    Ayurveda is considered by many scholars to be the oldest healing science. In sanskrit, Ayurveda means the Science of life. Ayurvedic knowledge originated in India more than 5000 years ago and is often called the Mother of healing. Ancient surgical science- Shalya Tantra is one of the vital components of Ayurveda science which involves surgical and para-surgical interventions. The Shalya chikitsa deals with different surgical approaches for the management of various diseases such as Bhagandra, Pilonidal sinus, Arshas etc. Shalya Tantra embraces all processes aiming at the removal of factors responsible for producing pain or misery to the body or mind. Acharya sushruta has mentioned Trividha Karma and Ashtavidha Shastra Karma as versatile approaches for therapeutic purposes. The concept of Ashtavidha karma is a unique contribution of Acharya Sushruta. These eight specific surgical procedures are useful in the management of all the diseases which require surgical intervention. In the present time though modern surgery has developed a lot but the basic procedures used in major conditions remained same. These eight basic surgical procedures mentioned by Sushruta are equally applied even today with required modifications to manage the diseases which require surgery including surgical emergency conditions. This article summarizes role of Ashtvidha Shastra Karma in Shalya karma for the management of various surgical problems

    Nowadays, esterases represent high growth potential in the World Industrial Enzymes Market. The industrial applications of esterases provide an immense contribution to the eco-friendly approaches towards nature as also in the food, textile industries as additives in detergents, agrochemical (herbicides, insecticides) industries, and bioremediation. The current review is focused mainly on different medical aspects of esterases because they find implications in treatment of cancer, cardiovascular diseases and neuropathy. The role of esterase has been observed in the treatment of hereditary angioedema (HAE). The arylesterase (ARE) has been found to show antioxidant activity in plasma cells and its low activity resulted in multiple myelomas accounting for 1% of neoplastic diseases and 13% of hematological malignancies in U. S. A. Lung cancer is one of the most lethal forms of cancers and reports have indicated that tumors of the lung, colon and liver had high esterase expressions and therefore could be exploited for selective drug conversion using an ester prodrug strategy. Human serum paraoxonase (PON1) and arylesterase (ARE) are lipophilic antioxidant enzymes. Hypercholesterolaemia has been found to be associated with reduced PON1 enzyme activities and patients are under increased oxidative stress. Esterases from animal and plant origins have been used as effective drugs and as detection probes for toxic compounds using chiral technology

    Case series of pubic bone diastasis causing severe pelvic girdle pain in pregnancy

    Pelvic girdle pain (PGP) is a pregnancy discomfort that causes pain and limitation of mobility and functioning in any of the three pelvic joints. The patient usually presents antenatally with persistent suprapubic pain which is exaggerated during moving, walking or climbing stairs. Intrapartum, this could be associated with disruption of sacroiliac joint, hematuria and bladder dysfunction in severe cases. Pelvic X-rays, ultrasound, and magnetic resonance imaging aid in confirmation of diagnosis by measuring the degree of separation of symphysis. Treatment modalities range from conservative management to orthopedic interventions in form of pelvic strapping, open reduction and internal fixation. Postpartum pain often masks clinicians to make the diagnosis of pubic symphysis diastasis. A case series of three cases which were diagnosed and confirmed with ultrasound and managed with orthopedic consultation. till their delivery and in post-partum period till recovery. Although there is still no specific consensus on treatment guideline, management generally of conservative management to surgical in the form of pelvic bracing or strapping. Awareness of this rare condition can help in management of pain and associated disability which improves post-delivery

    Correlation of severity of hyperandrogenism with ovarian stroma to area ratio and stromal echogenicity in polycystic ovary syndrome patients

