    Psychosocial factors during the first year after a coronary heart disease event in cases and referents. Secondary Prevention in Uppsala Primary Health Care Project (SUPRIM)

    <p>Abstract</p> <p>Background</p> <p>A large number of studies have reported on the psychosocial risk factor pattern prior to coronary heart disease events, but few have investigated the situation during the first year after an event, and none has been controlled. We therefore performed a case-referent study in which the prevalence of a number of psychosocial factors was evaluated.</p> <p>Methods</p> <p>Three hundred and forty-six coronary heart disease male and female cases no more than 75 years of age, discharged from hospital within the past 12 months, and 1038 referents from the general population, matched to the cases by age, sex and place of living, received a postal questionnaire in which information on lifestyle, psychosocial and quality of life measures were sought.</p> <p>Results</p> <p>The cases were, as expected, on sick leave to a larger extent than the referents, reported poorer fitness, poorer perceived health, fewer leisure time activities, but unexpectedly reported better social support, and more optimistic views of the future than the referents. There were no significant case-referent differences in everyday life stress, stressful life events, vital exhaustion, depressive mood, coping or life orientation test. However, women reported less favourable situations than men regarding stressful life events affecting others, vital exhaustion, depressive mood, coping, self-esteem, sleep, and symptom reporting, and female cases reported the most unfavourable situation of all groups.</p> <p>Conclusion</p> <p>In this first controlled study of the situation during the first year after a CHD event disease and gender status both appeared to be determinants of psychological well-being, with gender status apparently the strongest. This may have implications for cardiac rehabilitation programmes.</p

    Food systems sustainability : an examination of different viewpoints on food system change

    Global food insecurity levels remain stubbornly high. One of the surest ways to grasp the scale and consequence of global inequality is through a food systems lens. In a predominantly urban world, urban food systems present a useful lens to engage a wide variety of urban (and global) challenges—so called ‘wicked problems.’ This paper describes a collaborative research project between four urban food system research units, two European and two African. The project purpose was to seek out solutions to what lay between, across and within the different approaches applied in the understanding of each city’s food system challenges. Contextual differences and immediate (perceived) needs resulted in very different views on the nature of the challenge and the solutions required. Value positions of individuals and their disciplinary “enclaves” presented further boundaries. The paper argues that finding consensus provides false solutions. Rather the identification of novel approaches to such wicked problems is contingent of these differences being brought to the fore, being part of the conversation, as devices through which common positions can be discovered, where spaces are created for the realisation of new perspectives, but also, where difference is celebrated as opposed to censored

    Minimizing Variability of Cascade Impaction Measurements in Inhalers and Nebulizers

    The purpose of this article is to catalogue in a systematic way the available information about factors that may influence the outcome and variability of cascade impactor (CI) measurements of pharmaceutical aerosols for inhalation, such as those obtained from metered dose inhalers (MDIs), dry powder inhalers (DPIs) or products for nebulization; and to suggest ways to minimize the influence of such factors. To accomplish this task, the authors constructed a cause-and-effect Ishikawa diagram for a CI measurement and considered the influence of each root cause based on industry experience and thorough literature review. The results illustrate the intricate network of underlying causes of CI variability, with the potential for several multi-way statistical interactions. It was also found that significantly more quantitative information exists about impactor-related causes than about operator-derived influences, the contribution of drug assay methodology and product-related causes, suggesting a need for further research in those areas. The understanding and awareness of all these factors should aid in the development of optimized CI methods and appropriate quality control measures for aerodynamic particle size distribution (APSD) of pharmaceutical aerosols, in line with the current regulatory initiatives involving quality-by-design (QbD)

    Urban food systems as vehicles for sustainability transitions

    Strategies and action plans for sustainable food provisioning and urban food security are in progress in many urban regions both in the global North and South. A number of urgent challenges need to be confronted such as increasing uncertainty and unpredictability related to stronger dependence on a global market for food import, ongoing political unrest and environmental conflicts, increasing resource scarcity and climate warming making food production hazardous. There is an increased vulnerability with respect to food security for human societies, both in developing and developed countries. The food security dimension of access to healthy food is related to equality and poverty and is relevant for cities in the North via the segregation challenges. The food system issue is well-suited for assessing sustainable development since food provisioning is both a multiscale and cross-sectorial issue and thus addresses more than the three dimensions of social, economic and environmental sustainability. How is the planning for sustainable food strategies in urban regions in Europe concordant with the United Nations Global Sustainable Development Goals and with the transition towards sustainable futures? This paper deliberates on using the food system issues for sustainability transition, drawing on the forthcoming 2018 IPBES (Intergovernmental Science-Policy Platform on Biodiversity and Ecosystem Services) work on pathways for sustainable futures and a recent survey of existing urban food system strategies. Against this background, some reflections are given relevant for the ongoing work on a local urban food strategy for the city of Gothenburg, Sweden

    Performance of the Endemic Alpine Herb Primula scandinavica in a Changing European Mountain Landscape

    This paper reports on the performance of the red-listed Primula scandinavica, endemic to Scandinavia. The study took place in Jotunheimen, Norway. We evaluated occurrence, density and sexual reproduction variables (number of fertile individuals and number of seed capsules). Habitats were registered as land-cover categories (historical and current; broad habitat types) and habitat patch classes (semi-natural and natural; fine-grained habitat types). Spatial overlay analyses and maximum likelihood tests of generalized linear mixed models (GLMMs) were performed to assess the habitat quality of the land-cover categories and the habitat patch classes. In most (96%) of the observed occurrences the species was located at a site that had open vegetation 40 years ago but is now becoming forested or covered by shrubs. The highest densities of P. scandinavica were found in “heathland” and “grassland in transition into woodland”, while the potential for sexual reproduction was highest in “grassland” and lowest in “grassland in transition into woodland”. The results indicate that many of the populations in the study area are remnants and that the habitats of highest quality are semi-natural habitat patches. The changed land use practices in mountain areas have resulted and will result in fewer and smaller populations. However, as also climatic conditions will change it is not straightforward to predict the future performance of P. scandinavica.publishedVersio

    Natural resource conflicts and sustainable development

    Providing both a theoretical background and practical examples of natural resource conflict, this volume explores the pressures on natural resources leading to scarcity and conflict. It is shown that the causes and driving forces behind natural resource conflicts are diverse, complex and often interlinked, including global economic growth, exploding consumption, poor governance, poverty, unequal access to resources and power. The different interpretations of nature-culture and the role of humans in the ecosystem are often at the centre of the conflict. Natural resource conflicts range from armed conflicts to conflicts of interest between stakeholders in the North as well as in the South. The varying driving forces behind such disputes at different levels and scales are critically analysed, and approaches to facilitate and enforce mediation, transformation and collaboration at these levels and scales are presented and discussed. In order to transform existing resource conflicts, as well as to decrease the risk of future conflicts, approaches that enhance and enforce collaboration for sustainable development at global, regional, national and local levels are reviewed, and sustainable pathways suggested. A range of global examples is presented including water resources, fisheries, forests, human-wildlife conflicts, urban environments and the consequences of climate change. It will be a valuable text for advanced students of natural resource management, environment and development studies and peace and conflict management. The book will also be of interest to practitioners in the field of natural resource management
