    Face-to-Face with the Doctor Online: Phenomenological Analysis of Patient Experience of Teleconsultation

    The global crisis of the COVID-19 pandemic has considerably accelerated the adoption of teleconsultationā€”a form of consultation between patient and health care professional that occurs via videoconferencing platforms. For this reason, it is important to investigate the way in which this form of interaction modifies the nature of the clinical encounter and the extent to which this modification impacts the healing process. For this purpose, I will refer to insights into the clinical encounter as a face-to-face encounter drawn from the phenomenology of medicine (R. Zaner, K. Toombs, E. Pellegrino). I will also take into account a criticism that has been expressed by various contemporary phenomenologists (H. Dreyfus, T. Fuchs, L. Dolezal, H. Carel), namely, that due to the lack of physical proximity to the other in all types of online encounters, such encounters lack significant features that are present in face-to-face encounters, with the most important of these being the possibility of attaining an empathetic perception of the other and a sense of embodied risk. As these elements are essential features of the clinical encounter, the aim of this paper is to determine whether teleconsultation exhibits these features. To do that, I will integrate phenomenological philosophy with qualitative research drawing materials from both the philosophical tradition, particularly with respect to the concepts of the face-to-face encounter and embodied risk (A. Schutz and H. Dreyfus), and qualitative research study regarding patient experiences of teleconsultation.     Body objectified? Phenomenological perspective on patient objectification in teleconsultation

    The global crisis of COVID-19 pandemic has considerably accelerated the use of teleconsultation (consultation between the patient and the doctor via video platforms). While it has some obvious benefits and drawbacks for both the patient and the doctor, it is important to considerā€”how teleconsultation impacts the quality of the patient-doctor relationship? I will approach this question through the lens of phenomenology of the body, focusing on the questionā€”what happens to the patient objectification in teleconsultation? To answer this question I will adopt a phenomenological approach combining both insights drawn from the phenomenological tradition, i.e., the concepts of the lived body and the object body, and the results from the phenomenologically informed qualitative research study on the patient experience of teleconsultation. The theoretical background against which I have developed this study comprises discussions within the field of phenomenology of medicine regarding the different sources of patient objectification within clinical encounter and the arguments concerning the negative impact that objectification has on the quality of care. I will argue that a factor that has frequently been identified within phenomenology of medicine as the main source of patient objectification in clinical encounters, namely, the internalized gaze of the clinician, is diminished during teleconsultation, increasing patientā€™s sense of agency, decreasing her sense of alienation and opening up the possibility for a closer relationship between the patient and the health care provider, all of which lead to the transformation of the hierarchical patient-health care professional relationship.This research is funded by European Regional Development Fund, University of Latvia and the State budget/Post-Doctoral Research Aid, 4th Stage/1.1.1.2/VIAA/4/20/622/Healing at a distance: phenomenological analysis of patient experience of clinical encounter in telemedicin

    The four dimensions of embodiment and the experience of Illness

    Publisher Copyright: Ā© 2018 Centre for Philosophical Research.In this paper I will try to systematically lay out and describe the multiple dimensions of the embodied experience of illness, which until recently has been the main focus within the field of the phenomenology of medicine. In order to do this, I will turn to analysis of the nature of embodiment in Husserl's phenomenology. I will argue that based on Husserl's phenomenology of the body, one can distinguish four ways of experiencing one's body, or four dimensions of embodiment. I will distinguish between experience of one's body as 1) a bearer of sensations (the affective dimension of embodiment); 2) a seat of free movement, characterized by the faculty of "I can" (the functional dimension of embodiment); 3) a material thing in a causal relationship with the material world (the material dimension of embodiment); 4) a material thing embedded in a social context (the social dimension of embodiment). I will then apply the proposed classification of dimen-sions of embodiment to the experience of illness, focusing on the various ways these dimensions have been used within the phenomenology of medicine. This serves two pur-poses. Firstly, it provides a framework for classification of the existing interpretations of the experience of illness; secondly, it opens up a way for a comprehensive analysis of the experience of illness which includes both individual and social aspects of embodiment as well as their mutual intertwining.publishersversionPeer reviewe

    Prevention of Disease and the Absent Body: A Phenomenological Approach to Periodontitis

