    The Human Microbiome and Its Impacts on Health

    (e human microbiome comprises bacteria, archaea, viruses, and eukaryotes which reside within and outside our bodies. (ese organisms impact human physiology, both in health and in disease, contributing to the enhancement or impairment of metabolic and immune functions. Micro-organisms colonise various sites on and in the human body, where they adapt to specific features of each niche. Facultative anaerobes are more dominant in the gastrointestinal tract, whereas strict aerobes inhabit the respiratory tract, nasal cavity, and skin surface. (e indigenous organisms in the human body are well adapted to the immune system, due to the biological interaction of the organisms with the immune system over time. An alteration in the intestinal microbial community plays a major role in human health and disease pathogenesis. (ese alterations result from lifestyle and the presence of an underlying disease. Dysbiosis increases host susceptibility to infection, and the nature of which depends on the anatomical site involved. (e unique diversity of the human microbiota accounts for the specific metabolic activities and functions of these microorganisms within each body site. It is therefore important to understand the microbial composition and activities of the human microbiome as they contribute to health and disease

    Evaluation of Plasmodium falciparum K13 gene polymorphism and susceptibility to dihydroartemisinin in an endemic area

    Asia, and its reduced sensitivity has been reported in other regions. This study aims to determine parasite susceptibility to the bioactive form of artemisinin derivatives- dihydroartemisinin (DHA)-, and to detect the K13 polymorphism in isolates from an endemic area of Nigeria. Methods: Ex-vivo response in 55 parasites isolates obtained from malaria-positive patients were exposed to pulseDHA concentration and cultured for 66 hours ex-vivo. Parasite ring stage survival (RSAex-vivo) relative to unexposed matched control was determined by microscopy, and parasite growth was compared using Mann-Whitney U-test at a significance level of P<0.05. The Kelch propeller gene was amplified using specific primers, then sequenced and analyzed for single nucleotide polymorphisms (SNPs), which were compared to reference PF3D7_1343700. Results: Overall, 151 of 375 (40.2%) individuals were positive during the study period. In 55 selected isolates, there was increased growth in unexposed wells but growth was inhibited in DHA-exposed wells, with growth rate between 14.9 – 96.7%. The mean RSAex-vivo value was 0.18 � 0.09%, 95% CI (0.15-0.20). There was no significant mutation of the K13 gene in the parasite isolates evaluated. Conclusions: Plasmodium falciparum isolates from this endemic area show high sensitivity to dihydroartemisinin ex-vivo, with no mutations conferring artemisinin resistance. Continuous monitoring of parasite susceptibility to artemisinin combination drugs should be intensified to reduce chances of artemisinin resistance in endemic area

    Screening, isolation and biotechnological potentials of foodborne Lactobacillus fermentum strains MT903311 and MT903312

    Vitamins are an essential food source with excellent roles in the cellular metabolism and other essential nutrients required in food intake but cannot be synthesized by humans. There have been reports of some lactic acid bacteria (LAB) abilities with probiotic activities to produce food-grade vitamins. Our study aimed to investigate lactic acid bacteria (LAB) possessing antimicrobial activities and extracellular production of folate from different Nigerian fermented foods. LAB was assayed for their antimicrobial activities against clinical isolates of Escherichia coli and Salmonella typhimurium and their extracellular production of essential vitamins. Among the 43 isolates of LAB, two strains of Lactobacillus fermentum showed the highest inhibitions against the test bacteria and demonstrated the highest concentrations of extracellular vitamins production. The range of vitamins produced at 24 h was between 12.23 and 801.79 μg/ml, while the highest vitamin production of 801.79 and 310.55 μg/ml was observed for folate and vitamin B12 respectively, the lowest production was for B1+B2. Consistent vitamin production was typical with only L. fermentum MT903311 and L. fermentum MT903312, so were their antimicrobial activities. The L. fermentum strains isolated in this study could be exploited and applied in food products to substitute synthetic vitamin enrichment and fortification

