    Age at menarche associated with subsequent educational attainment and risk-taking behaviours: the Pelotas 1982 Birth Cohort.

    Background: Earlier age at menarche (AAM), a marker of puberty timing in females, has been associated with a higher likelihood of adolescent risk-taking behaviours and variably associated with educational attainment.Aim: To examine the association between AAM and educational attainment in the Pelotas, Brazil, 1982 Birth Cohort.Subjects and methods: AAM was categorised as Early (7-11 years), Average (12-13 years), or Late (14+ years). Primary outcome: years of education (age 30). Secondary outcomes: risk-taking behaviours, adult income and school grade failure.Results: In adjusted models, compared to Average AAM, Late AAM was associated with 0.64 fewer years of education (95% CI: -1.15, -0.13). Early AAM was associated with earlier age at first sexual intercourse (-0.25 years; 95% CI: -0.39, -0.12), whereas Late AAM was associated with 17% lower adult income (0.83; 95% CI: 0.71, 0.95) and 0.31 years older age at first alcohol consumption (95% CI: 0.10, 0.52).Conclusions: Our findings confirm the association between earlier puberty timing in females and a greater likelihood of risk-taking behaviours in this setting of recent secular changes towards earlier puberty. However, the association between Late AAM and lower education was surprising and may support a psychosocial rather than biological link between puberty timing and educational outcomes

    Validation of the Netherlands physical activity questionnaire in Brazilian children

    <p>Abstract</p> <p>Background</p> <p>Physical activity instruments can be subjective or objective. There is a need to assess the reliability of these instruments, especially for researches in children. The aim of this study was to determine the validity of the Netherlands Physical Activity Questionnaire (NPAQ).</p> <p>Methods</p> <p>Population under study were Brazilian children aged 4 to 11 years old, enrolled in a population-based study. Data collection took place in two distinct moments: 1) application of the NPAQ by face-to-face interviews with mothers' children and 2) utilization of accelerometers by children as the reference method. GT1M Actigraph accelerometer was worn for five consecutive days. Validity analyses were performed by sensitivity and specificity and ROC (Receiver Operator Characteristic) curve.</p> <p>Results</p> <p>Two hundred and thirty nine children participated in both phases of the study. A total of 73.2% children achieved the recommendation of 60 min/day of moderate to vigorous physical activity. The mean and median of the NPAQ score were 25.5 and 26, respectively. The score ranged from 7 to 35 points. The correlation coefficient between the NPAQ and the time spent in moderate to vigorous physical activities was 0.27. Based on the area under the ROC curve, the median value presented the best indicators of sensitivity (59.4%) and specificity (60.9%), and the area under curve was 0.63. The predictive capacity of the NPAQ to identify active children was high regardless the cut-off point chosen. This capacity was even higher if the score was higher than 30.</p> <p>Conclusions</p> <p>Based on sensitivity and specificity values, the NPAQ did not show satisfactory validity. The comparison of the reliability of the NPAQ with other instruments is limited, but correlation coefficients found in this study are similar to others. Physical activity level of children estimated from the NPAQ must be interpreted cautiously, and objective measures such as accelerometers should be encouraged.</p

    Stages of change for physical activity in adults from Southern Brazil: a population-based survey

    <p>Abstract</p> <p>Background</p> <p>There is evidence that physical activity (PA) interventions tailored to individual's stages of change (SoC) are more effective in promote behavior change than "one-size-fits-all" interventions. However, only a few researches have investigated these stages towards PA behavior in representative samples of the population. Thus, the purpose of this study was to determine the prevalence and factors associated with the SoC for PA in a probabilistic sample of adults aged 20 years or over.</p> <p>Methods</p> <p>A population-based survey was undertaken in Pelotas, Southern Brazil, in 2005. An algorithm was applied to evaluate the SoC for PA, and PA was defined as being engaged in moderate-to-vigorous PA for at least 20 minutes on three times per week. The covariates collected in the questionnaire were: sex, age, skin color, marital status, education level, economic status, family income, smoking, body mass index (BMI) and self-reported health status. Data analyses were performed through Poisson and multinomial regression, taking the sampling design into account.</p> <p>Results</p> <p>Face-to-face interviews were applied to 3136 individuals, corresponding to a response rate of 93.5%. The prevalence across the stages was as follows: 38.3% in precontemplation, 13.0% in contemplation, 19.5% in preparation, 5.2% in action and 24.0% in maintenance. The elderly, married, smokers, and those with lower socioeconomic status were less likely to adopt, initiate and maintain regular PA.</p> <p>Conclusion</p> <p>Despite the all benefits of PA, a high proportion of adults from Southern Brazil are physically inactive and do not present intention to engage in regular PA. The profile of those who are inactive but intend to do PA resembles those who are physically active. The findings of the present study can contribute to improve health behaviors and to plan health promotion strategies aimed at increasing the level of PA in the community.</p

