCs in a sample of four early-type
galaxies. There is some evidence that the fraction of LMXBs associated with GCs
(f_X-GC) increases along the Hubble sequence from spiral bulges to S0s to Es to
cDs. On the other hand, the fraction of globular clusters which contain X-ray
sources appears to be roughly constant at f_GC-X ~ 4%. There is a strong
tendency for the X-ray sources to be associated with the optically more
luminous GCs. However, this correlation is consistent with a constant
probability of finding a LMXB per unit optical luminosity; it seems to result
primarily from the larger number of stars in optically luminous GCs. The
probability of finding a bright LMXB per unit optical luminosity in the GCs is
about 1.5e-7 LMXBs per L_solar,I for L_X >~ 1e38 erg/s, and rises to about
2.0e-7 LMXBs per L_solar,I at lower X-ray luminosities, L_X >~ 3e37 erg/s. This
frequency appears to be roughly constant for different galaxies, including the
bulges of the Milky Way and M31. There is a tendency for the X-ray sources to
be found preferentially in redder GCs. This seems to indicate that the
evolution of X-ray binaries in a GC is affected either by the metallicity or
age of the GC, with younger and/or more metal rich GCs having more LMXBs. There
is a weak tendency for the brightest LMXBs, whose luminosities exceed the
Eddington luminosity for a 1.4 M_solar neutron star, to avoid GCs. That may
indicate that black hole X-ray are somewhat less likely to be found in GCs, as

STX209 (Arbaclofen) for Autism Spectrum Disorders: An 8-Week Open-Label Study
STX209 (arbaclofen), a selective GABA-B agonist, is hypothesized to modulate the balance of excitatory to inhibitory neurotransmission, and has shown preliminary evidence of benefit in fragile X syndrome. We evaluated its safety, tolerability, and efficacy in non-syndromic autism spectrum disorders, in an 8-week open-label trial enrolling 32 children and adolescents with either Autistic Disorder or Pervasive Developmental Disorder—Not Otherwise Specified, and a score ≥17 on the Aberrant Behavior Checklist (ABC)—Irritability subscale. STX209 was generally well-tolerated. The most common adverse events were agitation and irritability, which typically resolved without dose changes, and were often felt to represent spontaneous variation in underlying symptoms. Improvements were observed on several outcome measures in this exploratory trial, including the ABC-Irritability (the primary endpoint) and the Lethargy/Social Withdrawal subscales, the Social Responsiveness Scale, the CY-BOCS-PDD, and clinical global impression scales.