    The World Heal Organization (WHO) has identified malaria diagnosis as being pivotal to eradicating the disease by 2030 as stipulated in the Sustainable Development Goals (SDG). The data presented here was obtained from outpatients of a hospital in the South Western Region of Nigeria from November 2016 to May 2017. The data contains malaria incidence amongst asymptomatic and symptomatic outpatients in the period under review. Malaria incidence was obtained using two diagnostic test kits, Bioline SD (HRP-2) and ACON (HRP-2/Aldolase) alongside Microscopy as gold standard. Specificity, Sensitivity and Kappa statistic of each test device is presented in the tables herewith. Data presented here could be used alongside other data sources to assess the state of malaria diagnostics

    Ethanol extract of Tetrapleura tetraptera fruit peels: Chemical characterization, and antioxidant potentials against free radicals and lipid peroxidation in hepatic tissues

    The chemical and antioxidant properties of the ethanolic extract of Tetrapleura tetraptera fruit peels were investigated. Dried peels of T. tetraptera fruits were extracted with ethanol. The extract was subjected to preliminary phytochemical screening using standard procedures. GCâMS was used in identifying the secondary metabolites. The antioxidant properties of the extract were determined by its ferric reducing activity, 2,2â²-diphenyl-1-picrylhydrazyl (DPPH) and nitric oxide (NO) radicals scavenging activities, and the inhibition of lipid peroxidation in hepatic tissues of albino male rats. Preliminary phytochemical screening revealed the presence of flavonoids, phenols, tannins, saponins, terpenoids and phlebotannin. GCâMS analysis revealed the presence of D-fructose, piperazine, octodrine, glycidol, glyceraldehydes, 6-octadecenoic acid and 9,12-octadecenoic acid, with Dâfructose being the most predominant compound. The extract exhibited high antioxidant activities both in vitro and ex vivo, as indicated by its ability to scavenge DPPH and nitric oxide as well as inhibition of lipid peroxidation. This is further portrayed by its ferric reducing activity. These results suggest an antioxidant protective effect of the extract against oxidative hepatic damage and can be attributed to a synergetic action of the identified bioactive compounds.     Adaptation of the Wound Healing Questionnaire universal-reporter outcome measure for use in global surgery trials (TALON-1 study): mixed-methods study and Rasch analysis

    BackgroundThe Bluebelle Wound Healing Questionnaire (WHQ) is a universal-reporter outcome measure developed in the UK for remote detection of surgical-site infection after abdominal surgery. This study aimed to explore cross-cultural equivalence, acceptability, and content validity of the WHQ for use across low- and middle-income countries, and to make recommendations for its adaptation.MethodsThis was a mixed-methods study within a trial (SWAT) embedded in an international randomized trial, conducted according to best practice guidelines, and co-produced with community and patient partners (TALON-1). Structured interviews and focus groups were used to gather data regarding cross-cultural, cross-contextual equivalence of the individual items and scale, and conduct a translatability assessment. Translation was completed into five languages in accordance with Mapi recommendations. Next, data from a prospective cohort (SWAT) were interpreted using Rasch analysis to explore scaling and measurement properties of the WHQ. Finally, qualitative and quantitative data were triangulated using a modified, exploratory, instrumental design model.ResultsIn the qualitative phase, 10 structured interviews and six focus groups took place with a total of 47 investigators across six countries. Themes related to comprehension, response mapping, retrieval, and judgement were identified with rich cross-cultural insights. In the quantitative phase, an exploratory Rasch model was fitted to data from 537 patients (369 excluding extremes). Owing to the number of extreme (floor) values, the overall level of power was low. The single WHQ scale satisfied tests of unidimensionality, indicating validity of the ordinal total WHQ score. There was significant overall model misfit of five items (5, 9, 14, 15, 16) and local dependency in 11 item pairs. The person separation index was estimated as 0.48 suggesting weak discrimination between classes, whereas Cronbach's α was high at 0.86. Triangulation of qualitative data with the Rasch analysis supported recommendations for cross-cultural adaptation of the WHQ items 1 (redness), 3 (clear fluid), 7 (deep wound opening), 10 (pain), 11 (fever), 15 (antibiotics), 16 (debridement), 18 (drainage), and 19 (reoperation). Changes to three item response categories (1, not at all; 2, a little; 3, a lot) were adopted for symptom items 1 to 10, and two categories (0, no; 1, yes) for item 11 (fever).ConclusionThis study made recommendations for cross-cultural adaptation of the WHQ for use in global surgical research and practice, using co-produced mixed-methods data from three continents. Translations are now available for implementation into remote wound assessment pathways
