    Technical Note: A comparison of central and peripheral intraocular pressure using rebound tonometry

    Purpose: To compare central and peripheral intraocular pressure (IOP) readings obtained with rebound tonometry.Methods: Intraocular pressure was measured on the right eye of 153 patients (65 males, 88 females), aged from 21 to 85 years (mean +/- S.D., 55.5 +/- 15.2 years) with the ICare rebound tonometer at centre, and 2 mm from the limbus (in the nasal and temporal regions along the 0-180O corneal meridian).Results: Intraocular pressure values obtained with the ICare were 14.9 +/- 2.8; 14.1 +/- 2.5 and 14.5 +/- 2.7 mmHg at centre, nasal and temporal corneal locations, respectively. On average, nasal and temporal IOP readings were 0.75 and 0.37 mmHg lower than the central reading (p 0.05, respectively). A highly significant correlation was found between central and peripheral measurements in nasal (r(2) = 0.905; p < 0.001) and temporal (r(2) = 0.879; p < 0.001) regions along the horizontal meridian. Almost 80% of patients presented nasal IOP values within +/- 1 mmHg of the central value.Conclusions: Intraocular pressure values measured with the ICare (R) rebound tonometer on the nasal corneal region is slightly lower on average and highly correlated with IOP values recorded at corneal centre. Both nasal and temporal readings are in good agreement with central IOP, and could be used to obtain a reliable estimate of rebound IOP in corneas where central readings cannot be taken.- (undefined

    Cell Pattern in Adult Human Corneal Endothelium

    A review of the current data on the cell density of normal adult human endothelial cells was carried out in order to establish some common parameters appearing in the different considered populations. From the analysis of cell growth patterns, it is inferred that the cell aging rate is similar for each of the different considered populations. Also, the morphology, the cell distribution and the tendency to hexagonallity are studied. The results are consistent with the hypothesis that this phenomenon is analogous with cell behavior in other structures such as dry foams and grains in polycrystalline materials. Therefore, its driving force may be controlled by the surface tension and the mobility of the boundaries

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Determination of central corneal clearance in scleral lenses with an optical biometer and agreement with subjective evaluation

    Purpose: To compare three methods to measure central corneal clearance (CCC) during scleral lens wear: subjective (slit lamp), image processed (ImageJ) and with an optic biometer. The optic biometer technique was validated in comparison to an OCT in the first part of the study.Methods: Twenty-two eyes (11 subjects) with healthy corneas were recruited. Three measures of OCT with scleral lens and ten measurements of axial length (AL) with IOLMaster with and without lens were performed. For the second part, 61 eyes (35 subjects) enrolled in a clinical study were selected. Measurements of CCC were done with IOLMaster, SlitLamp and ImageJ.Results: The measurements of CCC indirectly obtained with IOLMaster had a strong correlation with AS-OCT measurements (r = 0.981), showing a mean difference of 122.18 +/- 46.05 mu m (higher with IOLMaster). Regarding the second part, measurements of CCC were 238.66 +/- 95.94 mu m, 250.16 +/- 124.31 mu m and 263.15 +/- 90.60 mu m, for the IOLMaster, SlitLamp and ImageJ, respectively. The correlations were higher for ImageJ vs Subjective measure (r = 0.891) than for IOL vs Subjective (r = 0.748) and IOL vs ImageJ (r = 0.745). Analysis of differences and correlations between SlitLamp and ImageJ through time showed a mean difference of -32.28 +/- 89.95 mu m (r = 0.683) at V1month, 12.53 +/- 59.46 mu m (r = 0.850) at V6months and 11.57 +/- 32.95 mu m (r = 0.940) at V12months.Conclusions: It is possible to measure CCC with IOLMaster, considering AL measured with and without lens and lens thickness. The three methods tested have good correspondence, showing that IOLMaster and ImageJ could be objective techniques to measure CCC.
