    Developing of tool for quality assurance of mental health inpacient services in Azerbaijan

    ABSTRACT: The quality of care in mental health institutions is a serious concern in modern psychiatry. People with mental disabilities often face violation of their human rights. They often expose physical and sexual abuse, neglect, stigma, discrimination. Despite implementing the Mental Health Strategy adopted in 2011 in Azerbaijan, there is a lack of monitoring and evaluation of the provided services. The main goal of the study is to develop an instrument to monitor and evaluate the quality of mental health services in the context of human rights in Azerbaijan and to determine the reference intervals associated with good quality. The study utilized observational cross-sectional design to examine the human rights, quality of life of inpatients and their satisfaction with the services in the Psychiatric Hospital N#1. A total number of 160 participants entered the study (100 inpatients, 30 family members and 30 staff members). The WHO Quality Rights Toolkit, the Schizophrenia Quality of Life Scale-Revision 4 and the Client Satisfaction Questionnaire were used as the main tools. The results showed that the quality of services and the human rights the hospital were not fully met, precluding the provision of comprehensive and high-quality services for the persons with severe mental disorders. Unmet needs were higher in areas such as the right to exercise legal capacity, the right to personal liberty and security, and the right to live independently and to be part of the community. Some improvements were also necessary to meet needs concerning the right to an adequate standard of living, to enjoyment of the highest attainable standard of health and freedom from torture or cruel, inhuman or degrading treatment or punishment are protected and respected. Despite the level of the quality of life and satisfaction with services are average, there was a positive improvement in comparison with previous studies. The findings can be used for further improvement of the psychiatric services at Azerbeijan.RESUMO: A qualidade dos cuidados prestados nas instituições de saúde mental é uma preocupação relevante na psiquiatria moderna. As pessoas com perturbações psiquiátricas muitas vezes enfrentam violações a nível dos seus direitos humanos, incluindo abuso físico e sexual, negligência, estigma e descriminação. Apesar da implementação da Estratégia de Saúde Mental adoptada em 2011 no Azerbaijão, existe uma falta de acompanhamento e avaliação dos serviços prestados. O principal objetivo deste estudo é desenvolver um instrumento para monitorizar e avaliar a qualidade dos serviços de saúde mental no contexto dos direitos humanos no Azerbaijão e determinar os intervalos de referência associados com a boa qualidade. Foi elaborado um estudo com desenho transversal e observacional, centrado na avaliação do cumprimento dos direitos humanos e da qualidade de vida dos pacientes internados, assim como a sua satisfação com os serviços prestado no Hospital Psiquiátrico N#1. A amostra estudada foi de 160 (100 pacientes internados + 30 familiares + 30 membros do staff). Os instrumentos de avaliação foram o WHO Quality Rights Toolkit, a Schizophrenia Quality of Life Scale-Revision 4 e o Client Satisfaction Questionnaire. Os resultados mostraram que a qualidade dos serviços e os direitos humanos no hospital não estavam plenamente atendidos, impedindo por isso a prestação de serviços integrados e de alta qualidade para as pessoas com transtornos mentais graves. As necessidades não satisfeitas eram mais elevadas em áreas como o direito ao exercício da capacidade jurídica, o direito à liberdade e segurança pessoal e o direito de viver de forma independente e fazer parte da comunidade. Também se evidenciaram necessidades não-totalmente satisfeitas em áreas relativas ao direito a um nível de vida adequado, ao gozo do mais alto nível possível de estado de saúde e à ausência de tortura ou de tratamentos ou penas cruéis, desumanos ou degradantes. Apesar do nível de qualidade de vida e da satisfação com os serviços serem apenas medianos, houve uma melhora positiva em relação aos estudos anteriores. Os resultados podem ser utilizados para a melhoria dos serviços de psiquiatria no Azerbaijão

Resolution 1325, one of the most important UN resolutions within the field of peace and security, was adopted unanimously on 31 October 2000. The resolution highlights the consequences of violent conflict on women and girls and the important role of women in peacebuilding and post-conflict processes. In 2002 and again in 2004, UN member states were invited to prepare national action plans in order to take strong steps towards the implementation of UNSCR 1325. This study examines the similarities and differences in the plans and compares the points identified in the plans to the relevant points in UNSCR 1325

    Comparison and optimisation of transfection of human dental follicle cells, a novel source of stem cells, with different chemical methods and electro-poration

    Introduction Human dental follicle cells (HDFCs) derived from human impacted third molars (wisdom teeth) have been shown to be a significant source of adult stem cells. Generation of mesenchymal stem cell-like cells from dental follicles causes minimal surgical stress. In vitro and in vivo reports showed that HDFCs can be utilized in gene and cell therapy applications which make them an attractive alternative source for different gene-cell therapy applications. However, there are currently no systematic comparative studies on transfection potential of HDFC cells using different chemical and electro-poration techniques. Methods Stem cells from impacted third tooth molars were isolated, and analyzed for expression of surface markers. Transfection efficiencies of four commercially available transfection reagents (Transfast, Escort V, Superfect and FuGene HD) and electro-poration on isolated stem cells were compared. Results Isolated HDFCs were stained positive for CD105, CD90, CD73, CD166, and negative for CD34, CD45, and CD133. Among the chemical transfection reagents used in this study, FuGene HD was the most efficient in transfecting HDFCs, even in the presence of 10% serum. Conclusion Electro-poration of HDFCs yield relatively high transfection rates and cell viability when compared to chemical transfection techniques. Our observations might be useful for developing gene and cell therapy applications using dental follicle stem cells. © 2009 Springer Science+Business Media, LLC

    Psychometric properties of the Opening Minds Stigma Scale for Health Care Providers in 32 European countries – A bifactor ESEM representation

