    Long-term impact of Global Health educational experiences in Rome. An attempt of measurement

    Background Global health education (GHE) is spreading in Europe and in other parts of the world. Since 2008, Sapienza University of Rome has offered activities to medical and other health profession students related to global health (GH), which is grounded in the theory of social determinants of health and inspired by social justice. The educational activities included elective courses as well as community and service-learning experiences, referred to as GH gyms. This study attempts to measure the long-term impact of these educational experiences, especially to demonstrate their influence on the perceived social responsibility of future health professionals. Methods A questionnaire was elaborated and tested on a small sample of participants. It was sent to participants by e-mail. Quantitative results were analysed through descriptive statistics and qualitative answers were carefully read and classified. Results A total of 758 students from different faculties took part to the educational experiences. Only 488 e-mail addresses were available. One hundred and five (21.5%) questionnaires were returned. Participation in GH gyms was perceived to have had a higher influence on future professional and personal choices, when compared to participation in elective GH courses. Conclusions The study shows that consideration of health and social issues related with inequities in health and the use of interactive teaching methodologies had important effects on social responsibility of a large number of students. As there could be a selection bias among respondents, more research is needed to understand the impact of GH educational experiences. The inclusion of global health education in health and social curricula and the use of interactive methodologies with a correct evaluation of results are the indications that emerge from this research, together with the necessity of a strong involvement of students, professors and the whole academic reality

    Medical education: an Italian contribution to the discussion on global health education

    Abstract Background In Italy an important contribution to the spread of global health education (GHE) grew from the establishment and work of the Italian Network for Global Health Education (INGHE). INGHE gave a national shared definition of global health (GH), grounded in the theory of determinants of health, inspired by a vision of social justice, and committed to reduce health inequities. The aim of this article is to share with the international community INGHE’s point of view on Medical Education. Methods To express its view of medical education at the national level, INGHE established a dedicated commission, which elaborated a first draft of the document and then shared and discussed it with all other members. Results INGHE elaborated a paper where it explained the need to change medical education in order to prepare future health professionals for the challenges of the globalized and unequal world. In this article the authors summarize the experience of INGHE and share with the international community its document. Conclusions The authors believe it is necessary now, more than ever, to insert this new approach to health at social and academic levels. Students should play a fundamental role in the spread of GHE, and activities related with GHE could be considered an important part of the third mission of universities to promote social justice

    A one-stage approach to the treatment of intravenous leiomyomatosis extending to the right heart

    This report describes the case of a 60-year-old woman with a history of hysterectomy for myomas, totally asymptomatic, with incidental evidence of a pelvic intracaval mass extending to the right atrium. She underwent a staged procedure (sternothomic and abdominal) through a thoracolaparotomic approach in circulatory arrest and deep hypothermia. Using a one-stage surgical approach, we were able to withdraw one portion of the mass from the right atrium and another from the abdominal inferior vena cava, thus minimizing the risk of unexpected venous or atrial wall injury during surgical manipulation

    Stem cell senescence: effects of REAC technology on telomerase-independent and telomerase-dependent pathways

    Decline in the gene expression of senescence repressor Bmi1, and telomerase, together with telomere shortening, underlay senescence of stem cells cultured for multiple passages. Here, we investigated whether the impairment of senescence preventing mechanisms can be efficiently counteracted by exposure of human adipose-derived stem cells to radio electric asymmetrically conveyed fields by an innovative technology, named Radio Electric Asymmetric Conveyer (REAC). Due to REAC exposure, the number of stem cells positively stained for senescence associated ß-galactosidase was significantly reduced along multiple culturing passages. After a 90-day culture, REAC-treated cells exhibited significantly higher transcription of Bmi1 and enhanced expression of other stem cell pluripotency genes and related proteins, compared to unexposed cells. Transcription of the catalytic telomerase subunit (TERT) was also increased in REAC-treated cells at all passages. Moreover, while telomere shortening occurred at early passages in both REAC-treated and untreated cells, a significant rescue of telomere length could be observed at late passages only in REAC-exposed cells. Thus, REAC-asymmetrically conveyed radio electric fields acted on a gene and protein expression program of both telomerase-independent and telomerase-dependent patterning to optimize stem cell ability to cope with senescence progression

    Definition of a methodology for gradual and sustainable safety improvements on farms and its preliminary applications

    In many productive sectors, ensuring a safe working environment is still an underestimated problem, and especially so in farming. A lack of attention to safety and poor risk awareness by operators represents a crucial problem, which results in numerous serious injuries and fatal accidents. The Demetra project, involving the collaboration of the Regional Directorate of INAIL (National Institute for Insurance against Accidents at Work), aims to devise operational solutions to evaluate the risk of accidents in agricultural work and analyze the dynamics of occupational accidents by using an observational method to help farmers ensure optimal safety levels.     Quality of images with toric intraocular lenses

