Rhegmatogenous retinal detachment masquerading as exudative panuveitis with intense anterior chamber inflammatory reaction.
Purpose:This is a retrospective case report illustrating the diagnostic and therapeutic challenges associated with a chronic rhegmatogenous retinal detachment masquerading as a severe panuveitis with intense anterior chamber inflammation. We have included clinical features, anterior segment and fundus photography, B-scan ultrasonography, fluorescein angiography, and intraoperative findings. Observations:A 26-year-old male presented with features of unilateral panuveitis: hypotony, anterior segment inflammation (posterior synechiae and anterior chamber cell with fibrin clumping), diffuse choroidal thickening, and retinal detachment. Laboratory investigations for infectious or rheumatologic processes were negative, and empiric systemic corticosteroid therapy was unsuccessful. This prompted suspicion for an alternate primary etiology, and pars plana vitrectomy revealed small retinal breaks as the underlying cause of the retinal detachment and inflammation. Conclusions:Rhegmatogenous retinal detachments are a known cause of intraocular inflammation. Nevertheless, it remains a challenge to recognize retinal breaks in this setting, particularly with robust anterior segment inflammation and posterior findings resembling severe exudative uveitis. Being aware of this unique presentation may prevent delays in diagnosis and have important prognostic implications
Health-related quality of life and depression among participants in the Sjögren's International Collaborative Clinical Alliance registry.
ObjectiveTo examine health-related quality of life (HRQoL) and depression among participants in an international Sjögren's syndrome (SS) registry, comparing those with and without SS.MethodsCross-sectional study of participants in the Sjögren's International Collaborative Clinical Alliance (SICCA) registry. The 2016 American College of Rheumatology/European League Against Rheumatism SS classification criteria were used to determine disease status. HRQoL was assessed using the Short Form 12, version 2 Health Survey to derive scores for physical component summary (PCS) and mental component summary (MCS). Depression was assessed using the 9-Item Patient Health Questionnaire. Multivariate linear and logistic regression analyses were performed to identify predictors of HRQoL and depression while controlling for potential confounders.ResultsAmong 2401 SICCA participants who had symptoms of dry eyes and dry mouth, 1051 had SS (44%) and 1350 did not (56%). After controlling for confounders, when compared with non-SS participants, those with SS had better PCS (p<0.001, β=2.43, 95% CI 1.57 to 3.29), MCS (p=0.002, β=1.37, 95% CI 0.50 to 2.23) and lower adjusted odds of depression (p<0.001, OR 0.67, 95% CI 0.55 to 0.81). Other significant predictors of HRQoL and depression included employment, country of residence and use of medication with anticholinergic effect or for management of SS-related signs and symptoms.ConclusionOur results suggest that among symptomatic patients, having a diagnosis of SS may be associated with better emotional and psychological well-being compared with patients without a diagnosis. Having a definitive diagnosis of SS may encourage patients to obtain a better understanding of their disease and have coping mechanisms in place to better manage their symptoms
Metagenomic deep sequencing of aqueous fluid detects intraocular lymphomas.
IntroductionCurrently, the detection of pathogens or mutations associated with intraocular lymphomas heavily relies on prespecified, directed PCRs. With metagenomic deep sequencing (MDS), an unbiased high-throughput sequencing approach, all pathogens as well as all mutations present in the host's genome can be detected in the same small amount of ocular fluid.MethodsIn this cross-sectional case series, aqueous fluid samples from two patients were submitted to MDS to identify pathogens as well as common and rare cancer mutations.ResultsMDS of aqueous fluid from the first patient with vitreal lymphoma revealed the presence of both Epstein-Barr virus (HHV-4/EBV) and human herpes virus 8 (HHV-8) RNA. Aqueous fluid from the second patient with intraocular B-cell lymphoma demonstrated a less common mutation in the MYD88 gene associated with B-cell lymphoma.ConclusionMDS detects pathogens that, in some instances, may drive the development of intraocular lymphomas. Moreover, MDS is able to identify both common and rare mutations associated with lymphomas
Relación entre depresión y dolor crónico por osteoartrosis en adultos Hospital Santa Rosa – Piura en el periodo enero - diciembre 2017
La osteoartrosis se caracteriza por la alteración del cartílago y del hueso subcondral. que aparece con frecuencia a partir de los cuarenta años y edades avanzadas. La osteoartrosis de cadera es poco prevalente en Jamaica, Sudáfrica, y regiones de China (1-4%) en comparación con los países europeos, donde alcanza hasta un 25%. En lo concerniente a las rodillas, se demostró radiológicamente una mayor prevalencia de osteoartrosis entre mujeres de Beijing (China) en comparación con población caucásica americana. Se estima que el 70-90% de los mayores de 75 años tienen algún signo radiológico de OA de las manos, siendo sintomática un 10% de estos pacientes.