    Consumo de alimentos ultraprocessados e impacto na dieta de adultos jovens

    OBJETIVO Avaliar o consumo de alimentos ultraprocessados, os fatores associados e a sua influência na ingestão de nutrientes em adultos jovens. MÉTODOS Em 2004-2005, os participantes da Coorte de Nascimentos de Pelotas de 1982 foram identificados para entrevista domiciliar. Foram entrevistados 4.297 indivíduos (taxa de acompanhamento de 77,4%) e incluídos no estudo 4.202. O consumo alimentar foi avaliado por meio de questionário de frequência alimentar e estimada a proporção da ingestão calórica diária atribuída aos alimentos ultraprocessados, bem como a ingestão de macro e micronutrientes. A associação entre características dos indivíduos e consumo de alimentos ultraprocessados foi avaliada utilizando-se regressão linear. A análise de variância e o teste Qui-quadrado de Pearson foram utilizados na associação entre quintis de consumo de alimentos ultraprocessados, ingestão e na adequação da ingestão de nutrientes, respectivamente. RESULTADOS O consumo de alimentos ultraprocessados contribuiu com 51,2% das calorias totais ingeridas. A ingestão de alimentos ultraprocessados foi maior entre indivíduos: do sexo feminino; de maior escolaridade; que nunca foram pobres e eutróficos. Maior consumo de alimentos ultraprocessados foi positivamente associado ao consumo de gorduras, colesterol, sódio, ferro, cálcio e calorias (p &lt; 0,001) e negativamente associado ao consumo de carboidratos, proteínas e fibras alimentares (p &lt; 0,001). CONCLUSÕES O elevado consumo de alimentos ultraprocessados e sua relação positiva com a ingestão de sódio, colesterol e gorduras chama a atenção para a realização de intervenções visando a redução da ingestão desse grupo de alimentos.OBJECTIVE To evaluate the consumption of ultra-processed foods, its associated factors, and its influence on nutrient intake in young adults. METHODS In 2004-2005, the individuals belonging to the Pelotas birth cohort of 1982 were identified for a home interview. A total of 4,297 individuals were interviewed and 4,202 individuals were included in the study (follow-up rate of 77.4%). Diet was assessed using a questionnaire on dietary intake and the percentage of daily caloric intake attributed to ultra-processed foods as well as the intake of macro- and micronutrients were estimated. The association between cohort characteristics and the consumption of ultra-processed foods was assessed using linear regression. Analysis of variance and Pearson’s Chi-square test were used to evaluate the association between the quintiles of the consumption of ultra-processed food, nutrient intake and adequacy of nutrient intake, respectively. RESULTS The consumption of ultra-processed foods corresponded to 51.2% of the total caloric intake. The consumption of ultra-processed foods was higher among women, individuals with higher education, and individuals who were never poor and eutrophic. The increased consumption of ultra-processed foods was positively correlated with the consumption of fat, cholesterol, sodium, iron, calcium, and calories (p < 0.001) and was negatively correlated with the consumption of carbohydrates, protein, and dietary fiber (p < 0.001). CONCLUSIONS The high consumption of ultra-processed foods and its positive correlation with the intake of sodium, cholesterol, and fats underscores the need to perform interventions aimed at decreasing the intake of this food group

