    The Sleep Or Mood Novel Adjunctive therapy (SOMNA) trial: a study protocol for a randomised controlled trial evaluating an internet-delivered cognitive behavioural therapy program for insomnia on outcomes of standard treatment for depression in men

    BACKGROUND: Insomnia is a significant risk factor for depression onset, can result in more disabling depressive illness, and is a common residual symptom following treatment cessation that can increase the risk of relapse. Internet-based cognitive behavioural therapy for insomnia has demonstrated efficacy and acceptability to men who are less likely than women to seek help in standard care. We aim to evaluate whether internet delivered cognitive behavioural therapy for insomnia as an adjunct to a standard depression therapeutic plan can lead to improved mood outcomes.METHODS/DESIGN: Male participants aged 50 years or more, meeting Diagnostic and Statistical Manual of Mental Disorders criteria for current Major Depressive Episode and/or Dysthymia and self-reported insomnia symptoms, will be screened to participate in a single-centre double-blind randomised controlled trial with two parallel groups involving adjunctive internet-delivered cognitive behavioural therapy for insomnia and an internet-based control program. The trial will consist of a nine-week insomnia intervention period with a six-month follow-up period. During the insomnia intervention period participants will have their depression management coordinated by a psychiatrist using standard guideline-based depression treatments. The study will be conducted in urban New South Wales, Australia, where 80 participants from primary and secondary care and direct from the local community will be recruited. The primary outcome is change in the severity of depressive symptoms from baseline to week 12. DISCUSSION: This study will provide evidence on whether a widely accessible, evidence-based, internet-delivered cognitive behavioural therapy for insomnia intervention can lead to greater improvements than standard treatment for depression alone, in a group who traditionally do not readily access psychotherapy. The study is designed to establish effect size, feasibility and processes associated with implementing e-health solutions alongside standard clinical care, to warrant undertaking a larger more definitive clinical trial.Trial registration: Australian and New Zealand Clinical Trials Registry ACTRN12612000985886.The study is supported by beyondblue: the national depression and anxiety initiative National Priority Driven Research Program and funded through a donation from the Movember Foundation

    Prevalencia de la paramfistomiasis bovina en hatos lecheros de Colombia

    Se describen los resultados de prevalencia de la paramfistomiasis bovina en hatos lecheros de las regiones Andina, Caribe y Orinoquía colombiana. La enfermedad parasitaria, en vacas cursa con pérdida de peso, disminución de la producción de leche, debilidad y diarrea fétida y persistente. En 8 áreas lecheras se visitaron un total de 113 fincas, las cuales utilizan el sistema de pastoreo contínuo o rotacional, teniéndose en cuenta las medidas utilizadas para el control de parásitos gastrointestinales y hepáticos. En cada finca se tomaron 15 muestras de heces de vacas, las muestras se examinaron para determinar la presencia de huevos de Cotylophoron sp., usando la técnica descrita por Dennis et. al. De 1711 muestras analizadas, 154 resultaron positivas a huevos de Cotylophoron sp., que representa el 9.0 por ciento de los animales positivos. De 113 fincas examinadas, 29 resultaron positivas. El estudio de la prevalencia de la paramfistomiasis bovina en hatos lecheros de las regiones Andina, Caribe y Pidemonte Llanero de Colombia demostró que la entidad está presente en el país, siendo las zonas más afectadas los valles cálidos de la región Andina (22.6 por ciento) y el Piedemonte Llanero (42.9 por ciento). La temperatura sobre los 20 grados centígrados que predomina en las zonas anotadas hace suponer que es una de las condiciones que favorece el desarrollo del hospedador intermediario, el cual se desconoce en el medio.     Magnetization plateaus in spin chains: ``Haldane gap'' for half-integer spins

    We discuss zero-temperature quantum spin chains in a uniform magnetic field, with axial symmetry. For integer or half-integer spin, SS, the magnetization curve can have plateaus and we argue that the magnetization per site mm is topologically quantized as q(Sm)=integerq (S - m)= integer at the plateaus, where qq is the period of the groundstate. We also discuss conditions for the presence of the plateau at those quantized values. For S=3/2S=3/2 and m=1/2m=1/2, we study several models and find two distinct types of massive phases at the plateau.     Effects of air pollution and the introduction of the London Low Emission Zone on the prevalence of respiratory and allergic symptoms in schoolchildren in East London: a sequential cross-sectional study

    The adverse effects of traffic-related air pollution on children’s respiratory health have been widely reported, but few studies have evaluated the impact of traffic-control policies designed to reduce urban air pollution. We assessed associations between traffic-related air pollutants and respiratory/allergic symptoms amongst 8–9 year-old schoolchildren living within the London Low Emission Zone (LEZ). Information on respiratory/allergic symptoms was obtained using a parent-completed questionnaire and linked to modelled annual air pollutant concentrations based on the residential address of each child, using a multivariable mixed effects logistic regression analysis. Exposure to traffic-related air pollutants was associated with current rhinitis: NOx (OR 1.01, 95% CI 1.00–1.02), NO2 (1.03, 1.00–1.06), PM10 (1.16, 1.04–1.28) and PM2.5 (1.38, 1.08–1.78), all per μg/m3 of pollutant, but not with other respiratory/allergic symptoms. The LEZ did not reduce ambient air pollution levels, or affect the prevalence of respiratory/allergic symptoms over the period studied. These data confirm the previous association between traffic-related air pollutant exposures and symptoms of current rhinitis. Importantly, the London LEZ has not significantly improved air quality within the city, or the respiratory health of the resident population in its first three years of operation. This highlights the need for more robust measures to reduce traffic emissions

    Novel sources of variation in grain Zinc (Zn) concentration in bread wheat germplasm derived from Watkins landraces

    A diverse panel of 245 wheat genotypes, derived from crosses between landraces from the Watkins collection representing global diversity in the early 20th century and the modern wheat cultivar Paragon, was grown at two field sites in the UK in 2015-16 and the concentrations of zinc and iron determined in wholegrain using inductively coupled plasma-mass spectrometry (ICP-MS). Zinc concentrations in wholegrain varied from 24-49 mg kg-1 and were correlated with iron concentration (r = 0.64) and grain protein content (r = 0.14). However, the correlation with yield was low (r = -0.16) indicating little yield dilution. A sub-set of 24 wheat lines were selected from 245 wheat genotypes and characterised for Zn and Fe concentrations in wholegrain and white flour over two sites and years. White flours from 24 selected lines contained 8-15 mg kg-1 of zinc, which was positively correlated with the wholegrain Zn concentration (r = 0.79, averaged across sites and years). This demonstrates the potential to exploit the diversity in landraces to increase the concentration of Zn in wholegrain and flour of modern high yielding bread wheat cultivars

    Using monozygotic twins to dissect common genes in Posttraumatic stress disorder and migraine

    Epigenetic mechanisms have been associated with genes involved in Posttraumatic stress disorder (PTSD). PTSD often co-occurs with other health conditions such as depression, cardiovascular disorder and respiratory illnesses. PTSD and migraine have previously been reported to be symptomatically positively correlated with each other, but little is known about the genes involved. The aim of this study was to understand the comorbidity between PTSD and migraine using a monozygotic twin disease discordant study design in six pairs of monozygotic twins discordant for PTSD and 15 pairs of monozygotic twins discordant for migraine. DNA from peripheral blood was run on Illumina EPIC arrays and analyzed. Multiple testing correction was performed using the Bonferroni method and 10% false discovery rate (FDR).