Es importante y beneficioso para la salud pública, porque nos permitirá otorgar un correcto tratamiento del dolor crónico en el adulto mayor (antidepresivos, anticonvulsivantes) y de un apoyo psicológico, lo cual le permitirá mejorar sus relaciones familiares y de socialización, puesto que el dolor y la depresión son síntomas que se asocian y se solapan con frecuencia, siendo difícil diferenciar un síntoma de otro. La depresión es un estado de ánimo negativo, que hace que el paciente pierda la capacidad y el deseo de enfrentarse ante cualquier situación, y la vida cotidiana.

Osteoarthrosis of the hip is not very prevalent in Jamaica, South Africa, and regions of China (1-4%) compared to European countries, where it reaches up to 25%. With respect to the knees, a higher prevalence of osteoarthrosis was demonstrated radiologically among women from Beijing (China) compared to the Caucasian population of the United States. It is estimated that 70-90% of those older than 75 years have some radiological sign of OA of the hands, with 10% of these patients being symptomatic.

Probable zoonotic aortitis due to group C streptococcal infection
Human infections due to group C streptococcus (SGC) are unusual. Among them, vascular compromise, especially aortic involvement, is extremely rare. A case of microbial aortitis with aneurysm formation, likely secondary to a SGC soft tissue infection, in a 61-year-old patient who was caring for a purulent leg wound of his horse, is presented. He was successfully treated with antibiotics and in situ aortic replacement with a prosthetic graft and an omental wrap. He remains well almost 2 years after surgical intervention. Aortic infection caused by SGC is a rare condition that can be successfully treated with in situ prosthetic graft replacement
Fluoride and Phosphate Removal From Industrial and Domestic Wastewaters Using Cerium Chloride
The use of cerium chloride (CeCl3) to remove fluoride and phosphate from waters is addressed in this study. High concentrations of fluoride exist in groundwater especially in developing countries. Consumption drinking water containing high levels of fluoride can lead to serious cases of dental and skeletal fluorosis. Current defluoridation technologies are limited, especially for high levels of fluoride, and are expensive. Industrial wastewaters contribute to the highest fluoride contamination in the world. With the increasing production of electronic materials, the global fluoride concentration and fluoride-contaminated waters have grown tremendously. Excessive discharge of phosphate into the environment promotes eutrophication of lakes and rivers. The major sources of phosphate pollution are agricultural and urban runoff and domestic wastewater discharges. Cerium chloride, a newly available coagulant is expected to remove both fluoride and phosphate from waters A commercially available cerium chloride e (i.e. Sorbx-100) which is a non-hazardous rare-earth salt solution developed for faster coagulation and flocculation of phosphate in wastewater has been recently made available(Molycorp Minerals Incorporated, n.d.). The objectives of this research are to determine the efficiency efficacy of cerium chloride in removing fluoride and phosphate from waters. Batch jar testing was performed to evaluate fluoride removal under different conditions. Column testing was used to test the phosphate removal efficiency of various media impregnated with cerium chloride Batch tests were used to support central composite design (CCD) model to evaluate the impact of major parameters (e.g. fluoride concentration, pH, and cerium dose) on the removal of fluoride from industrial wastewater. In addition, batch tests were also used to investigate the interference of competing ions on the removal of fluoride using cerium chloride.
