Limb edema in patients with advanced disease — a pilot study of compression therapy combined with diuretics
Introduction. Management of edema due to advanced disease is seldom recognized in palliative care. Thepurpose of this pilot study was to assess the effectiveness and tolerance of conservative edema therapy.Material and methods. Prospective analysis of limb volumes, edema symptom burden and quality of life(QoL) in 119 patients diagnosed with edema was performed. Eighteen patients in their last days of lifecomplaining of edema were treated by limb elevation, supportive (palliative) bandaging, manual lymphaticdrainage, Kinesio Taping and/or controlled subcutaneous needle drainage. Forty-six with longer prognosiswere treated by one-week multilayer short-stretch limb bandaging and then re-evaluated. Twenty-eightof them with venous congestion resistant to previously given diuretics received additionally furosemideinfusion in hypertonic saline. Subgroups analysis of the patients with diuretic therapy and primary lymphaticcongestion was performed.Results. Within 46 patients re-evaluated the reduction of limb volume was accompanied by a decreaseof edema symptom intensity and an improvement of QoL. Subgroup analysis revealed both volume andsymptomatic improvement, equally effective in cases with suspected lymphatic congestion or when diureticswere added.Conclusion. Edema of advanced disease irrespective of the lymphatic component can be effectivelymanaged conservatively with short–stretch limb bandaging. In cases resistant to oral diuretic fluid overload,parenteral hypersaline furosemide addition to the compression may be the safe therapeutic optionenhancing treatment tolerance
Effectiveness of octreotide in advanced cancer patients with malignant bowel obstruction and intractable diarrhea
Malignant bowel obstruction and intractable diarrhea in advanced cancer are common devastating syndromes.
Apart from disease specific, causative treatment in patients with longer prognosis, there is a
possibility of conservative, non-operative symptom management in terminal phase. A successful control of
intractable vomiting and diarrhea using octreotide in two advanced cancer patients was presented, with
guidance through steps of the management. Pathophysiologic vicious circle of the bowel obstruction and
diarrhoea was described and the management algorithm was proposed. Due to the gut hyposecreting

Frailty - an overview for oncologists
Frailty - zespół słabości - występuje w populacji osób starszych z przewlekłymi problemami zdrowotnymi
i wiąże się z postępującym zmniejszaniem się sprawności fizycznej. Jest to syndrom fizjologiczny,
charakteryzujący się zmniejszeniem rezerw i odporności na czynniki stresogenne. Zespół słabości wiąże
się ze zwiększonym ryzykiem powikłań, takich jak utrata samodzielności, konieczność częstszych hospitalizacji,
podatność na częstsze zachorowania czy nawet śmierć z powodu niekiedy nawet początkowo
błahego powodu. Wzrost stężenia markerów zapalnych, zaburzenia krzepnięcia, związane z wiekiem
zmiany w układzie hormonalnym oraz utrata masy mięśniowej lub sarkopenia wydają się ściśle związane
z powstawaniem tego zespołu. Kliniczne kryteria rozpoznania to: zmniejszenie masy ciała, osłabienie,
wyczerpanie, spowolnienie chodu i obniżona aktywność fizyczna.

efficiency. It is a physiological syndrome consisted of the diminishing physical reserves and also decreasing
resistance for the various stressors. It can lead to the social dependence and the increased risk of
hospitalization, morbidity and mortality rate, even due to the trivial causes. High level of inflammatory mediators,
coagulation disturbances, age related changes in hormone levels, muscle loss or sarcopenia
appear to be connected with this syndrome. Clinical diagnosis consists of the weight loss, asthenia, fatigue,

The use of manual lymph drainage in vascular diseases
Ręczny drenaż limfatyczny (MLD) jest jedną z metod stosowaną w leczeniu obrzęków wynikających z zaburzeń
funkcji układu chłonnego — jego przeciążenia lub uszkodzenia. W pracy omówiono uzasadnienie fizjologiczne
MLD w terapii niewydolności chłonnej. Pomimo braku obiektywnego potwierdzenia efektu klinicznego drenażu
jako samodzielnej metody leczniczej pozostaje on cennym uzupełnieniem terapii, nie tylko w angiologii,
ale także w rehabilitacji, medycynie sportowej czy kosmetologii.

drainage (MLD). In this article the physiological basis of MLD usage in lymphatic insufficiency have been
described. Although studies show only a transient effect of MLD used alone, it may be beneficial as an adjunctive
therapy not only in angiology, but also in rehabilitation, sport medicine, and cosmetology.

