    A Artroplastia Total do Joelho (ATJ) tem vindo a assumir-se como a opção cirúrgica mais frequente no tratamento de doentes com alterações degenerativas da articulação do joelho devido aos ótimos resultados funcionais e melhoria da dor. No entanto, a percentagem de doentes com sintomas dolorosos após a realização deste procedimento atinge em algumas estatísticas publicadas valores significativos que oscilam entre os 10 e os 20%. Em alguns casos a dor não é facilmente explicável, representando um desafio para o cirurgião. Os autores relatam o caso de uma doente de 71 anos submetida a ATJ primária, no contexto de patologia degenerativa idiopática associado a desvio axial em varum. A dor localizava-se na face anteroexterna do joelho ao nível da interlinha articular e tiveram início no pós-operatório de forma gradual, associadas ao aumento da mobilidade. Devido à presença de um sintoma associado a atividade mecânica, foram colocadas várias hipóteses, entre as quais a possibilidade de conflito com resquício intra-articulares do compartimento externo do joelho. A doente foi submetida a artroscopia da articulação do joelho que revelou a existência de um fragmento meniscal que provocava conflito entre o côndilo externo do componente artroplástico femoral e o polietileno no movimento de extensão máxima do joelho. Procedeu-se a remoção artroscópica do fragmento com melhoria sintomática completa. Os autores pretendem com este caso evidenciar a importância da investigação diagnóstica para o tratamento da dor inexplicável na ATJ, e o valor terapêutico da artroscopia nos casos de provável conflito

    Decreased Survival in African Patients with Triple Negative Breast Cancer

    Abstract Introduction: Triple Negative Breast Carcinomas (TNBC) are more prevalent in younger women especially those with African Ancestry, in whom the disease appears to be more aggressive. Since there are no data on Africans living in continental Europe, we sought to analyse a sample of African women from a European country and determine if, like African Americans, they have more aggressive tumor biology and poorer outcomes. Methods: We performed a retrospective review of TNBC to compare clinical and pathological features and survival between African and non-African patients. All women presented with breast cancer (BC), between 2005 and 2014, to a single general hospital, in Portugal. Results: A total of 144 (9.3% of the whole sample) TNBC patients were identified and amongst these, 17 were African (12%). African patients were not significantly younger than non-African patients (median age of 60 years vs 57.2 years, respectively, p=0.59). Regarding tumor size, nodal status and histologic grade at presentation, these variables were very similar between the two cohorts. Nevertheless, the prevalence of initially metastatic BC was significantly higher among the African population (41.2% vs 11%, p<0,005) and the outcome was worse for these patients (median survival: 62 vs 15 months, p<0.005). Conclusions: Our study demonstrated that African patients more frequently presented with late stage disease and worse survival outcome than the non-African population.     Polyphenolic and chromatic characterization of tropical red wines produced in the São Francisco river Valley (Northeast Brazil).

    Young varietal and experimental wines from Vitis vinifera - Petit Verdot,Tempranillo and Syrah - cultivated in a tropical semiarid region (Northeast Brazil) were evaluated with regard to their phenolic composition and chromatic characteristics during a storage period of 12-month. The wine was stored in bottles and evaluated using spectrophotometry and high efficiency liquid chromatography to determine color, total polyphenols, anthocyanins, flavanols and flavonols. The Petit Verdot had a less color variation when compared to wines from temperate climates. Although the wines followed the traditional pattem of decreasing polyphenol concentration during storage, the concentrations of these compounds remained higher than those reported in literature, such as for the anthocyanins in Petit Verdot wines and transresveratrolin Syrah, showing possible beneficial health effects even after storag

    Nos doentes com fratura osteoporótica da extremidade proximal do fémur, pouco é conhecido sobre a incidência e fatores de risco de fratura contralateral da extremidade proximal do fémur. O objetivo deste trabalho foi, através de um estudo retrospetivo determinar a incidência e os fatores de risco para fratura bilateral da extremidade proximal do fémur não contemporânea. Foram analisados os processos de 1911 doentes com fratura da extremidade proximal do fémur entre 2003 e 2009. Os dados recolhidos sobre as fraturas, tratamentos e comorbilidades foram trabalhados estatisticamente. Um total de 64 doentes (3,24%) teve fratura bilateral da extremidade proximal do fémur, com uma média de idades acima dos 80 anos. Determinou-se que existe uma relação direta entre o tipo da primeira e segunda fratura (intracapsular vs extraapsular), e que 70% das segundas fraturas ocorrem nos primeiros três anos após fratura. Das comorbilidades verificou-se que a doença de Parkinson, Hipertensão Arterial, doença Cardíaca, Anemia e alterações da Visão representam um risco acrescido para fratura contralateral da extremidade proximal do fémur. Propomos um follow-up mais rigoroso nos primeiros três anos após a primeira fratura e estabelecidas melhores formas de prevenção de fraturas e otimização das comorbilidades nos doentes com fatores de risco

    Outcomes of Long Pouch Gastric Bypass (LPGB): 4-Year Experience in Primary and Revision Cases

    Background: One of the most important complications of the one anastomosis gastric bypass (OAGB) is enterobilio acid reflux (EBAR). We report the concept of the long pouch Roux-en-Y gastric bypass (LPRYGB) meaning a Roux-en-Y with a long pouch and a 100-cm alimentary limb to avoid EBAR, with a long biliopancreatic limb to increase metabolic effects. Methods: A total of 300 LPRYGB cases in a 4-year period, with a 90% follow-up rate, were analysed. Anthropometric, technical feasibility, morbidity, weight loss and comorbidity outcomes were analysed. Results: The percentage total weight loss (%TWL) was 30.5% at 4 years of follow-up (32.3% in primary and 28.3% in revisions). Six intra-operative (2%) and 28 postoperative complications (9.3%) were seen. Out of this 28 complications, 11 (3.6%) were late complications. Reoperations were performed in 15 patients (5.0%). Clinically relevant EBAR was present in 3 cases only (1%) 4 years after the operation. Conclusions: The LPRYGB combines the main advantages of the OAGB (light restriction and moderate malabsorption) with the anti-reflux effect from the Roux-en-Y diversion.info:eu-repo/semantics/publishedVersio

    The organic acids (tartaric, malic, citric, lactic and succinic) are de main components responsible for the acidity in the wine. This method for simultaneous determination of organic acids and interfering peaks in wines can be achieved in 16 min. The sample preceded by a dilution and filtration step. The chromatographic separation required one reversed phase column, isocratic mobile phase (acetonitrila, formic acid in water) and detection wavelength was set at 212 nm. The validation confirmed good repeatability, recovery and application in red and white wines.Nota técnica
