    Quantitative Assessment of Desensitizing Agents in Occluding Dentine Tubules using Image Analysis

    Previous in vitro studies assessing the tubule occluding properties of various desensitizing agents in the dentine disc model appear to provide only qualitative data. The aim of this study was to establish a reliable and reproducible system to evaluate the in vitro effectiveness of three desensitizing agents. Six selected fields from SEM negatives (magnification x1000; working distance 10mm) of test and control dentine disc specimens treated with the desensitizing agents (Butler Protect, Colgate FluoriGard (GelKam), Macleans Sensitive) were evaluated. Fields were assessed using a Quantimet 520 Image analysis system (Leica UK) and the data recorded included patent tubule area, mean tubule diameter, mean patent/field area and number of tubules per unit area. Comparison of test and control specimens indicated that differences in the number of tubules, patent area, width of tubules and percentage of patent areas can be assessed quantitatively Furthermore, this methodology also demonstrated differences between the tubule occluding properties of the selected desensitizing agents

    On a Watson-like Uniqueness Theorem and Gevrey Expansions

    We present a maximal class of analytic functions, elements of which are in one-to-one correspondence with their asymptotic expansions. In recent decades it has been realized (B. Malgrange, J. Ecalle, J.-P. Ramis, Y. Sibuya et al.), that the formal power series solutions of a wide range of systems of ordinary (even non-linear) analytic differential equations are in fact the Gevrey expansions for the regular solutions. Watson's uniqueness theorem belongs to the foundations of this new theory. This paper contains a discussion of an extension of Watson's uniqueness theorem for classes of functions which admit a Gevrey expansion in angular regions of the complex plane with opening less than or equal to (\frac \pi k,) where (k) is the order of the Gevrey expansion. We present conditions which ensure uniqueness and which suggest an extension of Watson's representation theorem. These results may be applied for solutions of certain classes of differential equations to obtain the best accuracy estimate for the deviation of a solution from a finite sum of the corresponding Gevrey expansion.Comment: 18 pages, 4 figure

    Liposomal irinotecan and 5-fluorouracil/leucovorin in older patients with metastatic pancreatic cancer - A subgroup analysis of the pivotal NAPOLI-1 trial

Characterization of outcomes in these patients may help optimise treatment decisions. The global, phase 3 NAPOLI-1 trial ( NCT01494506 ) demonstrated an overall survival (OS) benefit with liposomal irinotecan and 5-flurouracil/leucovorin (nal-IRI + 5-FU/LV) versus 5-FU/LV. This subgroup analysis explored impact of age on outcomes in NAPOLI-1 patients, and nal-IRI + 5-FU/LV efficacy and safety in older patients. Materials and Methods This exploratory, post-hoc analysis of the NAPOLI-1 trial included patients aged ≥eighteen years (no upper limit) with metastatic pancreatic adenocarcinoma that had progressed on gemcitabine-based therapy. Patients were stratified by age (cut-offs at 65, 70, and 75 years); OS and progression-free survival (PFS) were estimated by Kaplan-Meier analysis. Results Of 417 randomized patients, 192 (46%), 110 (26%) and 43 (10%) were aged ≥65, ≥70 and ≥ 75 years, respectively. Mortality risk and risk of disease progression were similar in older and younger patients independent of treatment (HRs for median [m]OS/mPFS comparisons were 0.88/0.95 [ .25). Reduced mortality/morbidity risk with nal-IRI + 5-FU/LV in older subgroups was in line with the wider population. No additional toxicities with nal-IRI + 5-FU/LV were observed in older patients: 86% of patients ≥75 years versus 69% <75 years required a dose delay or reduction due to toxicities (43% versus 32% dose reductions). Discussion Results suggest that older patients with metastatic pancreatic adenocarcinoma that progressed on prior gemcitabine-based treatment can benefit from second-line therapy, supporting nal-IRI + 5-FU/LV treatment in older patients.The NAPOLI-1 trial (ClinicalTrials.govidentifier: NCT01494506) was sponsored by Merrimack Pharmaceuticals, Inc., Cambridge, MA, USA; Medical writing support was provided by Laura McMahon of Physicians World Europe GmbH, Mannheim, Germany, and funded by Shire International, Zug, Switzerland.     Prevalence and Pattern of Dentine Hypersensitivity in a population of patients at MGM Dental College

    Aim: The aim of this study was to determine the prevalence of Dentine Hypersensitivity (DH) in an Indian population and to assess the factors associated with this condition. Methods: 4200 subjects were examined in the Out-Patient Department (OPD) at the MGM Dental College, Navi Mumbai City. Patients who complained of dentine hypersensitivity were given the questionnaire to complete, in order to obtain the following information, demographics, dental history, eating habits and DH symptom data, and associated known factors relating to DH. In total 548 questionnaires were completed. Oral examinations assessed DH in participants who reported DH in at least one of their teeth. The subjects also quantified the severity of DH on 10-digit visual analogue scale. The diagnosis of DH was established by a short, sharp pain arising from exposed dentine in response to triple syringe air blast and explorer probe of the tooth surface. Results: The prevalence of self-reported DH was 13%; and that of clinically diagnosed DH was 8.2%. Numerically more males were examined compared to females although there were no differences in the prevalence of DH between the two groups. DH. The peak prevalence of DH was in the 30-39 years age group. Mandibular central incisors and first molars were significantly the most affected teeth. Cold (89.4%) was the most common cause of DH. Those subjects having moderate sensitivity for ≤ 6 months resorted to home remedies of brushing with a desensitizing paste to relieve the problem rather than visit their local dentist. However only 41 (8.8%) of the total participants claimed to have attended a dental practice. Conclusion: The results from the study would suggest that DH is a common prevalent dental condition in India. Although the condition appears to cause a degree of noticeable discomfort, most patients choose to either ignore the condition or simply tolerate it. Therefore, it can be assumed that the condition does not cause a significant effect on the quality of life or life style of the individual complaining of the problem

