    Xanthogranulomatous oophoritis presenting as a pseudotumor of the ovary: a clinical dilemma

    Background: Inflammatory Pseudotumor of ovary is a distinct benign entity characterized by the presence of spindle cells mixed with variable amount of lymphocytes and plasma cells. Female genital tract is an unusual location for xanthogranulomatous inflammation and if present usually involves the endometrium. xanthogranulomatous oophoritis is rare and only a handful of cases have been reported.Methods: At The Gujarat Cancer and Research Institute, Ahmedabad; a Regional Cancer Centre we came across two cases of Xanthogranulomatous oophoritis, a rare clinical condition. In view of the rarity of the clinical entity, the cases are summarized and discussed.Results: Xanthogranulomatous oophoritis is a rare clinical entity with very few reported cases. Its pathogenesis involves chronic inflammatory process of unknown origin. Pre-operative diagnosis has been challenging due to non-specific presenting symptoms and radiological imaging. Characteristic microscopic picture includes presence of well-differentiated spindle cells with variable amount of collagenous stroma and presence of inflammatory changes. Immunohistochemistry performed at our institute was found to be positive for CD 68, Vimentin and S-100 for xanthogranulomatous oophoritis.Conclusions: Data from radiological imaging for extra genital xanthogranulomatous inflammation is being extrapolated to determine a pre-operative diagnosis for xanthogranulomatous oophoritis of the genital tract. However, no pathognomic radiological findings are defined at present making histopathological diagnosis the mainstay for diagnosis of this condition. Immunohistochemistry panel markers play a helpful role in the diagnosis when in doubt. Surgery is the definitive mode of treatment providing a good prognosis post operatively

    A Study of Low Energy Ar+ Ion Bombardment Induced Lattice Damage in (100) n-GaAs by Channeling

    It has been found that sputter cleaning of GaAs single crystals results in damage or loss of crystallinity in the first few tens of nanometers of the crystal. The damage is a result of both - energetic Ar ions impinging on and being incorporated in the GaAs surface and the resultant preferential sputtering of As from the GaAs lattice. A study of this damage which was done at room temperature and at -110 °C was made by the use of Selected Area Electron Channeling Patterns. The relationship between the degree of surface disorder, as shown by the pattern degradation, and sputtering parameters (ion beam voltage and ion beam dose) was experimentally obtained. The energy regime investigated was 0.5 to 5 keV with sputtering times from 5 to 20 minutes. The results showed increasing contrast degradation in the selected area channeling patterns (SACPs) with increasing incident ion energies from 0.5 to 4 keV, a maxima in contrast degradation for a sputtering time of 10 minutes and greater contrast degradation at room temperature than at -110 °C

    Using the Systemic Immune-Inflammation Index (SII) as a mid-treatment marker for survival among patients with stage-III locally advanced non-small cell lung cancer (NSCLC)

    The Systemic Immune-Inflammation Index (SII) is an important marker of immune function, defined as the product of neutrophil-to-lymphocyte ratio (NLR) and platelet count (P). Higher baseline SII levels have been associated with improved survival in various types of cancers, including lung cancer. Data were obtained from PROCLAIM, a randomized phase III trial comparing two different chemotherapy regimens pemetrexed + cisplatin (PEM) vs. etoposide + cisplatin (ETO), in combination with radiotherapy (RT) for the treatment of stage III non-squamous non-small cell lung cancer (NSCLC). We aimed to determine if SII measured at the mid-treatment window for RT (weeks 3-4) is a significant predictor of survival, and if the effect of PEM vs. ETO differs by quartile (Q) level of SII. Hazard-ratios (HR) for survival were estimated using a proportional hazards model, accounting for the underlying correlated structure of the data. A total of 548 patients were included in our analysis. The median age at baseline was 59 years. Patients were followed for a median of 24 months. Adjusting for age, body mass index, sex, race, and chemotherapy regimen, SII was a significant mid-treatment predictor of both overall (adjusted HR (aHR) = 1.6

    Percutaneous transolecranon pinning and lateral pinning Vs lateral pinning in displaced supracondylar fractures of humerus in children: an observative study

    Background: Supracondylar fracture (humerus) is type of extra-articular fracture occurring in the distal metaphyseal site of humerus. It is almost exclusively a fracture of the immature skeleton, seen in children and young teenagers. Fractures around the elbow are a great challenge to orthopaedic surgeons. Clinical diagnosis may be difficult due to noncooperative patient and massive swelling around the elbow. Displaced type of supracondylar fractures poses problem not only in reduction but also in maintenance of reduced fracture   and   rapid   inclusion   of nerves and vessels.Methods: The present study was conducted on 30 cases of displaced supracondylar fracture humerus in children, aged 2-14 years, who were treated by CRPP with either lateral entry of k-wires or a lateral wire and a vertical wire through olecranon (transolecranon).Results: Both the Groups achieved 90% satisfactory results, but 10% unsatisfactory results recorded in Group A only rather than in Group B.Conclusions: Although the transolecranon wire has the disadvantage of limiting the flexion and extension of the elbow, this does not influence the final-outcome much as the elbow is fixed in a POP splint for minimum 3 weeks-in all patients in both groups

