    Environmental Conditions along Tuna Larval Dispersion: Insights on the Spawning Habitat and Impact on Their Development Stages

    Estimated larval backward trajectories of three Tuna species, namely, Atlantic Bluefin Tuna (Thunnus thynnus, Linnaeus, 1758), Bullet Tuna (Auxis Rochei, Risso, 1801) and Albacore Tuna (Thunnus alalunga, Bonnaterre, 1788) in the central Mediterranean Sea, were used to characterize their spawning habitats, and to assess the impact of changes due to the major environmental parameters (i.e., sea surface temperature and chlorophyll-a concentration) on larval development during their advection by surface currents. We assumed that the environmental variability experienced by larvae along their paths may have influenced their development, also affecting their survival. Our results showed that the Tuna larvae underwent an accelerated growth in favorable environmental conditions, impacting on the notochord development. In addition, further updated information on spawning and larval retention habitats of Atlantic Bluefin Tuna, Bullet and Albacore Tunas in the central Mediterranean Sea were delivered

    The Role of Obesity in Early and Long-Term Outcomes after Surgical Excision of Lung Oligometastases from Colorectal Cancer

    Obesity correlates with better outcomes in many neoplastic conditions. The aim of this study was to assess its role in the prognosis and morbidity of patients submitted to resection of lung oligometastases from colorectal cancer. Seventy-six patients undergoing a first pulmonary metastasectomy were retrospectively included in the study. Seventeen (22.3%) were obese (body mass index (BMI) >30 kg/m(2)). Assessed outcomes were overall survival, time to recurrence, and incidence of post-operative complications. Median follow-up was 33 months (IQR 16-53). At follow-up, 37 patients (48.6%) died, whereas 39 (51.4%) were alive. A significant difference was found in the 3-year overall survival (obese 80% vs. non-obese 56.8%, p = 0.035). Competing risk analysis shows that the cumulative incidence of recurrence was not different between the two groups. Multivariate analysis reveals that the number of metastases (p = 0.028), post-operative pneumonia (p = 0.042), and DFS (p = 0.007) were significant predictors of death. Competing risk regression shows that no independent risk factor for recurrence has been identified. The complication rate was not different between the two groups (17.6% vs. 13.6%, p = 0.70). Obesity is a positive prognostic factor for survival after pulmonary metastasectomy for colorectal cancer. Overweight patients do not experience more post-operative complications. Our results need to be confirmed by large multicenter studies

    Cytoreductive Surgery and Hyperthermic Intrathoracic Chemotherapy by Video-Assisted Surgery for Pleural Malignancies. Technical Aspects and Safety Profile from A Single Center

    Background Pleural malignancies are challenging conditions in terms of possibility of cure. Recent growing interest towards Hyperthermic Intrathoracic Chemotherapy (HITHOC) after Cytoreductive Surgery (CRS) has been referred. Minimally invasive approach (VATS) may be suggest in this context but evidence is still lacking. Methods A preliminary experience in seven patients submitted to cytoreductive surgery and HITHOC is described, with a focus on technical aspects related to VATS approach, operating median time and postoperative complication. Results A triportal VATS approach has been employed in all cases. Median time of surgery including pleural perfusion was 200 minutes (range 165-370). Mean blood losswas 217 cc (range 100 and 600). Thirty days’ mortality was nihil. Conclusions VATS cytoreductive surgery and HITHOC is a safeprocedure and could be proposed in the setting of a multimodality strategy employing adjuvant radio-chemotherapy in referral center

    Coupling Lagrangian simulation models and remote sensing to explore the environmental effect on larval growth rate: The Mediterranean case study of round sardinella (Sardinella aurita) early life stages

    The relationship between environmental conditions and early life-history traits of Sardinella aurita are investigated using material collected in two sites of the Central Mediterranean Sea. Individual mean daily growth during the planktonic phase has been determined by using otolith microstructure analysis, while Lagrangian simulation models allowed to estimate the daily position in space and time of each specimen from the hatching to the catch. Generalized Additive Mixed Models (GAMMs) have been implemented to explore the impact of environmental conditions at time t, t-1 day and t-2 days on the mean daily growth rate occurring at time t. Spatial analysis evidenced a wide dispersion of eggs and larvae in the coastal area of both sampling sites in correspondence to relatively warmer and chlorophyll-a enriched waters. Lagrangian simulations detected a complementary larval dispersal pathway able to transport larvae to a known retention area. Temperature at time t was the most important driver affecting the mean daily larval growth, followed by the food availability. On the other hand, models performed on lagged environmental covariates (t-1 and t-2) did not show any significant effect on the growth rate at time t. In addition to the sub-linear positive correlation between temperature and mean daily larval growth, model highlighted a decrease in the otolith core width at higher temperature that can be linked to an earlier stage of ontogeny at hatching. This study provided a useful methodological approach that takes advantage of available remote sensing data to perform ecological studies in support to fisheries management

    The Impact of Preoperative Inflammatory Markers on the Prognosis of Patients Undergoing Surgical Resection of Pulmonary Oligometastases

    The aim of this study was to assess the prognostic value of preoperative neutrophil-to-lymphocyte ratio (NLR) and C-reactive protein (CRP) levels in patients undergoing resection of pulmonary oligometastases. A retrospective analysis on 141 patients undergoing a first pulmonary metastasectomy in a single center was carried out. Two distinct analysis were performed subdividing patients according to their NLR ratio and CRP level. The main outcomes were survival and time to recurrence. At completion of follow-up 74 patients were still alive (52.5%). Subdividing patients according to their NLR yielded a significant difference in five-year progression-free survival (PFS, NLR = 4:18%, p = 0.01). When subdivided by their CRP levels, patients with preoperative CRP <5 mg/L demonstrated higher values of five-year overall survival (OS, 57% vs. 34%, p = 0.006) and five-year PFS (35% vs. 22%, p = 0.04). At multivariate analysis, level of neutrophils (p = 0.009) and lung comorbidities (p = 0.021) were independent predictors of death, whereas preoperative CRP (p = 0.002), multiple metastases (p = 0.003) and presence of lung comorbidities (p = 0.001) were independent predictors of recurrence. NLR and CRP are important predictors of prognostic outcome in patients undergoing pulmonary metastasectomy