    Global processes of anthropogenesis characterise the early Anthropocene in the Japanese Islands

    Although many scholars date the onset of the Anthropocene to the Industrial Revolution or the post-1945 ‘Great Acceleration’, there is growing interest in understanding earlier human impacts on the earth system. Research on the ‘Palaeoanthropocene’ has investigated the role of fire, agriculture, trade, urbanisation and other anthropogenic impacts. While there is increasing consensus that such impacts were more important than previously realised, geographical variation during the Palaeoanthropocene remains poorly understood. Here, we present a preliminary comparative analysis of claims that pre-industrial anthropogenic impacts in Japan were significantly reduced by four factors: the late arrival of agriculture, an emphasis on wet-rice farming limited to alluvial plains, a reliance on seafood rather than domesticated animals as a primary source of dietary protein, and cultural ideologies of environmental stewardship. We find that none of these claims of Japanese exceptionalism can be supported by the archaeological and historical records. We make some suggestions for further research but conclude that the Japanese sequence appears consistent with global trends towards increased anthropogenic impacts over the course of the Palaeoanthropocene.Introduction Claims for Japanese environmental exceptionalism - Claim 1: late arrival of agriculture. - Claim 2: the sustainability of rice. - Claim 3: domesticated animals versus marine foods. - Claim 4: cultural ideologies of stewardship.     Functional Brain Imagery and Jungian Analytical Psychology: An Interesting Dance?

    Jung’s original neuroscience research project looked at the neurophysiological responses to the word association test (WAT) in an effort to understand ‘complexes’, those emotionally laden fixations that bother us all, and can be inferred from certain painful responses in the WAT. He measured breathing rates, skin conductance and electrocardiography, but there was no brain functional imaging technology available at the time. One hundred years later, a wide range of brain functional technologies are available, and this chapter describes two studies in which the WAT was performed under functional magnetic resonance imaging and quantitative electroencephalography conditions. In essence, a complexed response first activates the amygdala (many right-sided). This is followed in the next 3 s by bilateral brain activity in the anterior insula, the supplementary motor area and the dorsal cingulum; the premotor mirror neuron areas, the so-called resonance circuitry, which is central to mindfulness (awareness of self) and empathy (sense of the other), negotiations between self-awareness and the ‘internal other’, and has been well described by Dan Siegel. But over the following 2 s, activity shifts to the left hemisphere, seemingly the way the brain deals with a complex in the moment, possibly to dull the pain of the complexed response

    Creative and Stylistic Devices Employed by Children During a Storybook Narrative Task: A Cross-Cultural Study

    Purpose: The purpose of this study was to analyze the effects of culture on the creative and stylistic features children employ when producing narratives based on wordless picture books. Method: Participants included 60 first- and second-grade African American, Latino American, and Caucasian children. A subset of narratives based on wordless picture books collected as part of a larger study was coded and analyzed for the following creative and stylistic conventions: organizational style (topic centered, linear, cyclical), dialogue (direct, indirect), reference to character relationships (nature, naming, conduct), embellishment (fantasy, suspense, conflict), and paralinguistic devices (expressive sounds, exclamatory utterances). Results: Many similarities and differences between ethnic groups were found. No significant differences were found between ethnic groups in organizational style or use of paralinguistic devices. African American children included more fantasy in their stories, Latino children named their characters more often, and Caucasian children made more references to the nature of character relationships. Conclusion: Even within the context of a highly structured narrative task based on wordless picture books, culture influences children’s production of narratives. Enhanced understanding of narrative structure, creativity, and style is necessary to provide ecologically valid narrative assessment and intervention for children from diverse cultural backgrounds

    A new echocardiographic model for quantifying three-dimensional endocardial surface area

    A new technique for quantitatively mapping the three-dimensional left ventricular endocardial surface was developed, using measurements from standard cross-sectional echocardiographic images. To validate the accuracy of this echocardiographic mapping technique in an animal model, the endocardial areas of 15 excised canine ventricles were calculated using measurements made from echocardiographic studies of the hearts and compared with areas determined with latex casts of the same ventricles. Close correlation (r = 0.87, p < 0.001) between these two measures of endocardial area provided preliminary confirmation of the accuracy of the maps.To further characterize the mapping algorithm, it was translated into computer format and used to map the surfaces of idealized hemiellipsoids. Areas measured with this mapping technique closely approximated the actual areas of idealized surfaces with a wide spectrum of shapes; maps were particularly accurate for ellipsoids with shapes similar to those of undistorted human ventricles. Also, the accuracies of area calculations were relatively insensitive to deviation from the assumed positions of the echocardiographic short-axis planes. Finally, although the accuracy of the mapping technique improved as data from more transverse planes were added, the procedure proved reliable for estimating surface areas when data from only three planes were used. These studies confirm the accuracy of the echocardiographic mapping technique, and they suggest that the resulting planar plots might be useful as templates for localizing and quantifying the overall extent of abnormal wall motion
