    Tuberculosis: an experience at government chest disease hospital

    Background: This retrospective study was carried out to find the incidence, clinical profiles and treatment outcome of pulmonary and extrapulmonary tuberculosis patients attending the only Chest Disease Hospital in Srinagar.Methods: A retrospective analysis of 613 patients having EPTB and PTB was undertaken from the chest disease hospital of Government Medical College Srinagar. Demographic characteristics, clinical features and treatment outcome were obtained from medical case records of all patients visiting the hospital for a period of two years from May 2016-May 2018.Results: The study comprised of 613 patients, out of which 365% were having pulmonary TB while 35% were having extrapulmonary TB. Majority of patients (44.5%) in TB group were in age group 10-30 while majority of patients (42%) in EPTB group were in age group 51-70. Males were seen more commonly affected. Majority (90%) of patients in EPTB group had pleural effusion. Majority of patients (71%) were smear positive by ZN staining in PTB group. Majority (90%) of PTB patients in smear negative group were BAL negative. In PTB group 8% were treatment failure while in EPTB group 2% were treatment failure. MDR was seen in 4.2% of total tuberculosis patient while XDR was seen in 0.32% of total patients. Out of treatment failure in PTB group MDR was seen in 64% followed by monoresistance to INH in 30% and XDR in 6%. In EPTB group MDR was seen in 100% of treatment failure. All patients with XDR died. Among the MDR patients majority are on treatment while 12% died, another 12% defaulter while 12% completed treatment.Conclusions: Among evaluated tuberculosis patients, maximum had PTB. Pleural tuberculosis was the most common presentation of EPTB. The study shows male preponderance in both groups. Treatment response was excellent with failure rate of 8% in PTB and 2% in EPTB. Most common cause of failure was MDR. XDR was seen in two patients and both of them died

    Localizing introgression on the chromosome of rice by genomic in situ hybridization (GISH)

    Genomic in situ hybridization was used to detect introgressed segment from Oryza australinesis onto the chromosomes of introgression line derived from the hybrid O. sativa x O. australinesis. Genomic DNA from Oryza australinesis was labeled with biotin and hybridized to the homologous sequences on the O. sativa chromosomes. The probe hybridization fluoresced green and non labeled O. sativa chromosomes appeared red or blue due to counterstaining with propidium iodide (PI) or 4,6-diamidino-2-phenylindole (DAPI). This differential painting of chromosomes unequivocally detected the introgressedsegment. Among the 200 cells analyzed, 6.5% of the cells showed  hybridization signal. Signal appeared on one chromosome in 5%, on two homologous chromosomes in 1% and on sister chromatids in 0.5%of the cells. Hybridization was seen on the short arm of the chromosome 12 of the introgression line

    Bicarbonate homeostasis in excitable tissues: role of AE3 Cl⁻/HCO⁻₃ exchanger and carbonic anhydrase XIV interaction

    Bicarbonate transport and metabolism are key elements of normal cellular function. Two alternate transcripts of anion exchanger 3 (AE3), full-length (AE3fl) and cardiac (AE3c), are expressed in central nervous system (CNS), where AE3 catalyzes electroneutral Cl⁻/HCO3− exchange across the plasma membrane of neuronal and glial cells of CNS. Anion exchanger isoforms, AE3fl and AE3c, associate with the carbonic anhydrases (CA) CAII and CAIV, forming a HCO3− transport metabolon, to maximize HCO3− flux across the plasma membrane. CAXIV, with catalytic domain anchored to the extracellular surface, is also expressed in CNS. Here physical association of AE3 and CAXIV was examined by coimmunoprecipitation experiments, using mouse brain and retinal lysates. CAXIV immunoprecipitated with anti-AE3 antibody, and both AE3 isoforms were immunoprecipitated using anti-CAXIV antibody, indicating CAXIV and AE3 interaction in the CNS. Confocal images revealed colocalization of CAXIV and AE3 in Muller and horizontal cells, in the mouse retina. Cl⁻/HCO3− exchange activity of AE3fl was investigated in transiently transfected human embryonic kidney 293 cells, using intracellular fluorescence measurements of BCECF, to monitor intracellular pH. CAXIV increased the rate of AE3fl-mediated HCO3− transport by up to 120%, which was suppressed by the CA inhibitor acetazolamide. Association of AE3 and CAXIV may represent a mechanism to enhance disposal of waste CO₂ and to balance pH in excitable tissues.Centro de Investigaciones Cardiovasculare

    Role of cytocentrifugation combined with nuclear fast picroindigocarmine staining in detecting cryptozoospermia in men diagnosed with azoospermia

    Purpose Azoospermia is defined as the absence of spermatozoa in the pellet of a centrifuged semen sample. In fact, when a basic semen analysis fails to detect sperm in the ejaculate, there is still the possibility of detecting rare sperm after centrifugation of the sample and examination of the pellet. In this study, we assessed the role of Cytospin centrifugation in combination with the nuclear fast picroindigocarmine (NF-PIC) staining in identifying sperm in azoospermic samples. Materials and Methods Semen samples of 251 men diagnosed as having azoospermia after standard examination were further analyzed by Cytospin centrifugation in combination with NF-PIC staining. Results Sperm were detected in 60 men (23.9%), thus changing their diagnosis to cryptozoospermia. Conclusions By identifying sperm in the semen of men who were thought to have total azoospermia, the Cytospin NF-PIC test can alter the diagnosis and further treatment of these men