    AimsTo measure the stigma of healthcare providers toward people suffering from mental illness, the Opening Minds Stigma Scale for Health Care Providers (OMS-HC) is a commonly applied instrument. However, this scale has not been thoroughly validated in many European countries, its psychometric properties are still unknown and data on practicing psychiatrists is lacking. Therefore, this multicenter study aimed to assess the psychometric characteristics of the 15-item OMS-HC in trainees and specialists in adult and child psychiatry in 32 countries across Europe.Materials and methodsThe OMS-HC was conducted as an anonymous online survey and sent via Email to European adult and child psychiatrists. Parallel analysis was used to estimate the number of OMS-HC dimensions. Separate for each country, the bifactor ESEM, a bifactor exploratory structural equation modeling approach, was applied to investigate the factor structure of the scale. Cross-cultural validation was done based on multigroup confirmatory factor analyses and reliability measures.ResultsA total of 4,245 practitioners were included, 2,826 (67%) female, 1,389 (33%) male. The majority (66%) of participants were specialists, with 78% working in adult psychiatry. When country data were analyzed separately, the bifactor model (higher-order factor solution with a general factor and three specific factors) showed the best model fit (for the total sample χ2/df = 9.760, RMSEA = 0.045 (0.042–0.049), CFI = 0.981; TLI = 0.960, WRMR = 1.200). The average proportion of variance explained by the general factor was high (ECV = 0.682). This suggests that the aspects of ‘attitude,’ ‘disclosure and help-seeking,’ and ‘social distance’ could be treated as a single dimension of stigma. Among the specific factors, the ‘disclosure and help-seeking’ factor explained a considerable unique proportion of variance in the observed scores.ConclusionThis international study has led to cross-cultural analysis of the OMS-HC on a large sample of practicing psychiatrists. The bifactor structure displayed the best overall model fit in each country. Rather than using the subscales, we recommend the total score to quantify the overall stigmatizing attitudes. Further studies are required to strengthen our findings in countries where the proposed model was found to be weak

    YÖNETİCİLERİN KİŞİSEL DEĞERLERİNİN SÜRDÜRÜLEBİLİRLİK STRATEJİSİYLE İLİŞKİSİ Çalışma sürdürülebilir işletmelerde yöneticilerin sahip oldukları değerlerin stratejiyle ilişkisini incelemektedir. Literatürde kurumsal sürdürülebilirlik, değerler ve strateji konuları sıklıkla araştırılan ve önem verilen konulardandır. Lakin bu üç konunun bir arada incelendiği araştırma sayısı oldukça azdır. Bu çalışmanın amaçı literatüre bu konuda katkı sağlamaktır. Çalışma kapsamında kişinin sahip olduğu değerleri ölçmek amacıyla, Schwartz`ın Portrait Değerler Ölçeği kullanılmıştır. İşletmede var olan kurumsal sürdürülebilirlik stratejilerini ölçmek amacıyla da Küresel Raporlama Girişimi (GRI)’nin oluşturduğu kurumsal sürdürülebilirlik raporlama ilkelerinden yararlanılmıştır. Ölçek çevresel, sosyal ve ekonomik sürdürülebilirlik boyutlarından oluşturulmuş ve işletmenin sahip olduğu sürdürülebilirlik stratejilerinin belirlenmesi için kullanılmıştır. Çalışma kapsamında anket formları Türkiye`de bulunan 82 sürdürülebilir şirkette 200 çalışan ile paylaşılmış ve eksiksiz bir şekilde geri dönüş yapılmıştır. Çalışma sonuçlarından elde edilen verilere esasen Schwartz`ın değer boyutlarından hangilerinin kurumsal sürdürülebilir yöneticiye daha uygun olduğu ve sonuçta hangi sürdürülebilir stratejilerin uygulandığı tespit edilmiş ve sürdürülebilirlik değerleri belirlenmiştir. Çalışma kapsamında elde edilen sonuçlar; Yönetsel Değerlere ilişkin boyutların neredeyse hepsinin Sürdürülebilirlik Stratejilerini oluşturan boyutlardan bağımsız olduğu görülmektedir. Ancak Güvenlik boyutu ile Finansal ve Toplumsal boyut birbirinden bağımsız değildir ve ilişkilidir sonucuna varılmıştır. Aynı şekilde Uyum boyutu ile Finansal ve Toplumsal boyutun birbirinden bağımsız olmadığı, Gelenekselcilik boyutu ile Toplumsal boyutun ve son olarak Hazcılık boyutu ile Finansal boyutun birbirinden bağımsız olmadığı, ilişkili olduğu sonucuna varılmıştır. The literature about corporate sustainability, values and strategy issues among the topics which are often explored and emphasis. But there is a few number of thesis which these three subjects research together. The aim of this study is to contribute to the literature about this subject. In the scope of the study was used Schwartz` Portrait Value Scale questionnaire to measure the value which a person owened. At the same time, using the corporate sustainability reporting guidelines which Global Reporting Initiative (GRI) formed, were created the questionaire to measure corporate sustainability strategies. The scale has been created from environmental, social and economic sustainability dimension and it is used to determine the sustainability of the business strategy. Questionnaires were shared with 82 sustainable company in Turkey and with 200 employees and has got completely answers. The datas obtained from the study results, the Schwartz` value dimensions are more appropriate with coorporate sustainable manager and which sustainable strategy applied has been identified and sustainability values were determined. The results obtained in this study, almost the dimensions related to Managerial Values seen to be independent from all forms of the sustainability strategy. However, from results were reached that financial, social dimensions aren`t independent with security dimension. At the same time, the compliance dimension isn`t independent with financial and social dimension and traditional dimension with the social dimension and finally hedonism isn`t indepent from the financial dimension