    A pilot study examining the prognostic utility of tumor shrinkage on cone-beam computed tomography (CBCT) for stage III locally advanced non-small cell lung cancer patients treated with definitive chemoradiation

    There has been growing interest in utilizing information from cone-beam computed tomography (CBCT) to help guide both treatment delivery and prognosis. In this assessment of locally advanced unresectable stage III non-small cell lung cancer (NSCLC) treated with definitive chemoradiation, we aimed to determine the survival advantage associated with using CBCT to measure tumor regression. Patient, tumor, and treatment characteristics were collected. The serial tumor shrinkage for each patient was determined from tumor volume contours on weekly CBCTs. Survival analysis was performed using the Kaplan-Meier technique and a Cox proportional hazards model. At least two-thirds of patients had a tumor volume reduction of at least 5% after each week of chemoradiation. A weekly reduction in tumor volume of 5% or greater seen on the CBCT images during radiation therapy was significantly associated with improved overall survival, which remained significant when adjusted for age, histology, grade, and T- and N-stages

    Based on the availability of many nutrients, Moringa oleifera tree leaves have been widely employed as nutrients and nutraceuticals in recent years. The leaves contain a small amount of anti-nutritional factors and are abundant in innumerable bioactive compounds. Recently, in several in vivo and in vitro investigations, moringa leaves’ bioactive components and functionality are highlighted. Moringa leaves provide several health advantages, including anti-diabetic, antibacterial, anti-cancer, and anti-inflammatory properties. The high content of phytochemicals, carotenoids, and glucosinolates is responsible for the majority of these activities as reported in the literature. Furthermore, there is growing interest in using moringa as a value-added ingredient in the development of functional foods. Despite substantial study into identifying and measuring these beneficial components from moringa leaves, bioaccessibility and bioavailability studies are lacking. This review emphasizes recent scientific evidence on the dietary and bioactive profiles of moringa leaves, bioavailability, health benefits, and applications in various food products. This study highlights new scientific data on the moringa leaves containing nutrient and bioactive profiles, bioavailability, health benefits, and uses in various food items. Moringa has been extensively used as a health-promoting food additive because of its potent protection against various diseases and the widespread presence of environmental toxins.     Pretreatment neutrophil-to-lymphocyte ratio as an important prognostic marker in stage III locally advanced non-small cell lung cancer: Confirmatory results from the PROCLAIM phase III clinical trial

    Background: Neutrophil-to-lymphocyte ratio (NLR) is an important pretreatment marker of systemic inflammation and tumor aggressiveness. Increased levels of this ratio have been associated with reduced survival in several observational studies of lung cancer. However, supporting analyses from large clinical trial data are lacking. Methods: To validate the prognostic role of NLR, the current study evaluated data from a randomized phase III study (PROCLAIM; clinicaltrial.gov ID: NCT00686959) of patients with stage IIIA/B, unresectable, non-squamous, non-small cell lung cancer (NSCLC), originally comparing combination pemetrexed-cisplatin chemoradiotherapy with etoposide-cisplatin chemoradiotherapy. Adjusted hazard ratios (aHR) and 95% confidence intervals (CI) for survival were estimated using a Cox proportional hazards model. Models were adjusted for age, race, sex, stage, treatment, and body mass index (BMI). Patients were followed for a median of 24 months. Results: Increased NLR levels at baseline were associated with reduced overall ( Conclusions: These findings provide substantiating evidence that NLR, which is routinely available from standard blood testing of patients diagnosed with NSCLC, is an important inflammation-based prognostic marker for survival among patients with locally advanced disease undergoing chemoradiation. Future research will benefit by assessing the prognostic potential of NLR in the context of genetic mutations and molecular markers

    Use of polyethylene glycol coatings for optical fibre humidity sensing

    Humidity induced change in the refractive index and thickness of the polyethylene glycol (PEG) coatings are in situ investigated for a range from 10 to 95%, using an optical waveguide spectroscopic technique. It is experimentally demonstrated that, upon humidity change, the optical and swelling characteristics of the PEG coatings can be employed to build a plastic fibre optic humidity sensor. The sensing mechanism is based on the humidity induced change in the refractive index of the PEG film, which is directly coated onto a polished segment of a plastic optical fibre with dip-coating method. It is observed that PEG, which is a highly hydrophilic material, shows no monotonic linear response to humidity but gives different characteristics for various ranges of humidity levels both in index of refraction and in thickness. It undergoes a physical phase change from a semi-crystal line structure to a gel one at around 80% relative humidity. At this phase change point, a drastic decrease occurs in the index of refraction as well as a drastic increase in the swelling of the PEG film. In addition, PEG coatings are hydrogenated in a vacuum chamber. It is observed that the hydrogen has a preventing effect on the humidity induced phase change in PEG coatings. Finally, the possibility of using PEG coatings in construction of a real plastic fibre optic humidity sensor is discussed. (C) 2008 The Optical Society of Japan