    High rate, fast timing Glass RPC for the high {\eta} CMS muon detectors

    The HL-LHC phase is designed to increase by an order of magnitude the amount of data to be collected by the LHC experiments. To achieve this goal in a reasonable time scale the instantaneous luminosity would also increase by an order of magnitude up to 6.1034cm−2s−16.10^{34} cm^{-2} s^{-1} . The region of the forward muon spectrometer (∣η∣>1.6|{\eta}| > 1.6) is not equipped with RPC stations. The increase of the expected particles rate up to 2kHz/cm22 kHz/cm^{2} (including a safety factor 3) motivates the installation of RPC chambers to guarantee redundancy with the CSC chambers already present. The actual RPC technology of CMS cannot sustain the expected background level. The new technology that will be chosen should have a high rate capability and provides a good spatial and timing resolution. A new generation of Glass-RPC (GRPC) using low-resistivity (LR) glass is proposed to equip at least the two most far away of the four high η{\eta} muon stations of CMS. First the design of small size prototypes and studies of their performance in high-rate particles flux is presented.     High ALDH Activity Identifies Chemotherapy-Resistant Ewing's Sarcoma Stem Cells That Retain Sensitivity to EWS-FLI1 Inhibition

    Cancer stem cells are a chemotherapy-resistant population capable of self-renewal and of regenerating the bulk tumor, thereby causing relapse and patient death. Ewing's sarcoma, the second most common form of bone tumor in adolescents and young adults, follows a clinical pattern consistent with the Cancer Stem Cell model - remission is easily achieved, even for patients with metastatic disease, but relapse remains frequent and is usually fatal.We have isolated a subpopulation of Ewing's sarcoma cells, from both human cell lines and human xenografts grown in immune deficient mice, which express high aldehyde dehydrogenase (ALDH(high)) activity and are enriched for clonogenicity, sphere-formation, and tumor initiation. The ALDH(high) cells are resistant to chemotherapy in vitro, but this can be overcome by the ATP binding cassette transport protein inhibitor, verapamil. Importantly, these cells are not resistant to YK-4-279, a small molecule inhibitor of EWS-FLI1 that is selectively toxic to Ewing's sarcoma cells both in vitro and in vivo.Ewing's sarcoma contains an ALDH(high) stem-like population of chemotherapy-resistant cells that retain sensitivity to EWS-FLI1 inhibition. Inhibiting the EWS-FLI1 oncoprotein may prove to be an effective means of improving patient outcomes by targeting Ewing's sarcoma stem cells that survive standard chemotherapy

    Performance of Resistive Plate Chambers installed during the first long shutdown of the CMS experiment

    The CMS experiment, located at the CERN Large Hadron Collider, has a redundant muon system composed by three different detector technologies: Cathode Strip Chambers (in the forward regions), Drift Tubes (in the central region) and Resistive Plate Chambers (both its central and forward regions). All three are used for muon reconstruction and triggering. During the first long shutdown (LS1) of the LHC (2013-2014) the CMS muon system has been upgraded with 144 newly installed RPCs on the forth forward stations. The new chambers ensure and enhance the muon trigger efficiency in the high luminosity conditions of the LHC Run2. The chambers have been successfully installed and commissioned. The system has been run successfully and experimental data has been collected and analyzed.     Controversy and consensus on the management of elevated sperm DNA fragmentation in male infertility: a global survey, current guidelines, and expert recommendations

    PURPOSE: Sperm DNA fragmentation (SDF) has been associated with male infertility and poor outcomes of assisted reproductive technology (ART). The purpose of this study was to investigate global practices related to the management of elevated SDF in infertile men, summarize the relevant professional society recommendations, and provide expert recommendations for managing this condition. MATERIALS AND METHODS: An online global survey on clinical practices related to SDF was disseminated to reproductive clinicians, according to the CHERRIES checklist criteria. Management protocols for various conditions associated with SDF were captured and compared to the relevant recommendations in professional society guidelines and the appropriate available evidence. Expert recommendations and consensus on the management of infertile men with elevated SDF were then formulated and adapted using the Delphi method. RESULTS: A total of 436 experts from 55 different countries submitted responses. As an initial approach, 79.1% of reproductive experts recommend lifestyle modifications for infertile men with elevated SDF, and 76.9% prescribe empiric antioxidants. Regarding antioxidant duration, 39.3% recommend 4-6 months and 38.1% recommend 3 months. For men with unexplained or idiopathic infertility, and couples experiencing recurrent miscarriages associated with elevated SDF, most respondents refer to ART 6 months after failure of conservative and empiric medical management. Infertile men with clinical varicocele, normal conventional semen parameters, and elevated SDF are offered varicocele repair immediately after diagnosis by 31.4%, and after failure of antioxidants and conservative measures by 40.9%. Sperm selection techniques and testicular sperm extraction are also management options for couples undergoing ART. For most questions, heterogenous practices were demonstrated. CONCLUSIONS: This paper presents the results of a large global survey on the management of infertile men with elevated SDF and reveals a lack of consensus among clinicians. Furthermore, it demonstrates the scarcity of professional society guidelines in this regard and attempts to highlight the relevant evidence.
