    On the Existence of an Optimum End-to-side Junctional Geometry in Peripheral Bypass Surgery—A Computer Generated Study

    AbstractBackground. To investigate hemodynamic flow changes associated with vein cuffs and patches that may be responsible for improved patency of prosthetic infrainguinal grafts.Methods. The role of the graft–artery junction angle was examined by computational fluid dynamics to assess the influence of anastomotic geometry on wall shear stress (WSS) distributions. Three geometrically different junction configurations were studied and the WSS and WSS gradient (WSSG) values were compared.Results. The inclusion of a patch or a cuff moves the bed stagnation point (BSP) distally, increasing the area on the bed of the junction which experiences a BSP and reducing the strength of the recirculation region opposite the heel of the junction by 54.8 and 50.8%, respectively. The patched geometry promotes earlier recovery of the flow in the distal outflow segment (DOS) than for the unpatched model. Also, the helical flow patterns in the DOS associated with the cuffed geometry are stronger. The net effect of these changes are that peak WSSG values for the patched and cuffed geometries are three times lower than those for the uncuffed geometry.Conclusion. This study provides some additional insights into the hemodynamics of graft–artery junction geometry which may influence future clinical practice

    Endovascular exclusion of iliac artery to iliac vein fistula after lumbar disk surgery

    AbstractIliac arteriovenous (AV) fistula is rare after lumbar disk surgery. Traditionally, open repair through the arterial lumen was performed. We report endovascular exclusion of an iliac AV fistula in a 41-year-old woman 8 years after lumbar diskectomy. An angiogram showed an AV fistula connecting the right common iliac artery and vein. This was repaired with placement of two covered wall stents in the right common artery and external iliac artery, and embolization of the right internal iliac artery. Contrast medium-enhanced computed tomography scan at 5 months confirmed elimination of the AV fistula and right iliac artery patency. This technique should be considered in management of iliac AV fistulas. (J Vasc Surg 2003;37:1091-3.

    Local Anaesthetic Flush Reduces Postoperative Pain and Haematoma Formation After Great Saphenous Vein Stripping—A Randomised Controlled Trial

    AbstractObjectivesTo observe the effect of local anaesthetic flush through the great saphenous vein (GSV) tunnel on postoperative pain and haematoma formation following saphenous vein stripping operations.DesignProspective, double-blind, randomised, control trial.MethodsOne hundred patients were randomized to receive 20ml of local anaesthetic (bupivacaine 0.25%+adrenaline) or saline control flush through the GSV tunnel after stripping in a double-blind study. Visual analogue pain scores were used to measure postoperative pain daily for the 1st week, then at 3 weeks and 6 weeks. Patients were examined during the 1st, 3rd and 6th week for haematoma formation.ResultsIn the control group the median postoperative pain score was 4 (range 0–7) in the immediate postoperative period compared to a median of 1 (range 0–4) in the LA group (p<0.001). The median pain score on day-4 was 4 (range 1–6) (control) vs. 1 (range 0–3) (LA group) (p<0.001, Mann–Whitney Utest) and on day-6 it was 1 (range 0–5) (control) vs. 0 (range 0–5) (LA group) (p<0.001, Mann–Whitney). Twelve patients (24%) developed a haematoma in the GSV tunnel in the control group compared to three patients (6%) in the LA group (p=0.007).ConclusionFlushing of the GSV tunnel with bupivacaine plus adrenaline significantly reduces postoperative pain and haematoma formation in patients undergoing GSV stripping for varicose veins

    Today globalization has penetrated all nations in the world including Indonesia. Globalization occurs in almost all aspects of life including the socio-economic field which can be said to have improved people's standard of living.  Theaim of this study is to find out the picture between hedonic lifestyle and consumptive behavior in MIK STIKes Santa Elisabeth Medan study program students.  The study used cross sectional design with random sampling and data collection was carried out through the distribution of online questionnaires via whatsapp.  The population used in this study was MIK Study Program students level 1 and 2 with a sample of 30 respondents and indicators of 30 statements.  Data collection techniques use questionnaires as primary data, and journals and books as secondary data. The results showed that the hedonic lifestyle of students was high, namely 17 (57%) students, this can be seen from the habits of students who like to buy luxury goods, the habit of buying food online and really like to follow trends, while the consumptive behavior of students is low, namely 18 (60%) Students, the level of consumption is low due to campus rules that prohibit using luxury items that are not needed. So it can be concluded that MIK STIKes Santa Elisabeth Medan study program students have a relatively high lifestyle in using luxury goods and relatively low consumptive behavior because they are bound by dormitory rules.  Abstract Pada era globalisasi ini sudah merambah ke seluruh negara-negara di dunia termasuk Indonesia. Globalisasi terjadi hampir diseluruh aspek kehidupan termasuk bidang sosial ekonomi yang dapat dikatakan telah meningkatkan taraf kehidupan masyarakat. Tujuan dari penelitian ini adalah untuk mengetahui gambaran antara gaya hidup hedonis dengan perilaku konsumtif pada mahasiswa prodi MIK STIKes Santa Elisabeth Medan. Penelitian menggunakan cross sectional design dengan random sampling dan pengumpulan data dilakukan melalui penyebaran kuesioner online melalui whatsapp. Populasi yang digunakan dalam penelitian ini adalah Mahasiswa Prodi MIK tingkat 1 dan 2 dengan jumlah sampel sebanyak 30 responden dan indikator sebanyak 30 pernyataan. Teknik pengumpulan data menggunakan angket sebagai data primer, serta jurnal dan buku sebagai data sekunder. Hasil penelitian menunjukan bahwa gaya hidup hedonis mahasiswa tergolong tinggi yaitu 17 (57%) mahasiswa, hal ini dapat dilihat dari kebiasaan mahasiswa yang suka membeli barang mewah, kebiasaan membeli makanan online dan sangat suka mengikuti trend, Sedangkan perilaku konsumtif mahasiswa tergolong rendah yaitu 18 (60%) mahasiswa, tingkat konsumtifnya rendah disebabkan aturan kampus yang melarang untuk menggunakan barang-barang mewah yang tidak dibutuhkan.Sehingga dapat disimpulkan bahwa mahasiswa prodi MIK STIKes Santa Elisabeth Medan memiliki gaya hidup yang relatif  tinggi dalam menggunakan barang mewah dan perilaku konsumtif yang relatif rendah karena terikat dengan aturan asrama. ABSTRAK Dewasa ini globalisasi sudah merambah ke seluruh bangsa-bangsa di dunia termasuk Indonesia.     Search for Gravitational Waves Associated with Gamma-Ray Bursts Detected by Fermi and Swift during the LIGO-Virgo Run O3b

    We search for gravitational-wave signals associated with gamma-ray bursts (GRBs) detected by the Fermi and Swift satellites during the second half of the third observing run of Advanced LIGO and Advanced Virgo (2019 November 1 15:00 UTC-2020 March 27 17:00 UTC). We conduct two independent searches: A generic gravitational-wave transients search to analyze 86 GRBs and an analysis to target binary mergers with at least one neutron star as short GRB progenitors for 17 events. We find no significant evidence for gravitational-wave signals associated with any of these GRBs. A weighted binomial test of the combined results finds no evidence for subthreshold gravitational-wave signals associated with this GRB ensemble either. We use several source types and signal morphologies during the searches, resulting in lower bounds on the estimated distance to each GRB. Finally, we constrain the population of low-luminosity short GRBs using results from the first to the third observing runs of Advanced LIGO and Advanced Virgo. The resulting population is in accordance with the local binary neutron star merger rate. © 2022. The Author(s). Published by the American Astronomical Society