    Prevalência de atividade física em adolescentes e fatores associados

    OBJECTIVE: To estimate the prevalence and identify correlates of physical activity among adolescents. METHODS: Cross-sectional study nested within a cohort of 4,325 subjects from the city of Pelotas, Southern Brazil, aged 14-15 years in 2008. Physical activity was analyzed using three different approaches: (1) prevalence of any leisure-time physical activity; (2) prevalence of any active commuting to school; and (3) prevalence of engaging in at least 300 minutes per week of both (1) and (2) combined. Independent variables included sociodemographic, behavioral, social, and biological characteristics, and number of different leisure-time physical activites practiced. Statistical analyses were carried out using Poisson regression. RESULTS: The proportion of adolescents involved in any type of leisure-time physical activity was 75.6%, while 73.4% displayed some form of active commuting to school. Prevalence of total physical activity score (>; 300 min/week) was 48.2%, being greater among boys (62.6%) than among girls (34.5%). Furthermore, prevalence increased along with the number of physical activity modalities practiced (pOBJETIVO: Estimar la prevalencia de actividad física entre adolescentes y identificar los factores asociados. MÉTODOS: Estudio transversal anidado en una cohorte con 4.325 individuos de 14-15 años en Pelota, Sur de Brasil, en 2008. La actividad física fue analizada por medio de tres diferentes abordajes: 1) prevalencia de alguna actividad física de ocio; 2) prevalencia de algún traslado activo para la escuela; 3) prevalencia de compromiso de por lo menos 300 minutos por semana en la combinación de ambos (1 y 2). Variables independientes incluyeron características demográficas, socioeconómicas, comportamentales, sociales y biológicas y número de actividades físicas practicadas en el tiempo de ocio. Los análisis estadísticos fueron hechos por la regresión de Poisson. RESULTADOS: La proporción de adolescentes envueltos en alguna actividad física de ocio fue 75,6% mientras que 73,4% presentaron alguna forma de traslado activo para la escuela. La prevalencia de actividad física total (escore ³ 300 min/sem) fue 48,2% siendo mayor para los niños (62,6%) que para las niñas (34,5%). Además, la prevalencia aumentó de acuerdo con el número de actividades físicas practicadas (pOBJETIVO: Estimar a prevalência de atividade física entre adolescentes e identificar fatores associados. MÉTODOS: Estudo transversal aninhado em uma coorte com 4.325 indivíduos de 14-15 anos em Pelotas, RS, em 2008. As análises estatísticas foram feitas pela regressão de Poisson. A prevalência de atividade física total (escore >; 300 min/sem) foi 48,2%, sendo maior para os meninos (62,6%) do que para as meninas (34,5%). Além disso, a prevalência aumentou de acordo com o número de atividades físicas praticadas (p < 0,001). Os fatores associados à maior prática de atividade física (lazer + deslocamento) nos níveis recomendados foram: cor de pele não-branca, ter repetido de ano na escola e jogar videogame. As variáveis menor nível socioeconômico, maior tempo de uso de computador e atividade física dos pais estiveram associadas ao desfecho apenas entre as meninas. CONCLUSÕES: Menos da metade dos adolescentes atingiu as recomendações para a prática de atividade física, e essa proporção tende a diminuir entre os sujeitos de maior nível socioeconômico. Os fatores associados diferiram entre lazer e deslocamento. Engajamento em uma ampla variedade de atividades físicas deve ser encorajado desde a infância

    Impact of stressful life events on central adiposity in the Pelotas Birth Cohort

    OBJECTIVE: To investigate how stressful life events and social support relate to central adiposity in Southern Brazil. METHODS: Data included information from 802 participants in the 1982 Pelotas Birth Cohort that was collect in 2004–2005 and 2006. Stratifying by sex, we studied self-reported stressful life events during the year before 2004–2005 in relation to change in waist circumference between 2004–2005 and 2006 and waist-to-hip ratio in 2006, using both bivariate and multivariate linear regression models. RESULTS: In adjusted models, the experience of stressful life events during the year before 2004–2005 predicted a change in waist circumference in 2006 in men and a change in both waist-to-hip ratio in 2006 and waist circumference between 2004–2005 and 2006 in women. Men who experienced two or more stressful events had on average a one centimeter increase in their waist circumference between 2004–2005 and 2006 (β = 0.97, 95%CI 0.02–1.92), compared to those reporting no stressful events. For women, those who had one and those who had two or more stressful life events had over a 1 cm increase in their waist circumference from 2004–2005 to 2006 (β = 1.37, 95%CI 0.17–2.54; β = 1.26, 95%CI 0.11–2.40, respectively), compared to those who did not experience any stressful event. For both sexes, social support level was not significantly related to either waist-to-hip ratio or change in waist circumference, and it did not modify the association between stress and central adiposit