    Purpose: To objectively evaluate the image quality obtained with toric intraocular lenses (IOLs) when misaligned from the intended axis. Setting: University Eye Clinic and the Department of Industrial and Information Engineering, University of Trieste, Trieste, Italy. Design: Experimental study. Methods: An experimental optoelectronic test bench was created. It consisted of a high-resolution monitor to project target images and an artificial eye. The system simulates the optical and geometric characteristics of the human eye with an implanted toric IOL. A 3.00 diopters corneal astigmatism was simulated. Images reproduced by the optical system were captured according to different IOL axis positions. The quality of each image was analyzed using the visual information fidelity (VIF) criterion. The VIF reduction was calculated at each IOL rotational step. Results: A 5-degree IOL axis rotation from the intended position determined a decay in the image quality of 7.03%. Ten degrees of IOL rotation caused an 11.09% decay of relative VIF value. For a 30-degree rotation, the VIF decay value was 45.85%. Finally, the image decay with no toric correction was 56.70%. Conclusions: The more the objective quality of the image decays progressively, the further the axis of the IOL is rotated from its intended position. The reduction in image quality obtained after 30 degrees of toric IOL rotation was less than 50% and after 45 degrees, the image quality was the same as that of no toric correction

    Ubiquitination and sumoylation of the HTLV-2 Tax-2B protein regulate its NF-κB activity: a comparative study with the HTLV-1 Tax-1 protein.

    BACKGROUND:Retroviruses HTLV-1 and HTLV-2 have homologous genomic structures but differ significantly in pathogenicity. HTLV-1 is associated with Adult T cell Leukemia (ATL), whereas infection by HTLV-2 has no association with neoplasia. Transformation of T lymphocytes by HTLV-1 is linked to the capacity of its oncoprotein Tax-1 to alter cell survival and cell cycle control mechanisms. Among these functions, Tax-1-mediated activation of cellular gene expression via the NF-κB pathway depends on Tax-1 post-translational modifications by ubiquitination and sumoylation. The Tax-2 protein of HTLV-2B (Tax-2B) is also modified by ubiquitination and sumoylation and activates the NF-κB pathway to a level similar to that of Tax-1. The present study aims to understand whether ubiquitination and sumoylation modifications are involved in Tax-2B-mediated activation of the NF-κB pathway.RESULTS:The comparison of Tax-1 and Tax-2B lysine to arginine substitution mutants revealed conserved patterns and levels of ubiquitination with notable difference in the lysine usage for sumoylation. Neither Tax-1 nor Tax-2B ubiquitination and sumoylation deficient mutants could activate the NF-κB pathway and fusion of ubiquitin or SUMO-1 to the C-terminus of the ubiquitination and sumoylation deficient Tax-2B mutant strikingly restored transcriptional activity. In addition, ubiquitinated forms of Tax-2B colocalized with RelA and IKKγ in prominent cytoplasmic structures associated with the Golgi apparatus, whereas colocalization of Tax-2B with the RelA subunit of NF-κB and the transcriptional coactivator p300 in punctate nuclear structures was dependent on Tax-2B sumoylation, as previously observed for Tax-1.CONCLUSIONS:Both Tax-1 and Tax-2 activate the NF-κB pathway via similar mechanisms involving ubiquitination and sumoylation. Therefore, the different transforming potential of HTLV-1 and HTLV-2 is unlikely to be related to different modes of activation of the canonical NF-κB pathway

    MEET: A Monte Carlo Exploration-Exploitation Trade-off for Buffer Sampling

    Data selection is essential for any data-based optimization technique, such as Reinforcement Learning. State-of-the-art sampling strategies for the experience replay buffer improve the performance of the Reinforcement Learning agent. However, they do not incorporate uncertainty in the Q-Value estimation. Consequently, they cannot adapt the sampling strategies, including exploration and exploitation of transitions, to the complexity of the task. To address this, this paper proposes a new sampling strategy that leverages the exploration-exploitation trade-off. This is enabled by the uncertainty estimation of the Q-Value function, which guides the sampling to explore more significant transitions and, thus, learn a more efficient policy. Experiments on classical control environments demonstrate stable results across various environments. They show that the proposed method outperforms state-of-the-art sampling strategies for dense rewards w.r.t. convergence and peak performance by 26% on average.Comment: Accepted at ICASSP 202
