ISSLS PRIZE in Clinical Science 2023: comparison of degenerative MRI features of the intervertebral disc between those with and without chronic low back pain. An exploratory study of two large female populations using automated annotation
Objectives
The relationship of degeneration to symptoms has been questioned. MRI detects apparently similar disc degeneration and degenerative changes in subjects both with and without back pain. We aimed to overcome these problems by re-annotating MRIs from asymptomatic and symptomatics groups onto the same grading system.
Methods
We analysed disc degeneration in pre-existing large MRI datasets. Their MRIs were all originally annotated on different scales. We re-annotated all MRIs independent of their initial grading system, using a verified, rapid automated MRI annotation system (SpineNet) which reported degeneration on the Pfirrmann (1–5) scale, and other degenerative features (herniation, endplate defects, marrow signs, spinal stenosis) as binary present/absent. We compared prevalence of degenerative features between symptomatics and asymptomatics.
Results
Pfirrmann degeneration grades in relation to age and spinal level were very similar for the two independent groups of symptomatics over all ages and spinal levels. Severe degenerative changes were significantly more prevalent in discs of symptomatics than asymptomatics in the caudal but not the rostral lumbar discs in subjects
Conclusions
We confirmed age and disc level are significant in determining imaging differences between asymptomatic and symptomatic populations and should not be ignored. Automated analysis, by rapidly combining and comparing data from existing groups with MRIs and information on LBP, provides a way in which epidemiological and ‘big data’ analysis could be advanced without the expense of collecting new groups.

Experiences of living with chronic back pain: The physical disabilities
Purpose. Back-related functional limitations are largely assessed using lists of activities, each scored on a yes/no basis and the scores then summed. This provides little information about how chronic back pain (CBP) patients live with their condition. This study describes the consequences of living day-to-day with CBP and documents the 'insider' accounts of its impact on daily life.
Method. Unstructured interviews, using the 'Framework' approach with topic guide, were recorded and transcribed verbatim. Subjects were sampled for age, sex, ethnicity and occupation from new referrals with back pain to a rheumatology outpatient clinic. Eleven subjects (5 male, 6 female) were interviewed either in English (n = 9) or their preferred language (n = 2). Interviews were read in-depth twice to identify the topics. Data were extracted in phrases and sentences using thematic content analysis.
Results. Four themes emerged: sleep/rest, mobility, independence and leisure. All subjects reported issues about sleep and rest, nine about mobility, seven about independence and six on leisure. Most descriptions concerned loss and limitation in daily life. Strategies for coping with sleep disruption and physical limitations were described.
Conclusions. Subjects provided graphic 'in-depth' descriptions of experiences living with CBP every day; expressed regret at the loss of capabilities and distress at the functional consequences of those losses. Facilitating 'adjustment' to 'loss' may be more helpful than inferring the potential for a life free of pain as a result of therapeutic endeavours
Dysplasia epiphysealis hemimelica of the talus
Dysplasia epiphysealis hemimelica is a rare developmental disorder with unknown etiology affecting epiphysis in childhood. The lesion is an osteochondroma arising from the epiphysis and increasing in size until skeletal maturity is reached. Surgical treatment is mandatory when symptoms such as pain, joint impingement or deformation are present, and yields good results when the mass is juxtaarticular or extraarticular. In those cases where articular symptoms are not present and only mass evolution is observed, surgical treatment is not recommended before skeletal maturity has been reached. A case of DEH located in the talus and successfully treated with surgery is presented
Measurement of the velocity distribution of sputtered Na atoms from NaCl by Doppler shift laser fluorescence
The velocity distribution of sputtered Na atoms from NaCI during 15 keV, Ar+ bombardment was measured by a Doppler shift laser fluorescence technique. Contrary to other measurements, the sputtered Na atoms were found to have a velocity distribution identical to that of a three-dimensional Maxwell-Boltzmann gas.Peer reviewedElectrical and Computer Engineerin
Observation of single collisionally cooled trapped ions in a buffer gas
Individual Ba ions are trapped in a gas-filled linear ion trap and observed
with a high signal-to-noise ratio by resonance fluorescence. Single-ion storage
times of ~5 min (~1 min) are achieved using He (Ar) as a buffer gas at
pressures in the range 8e-5 - 4e-3 torr. Trap dynamics in buffer gases are
experimentally studied in the simple case of single ions. In particular, the
cooling effects of light gases such as He and Ar and the destabilizing
properties of heavier gases such as Xe are studied. A simple model is offered
to explain the observed phenomenology.Comment: 5 pages, 4 figures, accepted for publication in Phys. Rev. A. Minor

Measurement of the Spectral Shape of the beta-decay of 137Xe to the Ground State of 137Cs in EXO-200 and Comparison with Theory
We report on a comparison between the theoretically predicted and
experimentally measured spectra of the first-forbidden non-unique -decay
transition ^{137}\textrm{Xe}(7/2^-)\to\,^{137}\textrm{Cs}(7/2^+). The
experimental data were acquired by the EXO-200 experiment during a deployment
of an AmBe neutron source. The ultra-low background environment of EXO-200,
together with dedicated source deployment and analysis procedures, allowed for
collection of a pure sample of the decays, with an estimated
signal-to-background ratio of more than 99-to-1 in the energy range from 1075
to 4175 keV. In addition to providing a rare and accurate measurement of the
first-forbidden non-unique -decay shape, this work constitutes a novel
test of the calculated electron spectral shapes in the context of the reactor
antineutrino anomaly and spectral bump.Comment: Version as accepted by PR
'Searching for a needle in a haystack;' A Ba-tagging approach for an upgraded nEXO experiment
nEXO is a proposed experiment that will search for neutrinoless double-beta
decay (0) in 5-tonnes of liquid xenon (LXe), isotopically
enriched in Xe. A technique called Ba-tagging is being developed as a
potential future upgrade for nEXO to detect the Xe double-beta decay
daughter isotope, Ba. An efficient Ba-tagging technique has the
potential to boost nEXO's 0 sensitivity by essentially
suppressing non-double-beta decay background events. A conceptual approach for
the extraction from the detector volume, trapping, and identification of a
single Ba ion from 5 tonnes of LXe is presented, along with initial results
from the commissioning of one of its subsystems, a quadrupole mass filter.Comment: 4 pages, 2 figure
Investigation of radioactivity-induced backgrounds in EXO-200
The search for neutrinoless double-beta decay (0{\nu}{\beta}{\beta}) requires
extremely low background and a good understanding of their sources and their
influence on the rate in the region of parameter space relevant to the
0{\nu}{\beta}{\beta} signal. We report on studies of various {\beta}- and
{\gamma}-backgrounds in the liquid- xenon-based EXO-200 0{\nu}{\beta}{\beta}
experiment. With this work we try to better understand the location and
strength of specific background sources and compare the conclusions to
radioassay results taken before and during detector construction. Finally, we
discuss the implications of these studies for EXO-200 as well as for the
next-generation, tonne-scale nEXO detector.Comment: 9 pages, 7 figures, 3 table
