In this sequel, we develop some new properties on incomplete extended Riemann-Liouville fractional derivative operators with the help of new incomplete extended Beta functions.     Juvenile Onset Clinically Amyopathic Dermatomyositis Presenting as Calcinosis Cutis

    Dermatomyositis (DM) is an autoimmune inflammatory disorder characterized by involvement of muscles and skin. Classical dermatomyositis (CDM) patients display the hallmark cutaneous manifestations of dermatomyositis (DM), proximal muscle weakness, and laboratory evidence of myositis. Rarely, DM presents with cutaneous features of the disease without muscle involvement for a period of more than six months. Such cases are classified into a category of clinically amyopathic dermatomyositis (CADM) which includes amyopathic DM and hypomyopathic DM. We present a case of a 14-year-old child who presented with calcinosis cutis and cutaneous findings suggestive of the disease without muscle involvement.     Maternal Health Situation in India: A Case Study

    Since the beginning of the Safe Motherhood Initiative, India has accounted for at least a quarter of maternal deaths reported globally. India's goal is to lower maternal mortality to less than 100 per 100,000 livebirths but that is still far away despite its programmatic efforts and rapid economic progress over the past two decades. Geographical vastness and sociocultural diversity mean that maternal mortality varies across the states, and uniform implementation of health-sector reforms is not possible. The case study analyzes the trends in maternal mortality nationally, the maternal healthcare-delivery system at different levels, and the implementation of national maternal health programmes, including recent innovative strategies. It identifies the causes for limited success in improving maternal health and suggests measures to rectify them. It recommends better reporting of maternal deaths and implementation of evidence-based, focused strategies along with effective monitoring for rapid progress. It also stresses the need for regulation of the private sector and encourages further public-private partnerships and policies, along with a strong political will and improved management capacity for improving maternal health

    Comparative Evaluation of the Complementary and Alternative Medicine Therapy and Conventional Therapy Use for Musculoskeletal Disorders Management and Its Association with Job Satisfaction among Dentists of West India

    ABSTRACTMusculoskeletal problems have become a significant issue in the profession of dentistry. There are currently no recommended effective disease-preventing and modifying remedies. High prevalence rates for musculoskeletal disorders (MSDs) among dentists have been reported in the literature. Complementary and alternative medicine can be helpful in managing and preventing the MSDs. The purpose of this study was to determine if dentists in the western part of India are using complementary and alternative medicine therapies for MSDs, and also to find if those who use complementary and alternative medicine therapies have greater job/career satisfaction compared to conventional therapy (CT) users. Dentists of western India registered under the Dental Council of India (N=2166) were recruited for the study. Data were analyzed using univariate and bivariate analyses and logistic regression. A response rate of 73% (n=1581) was obtained, of which 79% (n=1249) was suffering from MSDs. The use of complementary and alternative medicine or CT was reported by 90% (n=1124) of dentists with MSDs. Dentists using complementary and alternative medicine reported greater health (P<0.001) and carrier satisfaction (P<0.001) and were able to work as many hours they wanted (P<0.001) compared to CT users. Complementary and alternative medicine therapies may improve the quality of life and enhance job satisfaction for a dentist who suffers from MSDs

    Proteomic and Phospho-Proteomic Profile of Human Platelets in Basal, Resting State: Insights into Integrin Signaling

    During atherogenesis and vascular inflammation quiescent platelets are activated to increase the surface expression and ligand affinity of the integrin αIIbβ3 via inside-out signaling. Diverse signals such as thrombin, ADP and epinephrine transduce signals through their respective GPCRs to activate protein kinases that ultimately lead to the phosphorylation of the cytoplasmic tail of the integrin αIIbβ3 and augment its function. The signaling pathways that transmit signals from the GPCR to the cytosolic domain of the integrin are not well defined. In an effort to better understand these pathways, we employed a combination of proteomic profiling and computational analyses of isolated human platelets. We analyzed ten independent human samples and identified a total of 1507 unique proteins in platelets. This is the most comprehensive platelet proteome assembled to date and includes 190 membrane-associated and 262 phosphorylated proteins, which were identified via independent proteomic and phospho-proteomic profiling. We used this proteomic dataset to create a platelet protein-protein interaction (PPI) network and applied novel contextual information about the phosphorylation step to introduce limited directionality in the PPI graph. This newly developed contextual PPI network computationally recapitulated an integrin signaling pathway. Most importantly, our approach not only provided insights into the mechanism of integrin αIIbβ3 activation in resting platelets but also provides an improved model for analysis and discovery of PPI dynamics and signaling pathways in the future

    Enhancing accuracy in brain stroke detection: Multi-layer perceptron with Adadelta, RMSProp and AdaMax optimizers

    The human brain is an extremely intricate and fascinating organ that is made up of the cerebrum, cerebellum, and brainstem and is protected by the skull. Brain stroke is recognized as a potentially fatal condition brought on by an unfavorable obstruction in the arteries supplying the brain. The severity of brain stroke may be reduced or controlled with its early prognosis to lessen the mortality rate and lead to good health. This paper proposed a technique to predict brain strokes with high accuracy. The model was constructed using data related to brain strokes. The aim of this work is to use Multi Layer Perceptron (MLP) as a classification technique for stroke data and used multi-optimizers that include Adaptive moment estimation with Maximum (AdaMax), Root Mean Squared Propagation (RMSProp) and Adaptive learning rate method (Adadelta). The experiment shows RMSProp optimizer is best with a data training accuracy of 95.8% and a value for data testing accuracy of 94.9%. The novelty of work is to incorporate multiple optimizers alongside the MLP classifier which offers a comprehensive approach to stroke prediction, providing a more robust and accurate solution. The obtained results underscore the effectiveness of the proposed methodology in enhancing the accuracy of brain stroke detection, thereby paving the way for potential advancements in medical diagnosis and treatment

    Dermatopathia pigmentosa reticularis is a rare ectodermal dysplasia with a triad of generalized reticulate hyperpigmentation, noncicatricial alopecia, and onychodystrophy. We report a case of a 21 year old woman who had generalized reticulate pigmentation, diffuse noncicatricial alopecia and onychodystrophy of finger and toe nails. Along with this triad she had palmoplantar keratoderma and poorly developed dermatoglyphics. There was no evidence of involvement of other ectodermally derived organ

    Aggressive digital papillary adenocarcinoma (ADPA) is a rare tumour of eccrine origin. They present as asymptomatic nodules with flattened and thickened surface skin, most commonly on fingers and toes. It mimics many benign entities and tumours, hence leading to a delay in diagnosis. It has a high metastasizing potential, but is slowly progressive. Histopathologically, it has both a nodular and a cystic component with papillary projections into cystic spaces. We present a 54-year man with a tumorous mass on right heel, which was diagnosed by histopathology and immunohistochemistry as ADPA. Our case had ADPA lesion, a rarely reported malignancy on an uncommon site with distant metastasis