Edmonton Symptom Assessment System: Psychometric validation of six-point Verbal Rating Scale in Polish hospice setting
Introduction: The Edmonton Symptom Assessment System (ESAS) quantifying the most common symptoms rated in the numerical rating scale (NRS) for some respondents are cumbersome or incomprehensible. The study aimed to introduce, implement and validate psychometric properties of the ESAS (revisited) six-point Verbal Rating Scale — Polish version (ESAS-r 6VRS-PL) into a Polish hospice setting.
Methods: In a cross-sectional, observational prospective psychometric validation study the adult advanced cancer patients admitted to a hospice were evaluated twice for test-retest reliability. Each patient’s evaluation encompassed the ESAS-r 6VRS-PL, which was compared on the first day with the validated Polish version of the European Organisation for Research and Treatment of Cancer QLQ-C15-PAL, considered the gold standard, to accomplish its external construct validity.
Results: Of the 57 patients enrolled 48 were evaluated twice and 9 (15.5%) dropped out. All respondents were able to describe their symptoms in < 15 minutes independently without any need to clarify the meaning of the questions and only occasionally with assistance in reading the questions or answers. The high intraclass correlation coefficient of the ESAS Global Distress Score was noted (0.825; 95%CI 0.723–0.888). Similar items of the ESAS-r 6VRS-PL and QLQ-C15-PAL showed moderate correlations (range 0.59–0.65; p < 0.001). The internal consistency of the ESAS-r 6VRS-PL measured using Cronbach’s alpha coefficient was fairly high, reaching 0.723 (item range 0.664–0.734).
Conclusion: The ESAS-r 6VRS-PL is a simple, valid, reliable and feasible test that allows estimating the QoL in advanced cancer patients
Postępowanie w profilaktyce i leczeniu zachowawczym obrzęku chłonnego — przegląd piśmiennictwa
Lymphoedema, which is understood as a result of mechanical (low output) failure of the lymphatic system, may be a result of abnormalities of structure and function of the lymphatic system (primary lymphoedema), injuries, inflammation, chronic vein insufficiency or related to cancer (secondary lymphoedema). Because of chronic nature of the illness and the lack of possibilities to be cured, education and prevention play an important role. In the developed lymphoedema complex physical therapy is used which an indispensable factor is compression therapy. This paper presents current review of knowledge using in the prevention and the treatment of lymphatic insufficiency and also describes barriers in the sufficient management of lymphedema in the health care system.Obrzęk chłonny, rozumiany jako wynik mechanicznej niewydolności układu chłonnego, może być wynikiem nieprawidłowości budowy i funkcji układu chłonnego (postać pierwotna), urazów, zapalenia, przewlekłej niewydolności żylnej lub związany z chorobą nowotworową (postać wtórna). Ze względu na przewlekły charakter schorzenia i brak możliwości całkowitego wyleczenia, ważną rolę w postępowaniu odgrywają edukacja i profilaktyka. W rozwiniętym obrzęku chłonnym wykorzystuje się kompleksową fizjoterapię, której niezbędnym elementem jest kompresjoterapia. W pracy przedstawiono aktualne metody profilaktyczne i lecznicze, a także istniejące bariery w systemie opieki zdrowotnej w skutecznym postępowaniu z obrzękiem chłonnym
Skierowanie, jako narzędzie oceny wskazań do pilnego przyjęcia do opieki paliatywnej i hospicyjnej — prospektywne badanie obserwacyjne