    A large part of contemporary phenomenology of medicine has been devoted to accounts of health and illness, arguing that they contribute to the improvement of healthcare. Less focus has been paid to the issue of prevention of disease and the associated difficulty of adhering to health-promoting behaviours, which is arguably of equal importance. This article offers a phenomenological account of this disease prevention, focusing on how we ā€“ as embodied beings ā€“ engage with health-promoting behaviours. It specifically considers how we engage with oral hygiene regimes to prevent periodontitis and why we are not good at it. The article suggests that poor adherence to health-promoting behaviours can be explained with reference to the concept of the absent body, as prevention of disease is generally concerned with pre-symptomatic illness experience. The final section contains a discussion of some strategies for the improvement of disease prevention based on this viewpoint

    Anotācija Promocijas darba ā€žAbsolÅ«ta fenomena iespējamÄ«ba Žana-Lika Mariona fenomenoloÄ£ijāā€ mērÄ·is ir noskaidrot, vai un kādā nozÄ«mē absolÅ«ts fenomens ir iespējams franču fenomenologa Žana Lika Mariona fenomenoloÄ£ijas ietvaros. Ņemot vērā to, ka Mariona fenomenoloÄ£ijas ietvaros kā absolÅ«ts fenomens tiek raksturots piesātinātais fenomens (le phĆ©nomĆØne saturĆ©) ā€“ intuitÄ«va dotÄ«ba, kas rāda sevi sevÄ« un no sevis ā€“, darba pirmā un otrā daļa attiecÄ«gi ir veltÄ«tas piesātinātā fenomena jēdziena analÄ«zei un ar Å”o jēdzienu saistÄ«tu pieņēmumu uzrādÄ«Å”anai un izvērtÄ“Å”anai, secinot, ka, lai arÄ« Marions to nav darÄ«jis, piesātinātais fenomens ir jādiferencē, atsaucoties uz intuÄ«cijas raksturojumu un lomu. Var izŔķirt saturiski diferencētu intuitÄ«vu dotÄ«bu, saturiski nediferencētu intuitÄ«vu dotÄ«bu un neintuitÄ«vu dotÄ«bu jeb atklāsmes fenomenu. Balstoties Huserla fenomenoloÄ£ijā atrodamajās idejās, kā arÄ« kritikas, kas vērsta pret absolÅ«ta fenomena iespējamÄ«bu, izvērtējumā, darba treÅ”ajā daļā tiek argumentēts 1) par saturiski diferencētas intuitÄ«vās dotÄ«bas intencionālo raksturu, atceļot tās absolÅ«tumu; 2) par saturiski nediferencētas intuitÄ«vās dotÄ«bas neintencionālo raksturu, saglabājot tās absolÅ«tumu un 3) par neintuitÄ«vas dotÄ«bas nepieredzamÄ«bu, vienādojot to ar metafizisku pieņēmumu un atceļot tās absolÅ«tumu. Tiek secināts, ka, pretēji Mariona uzskatiem, ne visi piesātinātie fenomeni ir absolÅ«ti fenomeni ā€“ absolÅ«ts fenomens ir iespējams tikai kā saturiski nediferencēta, laiciski neizplesta un visaptveroÅ”a intuitÄ«va dotÄ«ba.Annotation The aim of this research is to ascertain whether and in what sense absolute phenomenon is possible within the framework of the French phenomenologist Jean-Luc Marionā€™s phenomenology. Because absolute phenomenon is characterized as the saturated phenomenon (le phĆ©nomĆØne saturĆ©) ā€“ intuitive givenness that shows itself in and from itself ā€“ the first and second parts of the work deal with the analysis and evaluation of the concept of the saturated phenomenon, concluding that, although Marion has not done so himself, one must differentiate the saturated phenomenon based on the understanding and role of intuition within it. It is possible to distinguish between internally differentiated intuitive givenness, internally undifferentiated intuitive givenness and non-intuitive givenness (or the phenomenon of revelation). Based on both the ideas found in Husserlā€™s phenomenology and the evaluation of the critique directed against the possibility of absolute phenomenon, the third part of the work argues for the intentional character of internally differentiated intuitive givenness, annulling its absoluteness, for the non-intentional character of internally undifferentiated intuitive givenness, establishing its absoluteness and for the non-experiencability of non-intuitive givenness, rendering it a speculative ideal and annulling its absoluteness. It is concluded that, contrary to Marionā€™s opinions, not all saturated phenomena are absolute phenomena ā€“ absolute phenomenon is possible only as internally undifferentiated, atemporal and all-embracing intuitive givenness