    Background: Polycystic ovary syndrome is one of the most common endocrine disorders of reproductive age group. Rotterdam consensus although includes ovarian volume in diagnosis of PCOS but its ovarian stroma area that corresponds histological and thus its clinical features.Methods: A prospective study conducted on 55 PCOS cases and 55 age and weight matched healthy volunteers. Detailed medical, menstrual history and examination for clinical hyperandrogenism and anthropometry was done. Fasting venous blood samples were obtained on day 2 for androgen profiles. All the patients were subjected to pelvic ultrasound and also a 3D ultrasound was done. The S/A (ovarian stroma to area ratio) were obtained by outlining the peripheral profile of the stroma with a caliper and similarly total ovarian area by outlining in the maximum plane section. Stromal echogenicity was assessed semi quantatively.Results: S/A exhibits positive and significant correlation with DHEAS (r = 0.565, p0.05) in cases, whereas stoma echogenicity was increased in 78.2% cases and 20% controls and correlated significantly with DHEAS levels and not H score, probably due to small sample size.Conclusions: S/A bears a positive significant correlation with androgenic parameters studied and can be considered as one of the best predictor of hyperandrogenism. The stroma echogenicity measured semiquantatively can act as a clue for PCOS diagnosis. Their measurement and follow up during course of treatment may show significant role in defining the severity, progression or regression of the disease

    Semiotic analysis of Indian television advertisements and its impact on consumers: an exploratory study

    Everyday consumers are exposed to a huge and wide variety of advertisements. These advertisements fall into different categories of communication media such as television, print media e.g., magazines and newspapers, cinema or billboards, radio etc. Advertising now reaches far more people than it used to with traditional media since it has major presence in new digital media which has also transformed immensely in recent time. These advertisements deliver and also utilise a wide range of meaning, symbols and messages also called semiotics in their advertisements. Importantly, large part of any individual is surrounded by lot of signs and symbols, however, the way they comprehend these sign, symbols and meaning differ from one to other individual. Since, India is a vast country with vivid and varied culture and demographics, it becomes a challenge for advertisers to target and attract right consumers through their advertisements. In such a situation it is essential for advertisers to understand the choice consumers have for advertisements and the differential impact it has on the consumers. Our study shows the differential impact advertisements have through their themes, colours, to be more specific, impact that semiotics have on consumers and how it can be made more effective and targeted by understanding the language and impact of semiotics on consumers in India

    The Bethesda system for reporting of thyroid cytopathology and its category wise management

    Background: Fine-needle aspiration cytology (FNAC) of thyroid is a rapid diagnostic test for thyroid nodules. The Bethesda system classifies thyroid FNAC into six categories. Each category is linked to a malignancy risk and has recommended clinical management. The aim of this study was to analyze the thyroid cytology smears by Bethesda system and to assess the frequency of various categories and to correlate it previous published studies. Category wise management is also discussed in detail.Methods: A total of 388 thyroid FNAC samples were examined from January 2017 to June 2018 in the Department of Pathology, in Medanta- The Medicity Gurugram.Results: A total of 388 cases were reported according to Bethesda system of reporting. Categories were as follows: I (14.69%), II (61.85%), III (11.34%), IV (2.57%), V (2.660%), VI (4.91%) nodules.Conclusions: The Bethesda System for reporting thyroid cytopathology proved to be an excellent reporting system and it puts clinician and the cytopathologist on the same page and easier to communicate regarding diagnosis

    TRIVIDHA KARMA IN SURGICAL PARLANCE- A CONCEPTUAL STUDY

    In Ayurvedic classics there are various types of treatment and Shastra chikitsa is one among them. Purva Karma Pradhana Karma, Paschat Karma are Trividha Karma. According to Acharya Sushruta, Purva Karma means preparation of patient along with collecting all the materials needed during the Pradhana karma. Ashtaviddha Shastra Karma is included in Pradhana Karma. Paschat Karma includes post operative care. Sushruta division of surgical activity into three parts i.e., pre- operative, operative and post-operative based on sound scientific principles. Sushruta has also described the pre-operative appreciation of foreign body, its size, shape, and exact location within the body and appropriate instrument for its removal should be selected pre-operatively. He has also mentioned the pre-operative diet and starvation for various types of surgeries. He has also emphasized that asepsis and antisepsis precaution should be taken, wound should be protected from dangerous and invisible creatures (Nishachara). Fumigation of Vranitaagara should be done for ten days, twice a day. By virtue of this article, we can understand the systematic method of arranging the surgical experience of arranging the surgical experience of the older surgeon, about preliminary measures, principal measures and after measures. All the procedures included under these three headings i.e., Trividdha karma plays an important role in successful and complication free surgery