Qualification to palliative care is more demanding when the number of referred patients exceeds the admission capabilities of an institution. In such cases the selection of urgent admission needs (when management aside from hospice care is problematic) may be essential. For this purpose the palliative care referral form (PCRF) containing additional information on disease stage, patient’s performance, symptoms of suffering, previously applied therapies and social concerns was developed and prospectively analyzed.
The aim of the study was to assess the qualification to palliative care accuracy in a group of 41 patients based on the PCRF. At first the qualification was based solely on the PCRF data, then (on the second day after admission) a structured interview with an attending physician was conducted. Pairs of these data were compared and analyzed.
All palliative referrals were legitimized on admission. Among 26 patients triaged as “urgent”, 19 were such on admission, within 15 staged as “stable” 13 were confirmed later. Also, the disease stage and patients’ performance were usually confirmed on admission. The high PCRF ability to predict the need of urgent implementation of palliative care was observed. (positive predictive value of 73,1%, sensitivity 90,5%)
The PCFR which is legitimate, provides the essential information for preliminary qualification to palliative care and accurate assessment of its urgent indications. It may simplify and accelerate the process thus lowering costs. Wider practical application of the PCRF in the hospice care is proposed.Wstęp. Kwalifikowanie pacjentów do objęcia opieką paliatywną i hospicyjną nabiera szczególnego znaczenia, kiedy zapotrzebowanie na nią przewyższa możliwości ośrodków oph. Powstaje wtedy konieczność selekcji kierowanych chorych tak, by w pierwszej kolejności udzielać świadczeń zdrowotnych tym, których potrzeby (np. trudne do opanowania objawy) nie mogą być zabezpieczone w innych warunkach. W tym celu opracowano druk skierowania rozszerzony o dodatkowe informacje (w tym etap choroby, stopień sprawności, objawy cierpienia, terapia i problemy społeczne), którego przydatność poddano prospektywnej analizie.
Materiał i metody. Celem pracy była ocena trafności kwalifikacji chorych do pilnego przyjęcia do stacjonarnej oph na podstawie informacji zawartych na badanym skierowaniu. Badaniem objęto 41 osób. W pierwszej kolejności dokonano oceny wskazań do pilnego przyjęcia wyłącznie na podstawie skierowań. Następnie, w drugiej dobie po przyjęciu chorego, przeprowadzono ustrukturyzowane wywiady z lekarzami przyjmującymi pacjentów, gromadząc dane podobnie, jak na skierowaniu. Zebrane pary danych zestawiono ze sobą i poddano analizie.
Wyniki. U wszystkich badanych osób potwierdzono przy przyjęciu wskazania do objęcia oph. Na 26 chorych zakwalifikowanych do przyjęcia ze wskazań pilnych potwierdzono ten stan u 19. W grupie 15 zakwalifikowanych jako „stabilni”, po przyjęciu podobną kwalifikację uzyskało 13. Przy przyjęciu zwykle potwierdzano także zarówno etap choroby, jak i stopień sprawności pacjenta. Stwierdzono wysoką zdolność do przewidywania potrzeby pilnego przyjęcia na podstawie badanego druku skierowania (dodatnia wartość predykcyjna 73,1% przy czułości 90,5%).
Wnioski. Opracowany druk umożliwia zgodne z przepisami zebranie informacji niezbędnych do wstępnej kwalifikacji do oph, trafnego określenia wskazań pilnych oraz znacząco upraszcza i przyspiesza proces kwalifikowania pacjentów, obniżając jego koszty. Proponuje się szersze wykorzystanie druku w hospicyjnej praktyce klinicznej
Zgoda chorego na leczenie u kresu życia
W pracy przedstawiono problem informowania chorych kierowanych do opieki paliatywnej oraz prawnoetyczne
argumenty przemawiające za uzyskiwaniem uświadomionej zgody chorego na tę opiekę. Omówiono
także aspekty zgody zastępczej oraz przyjmowania chorych niekompetentnych do hospicjów.