The CCD model achieved an R-squared = 0.8615 and adjusted R-squared = 0.7368. The model was deemed to be statistically significant. The results showed that the highest removal (\u3e 90%) was achieved at pH of 4.75 at cerium dose of 25 mM, regardless fluoride concentration. The results revealed that sulfate and phosphate have a positive impact on fluoride removal. Bicarbonate was shown to have as a negative impact on fluoride removal. For an actual industrial wastewater containing fluoride and with high alkalinity, no fluoride removal was observed with addition of cerium chloride. The observed fluoride removals can be attributed to two mechanisms: direction precipitation of cerium fluoride, and adsorption of fluoride ions and fluoride complexes onto the surfaces of cerium hydroxides and/or cerium carbonate. Impregnation of various filter media with cerium chloride and using various techniques was proven unsuccessful for phosphate (PO4-3-P) removal. Analysis of the cerium on the column effluent samples showed cerium leaching out of the media. Therefore, all methods for preparing impregnated media were ineffective
“What’s this got to do with PE? Why can’t we just play?”: Exploring the effects on intentionally implementing social emotional learning through physical education classes
Physical Education (PE) has become an increasingly complex learning environment. Prior research emphasized the propitious role of PE in teaching personal and social responsibility, skills, physical activity, and health. The purpose of this thesis was to explore the integration of Social Emotional Learning (SEL; CASEL, 2020) concepts and activities through mainstream PE classes, which utilized the teaching personal and social responsibility (TPSR; Hellison, 2011) framework, national and state standards, and an established school curriculum. The study aimed to examine the effect of intentionally implementing SEL in PE from a practitioner perspective and gathered feedback from students about the effect and class design. Teacher Action Research (Pine, 2009) was conducted over one semester, in which twenty-four lessons were implemented in an 8th-grade class in a Boston area public middle school. This period of adolescent development is often considered a complex and challenging time due to a combination of complex, overlapping determinants (Hall, 1904).
Observations were made, student voices provided feedback and input on the class design, and additional sources of qualitative data were collected and triangulated for rigor.
The findings of this study shed light on the inclusion of SEL in PE classes, identifying mechanisms for facilitating their delivery and promoting meaningful engagement among students. The research also provided valuable insights into student development, personal growth, and refined methods of practice for the researcher.
Through intentional integration of SEL in PE, this study contributes to the development of more inclusive and meaningful experiences for all students, aligning with the goal of fostering physically literate individuals who possess the knowledge, skills, and confidence to enjoy a lifetime of healthful physical activity. The insights provide a foundation for further exploration of SEL implementation in PE settings and offer practical implications for physical education teachers and coaches seeking to enhance their curriculum and support students' holistic development
The TRENDS High-Contrast Imaging Survey. VI. Discovery of a Mass, Age, and Metallicity Benchmark Brown Dwarf
The mass and age of substellar objects are degenerate parameters leaving the
evolutionary state of brown dwarfs ambiguous without additional information.
Theoretical models are normally used to help distinguish between old, massive
brown dwarfs and young, low mass brown dwarfs but these models have yet to be
properly calibrated. We have carried out an infrared high-contrast imaging
program with the goal of detecting substellar objects as companions to nearby
stars to help break degeneracies in inferred physical properties such as mass,
age, and composition. Rather than using imaging observations alone, our targets
are pre-selected based on the existence of dynamical accelerations informed
from years of stellar radial velocity (RV) measurements. In this paper, we
present the discovery of a rare benchmark brown dwarf orbiting the nearby
( pc), solar-type (G9V) star HD 4747 ([Fe/H]=)
with a projected separation of only AU (
0.6"). Precise Doppler measurements taken over 18 years reveal the companion's
orbit and allow us to place strong constraints on its mass using dynamics (). Relative photometry (,
, ) indicates that HD 4747 B is
most-likely a late-type L-dwarf and, if near the L/T transition, an intriguing
source for studying cloud physics, variability, and polarization. We estimate a
model-dependent mass of for an age of
Gyr based on gyrochronology. Combining astrometric measurements with RV data,
we calculate the companion dynamical mass () and orbit
() directly. As a new mass, age, and metallicity benchmark, HD
4747 B will serve as a laboratory for precision astrophysics to test