    Given the growing recognition of the importance of the life course approach for the determination of chronic diseases, birth cohort studies are becoming increasingly important. This paper describes the methods used in the 1982 Pelotas (Brazil) birth cohort study, one of the largest and longest studies of this type in developing countries. All 5,914 hospital births occurring in Pelotas in 1982 (over 99% of all deliveries) were studied prospectively. The main stages of the study took place in 1983, 1984, 1986, 1995, 1997, 2000, and 2001. More than two thousand variables are available for each subject who participated in all stages of the study. Recent phases of the study included the examination of 2,250 males when presenting for the army recruitment exam in 2000, the study of a 27% sample of men and women in 2001 through household visits, and the study of over 400 children born to the cohort women. Follow-up rates in the recent stages of the cohort were 78.9% for the army examination and 69.0% for the household visits. Ethnographic and oral health studies were conducted in sub-samples. Some recent results on blood pressure, adolescent pregnancy, and asthma are presented as examples of utilization of the data. Suggestions on lessons learned for other cohort studies are proposed

    Determinantes precoces da pressão arterial em adultos da coorte de nascimentos de 1982, Pelotas, RS

    OBJECTIVE: To evaluate the effects of exposure occurring during pregnancy or the first years of life on blood pressure. METHODS: Cohort study on all children born in 1982 in maternity hospitals in the city of Pelotas, Southern Brazil. The mothers living in the urban area were interviewed and the children were followed up on different occasions. In 2004-5, all the individuals in the cohort were sought for monitoring. Their blood pressure was measured twice, at the start and end of the interview, using a digital wrist sphygmomanometer. Associations between blood pressure and the following variables were evaluated: skin color; maternal schooling level; family income at birth; change in income between birth and 23 years of age; birth weight; and duration of breastfeeding. Analysis of variance was used to compare the means and a generalized linear model was used in the adjusted analysis. RESULTS: Blood pressure measurements were obtained from 4,291 individuals: 2,208 males and 2,083 females. The mean systolic blood pressure was 117.5 ± 15.0 mmHg and the mean diastolic was 73.6 ± 11.5 mmHg. Among the men, systolic blood pressure was higher among those of black or brown skin color and among those who were never considered poor. Diastolic pressure was only associated with birth weight. Among the women, systolic blood pressure was greater among those of black or brown skin color whose mothers' schooling level was greater than or equal to 12 years or whose birth weight was less than 4,000 g. CONCLUSIONS: For both sexes, only skin color was associated with blood pressure. Breastfeeding did not have any long-term effect on blood pressure and the associations for birth weight and socioeconomic level were inconsistent.OBJETIVO: Evaluar los efectos de exposiciones ocurridas en la gestación o en los primeros años de vida sobre la presión arterial. MÉTODOS: Estudio de cohorte con todos los niños nacidos en 1982 en las maternidades de la ciudad de Pelotas (Sur de Brasil). Las madres residentes en área urbana fueron entrevistadas y los niños, acompañados en diferentes ocasiones. En 2004-5, se buscó acompañar todos los individuos de la cohorte. La presión arterial fue medida dos veces, en el inicio y al final de la entrevista, con esfigmomanómetro digital de pulso. Fue evaluada la asociación entre presión arterial y las siguientes variables: color de la piel, escolaridad materna, renta familiar al nacer, cambio de renta entre el nacimiento y los 23 años de edad, peso al nacer y duración del amamantamiento. El análisis de varianza fue utilizado para la comparación de promedio y los modelos lineares generalizados en el análisis ajustado. RESULTADOS: Se obtuvo la medida de presión arterial para 4.291 individuos, 2.208 del sexo masculino y 2.083 del sexo femenino. El promedio de las medidas de presión arterial sistólica fue de 117,5±15,0 mmHg y de la diastólica, de 73,6±11,5 mmHg. Entre los hombres, la presión arterial sistólica fue mayor en los individuos de color de piel negra o parda y en aquellos que nunca fueron considerados pobres. La presión diastólica estuvo asociada apenas con el peso al nacer. Entre las mujeres, la presión arterial sistólica fue mayor en las de color de piel negra o parda, cuya escolaridad materna era mayor o igual a 12 años o con peso al nacer menor de 4.000 g. CONCLUSIONES: Para ambos sexos apenas el color de la piel estuvo asociado con la presión arterial. El amamantamiento no tuvo efecto a largo plazo sobre la presión arterial, y para el peso al nacer y el nivel socioeconómico, las asociaciones no fueron consistentes.OBJETIVO: Avaliar os efeitos de exposições ocorridas na gestação ou nos primeiros anos de vida sobre a pressão arterial. MÉTODOS: Estudo de coorte com todas as crianças nascidas em 1982 nas maternidades da cidade de Pelotas, RS. As mães residentes em área urbana foram entrevistadas e as crianças, acompanhadas em diferentes ocasiões. Em 2004-5, buscou-se acompanhar todos os indivíduos da coorte. A pressão arterial foi medida duas vezes, no início e no final da entrevista, com esfigmomanômetro digital de pulso. Foi avaliada a associação entre pressão arterial e as seguintes variáveis: cor da pele; escolaridade materna; renda familiar ao nascer; mudança de renda entre o nascimento e os 23 anos de idade; peso ao nascer e duração da amamentação. A análise de variância foi utilizada para a comparação de médias e os modelos lineares generalizados na análise ajustada. RESULTADOS: Obteve-se a medida da pressão arterial para 4.291 indivíduos, 2.208 do sexo masculino e 2.083 do sexo feminino. A média das medidas da pressão arterial sistólica foi de 117,5±15,0 mmHg e da diastólica, de 73,6±11,5 mmHg. Entre os homens, a pressão arterial sistólica foi maior nos indivíduos de cor da pele preta ou parda e naqueles que nunca foram considerados pobres. A pressão diastólica esteve associada apenas com o peso ao nascer. Entre as mulheres, a pressão arterial sistólica foi maior nas de cor da pele preta ou parda, cuja escolaridade materna era maior ou igual a 12 anos ou com peso ao nascer menor do que 4.000 g. CONCLUSÕES:Para ambos os sexos apenas a cor da pele esteve associada com a pressão arterial. A amamentação não teve efeito em longo prazo sobre a pressão arterial, e para o peso ao nascer e o nível socioeconômico, as associações não foram consistentes

