    Le projet Clothilde est un partenariat franco-suisse entre deux universitĂ©s et deux entreprises. Ce projet vise Ă  minimiser les risques opĂ©ratoires en proposant des dispositifs originaux et fiables pour la chirurgie orthopĂ©dique de l’épaule. Pour rĂ©pondre Ă  cette problĂ©matique, u nimplant d’ostĂ©osynthĂšse permettant de traiter des fractures traumatiques proximales et diaphysaires rĂ©alisĂ© en impression mĂ©tal est proposĂ©. Ce dispositif est composĂ© d’un verrouillage distal interne Ă  foyer fermĂ© combinĂ© Ă  un verrouillage proximal offrant une grande flexibilitĂ© dans la reconstruction des fragments osseux. Des outils spĂ©cifiques ont Ă©tĂ© dĂ©veloppĂ©s afin de confirmer la tenue mĂ©canique du dispositif de soin. La procĂ©dure chirurgicale a Ă©tĂ© validĂ©e en laboratoire d’anatomie par un panel de chirurgiens.     The main challenges of this project are to achieve in less than 3 years, an innovative implant for retaining as much as possible bone mass (promote the use of an implant rather than a prosthesis which requires the removal of the head of the humerus), anticipate complications of bone necrosis, to minimize the size of the incisions (minimally invasive technique) for a cost and a surgery time comparable or lower than current implants. Two major types of solutions are envisaged to treat the fracture: A customized implant (adapted to the morphology of patients and various fractures), which will be removed after 18 months to 2 years with a range of adaptable to all morphologies prostheses with a number of references as limited as possible. Both solutions will be supported by the same standard attachment part in healthy diaphyseal portion of the humerus. The specific scope of OneOrtho-Medical in terms of rapid prototyping manufacturing by laser fusion, will enable us to achieve quickly some parts of this custom implant to consider a settlement within 48 hours after trauma.Les principaux enjeux de ce projet sont de pouvoir proposer aux termes des trois annĂ©es, un implant innovant permettant de conserver le plus souvent possible le capital osseux (donc de favoriser l’utilisation d’un implant plutĂŽt que d’une prothĂšse qui nĂ©cessite l’ablation de la tĂȘte de l’humĂ©rus), d'anticiper les complications liĂ©es Ă  la nĂ©crose osseuse, de minimiser la taille des incisions (technique mini invasive) pour un coĂ»t et un temps d'intervention comparable ou infĂ©rieurs aux implants actuels. Deux grands types de solutions sont envisagĂ©s pour traiter la fracture : Un implant sur mesure (adaptĂ© Ă  la morphologie des patients et au type de fracture), qui sera dĂ©s-implantĂ© au bout de 18 mois Ă  2 ans et une gamme de prothĂšses adaptable Ă  toutes morphologies avec un nombre de rĂ©fĂ©rence le plus limitĂ© possible. Ces deux solutions seront supportĂ©es par une mĂȘme partie standard de fixation dans la partie diaphysaire saine de l’humĂ©rus. Les compĂ©tences spĂ©cifiques de la sociĂ©tĂ© OneOrtho-Medical en termes de fabrication rapide par prototypage par fusion laser vont nous permettre d’envisager de rĂ©aliser en des temps record tout ou partie de cet implant sur mesure afin d'envisager une implantation sous 48h aprĂšs traumatisme