    OBJECTIVE: To analyze the influence of biological and socioeconomic factors throughout life on entry into the university and insertion in the work force of young adults from the 1982 birth cohort. METHODS: Longitudinal study of 5,914 births that took place in the city of Pelotas, Southern Brazil, in 1982. Data was collected by means of questionnaires applied on young adults when accompanying the 1982 cohort in 2004-5. Information was gathered concerning educational level and insertion in the labor market. Poisson Regression was utilized to study the effect of demographic and socioeconomic variables, as well as birth weight and maternal breastfeeding, on the outcomes. RESULTS: On the average, these young adults had 9.4 (± 3.1) years of schooling and 42% of them were attending school in 2004-5. One in five young adults had entered a university and approximately two thirds were working during the month prior to the interview. Entry in the university was determined by economic conditions. Furthermore, women's birth weight and breastfeeding among men influenced this outcome. Insertion in the labor market was more frequent among the poorer men, but this did not affect women's outcomes in this respect. CONCLUSIONS: The low inclusion in the university and the need to enter the labor market among the poor families maintains a vicious circle that reproduces the dominant social hierarchy.OBJETIVO: Analizar la influencia de factores socioeconómicos y biológicos precoces a lo largo de la vida sobre el ingreso en la universidad y la inserción en el mercado de trabajo de los jóvenes de la cohorte de nacimiento de 1982. MÉTODOS: Estudio longitudinal de 5.914 nacimientos de la ciudad de Pelotas (Sur de Brasil), en 1982. Utilizándose cuestionarios aplicados al joven, fueron colectadas informaciones sobre el nivel educacional y la inserción en el mercado de trabajo durante acompañamiento de la cohorte realizado en 2004-5. Regresión de Poisson fue utilizada para estudiar el efecto de las variables demográficas, socioeconómicas, peso al nacer y amamantamiento materno sobre los desenlaces. RESULTADOS: La escolaridad promedio fue de 9,4 años (± 3,1) y 42% de los jóvenes estaban frecuentando la escuela en 2004-5. Uno de cada cinco jóvenes había ingresado en la universidad y cerca de dos tercios estaban trabajando en el mes anterior a la entrevista. El ingreso en la universidad fue determinado por las condiciones económicas, y tuvo influencia de peso al nacer en las mujeres y del amamantamiento en los hombres. La inserción en el mercado de trabajo fue más frecuente entre los hombres más pobres, pero no para las mujeres. CONCLUSIONES: La baja inclusión universitaria y la necesidad de inserción en el mercado de trabajo de los jóvenes de familias más pobres mantienen un círculo vicioso que reproduce la jerarquía social dominante.OBJETIVO: Analisar a influência de fatores socioeconômicos e biológicos precoces ao longo da vida sobre o ingresso na universidade e a inserção no mercado de trabalho dos jovens da coorte de nascimento de 1982.
